RESUMEN

MK-927 is a new topical carboanhydrase inhibitor with proven ability to lower intraocular pressure in glaucoma patients. Subjective and objective symptoms following topical application of MK-927 were evaluated in a randomized, double-blind, placebo-controlled study in which 24 healthy male volunteers took part. Symptoms were rated according to a visual analog scale by the volunteers and according to a grading scale by the examiner. The study showed good local tolerance of MK-927, comparable to that of Betaxolol.

RESUMEN

Oculo-oscillodynamography after Ulrich was performed in 27 patients suffering from glaucoma without hypertension (so-called low-tension glaucoma). Patients who had severe systemic disease or were receiving systemic medication which might influence IOP, and patients with narrow angle and an IOP higher than 22 mm Hg were excluded. In 85% of the patients a severe decrease in systolic ciliary perfusion pressure was found, whereas the systolic retinal and diastolic ocular perfusion pressures were significantly lower in almost 30% of the cases. Glaucoma without hypertension appears to be caused by the vascular change at the disk.

RESUMEN

Oculo-oscillodynamography (OODG) after Ulrich was performed on 21 patients suffering from unilateral hemodynamically relevant carotid stenosis. In ten patients the perfusion pressure was low on the affected side. In the other eleven patients there was no difference between the two sides and these patients were thought to have well-developed collateral anastomoses. In nine patients with temporal arteriitis low perfusion pressure was found by OODG on the side with the lower visual acuity. After systemic therapy with corticosteroids the perfusion pressure normalized on both sides.

RESUMEN

A pilot study was conducted to find out whether it is possible to achieve a further drop in intraocular pressure in eyes with glaucoma, which is about 22 mmHg after treatment with 0.5% timolol, by employing a combination preparation of timolol 0.5% and pilocarpine 2% or timolol 0.5% and pilocarpine 4% respectively, administered as eyedrops. This resulted in an average additional lowering of 1.9 mmHg after 2 hrs and by 2.1 mmHg after 6 hours (TP2) or by 3.0 mmHg after 2 and 3.5 mmHg after 6 and by 2.7 mmHg after 12 hours (TP4), the therapeutically relevant duration of effectivity being 6 (TP2) or 12 (TP4) hours, respectively.

RESUMEN

A study was conducted to investigate the influence of a vasodilating beta receptor blocking substance on IOP in eyes with open-angle glaucoma. The study proceeded from the hypothesis that throttling of the production of aqueous humor by beta blockers and reduction of intraocular pressure is caused by vasoconstriction of the afferent vessels. The authors found that the degree of reduction of intraocular pressure corresponded to the effect of beta blockers routinely applied in glaucoma therapy. However, the duration of the effect was shorter. The above-mentioned hypothesis would thus seem to be disproved.

RESUMEN

Examples of "endogenous" or direct and "endogenous" or indirect endothelial trauma are presented. Corneal contusion, McCannel suture, YAG laser iridectomy, congenital luxation of the lens, and chronic over wearing of contact lenses all cause traumatic changes. The etiology and effects of these changes are discussed. Specular microscopy of the corneal endothelium permits good clinical control of both the trauma and wound healing.

Fifty patients with open-angle glaucoma were treated twice daily for one year with topical 0.5% levobunolol, 1% levobunolol, or 0.5% timolol. Both concentrations of levobunolol were as effective as timolol in reducing intraocular pressure over the one-year period. At the concentrations tested, levobunolol and timolol decreased heart rate to a similar extent, suggesting that systemic absorption occurred after topical instillation. In all three treatment groups, clinically insignificant changes in blood pressure were observed sporadically throughout the one-year period. Very few clinically significant toxic ocular reactions were observed.

In a prospective study of 44 eyes with open angle-glaucoma and 27 with pseudocapsular glaucoma, the aqueous humor outflow facility was controlled for up to 12 months following argon laser trabeculoplasty (ALT). The average increase in the facility of outflow, which was 70% in open-angle glaucoma, was 25% higher in pseudocapsular glaucoma. In successful ALT, this increase in outflow facility was established within the first 20 hours after coagulation and did not decrease during the following year. There were no major complications such as transitory elevations of intraocular pressure, hyphema, or severe uveitis; this fact is interpreted as a result of the authors' technique, in which only the light-pigmented trabecular meshwork is coagulated.

In a randomized and controlled study (already published) the efficacy and safety of Pindolol eye drops 1% were compared with Timolol eye drops 0.5% in the treatment of open-angle glaucoma over a period of 6 months. With the exception of some allergic reactions there was no difference between the two with regard to IOP and the other parameters documented, such as pulse, blood pressure, visual acuity, refraction, diameter of pupils, tear production, visual field and biomicroscopic and ophthalmoscopic. In this article, the authors report on the results of prolongation of treatment in the 33 patients of the Pindolol group from month 7 to month 12 in an open-case study. The effect on IOP after 12 months was no different from that after month 6. There was no difference with regard to pulse and blood pressure, either. Four patients had to discontinue the treatment, in one case owing to drug-related side effects on the lids and conjunctiva. The other parameters documented (visual acuity, visual field, refraction, diameter of pupils, tear production, corneal sensitivity and biomicroscopic and ophthalmoscopic examination) revealed no difference between the first and the second 6 months. A diminution of the effect of the drug on the level and duration of IOP was ruled out.

RESUMEN

The results of a 3-month study of Befunolol eye drops 0.25% and 0.5% have already been published. The continuation of the study has shown that there is no diminution of the effect of the drug on IOP after a year of treatment. The absence of significant influences on pulse, systolic and diastolic blood pressure, corneal sensitivity, tear production, pupil diameter, visual acuity or visual field, which was established within the first 12 weeks, was also confirmed after 12 months.

In results of studies published hitherto it has been found that Pindolol eye drops 1%, a nonselective beta-blocking agent with intrinsic sympathomimetic activity have little or no effect on the bronchopulmonary system. Therefore, a long-term study of the pressure-lowering and other effects of this drug seems in open-angle glaucoma appeared desirable. In a multicenter double-blind trial 41 patients were treated with Pindolol eye drops 1% and 40 with Timolol eye drops 0,5%. A comparison of the two groups showed that their effect in IOP was almost identical. The other parameters of the examination revealed no significant differences between the two groups. - Five patients in the Pindolol group had to discontinue the study due to allergic reactions of the lids and/or conjunctiva .

The effect and possible side effects of the new beta-blocker Befunolol 0.25% and 0.5% were investigated for the first time in Europe in an open case study lasting 3 months. Good reduction of intraocular pressure was achieved with both concentrations. The statistical evaluation gave no indications of any significant influences on pulse, blood pressure, corneal sensitivity, tear production, pupil diameter, ECG, visual acuity or perimetry. No local side effects were observed and subjective tolerance was good. There was no diminution of the effect on IOP during the 3 months of the study.

RESUMEN

In a short-term study the pressure-lowering effects of Metipranolol 0.3% and Timolol 0.25% were compared in eyes with open-angle glaucoma by means of one-drop curves, diurnal pressure curves and tonography. It appears that these two drugs may be regarded as equal in respect of extent and duration of action. Aqueous humor outflow was facilitated slightly by both drugs, although increase was only statistically significant with Metipranolol.
