RESUMEN

Cervical cancer is the leading gynecological tumor associated with pregnancy accounting for 1.2-4.5 cases per 10,000 births. Precancerous diseases of the cervix, which include cervical intraepithelial neoplasia of high severity (H-SIL) combined with pregnancy, are even more relevant since the frequency of their occurrence can achieve 5.0%. Besides the peak of dysplastic cervical changes occurs in the mean age at delivery in the Russian Federation (28 years). The features of diagnosis and management of these patients during pregnancy, delivery and postpartum require a multidisciplinary approach from doctors of different specialties.

RESUMEN

There are presented data of literature and own observations of the treatment for recurrent ovarian cancer using hyperthermic intraperitoneal chemoperfusion. The possible complications during hyperthermic chemoperfusion are discussed and the effectiveness of the method is analyzed. Further studies are needed to obtain more certain criteria for abdominal chemotherapy in these patients.

RESUMEN

The efficiency of laparoscopic para-aortic lymphadenectomy and radiodiagnostic methods in pre-treatment clarification of tumor lesion extent in stage IB2-III cervical cancer patients was compared. The Magnetic Resonance or sonographic imaging data was found to be inferior to lymph nodes morphological investigation followed by morphological study of distant lymph nodes. The results demonstrate the adequacy of surgical staging by laparoscopic para-aortic lymphadenectomy in patients with locally advanced cervical cancer for therapy tactics decision making.

Comparison was made of changes in such morphological characteristics of tumor as cell differentiation and mitotic activity in 142 patients with endometrial carcinoma in the course of treatment, using tamoxifen or oxyprogesterone capronate or both, and those of estrogen background (karyopicnotic, eosinophilic and maturity indices, numerical index of maturity and blood-estradiol level). Three levels of estrogen background-low, medium and high-were identified by assay of the colpocytological indices in postmenopausal patients with endometrial carcinoma. Levels of the colpocytological indices observed at high indices of estrogen background proved a reliable numerical criterion for evaluating the sensitivity of the patients to neoadjuvant hormone therapy. A significant correlation was established between high estrogen background, pathogenetic variant I and high differentiation of tumor cells. The high incidence of hormonal pathology in tumor tissue was registered when the colpocytological indices dropped by half after oxyprogesterone capronate treatment, and by third-after combined therapy (tamoxifen + oxyprogesterone capronate). In cases free from pronounced pathomorphism, said indices dropped by not more than 20%.

RESUMEN

Multiple primary tumors have been detected in 192 (5.0%) cases out of 3,812 patients with primary cervical malignancies, 100 (2.6%) cases having been traced to radiation-related origin. The peculiarities of the syndrome of radiation-induced multiple tumors of the rectum, vagina and corpus uteri, duration of their development, histological patterns, degree of cell differentiation, depth of invasion and patterns of metastasis spreading have been identified. Relative cumulative risk of secondary tumor development and causal factors have been established.

RESUMEN

The study involved 1,256 cases of endometrial carcinoma. Minimal tumor (up to 2 cm in diameter, invasion- up to 0.5 cm) was diagnosed in 30%. This pattern showed a low potential for progression and metastasis spreading. Feasibility of identification of minimal cancer of the endometrium in the course of screening for hormone-dependent tumors of the reproductive system is discussed.
