RESUMEN

Objective@#To retrospectively analyze the potential correlation between cardiac magnetic resonance (CMR) imaging and clinical features and idiopathic arrhythmia in patients with straight back syndrome (SBS).@*Methods@#Patients receiving CMR imaging examination from April 2015 to March 2016 at our department (n=1 432) were screened, 76 patients met the diagnosis criteria of flat chest (anteroposterior diameter/transthoracic diameter (APD/TTD) ratio<0.37 at the T8 vertebra). After excluding 33 patients with structural heart disease, 43 SBS patients were divided into two groups: SBS without obvious morphological change in the heart (group A, n=19) and SBS with morphological change of the heart (group B, n=24). CMR images were analyzed, focusing the heart morphological changes induced by SBS. The clinical data were collected to comprehensively analyze the medical history, electrocardiogram and electrophysiological examination in order to observe the relationship between SBS induced heart morphological change and the arrhythmia type and origin.@*Results@#There were 21 male patients in this cohort, mean age was (28.5±11.5) years (13-58 years). APD/TTD ratio was similar between the two groups (0.30±0.03 vs. 0.29±0.04, P>0.05). LVEF tended to be lower in group B than in group A ((47.48±12.77)%vs. (59.31±9.04)%, P>0.05) . In group B, there were 15 patients with left ventricular enlargement, 2 with left ventricular wall thickening, 5 with uncoordinated ventricular wall motion, 5 with tricuspid regurgitation, 3 with mitral regurgitation, 2 with myocardial fibrosis, 5 with increased trabecular and 16 with decreased left ventricular function. Direct compression sign of right ventricle (disappeared precordial fat tissue space, secondary right atria enlargement and tricuspid regurgitation) and left atria (with or without secondary left ventricular enlargement and mitral regurgitation) were evidenced in patients of group B. CMR revealed that the arrhythmia origin corresponded the compression site of the heart in 8 cases (42.1%) in group A and 13 cases (54.2%) in group B, not corresponded to the compression site in 6 patients (31.6%) in group A and in 7 patients (29.2%) in group B, not attributable in 5 patients (26.3%) in group A and 4 patients (16.7%) in group B. The percent of arrhythmia origin corresponded the compression site of the heart tended to be higher in group B as compared to group A (P>0.05).@*Conclusion@#SBS can induce changes of cardiac morphology and cardiac function. SBS induced cardiac compression is linked with the development of arrhythmias and might be one of the reasons of arrhythmias in these patients.

RESUMEN

Objective To evaluate the apparant diffusion coefficient (ADC) values of cerebellar and the middle cerebellar peduncles in the differential diagnosis of multiple system atrophy (MSA) and Parkinson disease (PD). Methods Conventional MRI and DWI were performed in 18 clinically proved MSA patients with 7 cases of early cases (early-stage MSA group), 19 PD patients (PD group) and 18 agematched normal controls (the control group). DWI was performed using a single shot-spin echo-echo planar imaging sequences, and ADC values were measured in the ROIs (0. 16 cm2) of the bilateral cerebellum, the middle cerebellar peduncles and cerebral white matter. Then one way ANOVA test was used for statistical analysis. Results Of the 18 MSA patients, 11 had MR abnormalities, 8 had hot-cross bun sign in the pens on T2-weighted images, 11 patients had pontine, cerebellar and medulla oblongata atrophy, 10 patients had atrophy of the middle cerebellar peduncles, 2 patients had hyperintense rim of the putamen and putaminal atrophy on T2-weighted images. The ADC values in the middle cerebellar peduncles were significantly increased in the MSA group[ (0. 98 ±0. 07) × 103 mm2/s] and early-stnge MSA group [ (0. 95 ±0. 05) ×103 mm2/s] as compared to PD group [ (0. 77 ±0. 04) × 103 mm2/s] and control group[ (0. 78 ±0. 04) ×103 mm2/s]. There was statistical significant difference among them (F = 91.049,55. 301, P < 0.01 ).There was no overlap in the distribution of ADC values of the middle cerebellar peduncles among the MSA group [ (0.86-1.13 ) × 103 mm2/s ], early-stage MSA group [ (0. 86-1.02 ) × 103 mm2/s ] and PD group [ (0. 68-0. 84) × 103 mm2/s] and the control group [ (0. 69-0. 82) × 103 mm2/s]. The ADC values in the cerebellum were significantly increased in the MSA group[ (0. 95 ±0. 09) × 103 mm2/s] and early-stage MSA group [ (0. 92 ±0. 07) × 103 mm2/s] as compared to PD group [ (0. 78 ±0. 05) × 103 mm2/s] and control group[ (0. 79 ± 0. 05 ) × 103 mm2/s ]. Statistically significant difference was found among them (F =39. 274,18. 623 ,P <0. 01 ). There was overlap in the distribution of ADC values of the cerebellum [ MSAgroup(0. 80-1.10) × 103 mm2/s,early stage MSA group (0. 80-0. 99) × 103 mm2/s,PD group(0. 72-0. 90) × 103 mm2/s,control group (0. 71-0. 87) × 103 mm2/s]. There was no significant difference among the ADC values of MSA group, MSA group(early stages) and PD group and the control group in the cerebral white matter( P > 0. 05 ). RESUMEN

Objective: To study the correlation between MR perfusion imaging and pathology after transcatheter arterial chemoembolization (TACE) using VX2 liver cancer model and to provide a theoretical basis to evaluate the curative effect of TACE. Methods; Fifteen New Zealand white rabbits (weight: 2.5-3.0kg) were randomly divided into three groups, with 5 in each group (group 1, pre-TACE; group2, 3 days after TACE; group 3,1 week after TACE). The rabbit VX2 hepatic carcinoma models were presented in all rabbits. All of the three groups received TACE at three weeks after the tumor was implanted. The MR perfusion imaging was performed before chemoembolization, at 3 days and 1 week after chemoembolization respectively for group 1, 2 and 3. Each animal was then sacrificed for pathology observation after MR examination. Results: The lesions assessed before TACE were hyperintense compared with the surrounding liver parenchyma on DWI images. The volume of neoplastic cells became large. Nucleus was hypertrophic with different size and shape. Phase of nucleous mitosis showed in many cells and necrosis was hardly seen. No obvious difference was found between the peripheral area and the core area. At 3 days after TACE, the heterogeneous hypo-intense was observed on DWI images. Many nuclear fragmentation and caryolysis appeared on pathology. Neoplasm necrosis was seen. At 1 week after TACE, the heterogeneous hypo-intense areas became larger. Light microscopy showed incomplete necrosis. There were increased karyopycnosis and nuclear fragmentation. Conclusion: MR perfusion imaging of VX2 liver cancer corresponds well with pathology and can reflect the outcome of liver cancer after TACE.

Objective To evaluate the value of dynamic susceptibility contrast-enhanced MR imaging (DSC)in evaluation of brain tumor after operation. Methods 41 cases with brain tumors verified histologically, including 8 cases of high-grade gliomas,16 of low-grade gliomas,7 of metastases and 10 of meningiomas underwent DSC examination. All the cases at 3.0T MR unit. All primitive data were sent to ADW4.3 workstation for post-processing.The mean maximal rCBV and rCBF ratios of tumors,1 cm and 2 cm regions of peri-tumor were obtained respectively. The relativity of MR perfusion parameters(rCBV and rCBF)between diferent tumors were statistically evaluated by using Pearson correlative analysis. Results In the solid region of tumors,rCBV and rCBF were statistically significant differences (P<0.05)between low-grade gliomas and the other three groups of tumors; in 1 cm region of peri-tumor between high-grade gliomas and the other three groups, and between low-grade gliomas and metastases, between the solid region and both regions of 1 cm and 2 cm peri-tumors at the same tumor were all of statistically significant differences(P<0.05),between 1 cm and 2 cm region of peri-tumors at high-grade gliomas and metastases, the rCBV and rCBF were also of statistically significant differences.Pearson correlative analysis showed that there were significant positive correlations between the mean maximal rCBV and rCBF ratios at the same region in the same tumor's group. Conclusion DSC is very valuable in evaluation of the histopathological grade and the differentiation of brain tumor.

RESUMEN

Objective To quantitatively study the features of cerebromspinal fluid(CSF)flow dynamies in normal sella region in MRI with phase-contrast method. Methods Seventeen healthy volunteers were studied. The CSF flow image in sella region was positioned at the middle sagittal T1WI or T2WI image. This pulse sequence used a encoding velocity of 20 cm/s.The waveforms were analyzed for the maximum flow velocity, flow volume rate and the change of the figures. From the velocity and area measurements on the cine images, mean CSF flow was calculated in millimeters per second and milliliters per cardiac cycle. Results The normal CSF flow of the sella region had two directions which was downward(caudal)flow during thesystolic period and upward(cranial)flow during the diastolic period of the cardiac cycle. The downward and upward peak flow velocity, mean downward and upward flow velocity and mean flow velocity was(1.44±0.99)cm/s,(302.71±248.15)ms,(1.16±0.64)cm/s,(331.00±225.38)ms,(0.49±0.39)cm/s.(0.67±0.44)cm/s,(0.54±O.30)cm/s,respectively.The downward and upward peak flow volume rate.mean downward and upward flow volume rate and mean flow volume rate was (0.014±0.009)ml/s.(0.012±0.006)ml/s,(0.047±0.041)ml/s,(0.053±0.003)ml/s,(0.005±0.003)ml/s,(0.034±0.031)ml/s,respectively.The mean cycle was(775.25±173.06)ms.Conclusion Phase-contrast method in MRI cine is a noninvasive method to study the CSF flow in physiological and pathological conditions for determining the pattern, direction, speed and quantity of the CSF flow. Therefore it is better than other invasive research modalities and has an important value in clinical application.
