RESUMEN

Aim: This study aimed to investigate the incidence of BRAF mutation in cutaneous melanoma in the Ukrainian population with respect to clinical and histopathological data. Materials & methods: This single-center retrospective cohort study enrolled 299 primary CM with known BRAF status assessed by RT-PCR. Results: The overall BRAF mutation rate was 56.5% in CM and demonstrated a link with the younger age (p < 0.001), anatomical site (p < 0.001) and histological type of CM (p = 0.022). BRAF-positive CM possessed a slightly higher mitotic rate (p = 0.015) and Breslow thickness (p = 0.028) but did not relate to tumor-infiltrating lymphocytes. Conclusion: The high rate of BRAF mutations in CM patients in the Ukrainian cohort was associated with superficial spreading histology, higher depth of invasion and proliferation.

OBJECTIVE: The aim: Of this publication is to evaluate the results of EPEA in functionally healthy respondents and patients with NCDs to deepen fundamental knowledge of Systemic Medicine, increase the effectiveness of measures to prevent and treat NCDs by improving their diagnosis and prevention through the introduction of modern science-intensive technologies. PATIENTS AND METHODS: Materials and methods: 114 respondents were included in an open non-randomized controlled study. 66 сcomorbid patients (71% - men; median age - 61(43; 80) years) with verified Ischemic Heart Disease: functional class II-III angina pectoris were included in the Main Group. 78 functionally healthy young people (23% - men; median age - 23(20; 30) years) who did not play sports regularly were a Control Group. EPEA was made on a digital software hardware device Bio-Well 2.0 (Bio-Well 2.0, Russia-USA). RESULTS: Results: Significant differences in AEFE indicators in functionally healthy individuals and patients with NCDs were established. Qualitative indicators of AEFE (indicators of area, intensity, energy of emission) were significantly higher in functionally healthy individuals compared to patients with NCDs. Geometric indicators of AEFE (indicators of length of contours and radii of luminescence) were significantly higher in patients with NCDs than in healthy individuals, which confirmed the visually uneven and more complex contour of luminescence of the fingers in them. CONCLUSION: Conclusions: AEFE parameters have significant differences in functionally healthy individuals and patients with NCDs. AEFE is a fundamentally new promising approach to assessing the level of activity of metabolic processes at the tissue level in living biological systems, including in the normal human body and in patients with NCDs.

RESUMEN

OBJECTIVE: The aim of the publication is to substantiate a conceptual system of views on the role of internal electromagnetic fields and the scientific feasibility of studying the direction of their possible use as a diagnostic indicator in the examination of patients with NCDs. PATIENTS AND METHODS: Materials and methods: The of the presented data was carried out during a search study of methods for the rapid assessment of valeological status, prenosological early diagnosis of NCDs, analysis of existing methods for studying the electromagnetic phenomena of the human body. RESULTS: Review: The authors systematized the existing technical capabilities and instrumental methods in the framework of systemic medicine. The article proposes a working classification of methods, justifies the choice of methods for further research. CONCLUSION: Conclusions: 1) The study of the electromagnetic component of the human body as a diagnostic indicator in the examination of patients with NCDs is a relevant and promising scientific direction at the present stage. 2) Methods for assessing heart rate variability in a short recording, computerized hardware segmental diagnostics, a computerized digital study of Gas-discharge Electrophotonic Analysis, determination of the PBS-activity of body tissues were selected to further examine the value of their diagnostic parameters in examining patients with NCDs in the first and second levels of therapeutic care.

OBJECTIVE: Introduction: The article discusses the problems of joint activities of public and private sectors to ensure the proper quality of medical services and jointly overcome financial and production risks. It is emphasized that if the financial and administrative issues of such interaction have been studied to a certain extent, then a quantitative assessment of the quality of medical care provided in a public-private partnership has so far raised questions The aim: Identify ways to use industrial quality criteria for providing medical care to patients in public-private partnership conditions. PATIENTS AND METHODS: Materials and methods: The results of rehabilitation of 300 patients with gastrointestinal pathology who underwent rehabilitation in the resort "Kvitka Poloniny" of the TOV "Suzirya" were studied. 160 patients underwent rehabilitation in the private part of the resort, 140 - in the state. RESULTS: Results and conclusions: The theoretical approaches to the use of scientific and technological tools for the implementation of public-private partnership in medical institutions, in particular in a rehabilitation center, are discussed. They are based on instrumental approaches to assessing the quality of medical care. The use of Pareto assessment techniques and the Taguchi loss function allows us to quantitatively characterize the improvement in the quality of care provided with the help of public-private partnership.

RESUMEN

The problems of ensuring continuity of medical care are considered. Information and cybernetic issues of continuity, in particular information standards, resource management systems of a medical institution, are analyzed. It emphasizes the need to develop standards for the continuity of care. It is concluded that long-term continuity of medical measures in rehabilitation is one of the main components of the effectiveness of medical care. Further qualitative research is needed, including some types of rehabilitation services (e. g. specialized medical care). At the same time the most important factors for ensuring continuity at the current stage of medicine development are the use of ontologies, representing the most general conceptual concepts of the modeled field of medicine, completely abstracted from specific models of knowledge representation and practical implementation. It is also shown that the specification of the patient management model in the rehabilitation period of treatment is provided by a wide application of telemedicine consultations. The index of continuity of medical aid is also proposed. It represents a simple additive function that reflects the change in the probability of an unfavorable outcome.
