BACKGROUND: Surgical site infections (SSIs), mainly caused by Staphylococcus aureus, pose a significant economic burden in Europe, leading to increased hospitalization duration, mortality, and treatment costs, particularly with drug-resistant strains such as meticillin-resistant S. aureus. AIM: To conduct a case-control study on the economic impact of S. aureus SSI in adult surgical patients across high-volume centres in France, Germany, Spain, and the UK, aiming to assess the overall and procedure-specific burden across Europe. METHODS: The SALT study is a multinational, retrospective cohort study with a nested case-control analysis focused on S. aureus SSI in Europe. The study included participants from France, Germany, Italy, Spain, and the UK who underwent invasive surgery in 2016 and employed a micro-costing approach to evaluate health economic factors, matching S. aureus SSI cases with controls. FINDINGS: In 2016, among 178,904 surgical patients in five European countries, 764 developed S. aureus SSI. Matching 744 cases to controls, the study revealed that S. aureus SSI cases incurred higher immediate hospitalization costs (€8,810), compared to controls (€6,032). Additionally, S. aureus SSI cases exhibited increased costs for readmissions within the first year post surgery (€7,961.6 versus €5,298.6), with significant differences observed. Factors associated with increased surgery-related costs included the cost of hospitalization immediately after surgery, first intensive care unit (ICU) admission within 12 months, and hospital readmission within 12 months, as identified through multivariable analysis. CONCLUSION: The higher rates of hospitalization, ICU admissions, and readmissions among S. aureus SSI cases highlight the severity of these infections and their impact on healthcare costs, emphasizing the potential benefits of evidence-based infection control measures and improved patient care to mitigate the economic burden.

BACKGROUND: Concerns have been raised about intraseasonal waning of the protection conferred by influenza vaccination. METHODS: During four influenza seasons, we consecutively recruited individuals aged 18years or older who had received seasonal influenza vaccine and were subsequently admitted to the hospital for influenza infection, asassessed by reverse transcription polymerase chain reaction. We estimated the adjusted odds ratio (aOR) of influenza infection by date of vaccination, defined by tertiles, as early, intermediate or late vaccination. We used a test-negative approach with early vaccination as reference to estimate the aOR of hospital admission with influenza among late vaccinees. We conducted sensitivity analyses by means of conditional logistic regression, Cox proportional hazards regression, and using days between vaccination and hospital admission rather than vaccination date. RESULTS: Among 3615 admitted vaccinees, 822 (23%) were positive for influenza. We observed a lower risk of influenza among late vaccinees during the 2011/2012 and 2014/2015A(H3N2)-dominant seasons: aOR=0.68 (95% CI: 0.47-1.00) and 0.69 (95% CI: 0.50-0.95). We found no differences in the risk of admission with influenza among late versus early vaccinees in the 2012/2013A(H1N1)pdm09-dominant or 2013/2014B/Yamagata lineage-dominant seasons: aOR=1.18 (95% CI: 0.58-2.41) and 0.98 (95% CI: 0.56-1.72). When we restricted our analysis to individuals aged 65years or older, we found a statistically significant lower risk of admission with influenza among late vaccinees during the 2011/2012 and 2014/2015A(H3N2)-dominant seasons: aOR=0.61 (95% CI: 0.41-0.91) and 0.69 (95% CI: 0.49-0.96). We observed 39% (95% CI: 9-59%) and 31% (95% CI: 5-50%) waning of vaccine effectiveness among participants aged 65years or older during the two A(H3N2)-dominant seasons. Similar results were obtained in the sensitivity analyses. CONCLUSION: Waning of vaccine protection was observed among individuals aged 65years old or over in two A(H3N2)-dominant influenza seasons.

OBJECTIVE: To determine the prevalence of Adverse Events related to Medication (AEM) in hospitals of the Valencian Community in the 2005-2013 study period, and to describe the associated risk factors and their impact. METHODS: This study is based on data and methodology of the Study of Prevalence of Adverse Events in hospitals (EPIDEA), since its inception in 2005 until 2013. AEM produced in each year were analyzed. RESULTS: We identified 344 AEM that occurred in 337 patients, among 35,103 patients studied, giving a prevalence of patients with AEM of 0.96% (IC95% 0.89-1.07). The most prevalent intrinsic risk factors for AEM were hypertension, diabetes and cancer. The most prevalent extrinsic risk factors were peripheral venous catheter, urinary catheter and central venous catheter. Therapeutic groups most frequently involved were systemic antibiotics, cardiovascular drugs and antineoplastics. The 61.17% of AEM was classified as moderate, followed by 27.18% as mild and 11.65% as severe. The 33.99% of EAM caused increase of the patient's stay and 39.90% of EAM caused the re-entry of patient. The 58.5% of AEM were avoidable. Mild AEM were avoidable in 46.3%, moderate AEM were avoidable in 60.3% and severe AEM were in 75% (p = 0.013). CONCLUSIONS: The prevalence of patients with AEM in hospitals of the Community of Valencia for the period 2005- 2013 was 0.96%. More than half of AEM were preventable, and preventability increases significantly with the severity of the event.

RESUMEN

Preliminary results for the 2014/15 season indicate low to null effect of vaccination against influenza A(H3N2)-related disease. As of week 5 2015, there have been 1,136 hospital admissions, 210 were due to influenza and 98% of subtype A strains were H3. Adjusted influenza vaccine effectiveness was 33% (range: 6-53%) overall and 40% (range: 13% to 59%) in those 65 years and older. Vaccination reduced by 44% (28-68%) the probability of admission with influenza.

Fundamento y objetivo: El aumento constante de la demanda asistencial en los servicios de hospitalización general, y más concretamente en los servicios pediátricos, justifica el interés por conocer su situación real en relación con las necesidades de atención pediátrica por parte de la población. El objetivo de este estudio es determinar la adecuación de las estancias e ingresos del Hospital Infantil «La Fe» (Valencia). Materiales y métodos: Estudio transversal descriptivo con componentes analíticos. La unidad de análisis fue un día de hospitalización (13 de mayo de 2007) ante los informes de alta del servicio de urgencias generales de dicho hospital durante la tercera semana de octubre de 2007, a los que se aplicó la versión pediátrica del protocolo de adecuación de ingresos y estancias PAEP (Paediatric Appropriateness Evaluation Protocol).Resultados: El porcentaje de inadecuación de ingresos fue del 10,5% y el de inadecuación de las estancias del 18,4%. La causa más frecuente de inadecuación de los ingresos fueron «las necesidades diagnósticas y terapéuticas pueden ser realizadas como paciente externo». La causa más frecuente de inadecuación de estancias fueron los «problemas de programación de pruebas o procedimientos quirúrgicos». Discusión: Confirmamos la hipótesis de la existencia de hospitalizaciones innecesarias en pediatría. El porcentaje de ingresos y estancias inadecuados fue similar al de otros estudios. El protocolo PAEP resultó ser un instrumento fácil de utilizar. El conocimiento de los factores de riesgo para los ingresos y las estancias inadecuados nos permitirá establecerlas medidas correctoras para optimizar la utilización de los recursos hospitalarios pediátricos en nuestro medio (AU)


Foundation and objective: The constant increase of the welfare demand in the departments of general hospitalization and more concretely in the pediatric department justifies the interest to know its real situation in relation to the needs of pediatric attention by part of the population. The objective of this study is to determine the adequacy of the stays and admissions of the infantile hospital La Fe (Valencia).Materials and methods: Transverse descriptive study with analytical components. The unit of analysis was a day of hospitalization (05/13/2007) before the reports of discharge of the general emergency unit of the infantile hospital La Fe during the third week of October 2007, to those to whom the pediatric version of the protocol of adequacy of admissions and stays (Pediatric Appropriateness Evaluation Protocol) PAEP was applied. Results: The inadequacy of admissions was of 10.5%. The percentage of inadequacy of stays was of 18.4 %. The more frequent reason of inadequacy of admissions was “the diagnostic and therapeutic needs can be conducted as external patient”. The most frequent reason of inadequacy of stays was “problems of programming test or surgical procedures”. Discussion: We confirm the hypothesis of the existence of unnecessary hospitalization in pediatrics. The percentage of inadequate admissions and stays was similar to that of other studies. The procotol PAEP turned out to be an easy to use instrument. The knowledge of the risk factors for inadequate admissions and stays will allow us to introduce correcting measures to optimize the use of the hospital pediatric resources in environment (AU)
