RESUMEN

Se presentan 2 casos, separados por un intervalo de más de 2500 años, de Enfermedad de Forestier - Rotés-Querol, uno de ellos procedentes de un enterramiento de la Atenas de Pericles y el otro actual para ejemplificar su presencia continua en la historia de la patología humana. Se realiza una sucinta revisión histórica sobre su separación de las espondilopatías inflamatorias anquilosantes, se revisan sus posibles manifestaciones clínicas y radiológicas y se menciona su importante papel en el desarrollo de la última película de Pedro Almodóvar, Dolor y Gloria


We present two cases of Forestier-Rotés-Querol disease, separated by an interval of more than 2500 years, one of them coming from a burial in the Athens of Pericles and the other from the present. This exemplify its continuous presence in the history of human pathology. A brief historical review of their separation from ankylosing inflammatory spondylopathies is carried out their possible clinical and radiological manifestations are reviewed, and it is mentioned an important role in the development of the latest film by Pedro Almodóvar, Dolor y Gloria

RESUMEN

Despite the potential benefits of outpatient care, most patients with pulmonary embolisms are treated in hospitals for fear of possible adverse events. However, there is a wealth of scientific evidence from studies covering more than 4000 outpatients, which has led the current clinical practice guidelines to recommend early discharge or outpatient treatment when a low risk of death or complications has been confirmed, when there are no comorbidities or aggravating processes present to warrant hospitalisation and when appropriate monitoring and treatment are observed. This approach minimises the complications that can arise in hospitals and represents considerable cost savings. When selecting these patients, the use of prognostic tools such as the Pulmonary Embolism Severity Index (PESI), its simplified version (sPESI) and the Hestia Criteria are of paramount importance. Using these tools, the short-term outcomes (30-90days) show low mortality (in general <3%) and a low incidence of other complications (rate of recurrence and major bleeding <2%). Based on the available evidence, outpatient treatment can be considered the most appropriate strategy at this time for most hemodynamically stable patients with pulmonary embolisms.

Se analizan los datos patobiográficos de 4 geniales artistas del siglo xx −Modigliani, Matisse, Portinari y Rebeca Horn− que vieron truncadas sus carreras artísticas en algún momento debido a enfermedades profesionales ocasionadas por los materiales que utilizaban. En el caso de Matisse, por manipulación debido a su propio temperamento de las cicatrices de una intervención quirúrgica. Sin embargo, lucharon contra tal contratiempo y su superación les permitió hallar nuevas vías de expresión artística para encauzar su creatividad(AU)

AIM: Cardiac rhythm alterations are a frequent cause of hospital admission. However, we do not know their characteristics and economic costs. We have analyzed the epidemiology and cost of hospitalizations due to cardiac arrhythmias in the National Health System. METHODS: The characteristics and costs were reviewed in patients admitted with a principal diagnosis of cardiac arrhythmia (1997-2010;diagnosis related groups [DRG] -138 and 139 of the National Health System minimum data base set). Atrial fibrillation/flutter accounted for 65% of these DRGs. The secondary diagnoses prevalent in such DRGs were also reviewed. RESULTS: Hospitalizations due to cardiac arrhythmias were approximately 26.000 per year and close to 1.6% for mortality. In 2010 there were 26.421 hospitalizations with an estimated cost of 65 million Euros. Frequent comorbidities were recorded, such as hypertension (43%), heart failure (12%) or diabetes mellitus (20%). A total of 43% were admitted to cardiology and 36% to internal medicine. CONCLUSIONS: During the period 1997-2010, there was a significant annual number of hospitalizations for cardiac arrhythmias (mainly atrial fibrillation), with measurable costs, in Spain. More than one third were attended by internists.

RESUMEN

Stroke is a terrible disease conditioning in an important manner the lives of patients. Recovery is, often, very laborious. The cases of six well-known artists who suffered it are presented and also their efforts to defeat it. Patobiographic data of the painters C.F. Friedrich, L. Krasner and C. Close, poet and artist G. Apollinaire, the film director and cartoonist F. Fellini and actor K. Douglas are discussed. The vital changes introduced by a such process, the strength to recover and the post-stroke artwork production are commented. The artists' premorbid personality influenced notably the recovery. These artists faced to the disease and with their masterpieces also bequeathed to the posterity their examples of courage.
