RESUMEN

Paracoccidioidomycosis (PCM) is a systemic mycosis that is diagnosed by visualizing the fungus in clinical samples or by other methods, like serological techniques. However, all PCM diagnostic methods have limitations. The aim of this study was to develop a diagnostic tool for PCM based on Fourier transform infrared (FTIR) spectroscopy. A total of 224 serum samples were included: 132 from PCM patients and 92 constituting the control group (50 from healthy blood donors and 42 from patients with other systemic mycoses). Samples were analyzed by attenuated total reflection (ATR) and a t-test was performed to find differences in the spectra of the two groups. The wavenumbers that had p < 0.05 had their diagnostic potential evaluated using receiver operating characteristic (ROC) curves. The spectral region with the lowest p value was used for variable selection through principal component analysis (PCA). The selected variables were used in a linear discriminant analysis (LDA). In univariate analysis, the ROC curves with the best performance were obtained in the region 1551-1095 cm-1. The wavenumber that had the highest AUC value was 1264 cm-1, achieving a sensitivity of 97.73%, specificity of 76.01%, and accuracy of 94.22%. The total separation of groups was obtained in the PCA performed with a spectral range of 1551-1095 cm-1. LDA performed with the eight wavenumbers with the greatest weight from the group discrimination in the PCA obtained 100% accuracy. The methodology proposed here is simple, fast, and highly accurate, proving its potential to be applied in the diagnosis of PCM. The proposed method is more accurate than the currently known diagnostic methods, which is particularly relevant for a neglected tropical mycosis such as paracoccidioidomycosis.

Recovering anthocyanins from black rice bran is a way of valuing this byproduct, by obtaining an extract with biological potential. The objective of this study was to recover anthocyanins using ultrasound-assisted extraction. Some of the extract was partially purified, and both (crude and partially purified) extracts were evaluated for their anthocyanin content, antioxidant activity, antidiabetic and antitumoral activities, cytotoxicity, and oxidative stress. An increase in the laboratory scale was also achieved, making possible to increase the extraction volume up to 20 times without significantly changing the content of anthocyanins (1.85 mg C3G/g DW). It was found that the purified sample presented a 4.2 times higher value of total anthocyanins compared to the crude sample. The best IC50 values for the purified sample were verified by DPPH and ABTS (0.76 and 0.33 mg/mL). The best results for antidiabetic activity were obtained for the partially purified sample: 0.82 µM C3G for α-glucosidase and 12.5 µM C3G for α-amylase. The extracts demonstrated protection (~70%) when subjected to the oxidative stress of L929 cells. An antitumoral effect of 25-30% for both extracts was found in A459 cells. The crude and partially purified extracts of black rice have antidiabetic and anticancer effects and more studies are needed to explore their potential.

Fungal melanin contributes to the survival and virulence of pathogenic fungi, such as Fonsecaea pedrosoi, which is responsible for causing chromoblastomycosis. The objective of this study was to employ Fourier transform infrared spectroscopy (FTIR) to predict the melanin content of F. pedrosoi. The melanin content, in percentage, was previously determined using gravimetry for twenty-six clinical isolates. Quintuplicate spectra of each isolate were obtained using attenuated total reflection (ATR) within the range of 4000 to 650 cm-1. To predict the melanin content, modeling was performed using partial least squares regression (PLS) in the region 1800 - 750 cm-1. Two models were tested: PLS and successive projections algorithms for interval selection in partial least squares (iSPA-PLS). The best modeling results were achieved using iSPA-PLS with one factor. The calibration set exhibited a determination coefficient (R2) of 0.9745 and a root mean square error of cross-validation (RMSECV) of 0.0977. In the prediction set, the R2 value was 0.9711, and the root mean square error of prediction (RMSEP) was 0.0999. Modeling with FTIR and multivariate calibration provides a valuable means of predicting fungal melanin content, which is simpler and more robust, thereby contributing to the advancement of this field of study.

Diffuse hemispheric glioma (DHG), H3 G34 mutant was included in the 5th edition of the World Health Organization Classification of Tumors of the Central Nervous System recently published. Given the recent inclusion in the current classification and its rarity in adult patients, there are scarce data on clinical-radiological characteristics, survival, and outcome. The authors report the case of a 35-year-old female with DHG, H3 G34-mutant characteristics and outcomes with an unusual presentation, recurrence, and prolonged survival. In conclusion, our case report demonstrates relevant details that should be observed in patients with suspicion or confirmation of the diagnosis of DHG, H3 G34 mutant, not only in the initial presentation but also in the evolution to ensure more personalized treatment.

BACKGROUND: Leprosy is an infectious and contagious disease caused by Mycobacterium leprae and is mainly characterized by lesions in the skin and peripheral nerves. In Brazil, it is a public health problem due to its high endemicity. However, the state of Rio Grande do Sul presents low endemicity of this disease. OBJECTIVE: To characterize the epidemiological profile of leprosy in the state of Rio Grande do Sul from 2000 to 2019. METHODS: This was a retrospective observational study. Epidemiological data were collected from the Notifiable Diseases Information System (SINAN, Sistema de Informação de Agravos de Notificação). RESULTS: Among the 497 municipalities in the state, 357 (71.8%) registered cases of leprosy in the assessed period, with an average of 212 (81.5%) new cases per year. The average detection rate was 1.61 new cases per 100,000 inhabitants. The male sex was predominant (51.9%) and the mean age was 50.4 years. Regarding the epidemiological clinical profile; 79.0% of the patients were multibacillary; 37.5% presented the borderline clinical form; 16% had grade 2 physical disability at diagnosis and bacilloscopy was positive in 35.4% of cases. As for treatment, 73.8% of the cases were treated with the standard multibacillary therapeutic regimen. STUDY LIMITATIONS: There were missing/inconsistent data in the database available. CONCLUSIONS: The findings observed in this study indicate that the state presents a low endemicity profile of the disease and these results can support adequate health policies relevant to the reality of Rio Grande do Sul, inserted in a national scenario of highly endemic leprosy.

Biosurfactants are amphipathic molecules that can be applied in a wide range of areas. The cost of production limits the industrial application of biosurfactants. Nevertheless, the biosurfactant productivity can be easily enhanced by inducers. This work aimed to investigate the effect of hydrophobic inducers on surfactin production by B. subtilis ATCC 6633 using cassava wastewater as low-cost culture medium. The submerged cultivation was carried out at 30 °C, 150 rpm for 72 h. The fermentation parameters used were bacterial growth, consumption of sugars, and surfactin production, including surfactin homologues. The surface tension decreased by 40% after 12 h, when compared to control. Depletion of sugars was observed in all experiments. Palmitic acid led to the highest yield in terms of surfactin production (≈ 1.3 g·L- 1 of pure surfactin). The inducers triggered the production of new surfactin homologues, that represent, potentially, new biological activities.

BACKGROUND: Establishing the risk of malignant transformation (MT) in oral leukoplakia is usually based on grading oral epithelial dysplasia (OED) on biopsy tissue, for which two systems are proposed: a 3-tier and a binary system. Only very few actuarial studies have tested the accuracy of such methods in predicting MT, especially for the binary system. This study aimed to assess the accuracy of the two grading systems in predicting MT in a cohort of oral leukoplakia (OL) from Brazil, with follow-up data. METHODS: The sample comprised 878 individuals diagnosed with OL from 2005 to 2018. Follow-up data were obtained both locally and from the regional cancer registry. All lesions were graded using both the 3-tier and the binary systems. Kaplan-Meier curves (Log-rank Mantel-Cox) were used to assess risk and kappa to assess interobserver agreement. RESULTS: Thirty-five individuals underwent MT (4%). Both systems demonstrated prognostic value, though the 3-tier system proved superior, with OR 9.23 (3.42-23.69), PPV 0.152, NPV 0.98, compared to binary OR 3.49 (1.79-6.79), PPV 0.079, NPV 0.976. Interobserver agreement was also superior in the 3-tier system (0.47, p < 0.05) compared to the binary system (0.139, p = 0.39). Combining the two systems enhanced prognostic values (OR 14.28, PPV 0.217, NPV 0.981). CONCLUSION: The 3-tier system presented superior prognostic value to the binary system. Combining both systems to double-grade intermediate lesions might enhance risk assessment.

PURPOSE: A nationwide lockdown was enforced in Brazil starting in March 2020 because of the COVID-19 pandemic when cancer screening activities were reduced. In this study, we evaluated the impact of the COVID-19 pandemic on breast cancer (BC) diagnosis. METHODS: We extracted data from the medical records of patients age older than 18 years who were diagnosed with BC and started treatment or follow-up in private oncology institutions in Brazil between 2018 and 2021. The primary objective was to compare the stage distribution during the COVID-19 pandemic (2020-2021) with a historical prepandemic control cohort (2018-2019). Early BC was defined as stage I-II and advanced disease as stage IV. RESULTS: We collected data for 11,753 patients with an initial diagnosis of BC, with 6,493 patients in the pandemic (2020-2021) and 5,260 patients in the prepandemic period (2018-2019). We observed a lower prevalence of early-stage BC (63.6% v 68.4%) and a higher prevalence of advanced-stage BC (16.9 v 12.7%), after the onset of the pandemic (both P < .01). This pattern was similar for both estrogen receptor-positive/human epidermal growth factor receptor 2-negative and human epidermal growth factor receptor 2-positive tumors: significantly decreased in the early stage from 69% to 67% and 68% to 58%, respectively, and a considerable increase in advanced-stage disease from 13% to 15% and 13% to 20%, respectively. For triple-negative BC, there was a significantly higher percentage of patients with advanced-stage disease during the pandemic (17% v 11%). Overall, age 50 years or older and postmenopausal status were associated with a greater risk of advanced stage at diagnosis during the pandemic period. CONCLUSION: We observed a substantial increase in the number of cases of advanced-stage BC in Brazil during the COVID-19 pandemic.

Objective: To assess the evidence available and knowledge gaps in photobiomodulation (PBM) for oral facial pain. Background data: Effective identification of a noninvasive resource for oral facial pain such as PBM may mitigate the risks of invasive therapeutic resources. Methods: Nine electronic databases were searched for systematic reviews reporting oral facial pain outcome for PBM. The 3iE (International Initiative for Impact Evaluation) evidence gap map methodology with the tableau was used to graphically display the parameters analyzed in the research. Results: Several wavelengths within the range of infrared were used in 37.6% of the studies, accompanied by the 32.4% in the red range. The quality of the effect was positive in 61.4% of the studies, whereas the impact degree was low, according to the measurement tool used to assess systematic reviews 2 (AMSTAR 2), in 60.2%. Conclusions: Despite the positive potential of PBM in the treatment and control of pain in diseases of the oral cavity, complete information on dosimetry in published studies with PBM is still lacking, making it difficult to reproduce the results found.

BACKGROUND: Paracoccidoidomycosis (PCM) is a granulomatous systemic mycosis endemic in southern Brazil. OBJECTIVES: Analyze the clinical and epidemiological characteristics of cases of PCM with head and neck manifestations in southern Brazil. MATERIAL AND METHODS: Retrospective study of histopathologically diagnosed cases of PCM with head and neck manifestations referred to two medical centers in the municipality of Santa Cruz do Sul, state of Rio Grande do Sul, Brazil, during a 10-year period (2011-2020). RESULTS: Twenty-eight patients were selected. The number of cases usually ranged from one to four per year, although 11 cases were diagnosed in 2019. Most patients were between 40 and 59 years old. In total, 21 patients were men and seven were women (male:female ratio 3:1). Most (92%) were Caucasian and 46% were smokers. Patients were from 12 municipalities in the central-eastern region of the state. The most frequent site of the lesions was the larynx. Associated HIV and squamous cell carcinoma were both present in three cases. CONCLUSIONS: This is the first study to analyze PCM cases from central-eastern Rio Grande do Sul. PCM is endemic in southern Brazil, and more epidemiological data are needed, considering that its incidence may be higher than currently estimated, as demonstrated by our results.

Leprosy is an infectious and contagious disease affecting skin and nerves. The number of cases in individuals under 15 years old is one of the parameters used in Brazil as an indicator of endemic permanence of the disease and its continuous transmission. Rio Grande do Sul State, in Southern Brazil, is low-endemic to leprosy. However, the disease remains a public health problem. This is a retrospective, observational and analytical study of a historical series of new cases of leprosy in children under 15 years old diagnosed in the period from 2000 to 2019, in all health units in Rio Grande do Sul State. Seventy-seven new cases were notified. The male gender was predominant in 53.2% of the cases (n=41). The average age was 10.4 years (standard deviation of 2.9), with predominance of the age group between 10 and 15 incomplete years old. The most frequent operational classification was multibacillary, in 62.3% of cases (n=48), and the most common clinical form was borderline, in 38.9% of cases (n=28). The predominant disability degree in the sample was grade zero, in 80.0% of the cases (n=60), but in 4.0% (n=3) the grade assessed was 2. In 54.0% of cases (n=27), bacilloscopy was performed, with positive results in 36.0% (n=9) of the exams. Multibacillary cases, with physical disability and/or positive bacilloscopy, draws attention that that the diagnosis is frequently not made in early stages.

Objective: the present report describes the clinical, radiographic, and histopathological features of an ameloblastic fibro-odontoma (AFO) lesion. Case report: we report a clinical case of a 14-year-old boy with asymptomatic edema. Panoramic radiography detected a unilocular lesion with defined margins located in the posterior region of the mandible. The internal structure of the lesion presented several degrees of radiopacity with the involvement of the third molar. Cone-beam computed tomography revealed expanded buccal and lingual cortical bones, perforation of the lingual cortical bone, and displacement of the mandibular canal. AFO was suspected based on the radiographic and clinical characteristics. Total excision was performed and histologically examined, confirming the diagnosis of AFO. No recurrence occurred during a 24-month follow-up period. Final considerations: the evaluation of the clinical, radiographic, and histopathologic findings needs to be accurate for a correct diagnosis and appropriate treatment for case of AFO since the presentation is often asymptomatic.(AU)


Objetivo: o presente relato descreve as características clínicas, radiográficas e histopatológicas de uma lesão de fibro-odontoma ameloblástico (FOA). Relato de caso: relatamos o caso clínico de um menino de 14 anos com edema assintomático. A radiografia panorâmica detectou lesão unilocular com margens definidas e localizada na região posterior da mandíbula. A estrutura interna da lesão apresentava vários graus de radiopacidade com envolvimento do terceiro molar. A tomografia computadorizada de feixe cônico revelou as corticais ósseas vestibular e lingual expandidas, perfuração da cortical óssea lingual e deslocamento do canal mandibular. FOA foi a hipótese diagnóstica com base nas características radiográficas e clínicas. A excisão total foi realizada e examinada histologicamente, confirmando o diagnóstico de FOA. Nenhuma recorrência ocorreu durante um período de acompanhamento de 24 meses. Considerações finais: a avaliação das características clínicas, radiográficas e histopatológicas contribuíram para um diagnóstico correto e o tratamento adequado para o caso de FOA, uma vez que a lesão é frequentemente assintomática.(AU)

Erythema nodosum leprosum (ENL) is an inflammatory complication caused by a dysregulated immune response to Mycobacterium leprae. Some Toll-like receptors (TLRs) have been identified as capable of recognizing antigens from M. leprae, triggering a wide antimicrobial and inflammatory response. Genetic polymorphisms in these receptors could influence in the appearance of ENL as well as in its treatment. Thus, the objective of this work was to evaluate the association of genetic variants of TLRs genes with the response to treatment of ENL with thalidomide and prednisone. A total of 162 ENL patients were recruited from different regions of Brazil and clinical information was collected from their medical records. Genomic DNA was isolated from blood and saliva samples and genetic variants in TLR1 (rs4833095), TLR2 (rs3804099), TLR4 (rs1927914), and TLR6 (rs5743810) genes were genotyped by TaqMan real-time PCR system. In order to evaluate the variants' association with the dose of the medications used during the treatment, we applied the Generalized Estimating Equations (GEE) analysis. In the present sample, 123 (75.9%) patients were men and 86 (53.1%) were in treatment for leprosy during the ENL episode. We found an association between polymorphisms in TLR1/rs4833095, TLR2/rs3804099, TLR4/rs1927914, and TLR6/rs5783810 with the dose variation of thalidomide in a time-dependent manner, i.e., the association with the genetic variant and the dose of the drug was different depending on the moment of the treatment evaluated. In addition, we identified that the association of polymorphisms in TLR1/rs4833095, TLR2/rs3804099, and TLR6/rs5783810 with the dose variation of prednisone also were time-dependent. Despite these associations, in all the interactions found, the influence of genetic variants on dose variation was not clinically relevant for therapeutic changes. The results obtained in this study show that TLRs polymorphism might play a role in the response to ENL treatment, however, in this context, they could not be considered as useful biomarkers in the clinical setting due small differences in medication doses. A larger sample size with patients with a more genetic profile is fundamental in order to estimate the association of genetic variants with the treatment of ENL and their clinical significance.

ABSTRACT BACKGROUND AND OBJECTIVES: Burning mouth syndrome is a dysfunctional state affecting physical, mental and social welfare, often contributing to chronic stress conditions. Despite the lack of objective data, patients experience pain-related discomfort with impact in their daily life. The objective of this study was to assess the impact of burning mouth syndrome on pain perception and quality of life. METHODS: A cross-sectional, observational, case-controlled study was performed on 76 individuals (38 in each group). The groups were sex- and age-matched. The Oral Health Impact Profile (OHIP-14) questionnaire was used to assess any changes in the quality of life. The visual analog scale was used to assess pain impact and intensity, as well as the Pain Catastrophizing Scale (PCS). The effect of sex and other risk factors associated with burning mouth syndrome were also associated. RESULTS: The age of participants was 41 to 85 years. The patients had a negative impact on quality of life with respect to all dimensions of OHIP-14 and PCS domains. Burning mouth syndrome patients complained about moderate (58%) or intense (42%) pain, while the control group participants experienced only mild pain by visual analog scale. The prevalence was predominant in females (a ratio of 3:1), and the most site involved was the tongue. Menopause, hormonal changes, and gastritis were identified as major risk factors. CONCLUSION: Burning mouth syndrome patients had significantly higher PCS and OHIP-14 scores for all domains, indicating an interaction between a higher burden of pain perception and worse quality of life which should therefore be adequately assessed, characterized and managed.


RESUMO JUSTIFICATIVA E OBJETIVOS: A síndrome de ardência bucal é um estado disfuncional que afeta o bem-estar físico, mental e social, contribuindo para condições de estresse crônico. Apesar da ausência de dados objetivos, os pacientes experimentam desconforto relacionado à dor com impacto na vida diária. O objetivo deste estudo foi avaliar o impacto da síndrome da boca ardente na percepção da dor e na qualidade de vida. MÉTODOS: Foi realizado um estudo transversal, observacional e caso-controle em 76 indivíduos, 38 em cada grupo, pareados por gênero e idade. Foram utilizados o questionário Oral Health Impact Profile (OHIP-14) para avaliar mudanças na qualidade de vida, a escala analógica visual para o impacto e intensidade da dor e a Pain Catastrophizing Scale (PCS). Foi avaliado também o efeito do sexo, idade e outros fatores de risco associados à síndrome de ardência bucal. RESULTADOS: A idade dos participantes foi de 41 a 85 anos. A síndrome de ardência bucal teve um impacto negativo na qualidade de vida em todas as dimensões dos domínios OHIP-14 e PCS. Cinquenta e oito por cento dos pacientes se queixaram de dor moderada e 42% de dor intensa, enquanto os controles experimentaram apenas dor leve pela escala analógica visual. A prevalência foi predominante no sexo feminino (3:1), e a língua foi o local envolvido mais comum. Menopausa, alterações hormonais e gastrite foram os maiores fatores de risco. CONCLUSÃO: Os pacientes com síndrome de ardência bucal apresentaram escores PCS e OHIP-14 mais altos para todos os domínios, indicando uma interação entre maior carga de percepção da dor e pior qualidade de vida, o que deve ser mais bem avaliado, caracterizado e gerenciado.

ABSTRACT BACKGROUND AND OBJECTIVES: The measurement of pain and discomfort from work-related musculoskeletal disorders is a conceptual and empirical challenge, considering the cognitive and subjective processes involved in the evaluation of somatic symptoms. The objective of this study was to construct a work-related musculoskeletal disorders index to evaluate musculoskeletal discomfort in workers with and without repetitive strain injuries/work-related musculoskeletal disorders, a syndrome that affects thousands of workers every year. METHODS: The sample was composed of 182 bank clerks from the State of Bahia, 91 of whom were diagnosed with the syndrome. RESULTS: The work-related musculoskeletal disorders index had a frequency distribution similar to the normal curve, with averages of 7.1 and 4.1 for the groups with and without a diagnosis, respectively. CONCLUSION: In addition to the adequate psychometric properties, the proposed index was able to differentiate, satisfactorily, workers with and without a diagnosis of repetitive strain injuries/work-related musculoskeletal disorders.


RESUMO JUSTIFICATIVA E OBJETIVOS: A mensuração da dor e do desconforto oriundos de distúrbios osteomusculares relacionados ao trabalho é um desafio conceitual e empírico, tendo em vista os processos cognitivos e subjetivos envolvidos na avaliação de sintomas somáticos. O objetivo deste estudo foi construir um índice de distúrbios osteomusculares relacionados ao trabalho para avaliar o desconforto osteomuscular entre trabalhadores com e sem lesões por esforço repetitivo/distúrbios osteomusculares relacionados com o trabalho, uma síndrome que acomete milhares de trabalhadores todo o ano. MÉTODOS: Participaram da amostra 182 bancários do Estado da Bahia, sendo que 91 deles tinham o diagnóstico da síndrome. RESULTADOS: O índice de distúrbios osteomusculares relacionados ao trabalho apresentou distribuição de frequência semelhante à da curva normal, com médias de 7,1 e 4,1 para os grupos com e sem diagnóstico, respectivamente. CONCLUSÃO: Além das propriedades psicométricas adequadas, o índice proposto se mostrou capaz de diferenciar, satisfatoriamente, trabalhadores com e sem diagnóstico de lesões por esforço repetitivo.

RESUMEN

Reaction of 5-bromo enones with pyrazoles provided a series of unexpected N,O-aminal derivatives, through a 1,4-conjugated addition at the ß-carbon of the 5-bromo enones instead of the expected nucleophilic substitution of the bromine. This reaction also furnished the 1,3-regioisomer of the pyrazole. A similar reaction of pyrazoles using 5-bromo enaminones furnished only N-alkylated pyrazoles-with high regioselectivity and at good yields-through nucleophilic substitution of the bromine.

PURPOSE: To evaluate three rigid, stable fixation methods for sagittal split ramus osteotomy (SSRO), using finite element analysis. The hypothesis is that a customized miniplate presents better stress concentration and distribution. MATERIALS AND METHODS: A 3D model of a hemimandible was created, and a 10-mm-advancement SSRO was simulated and fixed as follows: 3-DCP group - one custom miniplate fixed by eight screws; 4-H2P group - two miniplates of four holes each, fixed by eight screws; and 6-H2P group - two miniplates of six holes each fixed by 12 screws. After a vertical loading of 100 N, the values for von Mises stress, modified von Mises stress, and maximum and minimum principal stresses were measured. RESULTS: The area of maximum principal stress was similar for the three groups - located in the upper miniplate, in the screw near the proximal segment osteotomy. The maximum von Mises stresses were 1580.4 MPa, 1005 MPa, and 977.56 MPa for the 3DCP, 4-H2P, and 6-H2P groups, respectively, showing an allowable displacement of 2.57 mm, 1.62 mm, and 1.52 mm for the 3DCP, 4-H2P, and 6-H2P groups, respectively. CONCLUSION: The customized miniplate did not present better stress distribution than two commonly used types of fixation. Fixation with two straight miniplates, either with four or six holes, offers adequate resistance for 10 mm linear advancements.
