RESUMEN

SUMMARY: A diaphragmatic defect that permits abdominal contents to herniate into the right side of the chest is rare. In adults with right-sided diaphragmatic hernias, few occur without a history of trauma, and even fewer are symptomatic. This case report illustrates such a case and the rare entity of an anterolaterally located hernia. Although uncommon and easily missed, consequences of diaphragmatic hernias can be disastrous. The importance of a combination of high clinical suspicion and the use of computed tomography (CT) to aid diagnosis, and the dangers of a surgical condition being incorrectly assessed and admitted to a non-surgical specialty are highlighted in this case.

RESUMEN

Purine biosynthesis requires ten enzymatic transformations to generate inosine monophosphate. PurF, PurD, PurL, PurM, PurC, and PurB are common to all pathways, while PurN or PurT, PurK/PurE-I or PurE-II, PurH or PurP, and PurJ or PurO catalyze the same steps in different organisms. X-ray crystal structures are available for all 15 purine biosynthetic enzymes, including 7 ATP-dependent enzymes, 2 amidotransferases and 2 tetrahydrofolate-dependent enzymes. Here we summarize the structures of the purine biosynthetic enzymes, discuss similarities and differences, and present arguments for pathway evolution. Four of the ATP-dependent enzymes belong to the ATP-grasp superfamily and 2 to the PurM superfamily. The amidotransferases are unrelated, with one utilizing an N-terminal nucleophileglutaminase and the other utilizing a triad glutaminase. Likewise the tetrahydrofolate-dependent enzymes are unrelated. Ancestral proteins may have included a broad specificity enzyme instead of PurD, PurT, PurK, PurC, and PurP, and a separate enzyme instead of PurM and PurL.

RESUMEN

Ruminal zearalenone (ZEA) degradation in "in vitro" digestion was examined in different variants using the Hohenheimer Gas Test. First, the mycotoxin degradation was measured using squeezed rumen fluid from solid digesta of the dorsal sack and free rumen fluid from the ventral sack, respectively. Then free rumen, fluid of the ventral sac was used with addition of concentrate and mixtures of concentrate with sun-flower oil and starch, respectively. Within 24 hours ZEA was degraded down to 63% and 49% of the initial concentration when incubated with solid and fluid rumen digesta, respectively. Using additives and rumen fluid concentrate for incubation, concentrate, concentrate with oil and concentrate with starch, respectively, a reduction of ZEA to 46, 56 and 37% of the initial toxin concentration was observed.

Investigations on the mycological quality of feeds from Transylvanian farms are presented. The investigations had two objectives: 1. survey for mycological contamination and related chemical composition; 2. the evaluation of effects ofFusarium graminearum Schw. infested wheat (8% in the diet) on productivity and digestibility of nutrients in broilers. More than 50% of the samples showed water contents exceeding 16%, due to inadequate harvesting and storage conditions. This correlated with high my-cological contamination, with dominant species of the genusFusarium, Aspergillus, Alternaria,Penicillium andRhizopus. Feeding trials with broilers showed that live weight gain and digestibility coefficients of protein and fat in animals givenFusarium infested grain were depressed. As well N-retention and feed conversion rate (kg feeding stuff/ kg weight gain) were impaired.

Generic prescribing and generic substitution are mechanisms for reducing the cost of drugs. The purpose of this study was to assess the extent to which generic prescribing by private medical practitioners and generic substitution by private pharmacists is practised in South Africa and to estimate the potential savings from these two practices. Prescriptions from 10 pharmacists were collected on four randomly selected days. Computer printouts of all the prescriptions dispensed on these four days together with the original doctor's prescription were priced using a commercially available pharmacy dispensing computer package. A total of 1570 prescriptions with a total number of 4086 items were reviewed. Of the total prescriptions, 45.7% had at least one item for which there was a generic equivalent. Of the 961 drugs which had generic equivalents, 202 (21 %) were prescribed using the generic name of the drug. Only 0.3% of prescribers prohibited generic substitution. The cost of the prescription as dispensed was 1.4% (mean cost: R116.19 vs R117.84) below that of the original doctor's prescriptions, indicating the marginal benefit from the current low substitution rate of 13.9% by pharmacists. About 6.8% of the cost of the original doctor's prescriptions (mean cost: R117.84) could have been saved if total generic substitution (mean cost: R109.65) was practised. The cost of the prescriptions with only brand name items (mean cost: R120.49) would have been 9.9% higher than if generic drugs were used. Current restrictive prescribing and dispensing practices result in marginal cost savings from generic prescribing and generic substitution. Both these practices have a potential to reduce drug costs, if actively encouraged and practised to maximum capacity. It is noteworthy, however, that the potential savings from generic prescribing and substitution are at most 9.9% in the absence of any changes in types of drugs prescribed.

We have examined the in vitro phosphorylation of cellular src protein (pp60c-src) molecules associated with the polyoma virus middle-sized tumor antigen in polyoma virus-transformed cells. These pp60c-src molecules possessed an enhanced tyrosyl kinase activity, migrated aberrantly on NaDodSO4/polyacrylamide gels, and contained a novel site of tyrosine phosphorylation within the amino-terminal region of the molecule. The pp60c-src molecules not associated with the middle-sized tumor antigen were phosphorylated exclusively on a tyrosine residue within the carboxyl-terminal domain of pp60c-src. A similar modified form of the middle-sized tumor antigen-associated pp60c-src protein was detected in lysates from polyoma virus-transformed cells labeled in vivo with [32P]orthophosphate in the presence of sodium orthovanadate, an inhibitor of phosphotyrosyl phosphatases.

RESUMEN

Small molecular weight gag gene proteins p19 and p15 of human T-cell leukemia virus (HTLV) were purified to homogeneity from density banded virus, using differential affinities to phosphocellulose at pH 7.9 and 6.5. Using specific radioimmunoassays, p19 and p15 were shown to band with HTLV on sucrose gradients. These proteins were present only in HTLV-I and in cell lines expressing HTLV-I. They were not expressed in a B-cell line from patient CR, although his T cells were HTLV producers. No significant immunological cross-reactivity was detected between these proteins and HTLV-IIMo. P19 and p15 did not share any common antigenic determinants with each other or with HTLV p24 in competition radioimmunoassays. Sera of HTLV-positive leukemia-lymphoma patients and some of their relatives also had natural antibodies to p19 and p15.

RESUMEN

The "concrete" histological criteria for establishing the benignity or malignancy of cerebral tumours of neuroectodermic origin (gliomas and medulloblastomas, respectively granulocytomas) may be thus synthetized: 1) the benign tumours have a fibrocellular structure, the neoplastic cells being relatively monomorphous; vascular hyperplasia is absent, as well as necrotic and hemorrhagic foci; 2) the malignant tumours are hyper- and polymorphocellular, the fibrillar component being reduced or absent; vascular hyperplasia as well as the presence of necrotic and/or hemorrhagic foci is a constant characteristic; the tumoral cells often infiltrate the perivascular spaces.
