RESUMEN
Fundamento: las púrpuras son enfermedades hemorrágicas caracterizadas por la extravasación de sangre hacia los tejidos por debajo de la piel o las mucosas que producen petequias espontáneas o equímosis. La hemorragia anormal en la mucosa bucal, la piel, órganos internos y otros tejidos ocurre de manera súbita, excesiva, espontánea o después de una irritación y pone en riesgo la vida del paciente.Objetivo: reportar un caso de púrpura trombocitopénica inmunológica con manifestaciones bucales y cutáneas, así como su manejo estomatológico.Caso clínico: se reporta el caso de un paciente masculino de 80 años con púrpura trombocitopénica inmunológica, presentaba equímosis en ambos carrillos, paladar, en la piel de la cara, los miembros superiores e inferiores, tórax, abdomen y espalda. Se remitió al servicio de Hematología Clínica donde se le practicaron los exámenes de laboratorio, se diagnosticó la púrpura trombocitopénica inmunológica y se trató con esteroides y vitamina C, el cuadro purpúrico remitió después de las dos semanas. Recibió tratamiento estomatológico con medidas hemostáticas locales y bajo supervisión de su médico. El paciente evolucionó de manera favorable.Conclusiones: las manifestaciones bucales y cutáneas de la púrpura deben ser identificadas por el estomatólogo para poder realizar las modificaciones pertinentes en el plan de tratamiento estomatológico a fin de evitar contingencias letales en sus pacientes(AU)
Background: purpuras are hemorrhagic diseases characterized by the extravasation of blood towards tissues below the skin or the mucosa that produce spontaneous petechiae or ecchymosis. The abnormal bleeding in the oral mucosa, the skin, internal organs and other tissues occurs suddenly, excessive, spontaneously or after an irritation and puts at risk the life of the patient.Objective: to describe a case of purple thrombocytopenic immune with oral and skin manifestations as well as its stomatologic management.Case report: it is reported a case of an 80 year old male patient with immune thrombocytopenic purpura, who presented bruising on both cheeks, palate as well as in the skin of the face, both upper and lower limbs, thorax, abdomen and back. The patient was remitted to the Service of Clinical Hematology where laboratory tests were practiced. The patient was diagnosed with purple thrombocytopenic immune and was trated with steroids and vitamin C. The purpur manifestations remitted after two weeks. The patient received stomatologic treatment, taking into account local hemostatic considerations and under the supervision of medical doctor. The patient evolved favorably.Conclusions: the oral and cutaneous manifestations of the purple should be identified by the dentist in order to introduce changes in the treatment schedule and to avoid lethal contingencies in their patients(AU)
