RESUMEN

Low-level laser therapy (LLLT) is included in the Federal project «Fight against oncological diseases¼, which provides for equipping all specialized medical institutions in Russia with the appropriate devices until 2026. PURPOSE OF THE STUDY: Publications analysis on the use of LLLT in oncology. MATERIAL AND METHODS: To search for publications, databases and libraries were used: PubMed, Scopus, ResearchGate, Google Scholar, J-STAGE, eLibrary.ru for the period 2018-2021. For analysis, publications were selected that are of interest from the point of view of improving the methodology of LLLT, the prospects for the development of this method in the prevention, treatment and rehabilitation of patients. A total of 223 publications were found, including 39 reviews and meta-analyses, mostly in English (165) and Portuguese (41). RESULTS AND DISCUSSION: The inclusion of LLLT in the ISOO/MASCC/ASCO recommendations for the treatment of complications such as mucositis, salivary gland hypofunction and xerostomia caused by non-surgical methods of treating cancer patients should be considered as a promising trend. An extremely low level of quality of LLLT techniques was noted, most often the replacement with meaningless «doses¼, «fluences¼ instead of the necessary parameters: wavelength, operating mode, power, frequency, exposure, etc. The use of only local illumination instead of systemic methods of LLLT does not allow obtaining the best results of treatment and prevention. Eliminating the shortcomings of treatment protocols with a focus on Russian experience, when laser blood illumination (intravenously or externally) and laser acupuncture are also necessarily carried out with local illumination of the lesion, will bring the effectiveness of LLLT to a qualitatively new, higher level.

RESUMEN

In children, chronic inflammatory diseases of the pharyngeal lymphoid ring are the most common chronic nasopharyngeal diseases. Available therapies are not always effective, associated with an increase in treatment duration and possible complications and recurrences. Laser therapy (LT) techniques have long been successfully used in ENT practice, but they still need to be optimized and standardized. OBJECTIVE: To analyze treatment outcomes in children with chronic inflammatory diseases of the pharyngeal lymphoid ring in out-patient settings using standard methods of LT (by localization, technique, and time of laser exposure) with different lasers. MATERIALS AND METHODS: A total of 214 patients aged 1-15 years with chronic inflammatory diseases of the pharyngeal lymphoid ring were included in the study. The patients were divided into three main groups (1-3) and received LT using different lasers on the Lasmik device and a control group without LT. Parameters of laser radiation sources (groups) were the following: Group 1 wavelength (λ) 635 nm, pulsed mode (PM), pulse width 100 ns, pulse power (PP) 5 W, mean power (MP) 0.04 mW; Group2 λ=904 nm, PM, PP=5 W, MP=0.04 mW; Group 3 λ=635 nm, continuous mode, MP=15 mW. The outcomes were assessed by clinical improvement. RESULTS AND CONCLUSION: A comparative analysis of the comprehensive treatment outcomes showed significantly better outcomes in the 1-3 groups compared to the control group. A higher efficiency of pulsed low-intensity laser radiation (LIRI) in comparison with a continuous mode, mostly red spectrum (λ=635 nm, PM, MP=5 W) was shown. This laser exposure results in faster improvement and allows performing LIRI exposure percutaneously without using light guides, which require sterilization and are quickly degrading.

OBJECTIVE: To optimize postoperative rehabilitation by applying low-intensity laser irradiation (LILI) with different wavelengths in the early postoperative period to prevent inflammatory complications. MATERIAL AND METHODS: After radiological examination and ultrasound diagnostics of the periodontal tissue vessels, surgical methods of orthognathic treatment were performed, after completion of which a course of LILI was carried out. The VEGF its receptors (sVEGF-R1; sVEGF-R2) content was measured by enzyme immunoassay using standard reagent kits. The laser therapy using 635 nm laser light was applied directly to the vestibular and oral surfaces of the gingival tissues and in the operation area, changeable by the scanning method, for 1.5 minutes (5 W power); the pulsed infrared laser therapy (PILT) with the 904 nm wavelength (light pulse duration 100 s, power 15 W, 1500 Hz) applied epicutaneously to the operation projective zones (four control points of the upper and lower jaw) through the skin of the cheek, in stable method (1.5 minutes) with a time range between red and infrared wavelength LILI not exceeding 100 s (1.5 minutes). RESULTS: Vascular and endothelial dysfunction after laser irradiation with different wavelengths is better controlled by increasing the microcapillary blood flow (66.7% gain; p<0.05) in arteriolar and 70.3% in venular sections of capillaries (p<0.01), which is associated by vasodilatation: diameter increased by 26.9% compared to that under the influence of red laser radiation (by 13.0%) and infrared laser radiation (by 7.2%); p<0.01). CONCLUSIONS: Early laser therapy using the low-intensity laser irradiation with different wavelengths improves vasoactive processes of hemoregulation in dental tissues associated with the elimination of vasospasm caused by operative stress, activation of arteriolodilatory effects, contributes to the prevention of development of inflammatory complications.

RATIONALE: At sports submaximal loads accompanied by muscle fatigue due to metabolic transformations, the conditions are created for the development of combined endogenous and exogenous emotional stresses. An important role in the development of stress is played by the state of adaptation mechanisms, in which sintoxic and catatoxic programs are involved, as well as the GABA-dopaminergic system. There is insufficient information on the possibilities for reducing the manifestations of stress. PURPOSE: The objective of the presser study was to estimate the possibilities for the prevention of the development of psycho-emotional stress with the use of transcranial electrostimulation in the combination with serotonin laser phoresis. METHODS: The assessment of the psychological status before and after transcranial electrostimulation in the combination with serotonin laser phoresis was performed in 96 athletes who were engaged in weightlifting and athletic gymnastics, as well as in 37 athletes comprising the control group in the state of sparing rest. Transcranial electrostimulation was carried out during 14 days. The laser phoresis of serotonin was conducted with the application of the Matrix device and transcranial electrostimulation with the use of the Magnon-DKS apparatus. The assessment of the psychological status was carried out based on the Hospital Anxiety and Depression Scale (HADS) and the results of the well-being-activity-mood (WAM) questionnaire and in accordance with the Hildebrandt index, and the Spielberger-Hanin test. RESULTS: The two-week course of exposure to transcranial electrostimulation and serotonin laser phoresis of the patients comprising the main group resulted in a faster stabilization of the psychological status than in the control group. This effect was due to the multi-component involvement of the athletes in the adaptation programs including the management of homeostasis and the influence on the GABA-dopaminergic system via the serotonin and opioidergic mechanisms. CONCLUSION: The results of this study give reason to recommend the proposed method of transcranial electrostimulation and serotonin laser phoresis for the use in sports medicine.

AIM: The objective of the present study was the evaluation of the effectiveness of the combined treatment of the patients presenting with trophic ulcers associated with chronic venous insufficiency of the lower extremities of venous etiology with the application of low-intensity laser therapy (LILT) at different wavelengths. MATERIAL AND METHODS: The study included the patients presenting with chronic venous insufficiency (CVI) (class C6 in accordance with the CEAP clinical classification). The ultrasonic Doppler examination (USDG) of the lower extremities revealed valvular insufficiency in the saphenous veins in 98.8% of the examined patients, in the deep venous system in 58% of them, and in the perforating veins in 72% of the cases. The laser Doppler flowmetry technique was used to assess the state of the microcirculation. Group 1 was comprised of 34 (43.59%) patients undergoing the traditional conservative treatment that consisted of pharmacotherapy (including antibiotic therapy, depending on the sensitivity of microflora assessed by the culture technique), the local treatment in the form of dressing in compliance with the standard surgical treatment schemes depending on the phase of the wound healing process, and elastic compression of the lower extremities. The main group 2 consisted of 44 patients (56.41%) who were treated, in addition to the traditional conservative therapy, with the use of LILT according to the new technique that combines the external laser exposure of the trophic ulcer region using the 'LASMIK' laser device during a single 2 minute session per zone in the pulsed mode (light pulse duration of 100-130 ns, frequency 80 Hz), at a wavelength of 635 nm, by a matrix emitter (consisting of eight laser diodes with the surface area of 8 cm2), at a distance up to 7 cm, with pulsed power of 40 W, power density of 5 W/cm2, and the intravenous laser blood illumination (fiber output power 2 mW) with a wavelength of 365 nm (UV range) and 525 nm (green spectrum) alternately, every other day. Each patient underwent 12 daily procedures per course. RESULTS: Combined low-intensity laser therapy promoted the rapid recovery of the sympathetic regulation of the microvascular tone and normalization of arteriolar-venular relationships which contributed to the improvement of blood supply to the tissues, the reduction of inflammation, enhanced activation of the reparative processes, and acceleration of epithelialization of trophic ulcerous defects. CONCLUSION: The combined strategy makes it possible to carry out the more effective treatment of the patients presenting with trophic ulcers with the three-fold reduction of the ulcer healing time and the pronounced stimulation of the persistent adaptation of the physiological responses preventing the development of the relapses.

BACKGROUND: The prevalence of allergic reactions to the pharmaceutical products and the development of antibiotic resistance limit the provision of the high-quality medical care to the patients presenting with acute bacterial external otitis media (ABEO). AIM: The objective of the present study was to evaluate the effectiveness of the treatment of the patients suffering from ABEO with the application of the new technique based on intravenous blood illumination by low-intensity laser radiation at a wavelength of 445 nm (ILBI-445). MATERIAL AND METHODS: This open comparative clinical study was carried out from April 2017 till June 2017 and included the patients with ABEO who were divided into two groups. The patients of the main group underwent the standard treatment in the combination with low-intensity laser radiation at a wavelength of 445 nm. Those comprising the control group were given the conventional treatment alone. The end point of the study was the complete disappearance of local inflammatory signs in the region of the external auditory canal (EAC) and auricle, the restoration of the volume of the EAC, the absence of complaints estimated based on the visual analogue scale (VAS), and the complete disappearance of symptoms of intoxication. The local signs of inflammation in the region of the EAC and auricle were assessed from the otoscopic picture. The measurement of the volume of the external auditory canal were carried out by filling it with a furacilin solution at a temperature of 360C using a 2 ml syringe. The character of the inflammatory changes in the external auditory canal, the intensity of pain and itching, as well as the degree of hearing loss were estimated at 10 points based on the 10-point visual analogue scale (with 0 points standing for the absence of complaints and 10 points denoting the maximum severity of the complaints). The symptoms of intoxication were determined from the measurements of body temperature. RESULTS: The combined treatment of the patients with ABEO using ILBI-445 resulted in their earlier recovery in comparison with that achieved by means of the standard therapy. The differences were especially pronounced on the 5th day of treatment. Complaints of pain and the loss of hearing were documented only in the patients of the control group. Itching was a significantly less serious trouble in the patients of the main group than it was in those comprising the control group. The volume of the external auditory canal in the patients of the main group on the 5th day after the onset of the treatment was more than 16.7% greater than in the control one. CONCLUSIONS: The high therapeutic effectiveness and simplicity of the approach employed in the present study taken together with a minimum of contraindications for its application give reason to recommend intravenous blood illumination by low-intensity laser radiation at a wavelength of 445 nm for the treatment of the patients suffering from acute bacterial external otitis.

Male infertility is associated with a wide range of pathological conditions affecting both the sexual and other body systems: endocrine, nervous, blood, and immune. Laser therapy is a form of modern physiotherapy that applies low-intensity laser radiation in various parts of the body. It is widely used in all areas of modern medicine due to its high efficiency, ease of use, the absence of contraindications and side effects. The authors reviewed the results of 171 studies investigating laser therapy, of which 93 were experimental (20 in Russian and 73 international publications), 4 clinical and experimental (all in Russian), 74 clinical (73 in Russian and 1in English). Along with domestic literature, publications in Russian from Belarus, Georgia, Kyrgyzstan, Uzbekistan and Ukraine were studied. The article analyzes the experimental studies, somehow related to the problem of infertility and its treatment using low-intensity lasers. The analysis findings suggest that there are all prerequisites to develop a laser therapy as an effective modality for treating infertile men.

Male infertility is a multifactorial syndrome associated with a wide range of pathological conditions affecting both the sexual and other body systems: endocrine, nervous, blood, and immune. Laser therapy is a form of modern physiotherapy that applies low-intensity laser irradiation (LILI) in various parts of the body. It is widely used in all areas of modern medicine due to its high efficiency, ease of use, the absence of contraindications and side effects. The authors reviewed the results of 171 studies investigating laser therapy, of which 93 were experimental (20 in Russian and 73 international publications), four clinical and experimental (all in Russian), 74 clinical (73 in Russian and 1 in English).Along with domestic literature, publications in Russian from Belarus, Georgia, Kyrgyzstan, Uzbekistan, and Ukraine were also reviewed. The review suggests that laser therapy should be used as much as possible in the management of male infertility not just due to high effectiveness, but also because of the lack of alternatives. For local application, optimal laser irradiation regimens are pulsed LILI of red (635 nm) and infrared (904 nm) spectra. For intravenous laser blood irradiation, the authors recommend alternating a continuous LILI with a wavelength of 635 nm (red spectrum) and 365 nm (ultraviolet spectrum).

RESUMEN

One of the most widely employed methods of laser therapy is laser irradiation of blood (LIB). There are two modifications of this technique, one being intravenous low-intensity laser irradiation of blood (ILIB), the other non-invasive blood irradiation(NLIB). The two methods have been developing independently since either has its advantages and disadvantages. The present article was designed to review the main currently available techniques for laser irradiation of blood which are presented in the form of tables (charts). Replacing the UV irradiation of blood with UV lamps by laser ultraviolet irradiation of blood (LUVIB®) has made it possible to significantly simplify the technique and enhanced its efficiency. The most effective options for ILIB are the combined techniques: ILIB-635 + LUVIB® and ILIB-525 + LUVIB. The most effective technique for ELIB is believed to be the use of low-intensity pulsed laser light with a wavelength of 635 nm and output power up to 40 W.

Registration of the changes in the functional metabolic status of neutrophils under the influence of low-intensity laser radiation (LILR) can be useful for the choice of the exposure parameters in the studies designed to evaluate immunotropic effects. Such investigations, in their turn, are very promising in clinical terms since their results can be used to optimize the laser therapy techniques. The objective of the present study was to evaluate the degranulation potential of neutrophils and the intensity of their NADPH-oxidase reaction in response to the exposure to laser radiation as exemplified by the release of lysosomal granules in vitro. The source of radiation was the LASMIK device operated in the continuous mode at the following parameters: wavelength 635 nm, power density 0.12 mW/cm2, exposure time 10, 30, 90, 120, 150 s in the case of the automatic timer control and 100 s in the case of manual shutdown. It was shown that the maximum lysosomal activity and release of lysosomal granules took place at a 90-10 s exposure and a wavelength of 635 nm with the appearance of a plateau within 120 and 150 s after the onset of irradiation. In the case of a shorter exposure of neutrophil granulocytes to LILR (10 and 30 s) no pronounced effect was observed. It means that low-intensity laser radiation of the red spectrum with a wavelength of 635 nm is a physical stimulus reinforcing exocytosis of lysosomal granules by neutrophils in vitro. The reliable changes of NADPH-oxidase activity of neutrophil granulocytes isolated from donor peripheral blood are recorded at an optimum exposure time of 90-100 s. It is concluded that the laser therapy techniques, at least those designed to regulate neutrophils should be applied with the optimum exposure time of 90-100 s.

This scientific review highlights peculiarities of the delivery of laser radiation to the tissues of the athlete's body by means of laser electrophoresis of biologically active substances and the possibilities for electrolaser myostimulation. The main features of electrical myostimulation based on the application of laser-generated radiation are described. The authors emphasize the necessity of taking into account the neurophysiological dependence of active and passive contractile muscular activity during training. They provide the scientifically grounded rationale for the combined use of coherent laser radiation and electrical stimulation. The list of scientific papers that confirm the effectiveness of the method being considered is presented. The authors compare this method with other techniques for the induced ion transfer. The beneficial influence of laserphoresis of succinic and hyaluronic acids on the athlete's body is reported with special reference to alleviating the pain syndrome. A number of phyto-extracts used for the purpose of laser electrophoresis are characterized. The authors demonstrate the application of the optional versions of this technique in various sports during the training and competition periods.

UNLABELLED: Low-intensity laser radiation can be used as one of the methods for the non-specific regulation of the human mesenchymal stem cell (MSC) activity at the preliminary stage of their in vitro cultivation. The objective of the present study was to estimate the influence of the limiting regimes of continuous low-intensity laser radiation (CLIR) of red (635 nm) and green (525 nm) spectra. MATERIAL AND METHODS: The adhesive culture of human mesenchymal stem cells obtained from a donor's umbilical cord tissue was used in the experiments (following 4 passages). They were irradiated using a Lazmik-VLOK laser therapeutic device equipped with the KLO-635-40 (635 nm, 4,9 mW/cm(2)) and KLO-525-50 (525 nm, 5,4 mW/cm(2)) laser diode emitting heads operating in a continuous mode. A special nozzle (jar) for laser and vacuum massage (KB-5, 35 cm in diameter) was employed to fix the heads. The exposure time in all the irradiation regimes was 5 minutes. CONCLUSION: The study has demonstrated that neither the morphological features nor the viability of mesenchymal stem cells was altered under the influence of laser irradiation at the aforementioned energy and time parameters. The data obtained indicate that laser irradiation with the limiting levels of the chosen energy parameters produces no positive effect on the cell proliferative activity; more than that, it may cause its inhibition.

It is shown that the synchronization of energetic, spectral, frequency, and temporal parameters of technique with biorhythms of physiological processes in the organ, which is target of impact, and in the human body as a whole, allows to significantly improve efficiency, and achieve stable and reproducible results of treatment. The article presents the results of study including 62 patients with vasculogenic ED. The study design included the randomization of patients into 3 groups depending on the complex of the therapy with the use of combined techniques, including negative pressure and laser illumination (LLNP) as a part of combined therapy and as monotherapy. Significant increase in the peak flow velocity after a course of treatment was observed in all three groups of patients. Improvement in erectile function was observed in all groups; according to IIEF score, erectile function has increased by 22.3 +/- 0.05% in group 1, by 34 +/- 1.5% in the group 2, and by 19 +/- 1.7% in the group 3, indicating the best results of treatment in the group receiving combined therapy. Combination of LLNP with the administration of PDE5 inhibitors significantly increases the effectiveness of treatment of vasculogenic ED due to the influence of physical factors on the stabilization of hemodynamics in the main arteries of the penis. After a course of therapy, increase in systemic vascular elasticity by 39.8 +/- 1.5% was also noted. The efficiency of the LLNP methodology in the treatment of patients with vasculogenic ED is demonstrated. The best results were obtained in the group of patients treated with combined therapy, including the use of LLNP and PDE-5 inhibitor.

INTRODUCTION: Mesenchymal stem cells (MSC) have for a long time been an object of investigation with a view to elucidating the prospects for their application in clinical medicine and cosmetology. One of the approaches to the non-specific regulation of the activity of these cells at the stage of preliminary in vitro combination is the treatment with low-intensity laser radiation (LILR). The objective of the present study was to evaluate the possibility of using pulsed LILR of the infrared and red spectra for this purpose. MATERIAL AND METHODS: We used the 4th passage adhesive MSC cultures based at the umbilical tissue of a donor who gave the informed consent to participate in the study. The source of illumination was a Lazmik-VLOK laser therapeutic apparatus (RU No RZN 2014/1410 dated 06.02.2014) with the matrix laser infrared radiation heads (wavelength 904 nm, light pulse length 108 ns, frequency 1500 Hz). The apparatus was operated either in the multi-frequency Lazmik regime [Moskvin S.V., 2014] with mean power density 0.05 and 0.14 mW/cm2 and the red spectrum (wavelength 635 nm, light pulse length 144 ns, frequency 1500 Hz) or in the multi-frequency Lazmik regime [Moskvin S.V., 2014] with mean power density 0.03 and 0.12. The exposition was 5 min in both regimes. CONCLUSION: The study has demonstrated that neither the morphological structure nor the viability of mesenchymal stem cells changed under the influence of energy and time parameters used in experiments. The number of cells was shown to slightly increase in comparison with control. The most pronounced effect was documented after illumination with pulse infrared (904 nm) LILR in the multi-frequency Lazmik regime. The maximum effect was observed during a period between days 1 and 3 of cultivation.

The prevalence of cytomegalovirus infection (CMVI) dictates the necessity of its in-depth investigation in the patients presenting with the signs of chronic inflammatory diseases of sexual organs. The objective of the present study was to estimate the effectiveness of combined laser therapy of inflammatory diseases of the urogenital tract accompanied by the reactivation of CMVI concomitant with other infections. The examination of 158 women presenting with cytomegalovirus infection revealed clinical and laboratory characteristics of the microbiocenosis. These data may be used to improve the effectiveness of diagnostic and therapeutic strategies for such patients. The combined treatment of the patients with the reactivated form of CMVI using josamycin and doxycycline monohydrate in combination with panavir and low-intensity laser irradiation based at the "Matrix-Urolog" laser complex resulted in the favourable outcome of therapy in the majority of the patients.

Laserophoresis is a technique for the transcutaneous administration of biologically active compounds by means of low-intensity laser radiation (LFLR). It is currently regarded as a most promising method for the integrated application of a pharmaceutical substance and a physical factor. At present laserophoresis of various medicinal preparations is successfully used after preliminary experimental studies of their phoretic properties for the treatment of various inflammatory and dystrophic conditions as well as for the prevention of skin ageing. The most important route for the administration of the majority of drug preparations is through the shunts provided by perspiratory glands and hair follicles. Another essential factor determining the potential possibility of drug penetration through the skin is the characteristic of the substance chosen for the administration, such as its molecular weight, chemical structure, conformation, and hydrophilic properties. However, the most likely mechanism underlying the transport of the substance through the glandular cells of perspiratory glands and epithelial cells of hair follicles is pinocytosis, i.e. the process integrating exocytosis and endocytosis. To-day, the majority of the researchers lay emphasis on thermodynamic triggering of Ca2+-dependent processes as the primary mechanism behind the biological action of low-intensity laser radiation. Both exocytosis and endocytosis being the Ca2+-dependent processes, the liberation of Ca2+-ions under the influence of LFLR causes the activation of pinocytosis as a whole.

This study showed that it is highly undesirable to equip the devices for laser therapy with the dose-calculation function. In order to avoid mistakes, the operator should perform a strict sequence of actions as follows: to choose the needed wavelength and operating regime (the laser head block) of the LILR source, to set and measure the radiation power, the time and frequency of treatment, turn on the apparatus, control its operation and switch it off at the scheduled time. Meeting all these requirements eventually ensures obtaining a certain optimal dose density and guarantees that the entire procedure of laser irradiation is performed in a proper way. The equipment of the apparatus with the dose-calculation function is nothing more than a marketing ploy intended to earn extra money that apart from everything else creates additional problems for the customer.
