BACKGROUND: There is an increasing number of transradial approach (TRA) for carotid artery stenting (CAS), however, similar techniques and materials as for femoral access are used. We report the results of TRA lower profile technique for CAS using a 7 F Simmons guiding catheter, especially in terms of feasibility and procedural safety in a single center. MATERIALS AND METHODS: We retrospectively analyzed 68 consecutive patients with symptomatic extracranial carotid stenoses who underwent 75 CAS between January 2018 and December 2021. The success and crossover rate, procedural time, fluoroscopy, clinical outcomes, technical considerations, and procedural complications were analyzed. RESULTS: TRA CAS with Simmons guiding catheter was successful in 67/75 (89.3%) cases, with a 7 (9.3%) crossover rate. Fluoroscopy mean time was 15.8 minutes. Two forearm hematomas were described. No ischemic or surgical site complications were reported. CONCLUSIONS: In our experience frontline TRA with a 7 F Simmons guiding catheter is feasible with high procedural success and a low rate of access site complications.

Background Flow-diverter stents have been successfully used in the treatment of complex aneurysms with limited therapeutic alternatives. We report our experience using the Silk flow diverter (SFD; Balt Extrusion, Montmorency, France) for the treatment of complex aneurysms in four Argentine centers. Methods We conducted a retrospective review of 246 consecutive patients who were treated with the SFD at four Argentine centers between January 2009 and January 2017. The patient and aneurysm characteristics, as well as the details of the procedure, were analyzed. The angiographic and clinical findings were recorded during and immediately after the procedure and at 12-month follow-up. Results Angiography follow-up at 12 months was possible in 235 patients (95.5%) with 282 aneurysms. A total of 265 aneurysms (93.9%) presented with complete occlusion of the aneurysmal sac (class 1) and 17 aneurysms (6.1%) presented with partial occlusion (class 2). The 12-month clinical follow-up showed 11 patients with major events (seven, scale 2; five, scale 3; and two, scale 4). The morbidity and mortality rates were 4.2% (11/289) and 2.1% (5/289), respectively. Conclusions The treatment of aneurysms with the SFD was associated with a low rate of complications and a high percentage of aneurysmal occlusion. These findings suggest that SFD is an effective and safe alternative in the endovascular treatment of complex aneurysms.

RESUMEN

Primary meningioma of the orbit is very rare. A radical surgical treatment of primary meningioma of the orbit by a thirteen years old boy was carried out. Main symptoms communicated by the patient were amblyopia, which terminated in blindness, as well as increasing exophtalmos of a bulbus oculi. After the operation a regression of exophtalmos has been achieved while the blindness stayed unchanged.

RESUMEN

A twelve year boy was admitted to the Otolaryngological Clinic of the Medical Academy in Cracow with a paralysis of the peripheral facial nerve and a tumor on the right zygomatic arc. A CT investigation revealed an expansive process in the central fossa of the skull, which continuously passed into the squama of the temporal bone and the are of the zygomatic bone on the right side, causing a paralysis of the nerve VII. A neurosurgical intervention was carried out as well as a subsequent treatment by radiotherapy. A remission of the tumor was achieved. Osteoclastoma was diagnosed histologically.

RESUMEN

In the years 1986-91, five patients with neoplasms of neurolemmal sheath were treated in the Department of Otolaryngology, Medical University in Cracow. Two patients suffered from malignant neurinoma (malignant neurinoma of orbit or maxillary sinus) and the other three had neurinoma (neurinoma of the floor of mouth, nasal cavity and neck). There were four men and one woman. The age of patients ranged from 23 to 52 years. The tumors were surgically removed, patients with malignant neurinoma required additional irradiation. Currently, no tumor recurrence has been noticed.
