RESUMEN

PURPOSE: Electronic portal images are one of the most important tools to verify the ongoing radiotherapy treatment through comparison with a reference image generated during treatment planning. In this procedure, two images are geometrically matched by means of visible bone or other landmarks of interest such as implanted fiducials. However, the intrinsically poor contrast and low spatial resolution of portal images can limit image quality. METHODS: In this study, we have provided a multiresolution approach to enhance the quality of portal images acquired from the pelvis treatment fields. The main idea behind this work aims at removing some of the image artifacts that conceal the anatomical information. For this purpose, we have applied the homomorphic filtering on the approximation sub-band of wavelet decomposition to enhance local information. Moreover, in order to sharpen the bone edges, wavelet detail sub-bands were weighted to amplify important image details in the reconstruction of the desired enhanced image. The most appropriate image quality measure was chosen according to the image's characteristics in the spatial domain. By considering the characteristics of portal images as the random and nonperiodic texture, high level of noise, and a nonuniform background, three suitable quality measures of images were assessed: edge content, measure of enhancement, and measure of enhancement by entropy. RESULTS: The higher values of these measures indicate the quality improvement in the processed images through our proposed algorithm. Moreover, the subjective evaluation results indicate that the proposed multiresolution approach significantly enhances the perceived quality of images in comparison with original and the similar approach ( p < 0.001 $p < 0.001$ ). CONCLUSIONS: Our proposed wavelet-based enhancement algorithm successfully reduced image intensity nonuniformity and enhanced anatomical featured information, which drastically improved the objective metrics values. Subjective evaluation of enhanced image confirmed this quality improvement.

BACKGROUND AND OBJECTIVE: The present study aimed to investigate the effects of propolis and melatonin supplementation on inflammation, clinical outcomes, and oxidative stress markers in patients with primary pneumosepsis. MATERIALS AND METHODS: This pilot randomized controlled trial was conducted on 55 patients with primary pneumosepsis who were randomly assigned to the intervention and control groups. In the three intervention groups, the patients received propolis alone (1,000 mg/day), propolis (1,000 mg/day) plus melatonin (20 mg/day), and melatonin alone (20 mg/day). The control group received placebo. The inflammatory and oxidative stress markers as well as clinical outcomes were evaluated before and after the intervention, and the 28-day survival rate was also recorded. RESULTS: After the intervention, the combination of propolis and melatonin significantly reduced interleukin-6 (-55.282 pg/mL) and C-reactive protein (-21.656 mg/L) levels, while increasing gavage intake (326.680 mL/day) and improving some clinical outcomes (APACHE II, SOFA, and NUTRIC scores) compared to the control group. However, no significant difference was observed between the groups in terms of oxidative stress and hematological indices. In addition, there was no significant difference in the 28-day survival rate between the groups (p = 0.07). CONCLUSION: Supplementation with propolis and melatonin may improve clinical outcomes by reducing inflammation. Further investigations are required to confirm these findings.

RESUMEN

Radiotherapy of extensive malignant scalp lesions has always been a challenge for radiation oncologists considering the proximity of critical structures. We treated a 39-year-old patient with extensive scalp squamous cell carcinoma with application of adjuvant concurrent chemoradiation and external beam radiation therapy (EBRT), followed by high-dose-rate (HDR) surface brachytherapy boost using an exclusively designed mould, and assessed the radiation dose reaching planning target volume, brain, and optic structures. Comparison between conventional planning with sole EBRT and combined treatment planning with EBRT and brachytherapy, assured the use of the latter treatment to avoid high radiation doses from reaching critical organs at risk without compromising the required dose for planning target volume.

Background: Chitosan (CS) nanoparticles have attracted considerable attention as a non-viral and cationic carrier for delivery of therapeutic proteins and antigens and offer non-invasive routes of administration such as oral, nasal and ocular routes, and also show adjuvant characteristics for vaccines. Objectives: Preparation and formulation of CS nanoparticles as a capable carrier with immunoadjuvant properties to enhance the bioavailability of antigen and produce antibody with high affinity. Materials and Methods: CS nanoparticles were produced by ionic gelation process of sodium tripolyphosphate (TPP) with CS. Particle size and morphology of nanoparticles were determined using Dynamic Light Scattering (DLS) and Scanning Electron Microscopy (SEM) and also direct observation under light microscope. The influence of the initial BSA concentration and CS concentration on loading efficiency and release behavior was evaluated. The ε-toxin (derived from Clostridium perfringens type D) was loaded on CS nanoparticles and the complex was injected hypodermically into the rabbits for once. The anti ε-toxin antibody level in blood serum was evaluated using Dot Blot and ELISA methods. Results: The CS nanoparticles in different groups have a particle diameter (Z-average) in approximate ranges of 200-400, 300-600, 450-800 nm and a positive Zeta potential (32.4 - 48.6 mv). Optimum loading efficiency was achieved for CS at a concentration of 0.5 mg.mL-1 and TPP of 1.0 mg.mL-1. The results showed that the toxin-CS complex produces antitoxin at levels more than twice as high the control. Conclusion: The CS nanoparticles can be used as a good biodegradable carrier for protein and antigen delivery.

AIM: The aim was to measure doses of eyes and lenses in the presence and absence of shield during whole brain irradiation (WBI). In addition, the dose calculations accuracy of radiation therapy dose plan treatment planning system (TPS) in dose calculation of the eyes and lenses in WBI was evaluated. MATERIALS AND METHODS: To measure the eyes and lenses doses during WBI, an inhomogeneous phantom of human head was used. Then, the eyes and lenses doses in the presence and absence of shield were measured by EBT3 film. RESULTS: In single fraction with 200 cGy to reference point, average doses received by the left and right eyes in the absence of shield were 20 ± 1.5 and 22 ± 1.0 cGy, respectively, and for the left and right eyes in the presence of shield were 18 ± 2.2 and 21 ± 2 cGy, respectively. In addition, the average dose received by the left and right lenses in the absence of shield were 19.5 ± 0.5 and 18.5 ± 0.5 cGy, respectively, and for the left and right lenses in the presence of shield were 20.5 ± 1.5 and 19.5 ± 1.5 cGy, respectively. The results showed the TPS compared to the film underestimates doses for the eyes and lenses. CONCLUSION: The average dose received by the eyes and lenses during WBI were estimated around 9-11% of prescribed dose. According to the results, there is probability of radiation-induced cataractogenesis during WBI. By investigating the effect of shield on the lenses and eyes doses, using shield during WBI is not recommended. In addition, the results showed dose calculation accuracy of the TPS for the estimation of doses received by the eyes and lenses during WBI is not acceptable.

In this randomized controlled trial, diabetic patients with foot ulcers (Wagner grades 1 and 2) were randomly assigned to conventional therapies for diabetic foot ulcer plus topical propolis ointment (5%; twice daily) or conventional therapies alone. The process of ulcer healing was observed during 4 weeks and compared between the two groups regarding the size, erythema, exudates, white blood cell (WBC) count and erythrocyte sedimentation rate (ESR). The process of ulcer size reduction during the four-week period of study was significantly different between the groups. However, this difference was not significant between the third and fourth weeks. There was no significant difference between two groups regarding erythema and exudate reduction as well as WBC count and ESR. Administration of topical propolis ointment in addition to the conventional treatments of diabetic foot ulcer could reduce the size of ulcers with Wagner grades 1 and 2.

PURPOSE: Stereotactic radiosurgery is a high precision modality for conformally delivering high doses of radiation to the brain lesion with a large dose volume. Several studies for the quality control of this technique were performed to measure the dose delivered to the target with a homogenous head phantom and some dosimeters. Some studies were also performed with one or two instances of heterogeneity in the head phantom to measure the dose delivered to the target. But these studies assumed the head as a sphere and simple shape heterogeneity. The construction of an adult human head phantom with the same size, shape, and real inhomogeneity as an adult human head is needed. Only then is measuring the accurate dose delivered to the area of interest and comparison with the calculated dose possible. METHODS: According to the ICRU Report 44, polytetrafluoroethylene (PTFE) and methyl methacrylate were selected as a bone and soft tissue, respectively. A set of computed tomography (CT) scans from a standard human head were taken, and simplification of the CT images was used to design the layers of the phantom. The parts of each slice were cut and attached together. Tests of density and CT number were done to compare the material of the phantom with tissues of the head. The dose delivered to the target was measured with an EBT3 film. RESULTS: The density of the PTFE and Plexiglas that were inserted in the phantom are in good agreement with bone and soft tissue. Also, the CT numbers of these materials have a low difference. The dose distribution from the EBT3 film and the treatment planning system is similar. CONCLUSIONS: The constructed phantom with a size and inhomogeneity like an adult human head is suitable to measure the dose delivered to the area of interest. It also helps make an accurate comparison with the calculated dose by the treatment planning system. By using this phantom, the actual dose delivered to the target was obtained. This anthropomorphic head phantom can be used in other modalities of radiosurgery as well.

In recent years, Gafchromic films are used as an advanced instrument for dosimetry systems. The EBT3 films are a new generation of Gafchromic films. Our main interest is to compare the response of the EBT3 films exposed to gamma rays provided by the Theratron 780C as a conventional radiotherapy system and the Leksell Gamma Knife as a stereotactic radiotherapy system (SRS). Both systems use Cobalt-60 sources, thus using the same energy. However, other factors such as source-to-axis distance, number of sources, dose rate, direction of irradiation, shape of phantom, the field shape of radiation, and different scatter contribution may influence the calibration curve. Calibration curves for the 2 systems were measured and plotted for doses ranging from 0 to 40 Gy at the red and green channels. The best fitting curve was obtained with the Levenberg-Marquardt algorithm. Also, the component of dose uncertainty was obtained for any calibration curve. With the best fitting curve for the EBT3 films, we can use the calibration curve to measure the absolute dose in radiation therapy. Although there is a small deviation between the 2 curves, the p-value at any channel shows no significant difference between the 2 calibration curves. Therefore, the calibration curve for each system can be the same because of minor differences. The results show that with the best fitting curve from measured data, while considering the measurement uncertainties related to them, the EBT3 calibration curve can be used to measure the unknown dose both in SRS and in conventional radiotherapy.

Demand for high-performance energy storage materials has motivated research activities to develop nano-engineered composites that benefit from both high-rate and high-capacitance materials. Herein, NiMnO3 (NMO) nanoparticles have been synthesized through a facile co-precipitation method. As-prepared NMO samples are then employed for the synthesis of nano-composites with graphite (Gr) and reduced graphene oxide (RGO). Various samples, including pure NMO, NMO-graphite blend, as well as NMO/Gr and NMO/RGO nano-composites have been electrochemically investigated as active materials in supercapacitors. The NMO/RGO sample exhibited a high specific capacitance of 285 F g(-1) at a current density of 1 A g(-1), much higher than the other samples (237 F g(-1) for NMO/Gr, 170 F g(-1) for NMO-Gr and 70 F g(-1) for NMO). Moreover, the NMO/RGO nano-composite has shown excellent cycle stability with a 93.5% capacitance retention over 1000 cycles at 2 A g(-1) and still delivered around 87% of its initial capacitance after cycling for 4000 cycles. An NMO/RGO composite was assessed in practical applications by assembling NMO/RGO//NMO/RGO symmetric devices, exhibiting high specific energy (27.3 Wh kg(-1)), high specific power (7.5 kW kg(-1)), and good cycle stability over a broad working voltage of 1.5 V. All the obtained results demonstrate the promise of NMO/RGO nano-composite as a high-performance electrode material for supercapacitors.

BACKGROUND: Mucositis is one of the acute complications of radiotherapy which can ulcerate oral mucosa and cause severe pain and discomfort which can affect oral normal function. Propolis is a natural source of flavenoid which has antiulcer, antibacterial, antifungal, healing and anti-inflammatory effects. Using such an affordable compound without any bad smell or taste that has reasonable price can help the radiotherapy undergoing patients. OBJECTIVES: Our goal is assessing the preventing and therapeutic effect of propolis in radiotherapy induced mucositis in patients with head and neck cancer. PATIENTS AND METHODS: In a randomized triple blind clinical trial, 20 patient were selected randomly to swish and swallow 15 ml of water based extract of propolis mouth wash 3 times a day in the case group (n = 10) and 15 ml placebo mouth wash in control group (n = 10). we use NIC-CTC scale for determining mucositis grading. RESULTS: We use T-test, Man-Whitney, Chi-square, and Friedman as analyzing tests. Case group had significantly (P < 0.05) lower grade of mucositis in all of the follow-ups, but xerostomia is not significantly different in two groups (P > 0.05). CONCLUSIONS: This is a pilot study which shows water based extract of propolis efficiently prevents and heals radiotherapy induced mucositis.

Berberine was loaded in yeast cells of Saccharomyces cerevisiaeas a novel pharmaceutical carrier to improve the treatment ofmany diseases. The yeast-encapsulated active materialsshowedhigh stability and bioavailability due to the enhanced solubility and sustained releasing. In this study, different characteristics of prepared berberine loaded yeast cells (loading capacity, release kinetic order, MIC and stability) were evaluatedby different analytical methods (fluorescence spectroscopy, HPLC and SEM).The loading capacity was about 78% ± 0.6%.Berberine release patterns of microcapsules happened in two different stages and followed by zero and first-order kinetic,respectively. About 99% of all active material released during 34 h. MIC was improved by berberine loaded microcapsules in comparison withberberine powder. The microcapsules were completely stable. Berberine loaded Sac. Cerevisiae could be considered as a favorite sustained release drug delivery system. The yeast would be applied as an efficient carrier to improve various properties of different active materials.

Work-related musculoskeletal disorders (WMSDs) are common among drivers and official workers. Musculoskeletal disorders are frequent causes of absenteeism in many countries. The aim of this study was to determine the prevalence of musculoskeletal disorders and risk factors associated with these symptoms. A total of 346 workers and truck drivers were participated in this case-control study. All the participants were interviewed using a self- administered questionnaire containing demographic data and a Nordic questionnaire about presence site and characteristics of pain. Then the data were gathered, and the prevalence of the mentioned parameters and the relationship between variables in the questionnaire were analyzed statistically. The results of this study revealed that 78.6% out of truck drivers and 55.5% out of official workers had musculoskeletal disorders in on-year and there was a significant difference between two groups in this regard (P<0.001). On the whole, the most common symptoms were neck 47 (27.2%), followed by lumbar pain 42 (24.3%) in truck drivers and knee 63 (36.4%) and lumbar symptom 21 (12.1%) in one-year in official workers. In this study, musculoskeletal disorders showed statistically significant association with work duration, age and BMI (P<0.001). Within the limits of this study, it can be concluded that the musculoskeletal troubles have a high frequency among the drivers and official workers. Both groups usually remain on a prolonged uncomfortable postures and high static muscle load which may imply a risk for development of the troubles.

In order to establish diagnostic reference levels (DRLs) for multi-detector computed tomography (MDCT), four routine CT examinations were identified and a computer program was developed to collect data from 19 MDCT scanners in Iran. Mean values of Volume computed tomography dose index (CTDIvol) and dose-length product (DLP) in each site were calculated and the DRLs were defined as the 75th percentile of the distribution of the CTDIvol/DLP values for each examination. In terms of DLP, the DRLs of adult age group are 700, 290, 330, and 550 mGy cm for the Brain, Sinus, Chest, and Abdomen and Pelvis protocols, respectively. Although DRLs of this study are comparable to other international DRLs and in most cases are less than the international reference values, the great extent of dose distributions indicates that the CT imaging procedures in Iran should be optimized by applying diagnostic reference levels in order to decrease the radiation dose to patient undergoing CT examination.

BACKGROUND: Influenza A is a virus that affects a wide range of animals and also human beings. Avian influenza virus (AIV) subtype H9N2 has the potential to create influenza pandemic and vaccination is a common solution for this problem. The vaccine, used for rapid intervention, should be safe to use and highly effective, after a single administration. Chitosan nanoparticles (CNP) have already been recommended as a new adjuvant for inactivated AIV H9N2 vaccine immunization. OBJECTIVES: This study aimed at the evaluation and better understanding of optimum concentration of CNP preparations and also, assessment of loading capacity of AIV into CNP, as an adjuvant in specific pathogen-free (SPF) chickens. MATERIALS AND METHODS: For measurement of vaccine-antibody response, different types of CNP were injected intramuscularly, in a single dose, to 21-day-old specific pathogen-free chickens. Chickens were monitored for the efficacy of the nanoparticles and, also, their immune response, during a follow up of 7 weeks, by using hemagglutination-inhibition (HI) test. The CNP were prepared according to modified ionic gelation method and inactivated antigen was loaded in four hemagglutinin units (HAU) concentrations. Loading capacity of nanoparticles was determined by hemagglutination (HA) method. Inactivated A/H9N2 AIV was mixed with chitosan of low molecular weight. RESULTS: The CNP did not cause any mortality or side effects, when chickens were administered the prepared vaccine. The results strongly showed that this novel vaccine significantly enhances the immunogenicity of inactivated AIV, comparing with ISA70 (SEPPIC, Puteaux, France) adjuvant that is used routinely in the Razi Serum and Vaccine Research and Production Institute, Karaj, Iran, to reduce ISA70's side effects. CONCLUSIONS: The AIV loaded into CNP vaccines induce appropriate antibody titers, after a single immunization, while requiring a low dose of antigen. The CNP also represent an interesting new platform for antigen delivery and a promising adjuvant candidate for H9N2 inactivated influenza vaccine.

The pathogenicity and genetic characterizations of six Newcastle disease virus (NDV) isolates obtained from chicken farms in six different regions in Iran were carried out using conventional and molecular techniques. Based on the pathogenicity indices (MDT, ICPI, and IVPI), all of these isolates were found to be velogenic (highly virulent) strains. A sequence analysis of the full-length mRNA encoding the fusion glycoprotein precursor (F0) of the NDV's fusion proteins F1 and F2 in these six isolates showed the presence of point mutations in form of nucleic acid substitutions at positions 82((C→T)), 83((T→C)), 736((A→G)), and 1,633((G→A)). However, the nucleic acid residues at positions 330-347 of the precursor F0 gene, corresponding to the cleavage site of the F0 protein, were found to have remained conserved among the six NDV isolates. A phylogenetic comparison between the six Iranian isolates and the NDVs whose F0 gene sequences were previously deposited in GenBank Database showed that all of the newly characterized Iranian NDV isolates belonged to genotype VII.

In this study, we investigated the effects of hydrolyzed and non-hydrolyzed collagen and two-dimensional and three-dimensional collagen matrices on cell survival, attachment and neurite outgrowth of primary cultured nerve cells using 3-[4,5-dimethylthiazol-2-yl]-2,5-diphenyltetrazolium bromide (MTT) colorimetric assay and inverted microscopy. Hydrolyzed collagen facilitated nerve cell survival and neurite outgrowth, but it had no obvious influences on cell attachment. In contrast, non-hydrolyzed two-dimensional collagen matrix had no obvious effects on neurite outgrowth. These findings suggest that hydrolyzed collagen is an ideal nerve cell culture media.

In the present study, water extract of dried fruit of Zyzyphus Jujube was tested for its possible anticancer effect and induction of apoptosis on human tumor cell lines, HEp-2, HeLa and Jurkat cell lines. The inhibitory effect of water extract of this fruit on cell proliferation was assessed by MTT colorimetric assay. The induction of apoptosis of this extract was analyzed by DNA fragmentation analysis. Zyzyphus Jujube extract showed inhibitory effects on mentioned cell lines. Jurkat leukemic line was found the most sensitive cells with IC50 of 0.1 mug mL(-1). Our study also showed a typical DNA laddering in this cell line. The present study showed cytotoxic activity of Zyzyphus Jujube on tumor cells. Although Zyzyphus Jujube has useful compounds for medical applications.

The water extracts of propolis (WEP) could inhibit growth of different cell lines namely McCoy, HeLa, SP2/0, HEp-2, and BHK21 and stimulate growth of normal cell named human lymphocyte, rat kidney, rat liver, and rat spleen. In these experiments 1 and 2 mg of WEP were added to 1 ml RPMI media with 5% FCS. Cell counts and cell viability of propolis-treated and propolis-free cells were assessed by Trypan blue dye exclusion test and MTT assay. The results showed that in case of McCoy, HeLa, SP20, HEp-2, and BHK21 cell lines, the water extracts of propolis could inhibit cell growth as well as reduction on size of the cells. In contrast the same amount of WEP could stimulate growth of normal cells up to 60% with the same concentration used for cell lines. Thus our study indicates that although WEP consists only of the soluble part of propolis, it enables to inhibit different cell lines and increase growth of normal cells. This indicates also that WEP contains the specific compounds with bioactivity against cell lines. Although propolis contain different number of compounds it is clear that WEP has enough biological compounds useful for the treatment of some diseases, medical and related applications.

RESUMEN

The genotypes for Leptin, Kappa-Casein, Calpastatin and BoLA-DRB3 loci were determined by polymerase chain reaction and restriction enzyme digestion method in native Iranian breed cattle, Sistani. Blood samples were collected from Sistani Breeding Station located in Zehak, Zabol in Iran. The extraction of genomic DNA was based on Guanidin Thiocyanate-Silica gel method. After PCR reaction, amplicons were digested with appropriate restriction enzymes. The Calpastatin locus had 3 genotypes with frequencies of 0.62, 0.29 and 0.09 for MM, MN and NN, respectively; kappa-Casein and Leptin had 3 genotypes with frequencies of 0.27, 0.57 and 0.16 for kappa-Casein, 0.77, 0.22 and 0.01 for Leptin for AA, AB and BB genotypes, respectively. For BoLA-DRB3 we identified 19 alleles, that DRB3. 2*8 had the highest allelic frequency (22.4%) and DRB3. 2*3, *29, *37 and *51 had the lowest allelic frequency (1%). One of the 19 alleles had a new pattern. Average heterozygosity values for all loci were low. Chi2-test did not confirm the Hardy-Weinberg equilibrium for Leptin and Calpastatin in this population. These data provide evidence that Iranian's Sistani breed have a good genetic variability, which opens interesting prospects for future selection programs, especially marker-assistant selection.

RESUMEN

A protease isolated from Pseudomonas aeruginosa PD100 could act in the presence of SDS and Tween 80. This protease could be useful for degradation of protein in the presence of solvent, dehairing of cow skin and degradation of natural proteins. The immobilized protease showed 15-20% increases in temperature stability and the entrapped enzyme retained 83% of its initial activity after six cycles. With respect to properties of the enzyme and its capability for degradation of different protein sources, this protease finds potential application for waste treatment, used in detergents and leather industry.
