RESUMEN

Horse owners seek the advice and support of a number of equestrian professionals in carrying out their duty of care for their animal. In some instances, these professionals form a multi-disciplinary team (MDT). The aim of this study was to explore the experiences of the Society of Master Saddlers' qualified saddle fitters (SMSQSFs) working with other professionals and to understand the nature of inter-disciplinary working from an SMSQSF perspective. Semi-structured, one-to-one online interviews with fourteen SMSQSFs were completed. Areas explored included the nature of the participant's client base; the frequency and nature of their interactions with other professionals; their perceptions of horse owner expectations of an MDT approach; and any benefits, challenges, and barriers to an MDT approach within an equestrian setting. Interviews were video and audio recorded (MS Teams), transcribed verbatim (Otter ai), and imported into qualitative data analysis software (NVivo, version 12). Data were analysed using thematic analysis. Six themes were identified: (1) effective communication; (2) multidisciplinary expectations; (3) horse welfare; (4) professionalism; (5) relationships; (6) working together. Communication was recognised as a crucial component of an effective MDT. Most participants valued and desired an MDT approach. They felt they had a key role to play within the equestrian MDT, not only in the prevention of deterioration in horse welfare but also in improving the functionality and performance of the horse-rider partnership. Effective MDT working was also seen as having benefits to SMSQSFs and other professional stakeholders alike, although time and financial constraints were identified as barriers to MTD working. The role of the horse owner within the MDT was unclear and potentially complex, and this and other factors such as the professional identity of the SMSQSF, personal relationships, and input from others outside of the MDT team were identified as challenges to effective MDT working. This present study found that SMSQSFs experience similar benefits and challenges to an MDT approach as seen in human healthcare settings. The role of the horse owner, communication, and professional recognition are indicated as pivotal to MDT effectiveness in achieving optimal saddle fit.

This study follows a previously defined framework to investigate the impact of impairment on performance in Para dressage athletes. Twenty-one elite Para dressage athletes (grades I to V) and eleven non-disabled dressage athletes (competing at Prix St. Georges or Grand Prix) participated. Data were collected in two phases: performing a two minute custom dressage test on a riding simulator while kinematic data were synchronously collected using inertial measurement units (2000 Hz) and optical motion capture (100 Hz), and clinically assessed using a battery of impairment assessment tools administered by qualified therapists. Impairment and performance measures were compared between Para and non-disabled athletes. Significant differences between athlete groups were found for all impairment measures and two performance measures: simulator trunk harmonics (p = 0.027) and athlete trunk dynamic symmetry (p < 0.001). Impairment assessments of sitting function and muscle tone could predict 19 to 35% of the impact of impairment on performance in Para athletes but not in non-disabled athletes. These findings provide the basis for a robust, scientific evidence base, which can be used to aid in the refinement of the current classification system for Para dressage, to ensure that it is in line with the International Paralympic Committee's mandate for evidence-based systems of classification.

Static assessment and grouping of riders by competition level are prevalent in equestrian coaching practice and research. This study explored sagittal pelvic tilt in 35 competitive dressage riders to analyse the relationship between static and dynamic postures and assess the interaction of competition level. Riders were assessed using optical motion capture on a riding simulator at halt and in walk, trot, and left and right canter. Mean, minimum and maximum pelvic tilt, and range of motion (ROM) were measured as the pitch rotation of a rigid body formed by markers placed on the rider's left/right anterior and posterior superior iliac spines and sacrum, averaged over six time-normalised strides. Three key results emerged: (1) there are correlations between the rider's mean pelvic tilt in simulated walk, trot and canter, but not at halt; (2) mean pelvic tilt values are not significantly influenced by competition level (p = 0.233); and (3) the minimum and maximum pelvic tilt values illustrate individual strategies between gaits. Therefore, results from static assessment and grouping of riders by competition level should be interpreted with caution. Riders should be assessed as individuals, during dynamic riding-specific tasks, to understand their postural strategies.

The rider's ability to consistently coordinate their movements to their horse is a key determinant of performance in equestrian sport. This study investigated the inter-segmental coordination variability between the vertical displacement of a riding simulator and the pitch rotation of 28 competitive female dressage riders' head, trunk, pelvis, and left foot, in simulated medium and extended trot. A statistical non-parametric mapping three-way repeated-measures ANOVA investigated the influence of gait, competition level and segment on coordination variability. There was a significant main effect of gait and segment (p = 0.05), however, no significant effect of competition level. In medium trot, simulator-pelvis coupling was significantly (p < 0.001) less variable than simulator-head, -trunk, and -foot couplings. Significantly greater coordination variability of simulator-head and -foot relative to the trunk and pelvis suggested that riders can maintain stability in the saddle with their trunk and pelvis while allowing greater variability of their head and foot coupling to the simulator's vertical displacement. It is proposed that stronger coupling of the rider's pelvis relative to their other segments is one facet of the equestrian dressage skill of the independent seat. However, greater perturbations during simulated extended trot may necessitate a decrease in the independence of the rider's seat.

Water treadmill (WT) exercise is frequently used for training/rehabilitation of horses. There is limited study into the effect of water depth on limb/back kinematics warranting investigation. The objective was to determine the effect of walking in different water depths, at the same speed, on limb/back kinematics measured simultaneously in a group of horses. Six horses (age:15 ± 6.5 years) completed a standardized WT exercise session (19 minutes duration; speed:1.6 m/s; water depths: 0.0/7.5/21.0/32.0/47.0 cm). Ten waterproof light-emitting-diode tea-light-markers and reflective-spheres were affixed to the skin at predetermined locations; inertial-measurement-units were fixed to the poll/withers/left and right tubera coxae (TC)/sacrum to determine range-of-motion (ROM) changes of these locations. Univariable-mixed-effects-linear-regression-analyses were carried out, with a significance value of P ≤ .05. At maximum carpal/tarsal flexion during swing, regression analyses showed a clear and consistent nonlinear increase in carpal and tarsal flexion at increasing water depths (P < 0.0001 for both variables). As water depth increased there was a significant increase in thoracic spine flexion-extension ROM (P < 0.0001 at all thoracic sites) and increased dorsoventral and mediolateral ROM of the sacrum/left and right TC (P < 0.001 for all variables) as water depth increased. Results suggest that horses responded to an increase in water depth until a threshold depth was reached when the biomechanical response levelled off, and there was increased pelvic roll. In conclusion, changes in limb kinematics brought about by relatively modest increases in water depth at walking speed of 1.6 m/s are sufficient to induce significant changes in back/pelvic movement highlighting key issues with relevance for WT program design.

The EquiAmi Training Aid (ETA) is a popular training and rehabilitation tool, however knowledge about its effect on the equine gait is lacking. Understanding of its effects on equine kinematics, and the clinical relevance of these effects is vital to promote optimal use of training aids within training and rehabilitation programmes. Therefore, this study aimed to determine how the ETA influences horses' gait kinematics at walk and trot. Eight horses walked and trotted in-hand with and without the ETA. Optical motion capture was used to measure forelimb and hindlimb pro- and retraction angles, withers-croup angle, and stride length. Separate repeated-measures ANOVAs in each gait were used to assess the differences between gait kinematics and stride length variability with and without the ETA. The ETA did not significantly influence the horses' kinematics in walk or trot, however, individual differences in the effect of the ETA on the horses' angular and linear kinematics were found, with variation between gaits within the same horse observed. The ETA does not have the same effect on every horse, and its effect can vary within the same horse between gaits. Therefore, the individual characteristics and needs of the horse must be considered when applying training aids.

Water treadmill exercise has become popular in recent years for the training and rehabilitation of equine athletes. In 2019, an equine hydrotherapy working group was formed to establish what was commonly considered to be best practice in the use of the modality. This article describes the process by which general guidelines for the application of water treadmill exercise in training and rehabilitation programmes were produced by the working group. The guidelines describe the consensus reached to date on (1) the potential benefits of water treadmill exercise, (2) general good practice in water treadmill exercise, (3) introduction of horses to the exercise, (4) factors influencing selection of belt speed, water depth and duration of exercise, and (5) monitoring movement on the water treadmill. The long-term goal is to reach a consensus on the optimal use of the modality within a training or rehabilitation programme. Collaboration between clinicians, researchers and experienced users is needed to develop research programmes and further guidelines regarding the most appropriate application of the modality for specific veterinary conditions.

As a first step in achieving an evidence-based classification system for the sport of Para Dressage, there is a clear need to define elite dressage performance. Previous studies have attempted to quantify performance with able-bodied riders using scientific methods; however, definitive measures have yet to be established for the horse and/or the rider. This may be, in part, due to the variety of movements and gaits that are found within a dressage test and also due to the complexity of the horse-rider partnership. The aim of this review is therefore to identify objective measurements of horse performance in dressage and the functional abilities of the rider that may influence them to achieve higher scores. Five databases (SportDiscuss, CINAHL, MEDLINE, EMBASE, VetMed) were systematically searched from 1980 to May 2018. Studies were included if they fulfilled the following criteria: (1) English language; (2) employ objective, quantitative outcome measures for describing equine and human performance in dressage; (3) describe objective measures of superior horse performance using between-subject comparisons and/or relating outcome measures to competitive scoring methods; (4) describe demands of dressage using objective physiological and/or biomechanical measures from human athletes and/or how these demands are translated into superior performance. In total, 773 articles were identified. Title and abstract screening resulted in 155 articles that met the eligibility criteria, 97 were excluded during the full screening of articles, leaving 58 included articles (14 horse, 44 rider) involving 311 equine and 584 able-bodied human participants. Mean ± sd (%) quality scores were 63.5 ± 15.3 and 72.7 ± 14.7 for the equine and human articles respectively. Significant objective measures of horse performance (n = 12 articles) were grouped into themes and separated by gait/movement. A range of temporal variables that indicated superior performance were found in all gaits/movements. For the rider, n = 5 articles reported variables that identified significant differences in skill level, which included the postural position and ROM of the rider's pelvis, trunk, knee and head. The timing of rider pelvic and trunk motion in relation to the movement of the horse emerged as an important indicator of rider influence. As temporal variables in the horse are consistently linked to superior performance it could be surmised that better overall dressage performance requires minimal disruption from the rider whilst the horse maintains a specific gait/movement. Achieving the gait/movement in the first place depends upon the intrinsic characteristics of the horse, the level of training achieved and the ability of the rider to apply the correct aid. The information from this model will be used to develop an empirical study to test the relative strength of association between impairment and performance in able-bodied and Para Dressage riders.

Outcome measures (OMs) are a requirement of professional practice standards in human and canine physiotherapy practice for measurement of health status. Measures such as pain and functional capacity of specific regions are used to track treatment impact and can be used to develop optimal management strategies. To achieve comparable patient care in equine physiotherapy, OMs must be incorporated into practice; however, no reliable and valid OMs exist for equine rehabilitation. This study utilised the experience and opinion of a panel of experts working in the equine rehabilitation sphere to gain consensus on the core areas (domains) to be included in a model, to lead to an OM scale for horses undergoing rehabilitation. The Delphi method and content validity ratio testing was used to determine agreement with domains reaching the critical value required for inclusion. The expert panel agreed on ten domains to be included in the OM scale: lameness, pain at rest, pain during exercise, behaviour during exercise, muscular symmetry, performance/functional capacity, behaviour at rest, palpation, balance and proprioception. An OM with these domains would provide a holistic objective assessment tool which could be used by equine rehabilitation professionals in clinical practice.

Published guidelines indicate an English saddle tree should not extend beyond the 18th thoracic vertebra (T18). The study aimed to assess reliability of saddle fitters (SFs) to identify the T18 spinous process (SP). Part 1 investigated agreement between T18 (T18SF) as identified by three SFs using palpation and a veterinary surgeon (VS) using radiography (T18VS) in seven horses. Saddle fitter 1 and SF2 palpated the lumbosacral joint and counted cranially six SPs, whereas SF3 followed the rib curvature toward the dorsal midline. In part 2, SF1 and SF2 identified T18 by counting cranially five SPs in seven horses on two occasions. Agreement between SFs and VS was assessed using t tests and Bland-Altman plots. Interrater and intrarater reliability were estimated using intraclass correlation coefficients. In part 1, SF1 and SF2 found T18SF 4.3 cm (±4.1 and 4.0 cm, respectively) cranial to T18VS. Mean difference between T18SF3 and T18VS was 0.1 ± 4.9 cm (95% CI: -9.5 cm, 9.6 cm, P = .976). When counting cranially five SPs, mean difference between T18SF1 and T18VS was -1.5 ± 3.4 cm (95% CI: -8.3 cm; 5.1 cm; P = .265) and T18SF2 and T18VS was -0.3 cm ± 4.5 cm (95% CI: -8.8 cm; 8.5 cm; P = .847). Interrater reliability was "good" (ICC = 0.798). Intrarater reliability was "excellent" for SF1 (ICC = 0.905) and "good" for SF2 (ICC = 0.847). Counting cranially five SPs from the lumbosacral joint, when coupled with observation of the rib position and curvature should ensure a saddle is not placed beyond T18.
