RESUMEN

Surgical excision of solid tumors is required for local control of neoplasms. However, surgical trauma can stimulate the release of proangiogenic growth factors, suppressing cell-mediated immunity and favoring the development of micrometastases and progression of residual disease. The present study aimed to evaluate the intensity of the metabolic response to trauma induced via unilateral mastectomy in bitches with mammary neoplasia, the consequences of its joint performance with ovariohysterectomy, and their respective effects on the organic response. Two groups of animals were evaluated in seven perioperative moments, namely, unilateral mastectomy (G1) and unilateral mastectomy associated with ovariohysterectomy (G2). Thirty-two female dogs were selected, ten clinically healthy, and twenty-two diagnosed with mammary neoplasia. Surgical trauma reduced serum concentrations of albumin and interleukin-2 but increased blood levels of glucose and interleukin-6 in the postoperative of G1 and G2 patients. Moreover, serum cortisol levels increased after unilateral mastectomy associated with ovariohysterectomy. Our findings allowed us to conclude that unilateral mastectomy induces significant metabolic alterations in female dogs with mammary neoplasms and its joint performance with ovariohysterectomy increases the organic response to trauma.

PURPOSE: To evaluate whether using platelet-rich plasma (PRP) in the graft recipient bed after the resection of a neoplasia can influence its recurrence because this product stimulates angiogenesis, mitogenesis and chemotaxis. METHODS: A study with 30 rats Wistar (Rattus norvegicus albinus), which were separated into group A (induction of carcinogenesis, PRP in the postoperative period) and group B (induction of carcinogenesis, absence of PRP in the postoperative period), with 15 animals in each. Carcinogenesis was induced on the skin of the animals' chest by the topical application of 0.5% dimethylbenzantracene (DMBA) diluted in acetone. After surgical resection of the induced neoplasia, PRP was used to stimulate angiogenesis before surgical wound synthesis. Data on the control and experimental groups and macroscopic and microscopic variables were evaluated using analysis of variance and the Tukey's test (5%). RESULTS: It was possible to determine that the use of PRP is good in reconstructive surgeries, but it is contraindicated in patients during tumor resection, as it can cause changes in the surgical bed, in addition to stimulating recurrences and metastases. CONCLUSIONS: PRP may interact with tumour cells that were in the recipient site of the surgical wound during the resection of a neoplasia, and a local recurrence process can be triggered by applying this product.

Os retalhos cutâneos são técnicas de escolha para reconstrução de grandes defeitos. Objetivou-se relatar o emprego do retalho de padrão axial da artéria torácica lateral em paciente com ferida cirúrgica ocasionada imediatamente após exérese de neoformação na região da articulação do cotovelo. Foi atendido paciente canino macho, 10 anos, pesando 12 kg, apresentando neoformação em tecidos moles, de grande volume, localizada na região medial do cotovelo. Como tratamento definitivo, implementou-se a exérese marginal da neoplasma seguida de reconstrução com a técnica de retalho de padrão axial da artéria torácica lateral. A técnica possibilitou completa síntese e oclusão do defeito, obtendo-se ótima recuperação pós operatória, não sendo observada deiscência de sutura ou necrose do retalho. Conclui-se que a implementação do retalho da artéria torácica lateral foi uma técnica viável na reconstrução em cotovelo após a exérese do neoplasma no paciente relatado, observando resultados satisfatórios quanto aos aspectos funcionais e cosméticos, após o período cicatricial.


Skin flaps are suitable methods of wound closure in large cutaneous defects. I The present study aims to report the use of lateral thoracic axial pattern flap to repair an elbow wound secondary to a neoplasm resection. A 10-year-old male dog weighing 12 kg was presented with a large soft tissue neoplasm in the medial aspect of the elbow. The patient underwent surgical resection of the neoplasm using the lateral thoracic cutaneous flap to achieve a complete defect closure, resulting in an excellent postoperative wound healing without suture failure or skin flap necrosis. It is concluded that the implementation of the lateral thoracic artery flap was a viable technique in elbow reconstruction after neoplasm excision in the reported patient, with satisfactory results regarding functional and cosmetic aspects after the healing period.

Purpose To analyze changes in the thermal pattern in the skin graft receptor bed, after the use of therapeutic ultrasound through the thermographic images. Methods Eighteen Rattus norvegicus albinus Wistar, separated into two groups: GST groups (without tumor and without treatment with ultrasound) and GT (with tumor and treatment with ultrasound). In the GT group, induction of carcinogenesis was performed by single intradermal application of 0.05 ml DMBA at 0.5%, diluted in acetone. Subsequently, a technique of reconstructive grafting surgery of the mesh type was performed in both groups and treatment with therapeutic ultrasound was performed in the GT group the alternate day protocol at 3, 6, 10 and 15 days after the procedure. The thermographic evaluation occurred on days 3, 6, 10 and 15 after the grafting. Results There was a significant difference between the statistical evaluation of the temperature of the control group when compared to the treated group, on the different evaluation days (p <0.0001). Conclusion The thermographic analysis of the images was effective in evaluating the healing process, being the use of thermography feasible to evaluate changes in the thermal standard in the surgical bed, besides the beneficial effects of the US.

O objetivo do presente estudo é relatar a construção de um retalho direto de bolsa bipediculada com fixação apendicular à região torácica para o tratamento de lesões extensas no membro torácico direito de um gato. Foi tratado no Hospital Veterinário Unicesumar Maringá - Paraná, um felino de 1 ano de idade, apresentando lesão no membro torácico, após história de briga com outro animal da mesma espécie. Ao exame físico, apresentava fístula e edema no membro torácico direito. O paciente foi anestesiado e foi realizado o desbridamento das áreas desvitalizadas. Como terapia inicial, a lesão foi tratada como ferida aberta, até a formação de tecido de granulação, para posterior correção do defeito empregando retalho bipediculado em bolsa direta, com fixação temporária do membro à parede torácica. Após duas semanas, o membro foi desimplantado e os pontos foram removidos 21 dias após a aplicação do retalho. O uso do retalho permitiu completa cicatrização da ferida, sem ocorrência de hematomas, necrose ou deiscência de sutura. Conclui-se que a utilização do retalho bipedicular é aplicável ao tratamento de feridas traumáticas em membro torácico de felinos.


The aim of the present study is to report the construction of a direct bipedicled pouch flap with appendicular fixation to the thoracic region for the treatment of extensive lesions in the right thoracic limb of a cat. He was treated on Hospital Veterinário Unicesumar Maringá - Paraná, a 1-year-old feline, presenting injury to the thoracic limb after a history of fighting with another animal of the same species. On physical examination, he presented fistula and edema in the right thoracic limb. The patient was anesthetized and debridement of the devitalized areas was performed. As an initial therapy, the lesion was treated as an open wound until granulation tissue was formed for subsequent correction of the defect using a direct pouch bipedicled flap, with temporary fixation of the limb to the chest wall. After two weeks, the limb was unimplanted and the stitches were removed 21 days after flap application. The use of the flap allowed complete wound healing without bruising, necrosis or suture dehiscence. It is concluded that the use of bipedicular flap is applicable to the treatment of traumatic wounds in feline thoracic limb.

BACKGROUND: Photodynamic Therapy (PDT) using Aminolevulinic acid (ALA) and derivative molecules as topical medication and as a precursor of protoporphyrin (PPIX), is limited due to low permeation through skin or efficiency in porphyrin production. This behavior affects the production and homogeneity of PPIX distribution on superficial skin and in the deeper skin layers. Many authors propose alternatives to solve this such as, modification in the ALA and derivativemolecules, modifying the chemical properties of emulsion external phase or incorporating a delivery system to the emulsion. The goal of this study is to discuss what proportion of ALA and Methyl aminolevulinate (MAL) on mixtures increase the amount and uniformity of PPIX formation at superficial skin by fluorescence evaluations. METHODS: The study was conducted in vivo using a pig skin model. PPIX production was monitored using fluorescence spectroscopy and widefield fluorescence imaging on skin surface. 20% of ALA and MAL cream were done mixing the following proportions: ALA, M2 (80% ALA-20% MAL), M3 (60% ALA-40% MAL), M4 (50% ALA-MAL), M5 (40% ALA-60% MAL), M6 (20% ALA-80% MAL) and MAL. RESULTS: Mixtures M3, M4, and M5 showed the most PPIX production on skin by widefield fluorescence imaging and fluorescence spectroscopy in 3h of incubation. These results suggest that 50% of ALA and MAL in the same mixture increase the PPIX production in amount, homogeneity and time production when compared to ALA and MAL. This has a positive impact on photodynamic damage optimizing the PDT treatment.

Laryngeal and tracheal tumors are rare in pets; some piece of information on their disease behavior, therapy and evolution are limited. Neoplasms in this area are a diagnostic challenge. In many cases, they can be biopsied and excised using endoscopic instruments, but there is no report of this in canines. The goal of this study is to report a successful case of a laryngeal neoplasm removal through endoscopy. A head and neck radiogram revealed a mass in the laryngeal lumen protruding into the trachea. The patient then underwent an endoscopy to confirm the radiographic diagnosis and to surgically remove the tumor. The histopathological diagnosis was poorly differentiated carcinoma. The most appropriate treatment for laryngeal tumors is the resection of the submucosa or a partial laryngectomy however, partial and total laryngectomies are associated with many postoperative complications. In contrast, the endoscopic approach allows for highly magnified visualization of the lesion in situ, which facilitates the surgical removal of the mass through videosurgery. With little manipulation of the affected area, the chances of postoperative complications are reduced, leading to a more rapid recovery. .


Tumores de laringe e traqueia são raros em animais de estimação e as informações sobre o comportamento, terapia e evolução destas neoplasias são limitadas. Neoplasias nesta área são desafios diagnósticos. Em muitos casos, pode ser feita a biópsia e excisão da massa utilizando instrumentos endoscópicos, mas não existe relato deste tipo de procedimento em caninos. O objetivo deste estudo foi relatar um caso de sucesso da remoção endoscópica de um tumor de laringe. A radiografia da região cervical revelou uma massa na luz da laringe invadindo a traqueia. O paciente foi então submetido a uma endoscopia para confirmar o diagnóstico radiográfico e remover cirurgicamente o tumor. O diagnóstico histopatológico foi de carcinoma pouco diferenciado. O tratamento mais adequado para os tumores da laringe é a ressecção da submucosa ou uma laringectomia parcial, no entanto, estas estão associadas a muitas complicações pós-operatórias. Em contraste, a abordagem endoscópica permite a visualização da lesão in situ, o que facilita a remoção cirúrgica da massa através de videocirurgia. Com pouca manipulação da área afetada, as chances de complicações pós-operatórias são reduzidas, levando a uma recuperação mais rápida. .

A linfadenectomia laparoscópica é realizada de forma rotineira na medicina, contudo é pouco utilizada na veterinária. Neste relato, um canino fêmea apresentando tumores na cadeia mamária esquerda (M4 e M5), foi submetido à aplicação intradérmica do azul de metileno estéril, ao redor do maior tumor (M5), buscando-se demarcação dos vasos linfáticos e linfonodos regionais. Após 15 minutos, iniciou-se a linfadenectomia abdominal videolaparoscópica na região inguinal esquerda, seguida da ovário-histerectomia (OVH) lapararoscópica com três portais. Realizou-se ainda mastectomia total unilateral esquerda. Pela histologia, obtiveram-se dois linfonodos abdominais livres de células tumorais. A paciente não apresentou recidiva em 60 dias.


The laparoscopic lymphadenectomy is routinely performed in medicine, but not in veterinary medicine. This report describes a case of female canine with tumors in the left mammary chain (M4 and M5), which was subjected to intra-dermal application of sterile methylene blue, around the largest tumor (M5) which tried to demarcate the lymphatic vessels and regional lymph nodes. After 15 minutes, laparoscopic lymphadenectomy involving the abdominal in the left pelvic region started, followed by laparoscopic ovariohysterctomy (OVH) using three portals access. It was also performed an unilateral left total mastectomy. For histology, two abdominal lymph nodes were free of tumor cells. The patient had no recurrence in 60 days.

Cisplatin is an antineoplastic drug that is used to treat carcinomas and osteosarcomas in dogs. However, it can cause kidney damage which in turn leads to the loss of electrolytes. Concentration of magnesium was evaluated in the blood serum of eight dogs that had undergone therapy with cisplatin. The dogs were divided into two groups. Group 1 was supply with cisplatin (70 mg/m², iv) and to prevent nephrotoxcity, saline solution at 0.9% was administered (25 ml/kg/hour, iv in three hours). The animals in Group 2 were not supplied with cisplatin. There was no significant difference (P < 0.05) between the two groups and the results obtained indicated that the protocol used for Group 1 did not cause changes in the serum concentrations of magnesium. Therefore, it can be concluded that the administration of cisplatin does not cause variations if it is combined with the fuid therapy.


El cisplatino es un agente antineoplásico útil para el tratamiento de carcinomas y osteosarcomas en perros; sin embargo, puede causar daño renal que resulta con pérdida de electrólitos. Se evaluó la concentración de magnesio en suero sanguíneo de ocho caninos sometidos a terapia con cisplatino, distribuidos en dos grupos. A los perros del Grupo 1 se les administró cisplatino (70 mg/m², iv), para prevenir la nefrotoxicidad recibieron solución salina al 0.9% (25 ml/kg/h, iv durante tres horas). Los animales del Grupo 2 no recibieron cisplatino. No hubo diferencia significativa (P < 0.05) entre grupos, los resultados obtenidos indican que el protocolo utilizado para el Grupo 1 no ocasionó cambios en la concentración sérica de magnesio. Por tanto, se concluye que la administración de cisplatino no ocasiona estos cambios si se aplica juntamente con terapia de líquidos.

RESUMEN

O osteossarcoma (OSA) é um dos tumores malignos mais comum em cães. Geralmente acomete cães de raça grande e gigante, com idade média de sete anos. Esta neoplasia envolve, freqüentemente, a metáfise de ossos longos, sendo indicado à amputação radical do membro, acrescido de quimioterapia com cisplatina. A terapêutica desta neoplasia com localização na pelve toma-se difícil e normalmente não aceita pelos proprietários. O presente trabalho relata a incidência de osteossarcoma primário em ossos da pelve em cães, durante um período de 14 meses. Dos oito animais atendidos com esta neoplasia, observou-se maior freqüência nos cães da raça Rottweiler e Pastor Alemão. Dentre os ossos que compõem a pelve do animal se observou maior incidência de OSA acometendo o ílio (6 casos). Claudicação, impotência funcional de membro pélvico e dor foram os principais sinais clínicos observados. Em sete casos encontrou-se estágio avançado de comprometimento ósseo na imagem radiográfica, evidenciado, principalmente, pela lise da cortical óssea. Todos os animais foram submetidos à eutanásia por não apresentarem possibilidade de tratamento cirúrgico (hemipelvectomia) com margem de segurança. Também se levou em consideração para esta decisão à condição clínica dos pacientes, as chances de conferir melhora na qualidade de vida dos cães e a opção dos proprietários frente ao prognóstico.


The osteosarcoma (OSA) is one of the most common malignant tumors of dogs. Large and giant -breed dogs are affected and the mean age of these dogs is about 7 years. The tumor usually involves the metaphyses of long bones and the treatment is radical amputation and chemotherapy with cisplatin. The therapy of neoplasia located in pelvis is difficult and usually don't accept by owners. This report describes the incidence of primary osteosarcoma in pelvis of dogs, duting a period of fourteen months. Of the eight animals taken care with this neoplasia, a bigger frequency in the dogs of the race Rottweiler and German Pastor was observed. Amongst the bones that compose animal's pelvis was observed greater osteosarcoma incidence in ileum (6 cases). Lameness, functional impotence of pelvic member and pain had been the main observed clinical signals. In seven cases was found advanced involvement osseous, mainly, for lyses of the cortical. All the animals had been submitted to the euthanasia for not presenting possibility of surgical treatment with safety margin. Also one was taken in consideration for this decision to the clinical condition of the patients, the possibilities to confer improvement in the quality of life of the dogs and the option of the owners' front to the prognostic.
