RESUMEN

A case of toxigenic Corynebacterium ulcerans infection is presented. The diagnosis was delayed and no anti-toxin administered. A nasopharyngeal biopsy was complicated by severe haemorrhage necessitating a post nasal pack. A brief review of the pathology and treatment of Corynebacterium ulcerans is given.

RESUMEN

We report a case of a lateral pharyngeal diverticulum arising from the right vallecula. The aetiology and clinical features of these rare lesions are discussed. Diagnosis, which is difficult clinically, was made by barium swallow examination of the pharynx. The importance of contrast radiology in the investigation of pharyngeal symptoms is emphasized by this case.

RESUMEN

Six cases of bilateral abductor palsy with stridor are presented. Following unilateral intralaryngeal arytenoidectomy, four had adequate airways and reasonable voices. Two required bilateral arytenoidectomy, one acquired an adequate airway at the expense of a weakened voice, and the other still requires a tracheostomy. There was only one minor complication. Decannulation and discharge from hospital were usually rapid.

RESUMEN

Ten patients with allergic perennial rhinitis completed a 3-month course of reatment with flunisolide nasal spray. Biopsy of the nasal mucosa was carried out before the initiation of treatment and at the end of the treatment period. The flunisolilde was administered as a 0.025% solution twice daily at a total daily dose of 200 micrograms. No histological abnormalities which could have been attributed to the effects of the drug were found in the post-treatment biopsies. Comparison of the pre- and post-treatment histological features showed either that there had been no apparent changes or that there had been a reduction in the oedema and/or cellular infiltration which had been present initially. All except one patient improved clinically and in this case the pre-treatment nasal biopsy demonstrated features of atrophic rhinitis.

RESUMEN

Adequate pulmonary ventilation during general anaesthesia in completely relaxed patients during laryngoscopy has been provided with the aid of a readily available suction catheter and a manual oxygen valve. Operating conditions for the surgeon are excellent, clear photography through the operating microscope is facilitated and the exposure is a considerable improvement on other methods of anaesthesia.

RESUMEN

The results of Polytef injection of the vocal cords in 34 patients carried out over the last three years are reviewed. Two complications of respiratory distress were noted. It was shown that it is easy and safe to carry out this procedure under general anaesthesia using a shorter needle and a modified laryngoscope. There were no complications to the general anaesthetic.

RESUMEN

A double-blind investigation has been carried out using three different doses (20, 35 and 50 mg) of cocaine which were sprayed onto the nasal mucous membranes of forty-five patients for nasal surgery. The cocaine was administered after induction of general anaesthesia in all cases. The 20 mg dose of cocaine dissolved in 2 ml of saline without adrenaline was found to be safe and produced adequate nasal decongestion in the absence of other predominant factors. The efficacy of cocaine and its safety with regard to systemic absorption are assessed.
