RESUMEN

Our aim was to investigate the progress of wound healing after simple and surgical removal of the first two molars of the right and left maxillary segments, respectively, in Wistar rats with experimentally induced moderate chronic renal failure (CRF). Sixty Wistar rats were divided into two groups of 30 rats each: experimental and control. CRF was induced in the experimental group by an intraperitoneal injection of cisplatin, 5 mg/kg body weight (BW) initially and then with two maintenance doses of 2.5 mg/kg BW at intervals of one month. The teeth were extracted one month after the last dose of cisplatin. The sockets and the kidneys of all the rats of both groups were evaluated. The mandibles of the 15 rats in the experimental group that developed moderate CRF, together with those of two controls, were evaluated for abnormalities that suggested renal osteodystrophy. The histopathological examination showed: (a) that there were no significant differences in the pattern of wound healing no matter how the tooth was extracted; (b) there were no specific abnormalities in the mandible to indicate of secondary hyperparathyroidism or renal osteodystrophy; and (c) the kidneys of the rats of the experimental group underwent histopathological changes that were significantly different from those in the controls (P < 0.001). Our results indicate that moderate CRF does not have any appreciable or significant modifying effect on wound healing after tooth extraction in Wistar rats.

Three vertically HIV-infected children showed, in addition to oral candidiasis, HIV-gingivitis, which healed on antimycotic treatment. The intense linear gingival erythema of a fourth child was also clinically evaluated as a possible form of erythematous oral candidiasis. Direct microscopic examination of material from the gingival lesions of the latter disclosed yeast cells and hyphae. Subsequent culture, biochemical and serological tests identified the yeast as Candida dubliniensis. As the patient was on long-term prophylaxis with fluconazole, ketoconazole was administered and led to a good clinical response. This is the first report implicating this new Candida species as a pathogen in linear gingival erythema in a HIV-positive individual. The case reports presented provide evidence that linear gingival erythema may be of candidal origin. Further clinical and laboratory observations are required to establish whether this condition constitutes a variant of erythematous candidiasis associated with paediatric HIV infection.

RESUMEN

The first case of oral hairy leukoplakia (OHL) in an HIV-negative 56-year-old patient with acute lymphocytic leukemia (ALL) is reported. A white plaque was observed while the patient was in complete remission which followed the chemotherapeutic scheme. The clinical and histopathologic findings were typical for OHL and the polymerase chain reaction method was positive for Epstein-Barr virus DNA. Underdiagnosis and underreporting of OHL in patients with a malignant haematological disease and the apparent different environmental factors to which these non-AIDS patients have been exposed, probably constitute some of the reasons for the very few OHL cases reported in these patients.

Fifteen vertically HIV-infected children aged between 2 and 12 years were followed up for 1 year, weekly to monthly, to study the incidence of oral lesions. At the time of first examination, oral candidiasis (OC) was observed in nine children. Seven children presented with the erythematous type only and two with pseudomembranous oral candidiasis. Four cases of cheilitis were seen in association with the erythematous forms of oral candidiasis. One erythematous candidiasis progressed to pseudomembranous form. A second case of erythematous OC, after multiple recurrences in the form of erythematous OC, recurred as pseudomembranous OC. Another case of erythematous OC and one of pseudomembranous OC presented after multiple recurrences as a persistent, adherent pseudomembranous OC. An orofacial herpes-zoster infection, a hairy leukoplakia and a necrotic lingual ulcer were observed as second lesions and in association with oral candidiasis in three children. Erythematous oral candidiasis was the most frequent oral HIV-related lesion, was observed in different stages of HIV-infection, and in some cases progressed to pseudomembranous candidiasis. A different, selectively resistant, Candida clone was isolated in three cases of recurrent candidiasis.

RESUMEN

PURPOSE: To evaluate the effect of granulocyte-macrophage colony-stimulating factor (GM-CSF) in reduction of radiotherapy-induced oral mucositis. METHODS AND MATERIALS: Seventeen patients who were going to be irradiated with a total dose of 50-70 Gy for head and neck malignancies were included in the study. After the second week of radiotherapy, with the experience of oral pain, GM-CSF 400 microg was administered locally, once a day, until completion of radiotherapy. Patients were evaluated weekly for mucosal reaction and functional impairment. RESULTS: Three patients with gross and functional mucositis grade I after the second week, completed the planned radiotherapy showing mucositis grade I. Eleven patients who experienced, after 2 weeks of radiotherapy, mucositis grade II and III, presented after the third week with gross mucositis grade I and II and functional impairment grade I. One of these 11 patients was then lost to follow-up and the remaining 10 completed their planned radiotherapy having an almost asymptomatic mucositis grade I. The 15th patient with gross mucositis grade III after the 2 weeks of radiotherapy, had a 2-day interruption because of painful mucositis and then continued and completed radiotherapy with gross and functional mucositis grade I. The 16th patient with mucositis grade III after the second week, did not show any improvement, and completed her planned radiotherapy with mucositis grade III which finally healed after the administration of acyclovir. The last, 17th patient discontinued radiotherapy at the third week because of mucositis grade IV and severe ulceration in apposition to an extensive gold prosthesis. CONCLUSION: The local administration of GM-CSF significantly reduced and almost healed radiation-induced oral mucositis in 14 of 17 patients during the radiotherapy, which was completed within the preplanned time and without any significant patient weight loss or functional impairment.

RESUMEN

Polymorphous low-grade adenocarcinoma is a distinct type of salivary gland carcinoma arising most often intraorally and primarily in the palate. It is characterized by bland nuclear chromatin, variation in histomorphology and low-grade malignancy. We report a case of this recently described entity in a 66 year old male, presenting most of the characteristic clinical and histologic features. A short review of the current literature concerning this tumor is included.

RESUMEN

The clinical and histologic features of 87 cases of odontogenic keratocysts were reviewed. Males were affected more frequently than females. All age groups were affected, with a predilection for the fifth and sixth decades. The molar-angle-ramus area of the mandible was the most common location (29.8%). In 7 cases (8.8%), the cysts were part of the basal cell nevus syndrome. Recurrences were observed in 9 cases (10.4%). The cysts were lined by a thin, regular, stratified squamous, parakeratinized epithelium without rete ridges. Orthokeratin was observed in 24.14% of the cases. Mitotic figures were observed in approximately 50% of the cases. Separation of lining epithelium from underlying connective tissue was observed in 82.7%, with subepithelial hyalinization in 75.8% of the cases. Inflammation was observed in 72.4%, cholesterol clefts were observed in 11.5%, and Rushton hyaline bodies in 4.6% of the cases. Dental lamina rests were observed in 32.2% and satellite cysts in 20.7%. Dystrophic calcifications were found in 36.8%. Ameloblastomatous proliferation was observed in 2 cases.

A case of a spindle cell lipoma of the hard palate of a 58-year-old male is reported. The histologic diagnosis is discussed. Conservative surgical removal is the treatment of choice.

The effects of fractionated X-irradiation following isoproterenol (IPN) administration on rat parotid gland were studied 4, 8, and 12 weeks post-irradiation. Degenerative and necrotic changes of the acinar cells were observed at all three time intervals as a result of the radiation. These changes were more obvious 4 and 8 weeks post-irradiation and isoproterenol administration. The necrotic acinar cells were replaced by fibrous connective tissue, especially conspicuous at 12 weeks. Reparative changes were observed along with the degenerative and regressive phenomena of the gland. Enlarged acinar cells and nuclei were observed as early as 4 weeks post-irradiation. This change was focally distributed at the 4-week interval but more widespread 8 and 12 weeks post-irradiation. Mitochondrial hyperplasia with alteration of mitochondrial cristae was observed 12 weeks post-irradiation. Proliferation of cells of ductal origin and nodular proliferation of ducts were observed 12 weeks after isoproterenol administration plus X-irradiation. These proliferative phenomena may represent an early stage of tumor formation resulting from the combined effect of isoproterenol and X-irradiation on rat parotid gland.

Five cases of mucoepidermoid carcinoma of the parotid gland were studied ultrastructurally. In addition to epidermoid cells (tonofilament-rich) and mucous cells, three forms of intermediate cells were identified: mitochondria-rich cells, glycogen-rich cells, and ribosome-rich cells. Transitional forms of all cell types were observed. Ultrastructurally, cells rich in glycogen appeared to be the "clear cells" by light microscopy; cells rich in ribosomes appeared basophilic under light microscopy. Mitochondria-rich cells were lightly eosinophilic under light microscopy.
