[Safety and efficacy of the Relatox in the treatment of patients with cervical dystonia (results of a multicenter simple blind comparative randomized study)]. / Bezopasnost' i effektivnost' preparata Relatoks v lechenii patsientov s tservikal'noi distoniei (rezul'taty mul'titsentrovogo prostogo slepogo sravnitel'nogo randomizirovannogo issledovaniya).

OBJECTIVE: The analysis of evaluation of the efficacy and safety of Relatox in patients with cervical dystonia (CD). MATERIAL AND METHODS: Russian multicenter simple comparative randomized clinical trial. The study involved 180 patients of both sexes aged 18-65 years from 3 research centers of the Russian Federation. All patients had previously established diagnosis of CD and were randomized to one of two groups at 1 visit after the inclusion procedure and signing of informed consent. Patients in group 1 (n=90) got injections of Relatox, in group 2 (n=90) - injections of Botox for the first time or no earlier than 4 months after the previous injection of Dysport 500 units, Botox 200 units or Xeomin 200 units. The assessment of changes in cervical dystonia followed, including of determination of changes in the severity of CD, assessed on the TWSTRS scale by the total score and by the defining values of the blocks, as well as on the scales: the Patient Global Impression of Improvement (PGI-I) and of adverse events (local and systemic reactions). Based on the form of cervical dystonia (torticollis, laterocollis, etc.), the drug was injected under electromyographic control into the target muscles: sternocleidomastoid, belt, trapezoidal, shoulder-lifting muscle. RESULTS: Equal efficacy, good tolerability and safety comparable to the comparison drug Botox was demonstrated in 12 weeks after injection of Relatox. The number of registered local and systemic reactions in both groups was insignificant and did not differ significantly (p>0.05). There were no significant differences in the results of laboratory parameters in patients of both groups (p>0.05). Serious adverse events were also not detected during the study. CONCLUSION: Obtained results indicate the safety, good tolerability and efficacy of Relatox in patients with CD comparable to Botox and can be recomemded for using by practitioners.

[Intraocular pressure with miniscleral contact lenses]. / Oftal'motonus v usloviyakh nosheniya miniskleral'nykh kontaktnykh linz.

According to literature data, some experts do not exclude the possibility that scleral lens wear could influence intraocular pressure. PURPOSE: To evaluate the influence of rigid gas permeable miniscleral contact lenses on intraocular pressure (IOP), keratometry readings and corneal thickness, and to study the correlation between scleral (IOPs) and corneal (IOPc) intraocular pressure using the Icare ic100 tonometer (model TAO11, Icare Finland Oy). MATERIAL AND METHODS: The study included 99 volunteers without history of ocular diseases. The first group consisted of 66 participants (122 eyes) aged 22.3±2.2 years - IOPc and IOPs were measured by the Icare ic100 tonometer in order to determine the correlation. The second group (33 participants, aged 22.7±1.7 years) - day 1, diurnal IOPc and IOPs fluctuations were measured; on day 2, a miniscleral lens (diameter 14.9 mm) was placed on the study eye and was worn for 6 hours, the paired eye served as control. IOP was measured before, after lens placement, after 2 hours of lens wear, and before and after lens removal. Corneal topography was evaluated before and after lens removal. RESULTS: In the first group, there was a weak but significant correlation between IOPc and IOPs (Spearman correlation coefficient 0.285, p=0.001). In the second group, IOPc in the study eye before lens placement (14.8±3.8 mm Hg) and IOPc after its removal (13.6±3.9 mm Hg) were not different from those in the control eye. There were also no statistically significant changes in IOPs before, during lens wear, and after lens removal. The central corneal thickness increased by 2.9% (p<0.001) after 6 hours of lens wear. CONCLUSION: In young individuals without history of ocular diseases, wearing the miniscleral lens for 6 hours does not have significant influence on IOP and does not cause clinically significant corneal edema.

[Modern options for non-surgical correction of keratoconus]. / Sovremennye vozmozhnosti «nekhirurgicheskoi¼ korrektsii keratokonusa.

The article reviews data on the so called non-surgical methods of keratoconus (KC) correction. The combined approach to KC treatment entails three main tasks: prevention of progression, optical correction of induced refractive disorders, and structural repair. Measures taken to accomplish the first two tasks should be regarded as symptomatic treatment, while addressing the last task requires pathogenetically oriented solutions. Corneal cross-linking and optical correction of refractive disorders with rigid gas permeable (RGP) scleral contact lenses should be regarded as the first choice for symptomatic non-surgical correction of keratoconus.

The energy stored in a resonant cavity was extracted through four output ports and added in phase in a common line. Operation of a single switch provided synchronism and the power portions transmitted through the ports were combined in a waveguide turnstile junction. Estimation shows that the compressor peak power can reach a value eight times as much as the switched wave power, provided the output pulsewidth is shortened by the same factor with reference to the cavity double transit time. The performance of the X-band compressor prototype was investigated. Signals radiated through each of four output ports had identical envelope shapes and equal peak power values. The reflected wave did not accompany the power combining. The pulses of 1.2 MW peak power and 1.6 ns pulse width were obtained when the compressor was driven by the 50 kW pulse power magnetron generator.

The brief theoretical analysis shows the resonant microwave compressor provides the output pulse power higher than the traveling wave power in the storage cavity. The experimental study was made with the model of the S-band microwave compressor. The power of pulses generated by the device reached the value three times as much as the value of the traveling wave power in the storage cavity at the maximum amplification 23 dB, peak power 400 MW, and pulse width 4-5 ns.

[Peculiarities of erectile dysfunction in men with alcohol dependence].

To verify the etiopathogenic structure of erectile dysfunction in alcohol dependence, we have investigated 80 men with erectile dysfunction and alcohol dependence (the main group) and 40 men with erectile dysfunction without alcohol dependence (the control group). Endocrine and vasculogenic forms of erectile dysfunction were more frequent in men with alcohol dependence as compared to men without alcohol dependence, and the presentation of sexual disorder was complicated due to the increase in the number of accompanying syndromes. At the same time, as alcohol dependence progresses, the part of organic forms of erectile dysfunction and the number of syndromes which accompany erectile dysfunction increase.

[Hydrophilic regulator hexapeptides as inhibitors of tumor cell multiple drug resistance].

Multiple drug resistance (MDR) of tumor cells to cytostatics is one of the most often and severe complications of chemotherapy in oncological patients. The phenomenon of MDR could be due to a sharp increase of the activity of the ATP-dependent transport proteins of the ABC system, that provides pumping of the drug from the cells to the extracellular space. Up to now, all the attempts to design agents preventing MDR were of no success. One of the prospective trends is the use of hydrophilic regulator hexapeptides. Three regulator hydrophilic hexapeptides of the linear and cyclic structure were used as the MDR modulators. The sensitivity of the tumor cells to various cytostatics in the presence of the peptides was determined by the MTT-test and the direct counting of the survived cells. The effect of the hexapeptides on MCF7, KB8-5 and PC3 cells was investigated. It was concluded that the hydrophilic hexapeptides of the linear and cyclic structure increased the sensitivity to doxorubicin (a cytostatic). The tumor cell MDR inhibition was mediated by the ATP-dependent transport protein MRP. Such a characteristic of the hexapeptides is of interest for their use as agents preventing MDR.

Hydrophilic hexapeptide Imunofan as a hyperactive regulator of transport proteins for multiple drug resistance.

Hydrophilic hexapeptide Imunofan produces a modulatory effect on multiple drug resistance transport proteins depending on their functional activity. The hexapeptide inhibited multiple drug resistance proteins during substrate transport, but increased their activity in the latent period. By the inhibition of multiple drug resistance, specific activity of Imunofan more than 1000-fold surpassed that of other substrate analogues. Inhibitory analysis showed that regulatory effect of this hexapeptide depends on protein kinase C.

[Potential of different kinds of laser irradiation in treatment of benign and malignant tumors of facial skin].

The use of neodymium, holmium and diode lasers proved highly-effective in the treatment of facial skin cancers stage I-II (3,051)( basalioma--2,804, recurrent basalioma--165, squamous cell tumor--82) and benign tumors (2,157). Relapsed basaliomas after treatment from Pulsar-1,000 Ladoga- neodymium were reported in 1.6 and 3%, respectively. When pulsed neodymium lasers were used, repeat relapsed basalioma and recurrences of squamous cell cancer were detected in 4.2 and 4.9%, respectively. Overall malignancy recurrence frequency was 2,0% which was less than that after radiotherapy and surgery. Since the radiation penetration capacity of neodymium lasers is relatively high, their application is preferable in treatment of facial skin cancer stage I-II as well as exophytic or in-depth benign tumors. Holmium and diode lasers are better suited for destruction of superficial benign tumors. Good results should not be expected unless the right choice of radiation modality parameters, which take account of tumor localization and clinical course pattern, is made.
