RESUMEN

According to the literature short dental implants have often been used for the replacement of teeth in the daily praxis. The implantation of short dental implants raise a lot of questions. The autors's aim is to collect the most important experiences of current literature on the mentioned theme to oral implantologists. In the article 33 reviews and clinical studies have been overviewed. The analysis of the different studies suggest that the use of short implants--taking into account of indications and contraindications--decreases the incidence of complications and the patient's discomfort as well as the cost of treatment. The use of short dental implants is proposed as an alternative method in the daily dental praxis.

RESUMEN

Several scientific studies in the international literature discuss the practical application of preventive measures in implantation resulting in long-term success. These complex preventive principles are referred to as Tissue Care Concept (TCC). The authors' summarize the characteristics and prerequisites of TCC. Following several years of research, DenTi System Ltd. introduced a new dental implant, the Denti Bone Level (BL), in accordance with TTC requirements in 2008. In the last 3 years (between 2008 and 2011), the authors inserted 345 BL implants in 202 patients in either one or two sittings. Immediate, delayed, early or late implantations were performed following extraction. Fixation of crowns supported by implants resulted in immediate, early or delayed loading. The success rate of the implants was > 98% in the 3-year period.

RESUMEN

Infective endocarditis (IE) is a disease with serious, even fatal complications, often requiring long-term and expensive treatment. Therefore, prophylaxis has emerging importance. Previous guidelines suggested the use of prophylactic treatment for a wide range of patients and procedures. The Working Group of the European Society of Cardiology accepted a new guideline on the prevention, diagnosis, and treatment of infective endocarditis in 2009. One of its major point is a radical decrease in the type of procedures requiring prophylaxis. These changes also affect dental and oral surgical procedures. It is important for dentists and oral surgeons to come to know the changes and to apply them in their everyday practice.

Examination of the oral cavity in 2260 pregnant women revealed 12 cases of pregnancy epulis, an incidence of 0.48%. In 9 of these 12 cases, spontaneous regression and recovery occurred 1-4 months after delivery. Despite the continuous conservative therapy, several surgical interventions were necessary in the remaining 3 pregnant women, due to the large size of the epulis and the bleeding that was difficult to stop. The authors provide a detailed description of the clinical symptoms (bleeding, difficulty of oral closure, loosening and migration of teeth) caused by large, multiple epulis instances during two pregnancies of one woman, together with the surgical interventions, the histopathologic examination and the immunohistochemical characteristics of the epulis.

RESUMEN

Actinomycosis is a chronic, specific inflammation which is characterized by suppuration, abscess formation, tissue fibrosis and granuloma formation. Actinomycosis has three main forms (cervicofacial, which is the most frequent, approximately 60%, pulmonary and abdominal), but other regions of the body can be involved, too (e.g. neck, ovaries, bones), that is why its differential diagnosis becomes more and more relevant. Regarding its treatment, the majority of authors recommends the combination of surgical and antibiotic treatment. The authors of this article present a typical case of cervicofacial actinomycosis, in which the authors used the combination of surgical and antibiotic treatment. As a result of the treatment the healing process was completed successfully and without complications.

On the basis of the information gained from the literature the amoxicillin/clavulanic acid combination has attained the leading position in dentistry and outpatient oral surgery practices as regards the order of application of antibiotics. The explanation for this may be that in 60-70% of the cases of odontogenic inflammation, besides the pathogenic anaerobic bacteria there is an increasing necessity to eliminate beta-lactamase-producing bacteria as well. The authors applied the amoxicillin/clavulanic acid combination (Aktil Duo 625 mg) to support the--primarily surgical--treatment of 14 oral and dental surgery conditions on a total of 60 patients. After analysing the results and side effects they came to the conclusion that the application of the mentioned combination was very successful in dentistry and outpatient oral surgery practices.

RESUMEN

Crohn's disease is a systemic chronic inflammatory disorder, which may involve any part of the gastrointestinal tract, including the oral cavity. Bowel symptoms are the predominant manifestations, however during the course of the disease a lot of extra-intestinal complications may occur. The prevalence rates of oral manifestations varied between 5 to 20 per cent, but in pediatric patients the prevalence is much higher: 48 to 80 per cent. The review gives an overview of the oral findings observed in patients with Crohn's disease, and the potential implications of the disease for dental management are discussed.

RESUMEN

A rare case of an extremely large, unilateral incisive canal cyst (nasopalatine duct cyst), located between the teeth #11 and #25 is presented. The upper border of the cyst extended to the nasal cavity, the posterior border was the maxillary sinus while the lower border was 5 mm above the maxillary alveolar process. The apices of the vital teeth 21, 22, 23 were in the cyst. Cystectomy was combined with the implantation of an osteoconductive bone substitute material (beta-tricalcium-phosphate). The origin of the cyst was verified by histological examination. The authors also review the origin and pathology as well as the clinical and radiological symptoms and therapy of the non-odontogenic cysts.
