BACKGROUND: The aim of this systematic review was two-fold: (i) to evaluate the long-term (≥5 years) stability of the gingival margin position, keratinized tissue width (KTW) and gingival thickness (GT) in sites that underwent root coverage (RC) or gingival augmentation (GA); and (ii) to assess the influence of different local variables on the long-term stability of dental and gingival tissues. MATERIALS AND METHODS: Randomized controlled trials (RCTs) and non-RCTs reporting short-term (i.e., 6-12 months after baseline surgical intervention) and long-term (≥5 years) follow-up data after surgical treatment of adult patients presenting single or multiple mucogingival deformities, defined as sites presenting gingival recession defects (GRDs) and/or (KTW) deficiency (i.e., <2 mm), were considered eligible for inclusion. MEDLINE-PubMed, EMBASE and Cochrane Central Register of Controlled Trials databases were searched for articles published up to 15 May 2023. Mixed-effects multiple linear regression was used to assess the association between KTW, type of surgical procedure and time (i.e., independent variables) on the stability of the gingival margin in sites that received RC or GA therapy. RESULTS: Of the 2569 potentially eligible records, 41 (reporting 40 studies) met the eligibility criteria. Graphical estimates including data from all RC procedures found an upward trend in recession depth (RD) increase over time. Conversely, it was observed that in 63.63% of RC studies and in 59.32% of RC treatment arms KTW increased over time, particularly in sites treated with subepithelial connective tissue grafts (SCTGs). Conversely, sites that underwent GA procedures generally exhibited an overall reduction of KTW over time. However, sites treated with free gingival grafts (FGGs) showed a decrease in RD after 10 years of follow-up. Three main findings derived from the pooled estimates were identified: (i) Gingival margin stability was associated with the amount of KTW present during short-term assessment (i.e. the greater the KTW at 6-12 months after treatment, the more stable the gingival margin). (ii) The use of autogenous soft-tissue grafts was associated with lower RD increase over time. (iii) Treatment approaches that contribute to the three-dimensional enhancement of the gingival phenotype, as clearly demonstrated by FGG, were associated with gingival margin stability. CONCLUSIONS: The extent of apical migration of the gingival margin appears to be directly related to the amount of KTW and GT upon tissue maturation. Interventions involving the use of autogenous grafts, either SCTG or FGG, are associated with greater short-term KTW gain and lower RD increase over time.

RESUMEN El síndrome de Edwards o síndrome de trisomía 18 es un trastorno cromosómico autosómico que se caracteriza por la presencia de un cromosoma 18 extra, con rasgos clínicos distintivos. Se presenta un caso con diagnóstico prenatal basados en la ecografía morfológica obstétrica que condujeron a la realización ecocardiografía fetal donde resalta la displasia de la válvula pulmonar con doble lesión y la comunicación interventricular. Posteriormente se realiza cordocentesis para realización del cariotipo y asesoramiento genético.


ABSTRACT Edwards' syndrome, or trisomy 18 syndrome, is an autosomal chromosomal disorder characterized by the presence of an extra chromosome 18, with distinctive clinical features. We present a case with a prenatal diagnosis based on obstetrical morphological ultrasound that led to the performance of a fetal echocardiography with findings of a pulmonary valve dysplasia with double injury and interventricular communication. Subsequently, cordocentesis is performed for karyotyping and genetic counseling.

Two Bemisia tabaci (Gennadius) species, Middle East-Asia Minor 1 (MEAM1) and Mediterranean (MED), are major pests that are dispersed throughout the world. While MEAM1 was introduced in Brazil in the 1990s, MED was reported recently with limited spread. Here, a survey was performed to examine whether MED whiteflies are widely present in the Federal District region, in central Brazil. Whiteflies were collected in various locations in the Federal District and surroundings between 2018 and 2020, including garden centers and small- and large-scale farms. The species were identified using RFLPand sequencing of the mitochondrial cytochrome c oxidase I subunit gene region. Out of 108 whitefly batches, 63.89% were composed exclusively by MEAM1, followed by 16.67% presenting only MED, and another 7.40% containing unidentified whitefly species (NI). Plant varieties serving as hosts for more than one whitefly species were observed in 12.04% of the samples, either by MEAM1/MED, MEAM1/NI, or MED/NI. This study highlights the still limited presence of MED in the Federal District and surroundings, predominantly in garden centers and in the green belt of Brasília, closer to urban areas. In contrast, only MEAM1 was identified in large-scale cultivated areas.

BACKGROUND: Influenza vaccination (IV) and Pneumococcus vaccination (PV) are recommended for patients with cardiometabolic diseases. This study aimed to evaluate the immunization rate of ambulatory cardiometabolic patients during the COVID-19 pandemic in the Americas. METHODS: Electronic surveys were collected from 13 Spanish speaking countries between 15 June and 15 July 2020. RESULTS: 4216 patients were analyzed. Mean age 60 (±15) years and 49% females. Global IV rate was 46.5% and PV 24.6%. Vaccinated patients were older (IV = 63 vs. 58 years; PV = 68 vs. 59, p < 0.01) but without gender difference. Vaccination rates were greater in higher-risk groups (65+, diabetics, heart failure), but not in coronary artery disease patients. In the Southern cone, the rate of IV and PV was approximately double that in the tropical regions of the Americas. In a multivariate model, geographic zone (IV = OR 2.02, PV = OR 2.42, p < 0.001), age (IV = OR 1.023, PV = OR 1.035, p < 0.001), and incomes (IV = OR 1.28, PV = OR 1.58, p < 0.001) were predictors for vaccination. CONCLUSIONS: During the COVID-19 pandemic, ambulatory patients with cardiometabolic diseases from the Americas with no evidence of COVID-19 infection had lower-than-expected rates of IV and PV. Geographic, social, and cultural differences were found, and they should be explored in depth.

BACKGROUND: The COVID-19 pandemic's mental health consequences remain unknown. AIM: To assess the mental health status of ambulatory cardiometabolic patients during COVID-19 pandemic lockdown in Spanish speaking Latin American countries. METHODS: Cardiometabolic patients without COVID-19 evidence in 13 Latin American countries answered a survey between June 15th and July 15th, 2020. The Diagnosis Manual of Mental Disorders fifth edition was used to identify the presence of major depressive symptoms. RESULTS: The sample included 4216 patients, 1590 (37.71%; IC95% 36.24-39.19) were considered suffering major depression. Female gender, consuming ≥5 medications day, physical activity <100 minutes weekly, low fruits and vegetables intake, poor treatment adherence, reduced food consumption were independently associated to the presence of major depressive symptoms. CONCLUSIONS: The CorCOVID Latam Psy study showed that one-third of the Latin American Spanish speaking population is suffering from major depressive symptoms during the COVID-19 outbreak.

Replacement of missing teeth is an essential component of comprehensive dental care for patients suffering of edentulism. A popular option is implant-supported restorations. However, implant surfaces can become colonized with polymicrobial biofilms containing Candida species that may compromise peri-implant health. To prevent this, implant components may be treated with a variety of coatings to create surfaces that either repel the attachment of viable microorganisms or kill microorganisms on contact. These coatings may consist of nanoparticles of pure elements (more commonly silver, copper, and zinc), sanitizing agents and disinfectants (quaternary ammonium ions and chlorhexidine), antibiotics (cefalotin, vancomycin, and gentamicin), or antimicrobial peptides (AMPs). AMPs in bioactive coatings have a number of advantages. They elicit a protective action against pathogens, inhibit the formation of biofilms, are less toxic to host tissues, and do not prompt inflammatory responses. Furthermore, many of these coatings may involve unique delivery systems to direct their antimicrobial capacity against pathogens, but not commensals. Coatings may also contain multiple antimicrobial substances to widen antimicrobial activity across multiple microbial species. Here, we compiled relevant information about a variety of creative approaches used to generate antimicrobial prosthetic surfaces in the oral cavity with the purpose of facilitating implant integration and peri-implant tissue health.

BACKGROUND: Alpha-1-antitrypsin is a protein involved in avoidance of different processes that are seen in diabetic retinopathy pathogenesis. These processes include apoptosis, extracellular matrix remodeling and damage of vessel walls and capillaries. Furthermore, because of its anti-inflammatory effects, alpha-1-antitrypsin has been proposed as a possible therapeutic approach for diabetic retinopathy. Our group tested alpha-1-antitrypsin in a type 1 diabetes mouse model and observed a reduction of inflammation and retinal neurodegeneration. Thus, shedding light on the mechanism of action of alpha-1-antitrypsin at molecular level may explain how it works in the diabetic retinopathy context and show its potential for use in other retinal diseases. METHODS: In this work, we evaluated alpha-1-antitrypsin in an ARPE-19 human cell line exposed to high glucose. We explored the expression of different mediators on signaling pathways related to pro-inflammatory cytokines production, glucose metabolism, epithelial-mesenchymal transition and other proteins involved in the normal function of retinal pigment epithelium by RT-qPCR and Western Blot. RESULTS: We obtained different expression patterns for evaluated mediators altered with high glucose exposure and corrected with the use of alpha-1-antitrypsin. CONCLUSIONS: The expression profile obtained in vitro for the evaluated proteins and mRNA allowed us to explain our previous results obtained on mouse models and to hypothesize how alpha-1-antitrypsin hinder diabetic retinopathy progression on a complex network between different signaling pathways. GENERAL SIGNIFICANCE: This network helps to understand the way alpha-1-antitrypsin works in diabetic retinopathy and its scope of action.

RESUMEN

Abstract This reflection article seeks to define and establish the basic professionalism components required for the best practice of anesthesia. Some examples of unprofessional behavior will be identified, while exposing available tools that can be used for its correction. In consequence, professionalism is proposed as a key component for the development of anesthesiology as a medical specialty and the potential beneficial impact of its inclusion in curricular programs is assessed.


Resumen En el presente artículo de reflexión se busca definir y establecer los componentes fundamentales del profesionalismo requerido para el óptimo ejercicio de la anestesiología. De este modo, se identifican algunos ejemplos de comportamiento no profesional y de las posibles herramientas positivas que se pueden aprovechar. Se propone, entonces, el profesionalismo como eje fundamental para el desarrollo de la anestesiología como especialidad médica y se evalúa el impacto benéfico derivado de su inclusión en los programas curriculares.

PURPOSE: The objective is to analyze the antiangiogenic mechanism of suramab, a pharmaceutical compound of bevacizumab and suramin, in a rabbit model of corneal angiogenesis. MATERIAL AND METHODS: Corneal neovascularization was induced in four groups of six New Zealand White rabbits by applying a filter paper disk soaked in 1 M Na (OH) on the central cornea. Group one was treated after injury with intravenous suramab at a dose equivalent to 3 mg/kg of bevacizumab and 10 mg/kg of suramin. Group two was treated with intravenous bevacizumab (5 mg/kg). Group three was treated with 10 mg/kg of suramin while the control group received no treatment. Digital photographs were taken at days 9, 15, 21, and 35. Neovessel formation was quantified giving a 0-4 score to each quadrant according to the centripetal growth of the longest vessel (neovessel index, NVI). Animals were sacrificed at day 35. Corneas were processed for histology, immunohistochemistry, and Western-blot using primary antibodies against P2X2, basic fibroblast growth factor (bFGF), LYVE-1, PECAM-1, and vascular endothelial growth factor-A (VEGF-A). RESULTS: Suramab significantly reduced neovessel growth (mean NVI: 4.2) compared to bevacizumab (8.4), suramin (7.22), and control animals (12.2) at 35 days post-injury (p < 0.01). A lower protein expression of P2X2, bFGF, LYVE-1, PECAM-1, and VEGF-A was found in the cornea of suramab animals than in the other groups of animals. CONCLUSIONS: Joint downregulation of bFGF, P2X2, bFGF, and LYVE-1 constitutes a mechanism that induces greater and longer inhibition of corneal angiogenesis. Results might be relevant to ophthalmic care. Ocular administration of suramab is currently being investigated.

The association of cerebral ischemic attack with patent foramen ovale has not been extensively studied, and frequently the site of origin of embolism is not detected despite routine studies. We present the case of a young patient with ischemic stroke and permeable oval foramen in the context of May Thurner syndrome. The May Thurner syndrome is an entity scarcely studied in the medical literature and it has also been infrequently related to ischemic vascular cerebral accident, but in patients with permeable oval foramen without evidence of the emboligen source, it is interesting to rule it out as a cause of paradoxical embolism.

El desarrollo general del niño está asociado a la erupción dental. Los estudios epidemiológicos sobre la cronología de la erupción dental aportan información de las características de estas variaciones según la región o lugar donde se realice. La experiencia clínica ha demostrado que la población mexicana presenta patrones de erupción dental que no coinciden con los estándares establecidos. Objetivo:Estimar la prevalencia de variaciones cronológicas de la erupción de los incisivos centrales inferiores permanentes. Material y métodos: Estudio tipo observacional, transversal, descriptivo. Resultados: La muestra se conformó por 424 niños entre 5 y 9 años, siendo 203 (48 por ciento) hombres y 221 (52 por ciento) mujeres. Se observó un ligera ventaja de erupción dental en mujeres en comparación con hemiarcadas, no hubo cambios relevantes y en comparación con la tabla de Hurme se observó retraso en la edad de erupción dental. Conclusiones: El promedio de erupción dental está retrasado en nuestra población en comparación con la estudiada por Hurme. Para obtener resultados más generales y acordes con nuestra población es conveniente seguir realizando estudios más amplios en los que se considere: mayor población, ampliar el rango de edad y tomar en cuenta variables como estatura y peso. También es importante realizar tablas de cronología de erupción dental para niños mexicanos tomando como referencia los estudios realizados en otros países


There is a relation between children’s general development and tooth eruption. The information provided by epidemiological studies on the chronology of tooth eruption detail the characteristics of these variations according to the specific location or region where the study is conducted. Clinical experience has shown that Mexican population tooth eruption patterns do not match established standards. Objective:To estimate the prevalence of variations in the chronology of permanent lower-central incisor eruption. Material and methods: An observational, cross-sectional, descriptive study. results: The sample consisted of 424 children between 5 and 9 years old, 203 (48%) male and 221 (52%) female. The females observed a slightly more advanced eruption. The aim of this study was to evaluate the amount and quality of water in the Magdalena-Eslava river system and to propose alternatives for sustainable water use. The system is the last urban river in the vicinity of Mexico City that supplies surface water to the urban area. Historical flow data were analyzed (1973-2010), along with the physicochemical and bacteriological attributes, documenting the evolution of these variables over the course of five years (2008-2012) in both dry and rainy seasons. The analyses show that the flow regime has been significantly altered. The physicochemical variables show significant differences between the natural area, where the river originates, and the urban area, where the river receives untreated wastewater. Nutrient and conductivity concentrations in the river were equivalent to domestic wastewater. Fecal pollution indicators and various pathogens were present in elevated densities, demonstrating a threat to the population living near the river. Estimates of the value of the water lost as a result of mixing clean and contaminated water are presented. This urban river should be rehabilitated as a sustainability practice, and if possible, these efforts should be replicated in other areas. Because of the public health issues and in view of the population exposure where the river flows through the city, the river should be improved aesthetically and should be treated to allow its ecosystem services to recover. This river represents an iconic case for Mexico City because it connects the natural and urban areas in a socio-ecological system that can potentially provide clean water for human consumption. Contaminated water could be treated and reused for irrigation in one of the green areas of the city. Wastewater treatment plants and the operation of the existing purification plants are urgent priorities that could lead to better, more sustainable water use practices in Mexico City.

Durante los últimos años, los países en vía de desarrollo han experimentado retrasos en su responsabilidad con las necesidades de presupuesto para la educación en general, desde la educación secundaria hasta la educación superior. Esto se ve reflejado en los resultados negativos de pruebas académicas realizadas últimamente a los estudiantes, como lo son las pruebas PISA 2012 (Programme for International Student Assessment). Pese a toda explicación que se dé, es imperioso que la universidad no descuide el fomento de la cultura como uno de sus objetivos fundamentales en la formación integral del individuo. El conocimiento científico es cada vez más vasto y la vanidad más arrolladora. La universidad debe estar al servicio de la sociedad y viceversa. No basta con el análisis crítico y la construcción de nuevo conocimiento; se precisa que el desarrollo de un país cuente con el aporte de investigación y gestión en extensión de la universidad, lo que a su vez le generaría dividendos económicos.


Developing countries have been falling behind regarding their responsibility for educational budget needs regarding secondary to higher education; this has been reflected in the negative results of recent academic tests for students, such as PISA (Programme for International Student Assessment) during 2012. Despite the explanations being advanced for such poor performance, universities must not neglect the promotion of culture as one of their key objectives in training/educating people. Scientific knowledge is becoming more extensive and conceit more overwhelming. Universities must serve society and vice versa; they must offer provision for social extension / further education courses in addition to teaching, research and building knowledge which, in turn, will produce economic dividends for the societies funding them.

Diabetic retinopathy is one of the most important causes of blindness. The underlying mechanisms of this disease include inflammatory changes and remodeling processes of the extracellular-matrix (ECM) leading to pericyte and vascular endothelial cell damage that affects the retinal circulation. In turn, this causes hypoxia leading to release of vascular endothelial growth factor (VEGF) to induce the angiogenesis process. Alpha-1 antitrypsin (AAT) is the most important circulating inhibitor of serine proteases (SERPIN). Its targets include elastase, plasmin, thrombin, trypsin, chymotrypsin, proteinase 3 (PR-3) and plasminogen activator (PAI). AAT modulates the effect of protease-activated receptors (PARs) during inflammatory responses. Plasma levels of AAT can increase 4-fold during acute inflammation then is so-called acute phase protein (APPs). Individuals with low serum levels of AAT could develop disease in lung, liver and pancreas. AAT is involved in extracellular matrix remodeling and inflammation, particularly migration and chemotaxis of neutrophils. It can also suppress nitric oxide (NO) by nitric oxide sintase (NOS) inhibition. AAT binds their targets in an irreversible way resulting in product degradation. The aim of this review is to focus on the points of contact between multiple factors involved in diabetic retinopathy and AAT resembling pleiotropic effects that might be beneficial.

Diabetic retinopathy is one of the most important causes of blindness. The underlying mechanisms of this disease include inflammatory changes and remodeling processes of the extracellular-matrix (ECM) leading to pericyte and vascular endothelial cell damage that affects the retinal circulation. In turn, this causes hypoxia leading to release of vascular endothelial growth factor (VEGF) to induce the angiogenesis process. Alpha-1 antitrypsin (AAT) is the most important circulating inhibitor of serine proteases (SERPIN). Its targets include elastase, plasmin, thrombin, trypsin, chymotrypsin, proteinase 3 (PR-3) and plasminogen activator (PAI). AAT modulates the effect of protease-activated receptors (PARs) during inflammatory responses. Plasma levels of AAT can increase 4-fold during acute inflammation then is so-called acute phase protein (APPs). Individuals with low serum levels of AAT could develop disease in lung, liver and pancreas. AAT is involved in extracellular matrix remodeling and inflammation, particularly migration and chemotaxis of neutrophils. It can also suppress nitric oxide (NO) by nitric oxide sintase (NOS) inhibition. AAT binds their targets in an irreversible way resulting in product degradation. The aim of this review is to focus on the points of contact between multiple factors involved in diabetic retinopathy and AAT resembling pleiotropic effects that might be beneficial.

RESUMEN

La inequidad en salud es un fenómeno social que ha sido motivo de múltiples estudios y debates. La población obstétrica no es ajena a esta problemática. En especial se considera que la analgesia obstétrica no recibe el estándar de cuidado que debiera, derivada en muchas ocasiones por inequidad en el sistema de salud. En este artículo se pretende exponer el tema después de un análisis de la literatura y de la práctica diaria realizado por los autores, así como propuestas para afrontar el fenómeno y disminuir las grandes diferencias sociales que hay en este campo.


Healthcare inequity is a social phenomenon that has been subject of multiple studies and debates. Obstetric patients are not spared from its effects. The general belief is that obstetric analgesia in particular is substandard, usually because of healthcare inequities. This article intends to discuss the issue based on the authors' analysis of the literature and the current clinical practice, in addition to making proposals to cope with inequity and reduce the considerable social gap in this area.

BACKGROUND: The contemporary peak of diabetes seems to be related to obesity, sedentary lifestyle and diet. Diabetic retinopathy is the most leading cause of blindness in adulthood in industrialized countries. Our purpose was to evaluate the effect of a high-fat-diet (HFD) on the retina of diabetic rats. METHODS: Two groups of Wistar rats were injected with streptozotocin (STZ) two days after birth using 45 and 90 mg/kg, respectively. At 8 weeks the group on lower doses started to be fed on a HFD. Animals were sacrificed at 37 weeks of diabetes. A control group was made up of non-diabetic rats. Retinal flat mounts were examined using the trypsin digestion technique. Pericytes counts were compared between diabetic and control rats. Cross retinal sections were analyzed by histological techniques and immunohistochemistry and immunofluorescent technique. Primary antibodies against inflammatory and proangiogenic mediators such as RAGE, GFAP, 5-LO, VEGF and TNF-α were used for immunohistochemistry and Western Blot (WB) analyses. RESULTS: In the two diabetic groups we observed GFAP-positive cells with a morphology and spatial organization similar to those seen in Müller cells. Both diabetic groups had a significantly lower number of pericytes than non-diabetic animals.Increased retinal immunoreactivity of GFAP, RAGE, TNF-α, VEGF and 5-LO was seen in diabetic animals fed on HFD compared to the other groups of animals. WB analysis revealed a higher expression of 5-LO, VEGF, TNF-α and RAGE in the retina of diabetic rats on HFD than in controls and diabetics fed on a normal diet. The percentage of RAGE-stained ganglion cells and ganglion cells was found to be significantly lower in animals on a HFD than in the other animals. CONCLUSIONS: Diabetic animals fed on a HFD showed an increased upregulation of inflammatory and proangiogenic markers. This animal model may be useful to study mechanisms of diabetic retinopathy and therapeutic targets.

Se presenta el caso clínico de un paciente de 41 años de edad con antecedente de cambio valvular aórtico con una prótesis mecánica. Acude por presencia de disnea y palpitaciones a un centro hospitalario, donde le diagnostican fibrilación auricular paroxística y aparente fuga paravalvular severa, es enviado asintomático a un hospital de referencia. Negó síntomas anteriores al evento, se realiza ecocardiograma y estudios complementarios, encontrando aneurisma de raíz de aorta que simula la aurícula derecha.


We present the case report of a male aged 41 with a history of aortic valve replacement with a mechanical prosthesis. He presents dyspnea and palpitations in his local hospital, where they are diagnosed paroxysmal atrial fibrillation and severe paravalvular leak, He comes asymptomatic to our service, and denies symptoms prior to the event. It is performed echocardiography and complementary studies finding an aneurysm of the aortic root, simulating the right atrium.

RESUMEN

Lograr la certificación de la discapacidad en Colombia ha sido un reto que las personas con discapacidad se han impuesto y que el país ha asumido, en el marco de la legislación vigente y de los acuerdos internacionales a los que ha adherido Colombia. Para el efecto, se ha adoptado el modelo de clasificación internacional del funcionamiento, la discapacidad y la salud (CIF), en razón a que este instrumento, validado internacionalmente en diversos estudios, permite incorporar un estándar internacional de evaluación del estado funcional de los individuos. En este ensayo, inicialmente se define el concepto "discapacidad" y se ubica en el contexto histórico que lo precede, hasta llegar a la actual estructura de dominios y categorías de la CIF, instrumento que proporciona un marco común y generalizado, de forma tal que se permita proporcionar una adecuada asignación de servicios y beneficios generales, específicos, y a la vez se evalúa el diferencial existente entre el funcionamiento real y el potencial de las personas con discapacidad.
