RESUMEN

UNLABELLED: The aim of the research is to improve diagnosis and assessment of severity and prognosis with different forms of acute pancreatitis. In 79 patients were studied levels of leukocytes, lymphocytes, leucocyte intoxication index (LII), the content of lactate dehydrogenase (LDG), creatinephosphokinase (CPK), amylase, aspartate aminotransferase (AST, alanine aminotransferase (ALT). RESULTS: The level of leukocytes reflects the severity of the disease, but had no prognostic value. The level of lymphocytes, LII. LDG and lipase reflect the severity, of the disease and have prognostic value. The level of amylase, AST, ALT, CPK not always reflect the severity of the disease, but had prognostic value. CONCLUSION: The most readily available to assess the severity and prognosis in acute pancreatitis are the level of blood lymphocytes and LII. Indicators LDG. CPK, amylase, lipase, AST and ALT also reflect the course and prognosis of the disease.

RESUMEN

The comparative analysis of blood levels of leukocytes, lymphocytes, the leukocytic intoxication index, amylase, lipase, lactatdehydrogenase and creatinphosphokinase, measured in operated patients with the acute pancreatitis, demonstrated the general positive dynamics of the patients condition. The higher blood levels of the substances in died patients demonstrate the important prognostic value of them. The higher levels of amylase, lipase, lactatdehydrogenase and creatinphosphokinase by the end of the clinical treatment together with the normalization of the rest laboratory data may witness the higher risk of the chronisation of the pancreatitis.

RESUMEN

Investigation of white blood cell and lymphocyte concentration and white blood cell intoxication index (WBCII), integral systems APACHE II and Ranson reliably reflected the severity of clinical status in patients with acute pancreatitis and had prognostic value. At the same time white blood cell concentration and indices of APACHE II reflected to lesser extent the severity of clinical status, and white blood cell concentration didn't have any prognostic value. Application of lymphocyte concentration and WBCII can be recommended in urgent surgery for evaluation of clinical status and prognosis at early stages of diagnostics and treatment of acute pancreatitis.

RESUMEN

Investigation of indices of lactate dehydrogenase (LDG) and creatine phosphokinase (CPK) in a group of patients with extensive necrotic alterations in organs of the abdominal cavity and in a group of patients without extensive necroses has shown that in the first group there were higher levels of LDG and CPK. It shows that the LDG and CPK indices may be used as markers of tissue necrosis of the abdominal organs. In addition, in patients who died against the background of growing polyorganic insufficiency the LDG and CPK level by the end of treatment was statistically reliably higher than on admission of the same patients that may be an indicator of growing phenomena of dystrophy and micronecroses in vitals, so the indices of LDG and CPK may show the degree of severity of polyorganic insufficiency and determine its prognosis.

RESUMEN

Plasty of the anterior wall of the inguinal canal during herniotomy is considered the main risk factor for recurrent inguinal hernia. Plasty of the posterior wall of the inguinal canal for oblique inguinal hernia is simultaneously a prophylactic measure to prevent the development of direct inguinal hernia in future. 5 types of relapses are defined depending on the localization, tactical and technical mistakes during herniotomy. Plasty of the posterior wall of the inguinal canal and suturing of uniform tissues without tension are considered as prophylactic measures to prevent hernia relapse.

RESUMEN

In the literature there are reports on rare pancreatic tissue heterotopy that promotes the complicated course of duodenal or gastric ulcerative disease. In this connection, the authors have decided to share their observation of a 41-year-old female patient admitted to a hospital for perforated ulcer and ulcerous hemorrhage. Another ulcerative stenosis and ulcer penetration into the pancreas were found at surgery. Partial gastrectomy was carried out due to failure to suture the perforative opening. A gross biopsy specimen from the pyloric portion of the stomach displayed heterotopic parts in the pancreas with and without excretory ducts.

RESUMEN

Leukocytic index of intoxication (LII), number of leukocytes and lymphocytes in blood were compared with clinical symptoms and histological examination of removed organs in patients with acute pyodestructive diseases of abdominal organs. Correlation between LII and inflammation severity was revealed. It is also demonstrated that high LII with low level of lymphocytes is a negative prognostic sign.

Studies of the leukocytic intoxication index (LII), leukocytes, and lymphocytes in patients with inflammatory, purulent, and pyodestructive diseases of the abdomen, lung, and soft tissues at the height of disease have ascertained that LII progressively increased and the relative count of lymphocytes decreased with the worse health status in all groups of patients, particularly in ineffectively treated patients who subsequently died. However, in these patients, the absolute count of lymphocytes did not always tend to decrease. The persistent high LII concurrent with lymphocytopenia indicated a poor prognosis. Leukocytes had less pronounced changes in this respect.

RESUMEN

Examining the values of hemoglobin, red and white blood cells, differential count, and leukocytic index of intoxication (LII) in patients with tuberculosis, abscesses, pneumonias, and cancer of the lung indicated that in accordance with clinical and X-ray findings, LII and the count of lymphocytes reflected the degree of patients' health status to the greatest extent. These blood values were of the greatest prognostic value. In lung cancer, LII increased only when inflammatory and purulent complications occurred. Erythrocyte sedimentation rates had a greater tendency to increase in nonspecific processes, which may be occasionally of differential diagnostic value.

RESUMEN

While tumors stage III-IV of hollow organs involved lowered lymphocyte count matched by relatively high leukocyte index of intoxication, those parameters remained unchanged in parenchymatous cancer. Changes in lymphocyte count were both rarer and more irregular in all tumor sites. Similar changes occurred in post-operative pyo-destructive complications of hollow organs. This phenomenon may be instrumental in diagnosis of complication of both primary and postoperative pathology and assessment of surgical damage.

RESUMEN

The leukocytic index of intoxication (LII), hemoglobin, leukocytes, and lymphocytes were studied in patients with inflammatory, purulent, and pyodestructive diseases of the abdomen, lung, and soft tissues. It was shown that Lll progressively increased and the levels of lymphocytes decreased as the states deteriorated in all groups of patients, particularly in those who died due to ineffective treatment. This all testifies to the diagnostic and predictive value of these parameters and they may be wider used in the treatment of patients with inflammatory, purulent, and pyodestructive diseases at various sites. This is indicated by the values obtained by the end of treatment of patients.

RESUMEN

One hundred and twenty-five patients with acute and chronic ulnar and prepatellar bursitis were treated. Treatment policy was developed. If the first puncture of bursa's cavity without injection of obliterating drugs is not effective, drainage of bursa with perforated catheter is recommended. This policy permitted to reduce period of treatment and avoid bursectomy.

A formula was suggested to determine the leukocytic index of intoxication (LII), which is a correlation between the level of neutrophils and the content of other cells in the blood leukocytic composition. The above formula was used to estimate the minimal, maximal and mean LII indices in patients, who had no inflammatory or purulent pathologies, i.e. normal indices, which respectively amounted to 1, 3 and 1.9. Such approach to composing a formula is in line with the specific features of vital activity, unlike Ya.Ya. Kalf-Kalif formula, which (when it was composed) it was added a number of coefficients. The application of the offered formula to determine the LII at acute appendicitis showed that the more severe an acute appendicitis was so much the LII increase was more intensive. It is noteworthy, that the high LII indices did not always correlate with an increased quantity of blood leukocytes. The above stated proves that the offered LII formula can be used in practice.
