Mining areas may suffer long-term metal contamination and represent harmful remnants of former mining activities. In the northern Amazon of Ecuador, former mining waste pits are used in Oreochromis niloticus (Nile tilapia) fish farming. Given the high consumption of this species by the local population, we aimed to estimate human consumption risks by determining Cd, Cu, Cr, Pb, and Zn tissue bioaccumulation (liver, gills, and muscle) and genotoxicity (micronucleus essay) in tilapia cultivated in one former mining waste pit (S3) and compare the findings to tilapias reared in two non-mining areas (S1 and S2); 15 fish total. Tissue metal content was not significantly higher in S3 than in non-mining areas. Cu and Cd were higher in the gills of tilapias from S1 compared to the other study sites. Higher Cd and Zn were detected in the liver of tilapias from S1 compared to the other sampling sites. Cu was higher in the liver of fish from S1 and S2, and Cr, in the gills of fish from S1. The highest frequency of nuclear abnormalities was observed in fish from S3, indicating chronic exposure to metals at this sampling site. The consumption of fish reared at the three sampling sites results in a 200-fold higher Pb and Cd ingestion than their maximum tolerable intake thresholds. Calculated estimated weekly intakes (EWI), hazard quotients (THQ), and Carcinogenic Slope Factors (CSFing) denote potential human health risks, indicating the need for continuous monitoring in this area to ensure food safety not only in areas affected by mining, but in general farms in the region.

Gold mining (GM) is a major source of metals and metalloids in rivers, causing severe environmental pollution and increasing the exposure risks to the residents of surrounding areas. Mining in Ecuadorian Amazonia has dramatically increased in recent years, but its impacts on Indigenous local populations that make use of rivers are still unknown. The aim of this study was to assess the risks to adults and children caused by the exposure to metals and metalloids in freshwater ecosystems contaminated with tailings released by GM activities in 11 sites of the upper Napo River basin, Ecuador. We selected a carcinogenic and a noncarcinogenic risk assessment method to estimate the hazard index (HI) and total cancer risk (TCR). The concentration of Ag, Al, As, Cd, Cu, Fe, Mn, Pb, Zn, B, and V in water and sediment samples was considered to assess the risks to human health. The calculated HI was 23-352 times greater than the acceptable limits in all sites for both children and adults. Mn and Fe were the main contributors (75% in water and 99% in sediment) to the total calculated risk based on the HI. The calculated TCR for children and adults exceeded approximately one to three times the permissible threshold in all sites. As and Pb contributed up to 93% of the total calculated risk based on TCR for both children and adults. This study demonstrates that the emission and mobilization of metals and metalloids caused by mining activities increase the risk to human health, to which we recommend further monitoring of freshwater contamination in the area and the implementation of preventive health management measures. The effects of emerging contaminants on environmental health are of high concern, especially those potentially induced by mixtures. We assessed single and composite mixtures of triclosan (T), 17ß-estradiol (E2), sulfamethoxazole (SMX), and nicotine (N) at various concentrations, on neonates of Daphnia magna. When used in single exposure, T and N induced high toxicity (100% immobility, each one), compared to SMX and E2 (2.5% and 10% immobility, respectively). When T, E2, SMX and N were in mixture, T had the highest contribution to the overall toxicity in mixture exposures. The N toxicity lowered when in a fourfold exposure (85% immobility in fourfold exposure). Due to the high toxicity of T and N, both alone and in the mixtures, our results can serve as a warning about the use of these substances and their release in the aquatic ecosystem.

OBJECTIVE: According to DSM-5, catatonia and delirium are mutually exclusive clinical syndromes. The investigators explored the co-occurrence of delirium and catatonia (i.e., catatonic delirium) and the clinical significance of this syndrome with a sample of neurological patients. METHODS: This prospective study with consecutive sampling included patients diagnosed with delirium at the National Institute of Neurology and Neurosurgery of Mexico. DSM-5 criteria for delirium, the Confusion Assessment Method, and the Delirium Rating Scale-Revised-98 were used to select and characterize patients. Catatonia was assessed using the Bush-Francis Catatonia Rating Scale and DSM-5 diagnostic criteria. Logistic regression analysis was performed to identify etiological factors associated with catatonic delirium. RESULTS: A total of 264 patients with delirium were included, 61 (23%) of whom fulfilled the criteria for catatonia and delirium simultaneously. Brain tumors, subarachnoid hemorrhage, acute hydrocephalus, and ischemic stroke were associated with delirium without catatonic signs. Catatonic delirium was observed among patients with encephalitis, epilepsy, brain neoplasms, and brain tuberculosis. After multivariate analysis, the association between catatonic delirium and encephalitis (both viral and anti-N-methyl-d-aspartate receptor [NMDAR]) was confirmed. CONCLUSIONS: Delirium is a common complication of neurological diseases, and it can coexist with catatonia. The recognition of catatonic delirium has clinical significance in terms of etiology, as it was significantly associated with viral and anti-NMDAR encephalitis.

As the number of legal and illegal gold mining sites increases in the Andes-Amazonia region, integrative methods to evaluate the effects of mining pollution on freshwater ecosystems are of paramount importance. Here, we sampled water and sediments in 11 sites potentially affected by gold mining activities in the Napo province (Ecuador). The environmental impacts were evaluated using the following lines of evidence (LOEs): water physicochemical parameters, metal exposure concentrations, macroinvertebrate community response (AAMBI), and toxicity by conducting bioassays with Lactuca sativa and Daphnia magna. Dissolved oxygen and total suspended solids were under (<80%) and above (>130 mg/Ls) quality standards 65% of the sites. Ag, Al, As, Cd, Cu, Fe, Mn, Pb, and Zn in water and V, B, and Cr in sediments were detected above quality standards at sampled sites. Nine out of eleven sites were classified as having bad environmental quality based on the AAMBI. L. sativa seed germination in both water (37% to 70%) and sediment (0% to 65%) indicate significant toxicity. In five sites, neonates of D. magna showed a 25% reduction in survival compared to the control. Our integrated LOEs index ranked sites regarding their environmental degradation. We recommend environmental impact monitoring of the mining expansion at the Andes-Amazonia region using multiple LOEs.

This work is a study on the occurrence of emerging pollutants in the northeast Ecuadorian Amazon. Emerging contaminants (ECs)-caffeine, triclosan, estradiol, acetaminophen, nicotine, and ibuprofen-were quantified by gas chromatography-mass spectrometry in rivers and streams of the Amazon basin near the city of Tena, Ecuador. For that, a total of 16 natural water samples were taken in 8 locations. Sampling sites included areas impacted by discharges from inefficient sewage networks in urban areas, wastes from fish farming and non-functional landfill, a stream with few threats, tap water, and treated sewage. Caffeine was found in the 38% of the samples studied while trimethoprim and acetaminophen had an occurrence of 13%. Caffeine was detected at two sites receiving untreated sewage and one site receiving treated sewage with mean concentrations that ranged between 19 and 31.5 µg L-1. Acetaminophen (50.4 µg L-1) and trimethoprim (2 µg L-1) were only detected in the river receiving treated sewage effluent. This is the first assessment of emerging contaminants in the upper Ecuadorian Amazon basin, and our observations highlight the need for better sewage treatment and water quality monitoring in Amazonian cities.

BACKGROUND: Local classification models were used to establish Quantitative Structure- Activity Relationships (QSARs) of bioactive di-, tri- and tetrapeptides, with their capacity to inhibit Angiotensin Converting Enzyme (ACE). These discrete models can thus predict this activity for other peptides obtained from functional foods. These types of peptides allow some foods to be considered nutraceuticals. METHOD: A database of 313 molecules of di-, tri- and tetrapeptides was investigated and antihypertensive activities of peptides, expressed as log (1/IC50), were separated into two qualitative classes: low activity (inactive) was associated with experimental values under the 66th percentile and active peptides with values above this threshold. Chemicals were divided into a training set, including 70% of the peptides, and a test set for external validation. Genetic algorithms-variable subset selection coupled with the kNN and N3 local classifiers were applied to select the best subset of molecular descriptors from a pool of 953 Dragon descriptors. Both models were validated on the test peptides. RESULTS: The N3 model turned out to be superior to the kNN model when the classification focused on identifying the most active peptides.

Resumen Introducción: La catatonia y el delírium son 2 síndromes diferentes e independientes. La catatonia es un síndrome psicomotor asociado a una variedad de enfermedades de diferentes causas médicas y está caracterizado por ausencia de actividad, inducción de posturas pasivas contra gravedad, la oposición o ausencia de respuesta ante estímulos externos, flexibilidad cérea, estereotipias, manierismos y ecofenómenos, entre otros. El delirium se caracteriza por alteraciones de la conciencia y cognitivas, principalmente atención y orientación, habitualmente de aparición aguda, que tiende a fluctuar durante el día y con evidencia de que la alteración es una consecuencia fisiológica directa de una enfermedad, una intoxicación o la abstinencia de alguna sustancia. A pesar de las diferencias y que las clasificaciones excluyen la posibilidad de que estos síndromes puedan presentarse juntos, varios reportes de casos y estudios en grupos de pacientes han planteado que pueden darse las 2 condiciones conjuntamente. Material y métodos: En el presente estudio se detectó a 16 pacientes hospitalizados en quienes concomitaban ambos síndromes, identificados mediante la escala Delirium rating scale-R (DRS-98) y la escala de Bush y Francis de Catatonia (BFCRS). Resultados: Se siguió el desenlace durante la hospitalización y su condición clínica al egreso. Estos pacientes en su mayoría tenían diagnósticos neurológicos, tuvieron una hospitalización larga, requirieron tratamiento con antipsicóticos y benzodiacepinas y sufrieron frecuentes complicaciones. Conclusiones: Catatonia y delirium son síndromes que pueden presentarse al mismo tiempo, lo que lleva a que los pacientes tengan peor desenlace y mayor riesgo de complicaciones.

This study describes the prevalence, phenomenology, treatment, and outcome of neurological patients and psychiatric patients with catatonia at a tertiary neurological center. Clinical variables included nosological diagnoses and complications. Admission length and days with catatonia were used as outcome measures. Of 2,044 patients who were evaluated prospectively, 68 (3.32%) had catatonia, 42 (61.7%) were neurological patients, 19 (27.9%) were psychiatric patients, and 7 (10.2%) had drug-related diagnoses. Of all patients, the ratio of neurological to psychiatric patients was 3:1. Encephalitis was the most common diagnosis (N=26 [38.2%]), followed by schizophrenia (N=12 [17.6%]). Psychiatric patients exhibited a stuporous type of catatonia (15 [83.3%] versus 14 [33.3%], p>0.001), whereas neurological patients exhibited a mixed form of catatonia (25 [59.5%] versus 1 [5.6], p<0.001). Neurological patients had more complications, longer hospitalizations, and more days with catatonia. A total of 56 patients (82.3%) received lorazepam, and 14 patients (20.5%) underwent ECT. Second- and third-line treatments included amantadine, bromocriptine, and levodopa. Catatonia is a prevalent syndrome that can remit with proper and opportune treatment.

Introducción: La psicopatía se caracteriza por un patrón permanente de déficit afectivo y falta de respeto por los derechos de los otros y las normas sociales. La Lista de Chequeo de Psicopatía Revisada (PCL-R, por su sigla en inglés) es un instrumento de medición compuesto por 20 ítems y es uno de los más utilizados para investigación en psicopatía y trastorno de la personalidad antisocial. Objetivo: Validar la PCL-R en población colombiana carcelaria de sexo masculino. Método: Se hicieron dos traducciones al español y dos traducciones en sentido inverso de cada uno de los ítems. También, una traducción al español y en sentido inverso del instructivo. La validez de apariencia fue evaluada por un grupo de expertos. Se examinó la validez de contenido, consistencia interna, reproducibilidad prueba-reprueba e interevaluador. Resultados: En el análisis factorial para la validez de contenido se encontraron 4 dominios que explican el 61,1% de la varianza. La consistencia interna de la escala fue alta (alfa de Cronbach=0,94), al igual que la reproducibilidad pruebareprueba (CCI=0,83, IC 95%: 0,68-0,91) e interevaluador (CCI=0,92, IC 95% 0,86-0,96). Conclusiones: La versión en español de la PCL-R para la población carcelaria colombiana de sexo masculino muestra buenas propiedades psicométricas.


Introduction: Psychopathy is characterized by a pattern of constant affective deficit and lack of respect for other people's rights and social norms. The Psychopathy Check-List Revised (PCL-R) is an instrument of measurement composed of 20 items and it is one of the most used in psychopathy and antisocial personality disorder research. Objective: To validate the PCL-R in Colombian male inmates. Methods: Two translations into Spanish and two back-translations for each of the items were done. The instruction manual was also translated into Spanish and back-translated. Face validity was assessed by a group of experts. Content validity was examined as well as internal consistency, test-retest and inter-rater reproducibility. Results: In the factor analysis for the content validity, four domains were found which explained 61.1% of the variance. The internal consistency was high (Cronbach's Alpha=0.94). Similarly, the test-retest and inter-rater reproducibility were ICC=0.83, 95% CI: 0.68-0.91 and ICC=0.83, 95% CI: 0.86-0.96, respectively. Conclusions: The Spanish version of the PCL-R for Colombian male jail inmates shows good psychometric properties.
