RESUMEN Introducción: Hoy en día, las intervenciones terapéuticas son más especializadas y complejas. Se ha identificado que el Therapeutic Intervention Scoring System-28 (TISS-28) es un instrumento adecuado para planear las actividades de enfermería. Objetivo: Determinar mediante la aplicación del instrumento TISS-28 el grado de complejidad de las intervenciones realizadas al brindar cuidados de enfermería a pacientes en estado crítico. Métodos: Estudio descriptivo, 270 pacientes (adultos, pediátricos y neonatales) en estado crítico; se recabó información sociodemográfica tanto del personal de enfermería como de los pacientes a través de un instrumento, se aplicó el TISS-28 para identificar el grado de complejidad de las intervenciones que se le brindaron al paciente. El análisis estadístico fue descriptivo para variables sociodemográficas y laborales, para las acciones realizadas por enfermería mediante niveles de grados de complejidad se hizo uso del análisis bivariado y la regresión logística. Resultados: Las intervenciones brindadas por el personal de enfermería en el servicio de medicina interna en el Grado II es de 9.8 veces más que en el resto de los pacientes, grado III aumenta 68 veces cuando el paciente tiene una sobre estancia hospitalaria. Discusión: Los pacientes que se encuentran en los servicios de cuidados intensivos o quirúrgicos requieren mayores cuidados. Conclusión: El TISS-28 permite predecir el estado del paciente crítico y su evolución. Determina el tiempo de atención requerido según la gravedad de este, además facilita la asignación idónea de enfermera-paciente.


ABSTRACT Introduction: Currently, therapeutic interventions have become more complex and specialized, but the Therapeutic Intervention Scoring System-28 (TISS-28) has been identified as an adequate tool in the planning of the corresponding nursing activities. Objective: To estimate through the Therapeutic Intervention Scoring System-28 (TISS-28) the degree of complexity of the necessary nursing interventions to provide care to patients in critical status. Methods: This is a descriptive study on 270 adult, pediatric, and neonatal patients in critical status. The TISS-28, and nursing and patient sociodemographic instruments were used. A descriptive statistical analysis using the sociodemographic and work-related variables was carried out. Bi-variate analysis and logistic regression were calculated in order to analyze the nursing performance by levels of complexity. Results: The interventions provided by the nursing staff in the internal medicine service in Grade II is 9.8 times more than in the rest of the patients, grade III increases by 68 times when the patient has an excess hospital stay. Discussion: Patients in the intensive care or surgical services require more care. Conclusion: The TISS-28 offers an estimate on the evolution and attention time required by patients in critical status as well as the corresponding ideal nurse-patient ratios.


RESUMO Introdução: Atualmente as intervenções terapêuticas são mais especializadas e complexas, identificou-se que o Therapeutic Intervention Scoring System-28 (TISS-28) é um instrumento adequado para planejar as atividades de enfermagem. Objetivo: Determinar mediante a aplicação do instrumento TISS-28, o grau de complexidade das intervenções realizadas na assistência de cuidados de enfermagem a pacientes em estado crítico. Métodos: Estudo descritivo, 270 pacientes (adultos, pediátricos e neonatais) em estado crítico; foi coletada informação sociodemográfica tanto do pessoal de enfermagem quanto dos pacientes através de um instrumento, aplicou-se o TISS-28 para identificar o grau de complexidade das intervenções que foram prestadas ao paciente. A análise estatística foi descritiva para variáveis sociodemográficas e laborais; a análise bivariada e a regressão logística foi utilizada para as ações realizadas por enfermagem mediante níveis de graus de complexidade. Resultados: As intervenções realizadas pela equipe de enfermagem no serviço de clínica médica do Grau II são 9,8 vezes mais do que no restante dos pacientes, o grau III aumenta em 68 vezes quando o paciente tem um tempo de internação excessivo. Discussão: Os pacientes que se encontram nos serviços de cuidados intensivos ou cirúrgicos requerem maiores cuidados. Conclusão: O TISS-28 permite prever do estado do paciente crítico e sua evolução. Determina o tempo de atenção requerido conforme a gravidade deste, aliás facilita a atribuição idónea de enfermeira-paciente.

The determination of sex is an important hallmark in the life cycle of organisms, in which the fate of gonads and then the individual sex are defined. In gonochoristic teleost fish, this process is characterized by a high plasticity, considering that in spite of genotypic sex many environmental factors can cause shifts from one to another molecular pathway, resulting in organisms with mismatching genotypic and phenotypic sexes. Interestingly, in most instances, both female-to-male or male-to-female sex-reversed individuals develop functional gonads with normal gametogenesis and respective progenies with full viability. The study of these mechanisms is being spread to other non-model species or to those inhabiting more extreme environmental conditions. Although water temperature is an important mechanism involved in sex determination, there are other environmental stressors affected by the climate change which are also implicated in stress response-induced masculinization in fish. In this regard, the brain has emerged as the transducer of the environment input that can influence the gonadal fate. Furthermore, the evaluation of other environmental stressors or their synergic effect on sex determination at conditions that simulate the natural environments is growing gradually. Within such scope, the concerns related to climate change impacts rely on the fact that many of biotic and abiotic parameters reported to affect sex ratios are expected to increase concomitantly as a result of increased greenhouse gas emissions and, particularly worrying, many of them are related to male bias in the populations, such as high temperature, hypoxia, and acidity. These environmental changes can also generate epigenetic changes in sex-related genes affecting their expression, with implications on sex differentiation not only of exposed individuals but also in following generations. The co-analysis of multi-stressors with potential inter- and transgenerational effects is essential to allow researchers to perform long-term predictions on climate change impacts in wild populations and for establishing highly accurate monitoring tools and suitable mitigation strategies.

Resumen Introducción: La transición demográfica y epidemiológica mundial, aunado a los avances en la ciencia y tecnología aplicados en mejorar la calidad de vida de la población, han influido en la longevidad; los individuos viven más, pero con un bienestar deficiente debido a las limitaciones y comorbilidades inherentes al envejecimiento, con las que a menudo necesitan el cuidado de alguien. Actualmente, uno de cada cuatro adultos mayores requiere de ayuda para realizar sus actividades de la vida diaria. Objetivo: Evaluar el efecto de una intervención de enfermería por medio de la psicoeducación para facilitar el proceso de afrontamiento y adaptación al rol de cuidador familiar del adulto mayor. Material y Métodos: Estudio cuantitativo, cuasi experimental, realizado en una unidad de medicina familiar, con un grupo de 70 cuidadores familiares de adultos mayores, los criterios de inclusión: adultos de 20 a 59 años que supieran leer y escribir. La intervención psicoeducativa consistió en 10 sesiones, los instrumentos aplicados pre y post intervención fueron la escala sobre el proceso de afrontamiento y adaptación, así como la que evalúa habilidad de cuidado. Se utilizó estadística descriptiva e inferencial. Resultados: La edad de los cuidadores familiares tuvo una media de 50.6, femeninos 92.8%. La intervención fue efectiva, pues se obtuvieron diferencias estadísticamente significativas entre el pre y post en afrontamiento y adaptación (X2 Mc nemar p=0.00001), así como para la habilidad de cuidado (X2 p=0.01), lo que sugiere un efecto positivo y sostenido de la intervención. Conclusiones: Los cuidadores familiares de los adultos mayores que participaron en la intervención, mostraron un efecto positivo en la adaptación a su rol de cuidador familiar.


Abstract Introduction: The present demographic transition, influenced by science and technology advances aimed at improving the quality of life, features a greater longevity; however, older adults sometimes suffer from limitations and comorbidities which are related to their aging process and which make them require caring assistance. Currently, around 25% of older adults require some kind of aid in order to perform their daily living activities. Objective: To assess the effect of a nursing intervention using psychoeducation to support the older adult family caregiver adaptation and coping role. Methods and materials: This is a quantitative quasi-experimental study carried out in a unit of family medicine with 70 family caregivers of older adults. The inclusion criteria was: adults aged 20 to 59 who could read and write. The intervention included 10 sessions. The pre and post applied instruments were the Coping Adaptation Processing Scale, and a scale to assess the ability to provide care. Descriptive and inferential statistics were calculated. Results: The mean age of the family caregivers was 50.6 years. 65 were female. The intervention turned out to be effective as statistically significant differences in coping and adaptation measures (X2 Mc Nemar p=0.00001), as well as caregiving abilities (X2 p=0.01) measures, were obtained at the post intervention assessment. Conclusion: The older adult family caregivers who participated in this nursing intervention showed a positive effect in their role.

Atualmente, um de cada quatro idosos requerem de ajuda para realizar suas atividades da vida diária. Objetivo: Avaliar o efeito de uma intervenção de enfermagem através da psicoeducação para facilitar o processo de enfrentamento e adaptação no papel de cuidador familiar do idoso. Material e Métodos: Estudo quantitativo, quase experimental, realizado na unidade de medicina familiar, com um grupo de 70 cuidadores familiares de idosos, os critérios de inclusão: adultos de 20 a 59 anos, que soubessem ler e escrever. A intervenção psicoeducativa consistiu em 10 sessões, os instrumentos aplicados pre e post intervenção foram a escala sobre o processo de enfrentamento e adaptação, assim como a que avalia habilidade de cuidado. Utilizou-se estatística descritiva e inferencial. Resultados: A idade dos cuidadores familiares teve una média de 50.6, femininos 92.8%. A intervenção foi efetiva, pois se obtiveram diferenças estatisticamente significativas entre o pre e post em enfrentamento e adaptação (X2 Mc nemar p=0.00001), assim como, para a habilidade de cuidado (X2 p=0.01), o que sugere um efeito positivo e sustenido da intervenção. Conclusões: Os cuidadores familiares dos idosos que participaram na intervenção mostraram um efeito positivo na adaptação do seu papel de cuidador familiar.

PURPOSE: This study was conducted to identify the prevalence and sociodemographic factors associated with four forms of intimate partner violence (IPV) among HIV-positive women attending the Comprehensive Care Centre at the Kenyatta National Hospital in Nairobi, Kenya. METHODS: A cross-sectional study was conducted among 600 sexually active HIV-positive women aged 18-69 years from May to August of 2012. A structured questionnaire including questions pertaining to sociodemographic characteristics, health care decisions, and forms of IPV (controlling behavior, emotional abuse, physical violence, and sexual violence) was administered to each woman. RESULTS: All women reported experiencing emotional abuse; 20%, 17%, and 15% experienced controlling behavior, physical violence, and sexual violence, respectively. Women who reported low/below average socioeconomic status (SES) had a greater likelihood of experiencing controlling behavior than women with high/average SES (adjusted odds ratio [aOR] =1.62, 95% CI 1.05-2.51). Women who were unemployed had greater odds of experiencing physical violence than those who were employed (aOR =2.35, 95% CI 1.31-4.23). Non-Christian women had higher odds of experiencing controlling behavior, physical violence, and sexual violence than Christian women (aOR =4.41, 95% CI 1.81-10.76 and aOR =3.33, 95% CI 1.43-7.80). CONCLUSION: Based on the prevalence of IPV and the sociodemographic factors identified to be associated with IPV among women in this study it may be beneficial to include IPV screening as part of routine clinic visits for HIV-positive and other women. Furthermore, women who report emotional abuse or controlling behavior from spouse should not be overlooked, as these two forms of IPV may precede or accompany physical and sexual IPV. Women who report experiencing IPV during clinic visits may be referred to organizations and resources available to battered women in Kenya. Increased funding for anti-IPV programs and changes in policy may also contribute to a reduction in IPV among HIV-positive and other women in Kenya.

La mesalazina está aprobada como tratamiento de primera línea para inducción y mantenimiento a largo plazo en enfermedad inflamatoria intestinal de leve a moderadamente activa. Las reacciones adversas pulmonares documentadas en ensayos clínicos se consideran muy raras (< 1/10.000 pacientes), sin embargo, existen numerosos casos publicados que relacionan el fármaco con toxicidad pulmonar. La relación temporal entre el inicio del tratamiento y la aparición de síntomas respiratorios no siempre es evidente, si bien en la mayoría de publicaciones va desde 3 a 15 meses. Realizar un diagnóstico correcto y suspender precozmente el fármaco es fundamental para evitar que la afectación pulmonar subaguda y tratable progrese a crónica con hallazgos de fibrosis irreversibles. Se presenta el caso de un paciente de 26 años con enfermedad de Crohn y psoriasis, que desarrolla neumonitis por hipersensibilidad subaguda de forma secundaria al tratamiento con mesalazina, debutando los primeros síntomas tras 31 meses de tratamiento


Mesalamine is approved as a first line treatment for induction and long term maintenance for patients with mild to moderately active inflammatory bowel disease. Pulmonary adverse reactions documented in clinical trials are considered very rare (< 1/10,000 patients), however, there are numerous published cases that relate the drug to pulmonary toxicity. The temporal relationship between the onset of treatment and the onset of respiratory symptoms is not always evident, although in most publications it goes from 3 to 15 months. Making a correct diagnosis and stopping the drug early is essential to prevent subacute and treatable pulmonary involvement from progressing to chronic with irreversible fibrosis findings. We report the case of a 26-year-old patient with Crohn’s disease and psoriasis, who develops subacute hypersensitivity pneumonitis secondary to treatment with mesalamine, starting the first symptoms after 31 months of treatment

RESUMEN

Recently numerous proteomic approaches have been undertaken to identify sperm and seminal plasma (SP) proteins that can be used as potential biomarkers for sperm function, including fertilization ability. This review aims firstly to briefly introduce the proteomic technologies and workflows that can be successfully applied for sperm and SP proteomic analysis. Secondly, we summarize the current knowledge about boar SP and the sperm proteome, focusing mainly on its relevance to sperm preservation procedures (liquid storage or cryopreservation) and their outcomes in terms of sperm function and fertility.

Concentrations down to 3 nM of the rhS100A4 protein, associated with human tumor development, have been detected in undiluted urine using an integrated sensor based on microring resonators in the emerging Al2O3 photonic platform. The fabricated microrings were designed for operation in the C-band (λ = 1565 nm) and exhibited a high-quality factor in air of 3.2 × 105. The bulk refractive index sensitivity of the devices was ~100 nm/RIU (for TM polarization) with a limit of detection of ~10-6 RIU. A surface functionalization protocol was developed to allow for the selective binding of the monoclonal antibodies designed to capture the target biomarker to the surface of the Al2O3 microrings. The detection of rhS100A4 proteins at clinically relevant concentrations in urine is a big milestone towards the use of biosensors for the screening and early diagnosis of different cancers. Biosensors based on this microring technology can lead to portable, multiplexed and easy-to-use point of care devices.

Adsorption of heavy metals on modified orange biomass has been studied. This biomass was treated with NaOH and CaCl2 to improve its adsorption properties. Kinetic and thermodynamic studies of the adsorption of Cd2+, Ni2+, Cu2+, and Zn2+ were performed at different operating conditions, including competitive adsorption studies with binary metallic mixtures. Results show that this adsorption process was endothermic where an ion exchange mechanism played a relevant role. Adsorbent effectiveness decreased in binary solutions, indicating a strong antagonistic adsorption behavior caused by counter-ions. This antagonistic adsorption was highly dependent on the counter-ion type and its concentration. Multicomponent adsorption of Cu2+ ions was not significantly affected by the presence of other metallic counter-ions, whereas the adsorption of Ni2+ could be totally suppressed by the other ions. The modeling of binary adsorption isotherms was successful using the modified Langmuir equation, which outperformed the Ideal Absorbed Solution Theory-Sips and modified Redlich-Peterson models.

Mixed-lineage leukemia (MLL)-rearranged (MLLr) infant B-cell acute lymphoblastic leukemia (iMLLr-B-ALL) has a dismal prognosis and is associated with a pro-B/mixed phenotype, therapy refractoriness and frequent central nervous system (CNS) disease/relapse. Neuron-glial antigen 2 (NG2) is specifically expressed in MLLr leukemias and is used in leukemia immunophenotyping because of its predictive value for MLLr acute leukemias. NG2 is involved in melanoma metastasis and brain development; however, its role in MLL-mediated leukemogenesis remains elusive. Here we evaluated whether NG2 distinguishes leukemia-initiating/propagating cells (L-ICs) and/or CNS-infiltrating cells (CNS-ICs) in iMLLr-B-ALL. Clinical data from the Interfant cohort of iMLLr-B-ALL demonstrated that high NG2 expression associates with lower event-free survival, higher number of circulating blasts and more frequent CNS disease/relapse. Serial xenotransplantation of primary MLL-AF4+ leukemias indicated that NG2 is a malleable marker that does not enrich for L-IC or CNS-IC in iMLLr-B-All. However, NG2 expression was highly upregulated in blasts infiltrating extramedullar hematopoietic sites and CNS, and specific blockage of NG2 resulted in almost complete loss of engraftment. Indeed, gene expression profiling of primary blasts and primografts revealed a migratory signature of NG2+ blasts. This study provides new insights on the biology of NG2 in iMLLr-B-ALL and suggests NG2 as a potential therapeutic target to reduce the risk of CNS disease/relapse and to provide safer CNS-directed therapies for iMLLr-B-ALL.

Within the tumor, malignant and stromal cells support each other by secreting a wide variety of growth factors and cytokines, allowing tumor growth and disease progression. The identification and regulation of those key factors in this crosstalk has opened the opportunity to develop new therapeutic strategies that not only act on the tumor cells but also on the stroma. Among these factors, S100A7 protein has gained interest in the last years. With key roles in cell motility its expression correlates with increased tumor growth, angiogenesis and metastatic potential. This work aims to deepen in the role played by extracellular S100A7 in the tumor microenvironment, offering a new integrative insight of its mechanism of action on each cellular compartment (tumor, endothelial, immune and fibroblast). As a result, we demonstrate its implication in cell migration and invasion, and its important contribution to the formation of a proinflammatory and proangiogenic environment that favors tumor progression and metastasis. Furthermore, we define its possible role in the pre-metastatic niche formation. Considering the relevance of S100A7 in cancer progression, we have developed neutralizing monoclonal antibodies, reporting for the first time the proof of principle of this promising therapeutic strategy for cancer treatment.

In this work, we present an extension of the constant force approach [P. Orea and G. Odriozola, J. Chem. Phys. 138, 214105 (2013)] to the case of potentials with multiple discontinuities. To illustrate the method, we selected the square well potential of range λ=1.5 that exhibits two discontinuities. Square well single phase properties, vapor-liquid phase diagram, and surface tension were calculated and compared with available simulation data. Besides, we analyzed the internal energies of a square well plus a square shoulder potential having three discontinuities. For both potentials, a good agreement has been found when compared with results of other simulation techniques (discontinuous molecular dynamics and Monte Carlo methods). This extension can be easily implemented to more general and efficient continuous molecular dynamics packages (HOOMD, GROMACS, NAMD, etc.).

Clouded leopards in North American zoological institutions have a high frequency of pheochromocytomas and were identified in 32 of 70 (45%) animals necropsied. Archival sections of adrenal gland from 20 adult clouded leopards with unilateral or bilateral pheochromocytomas collected between 1984 and 2011 were examined by light microscopy and immunohistochemistry, and case demographics were reviewed. Affected leopards were older than 10 years of age (mean, 16 years; range, 11-19 years), and males were overrepresented (12 males, 8 females). Pedigree analysis yielded no evidence for heritability. Five clouded leopards had bilateral neoplasms. Pheochromocytoma was the cause of death due to invasion of the caudal vena cava and fatal hemorrhage in 4 cases. Most pheochromocytomas were well-demarcated, nodular, and expansile masses composed of cords and packets of neoplastic polygonal cells. Five pheochromocytomas had vascular invasion, of which 4 resulted in hemorrhage that was the cause of death. One of the latter pheochromocytomas also had pulmonary metastasis. Ultrastructurally, neoplastic cells had cytoplasmic structures consistent with both norepinephrine- and epinephrine-containing granules. In all cases, neoplasms were immunohistochemically positive for chromogranin A, protein gene product 9.5, and synaptophysin. A subset of neoplasms evaluated by tissue microarray were positive for met-enkephalin and ß-endorphin and negative for melan-A. Histologically, 7 of 20 (35%) clouded leopards with pheochromocytomas had retinal detachment, retinal degeneration, or intramyocardial muscular arteriosclerosis, suggestive of hypertension. Pheochromocytomas can cause mortality and may be a source of clinically significant hypertension in clouded leopards. These neoplasms share similar histologic, immunohistochemical, and ultrastructural characteristics with those of other species.

T regulatory (Treg) cells have a key role in immune homeostasis and the pathogenesis of chronic inflammatory and autoimmune diseases. CD69 is an early leukocyte activation molecule that under steady state conditions is detected in a small proportion of lymphocytes in peripheral blood and lymphoid tissues. Although it has been reported that a subset of CD69(+) T cells behaves as Treg lymphocytes, the possible relationship between CD69(+) Treg cells and CD4(+)NKG2D(+) T lymphocytes, which also exert immunosuppressive activity, has not been explored. In this study, we analyzed the expression of CD69 and NKG2D by T lymphocytes from the peripheral blood of twenty-five healthy subjects by multi-parametric flow cytometry analysis, and their suppressive activity by an assay of inhibition of lymphocyte activation (CD40L expression) and proliferation (carboxyfluorescein partition assay). We found a very small percentage of CD4(+)CD69(+)NKG2D(+) T cells (median 0.002%, Q1-Q3, 0.001-0.004%), which also expressed TGF-ß (Latency Associated Peptide or LAP) and IL-10, in all samples analyzed. These cells exerted an important in vitro suppressive effect on both activation and proliferation of T effector cells. Our data suggest that at very small numbers, CD4(+)CD69(+)NKG2D(+) lymphocytes seem to exert a relevant functional immune-regulatory role in healthy subjects.

The signaling lymphocytic activation molecule SLAMF1 (CD150) is a co-stimulatory molecule that is expressed by most immune cells, including T regulatory (Treg) lymphocytes. Since different abnormalities have been reported regarding the number and function of Foxp3+ Treg cells in patients with systemic lupus erythematosus (SLE), we decided to analyze the expression and function of CD150 in these regulatory lymphocytes in this condition. We isolated peripheral blood mononuclear cells from 20 patients with SLE, and 20 healthy controls. The expression of SLAMF1 was determined by multi-parametric flow cytometry and the suppressive function of CD4+CD25+ lymphocytes, upon engagement or not of CD150 with an agonistic monoclonal antibody, was analyzed by an assay of inhibition of cell proliferation. We observed a significantly increased expression of SLAMF1 by CD3+CD4+ helper T cells and CD19+ B cells in patients with SLE and active disease. However, similar levels of SLAMF1 expression were detected in Foxp3+ Treg cells from patients and controls. In contrast, a higher proportion of SLE patients increased their suppressive function of Treg cells upon CD150 engagement compared to healthy controls. Our data suggest that SLAMF1 is another significant piece in the intricate defective immune-regulatory function of patients with SLE.
