RESUMEN

Concomitance of infiltrative tuberculosis and lung cancer cannot be considered to be rare and sporadic. Thirty-seven patients with concomitance of these two diseases were followed up at the phthisiosurgical department of the Kostroma regional tuberculosis dispensary in 1979 to August 2005. Uni- and bilateral tuberculosis affliction occurred in 14 and 23 patients, respectively. Thirty-four cases developed cancer in the lung that exhibited tuberculous changes. In this group of patients, the central form of cancer prevailed over the peripheral one (33 versus 4 patients). Stages I, II, III, and IV cancer were established in 1, 3, 22, and 11 patients, respectively. Eleven patients were operated on.

RESUMEN

The paper describes a case of diffuse leiomyomatosis of the lung in a 45-year-old female. Leiomyomatosis, a disease noted in females, is considered to include 4 types of multiple smooth muscle lesions: 1) benign metastasizing leiomyoma; 2) lymphangioleiomyomatosis; 3) leiomyomatous peritoneal dissemination; and 4) intravenous leiomyomatosis. The authors present a classification proposed by E. Martin who attempts to pool in it all multiple leiomyomatous lesions observed in females, males, and children.

RESUMEN

AIM: To define diagnostic value of parotid gland (PG) sonography in Sjogren's syndrome (SS) as compared to sialography and morphological changes of labial salivary glands (LSG). MATERIALS AND METHODS: Examination of 50 females with primary (20 patients) and secondary (30 patients) SS consisted of clinical, ultrasonic, x-ray and morphological investigations. RESULTS: Sonographically, PG in SS is characterized by nonhomogeneity of parenchymal picture detected in 75% of patients with primary and 50% of patients with secondary SS. Moderate and severe nonhomogeneity of PG parenchyma was seen in the stage of marked manifestations of chronic parenchymatous parotitis. CONCLUSION: Relationship between PG sonographic image in SS and morphological changes in LSG need further study. PG sonography may help in combined examination of SS patients.

RESUMEN

A study of 35 gastrobioptates sampled from benign epithelial proliferates, adenomas with advanced epithelial dysplasia and adenocarcinomas was conducted using standard original procedures of staining and DNA identification. Also, the method of direct immunoluminescence using fluoroesceinisothiocymate-labeled monoclonal antibodies against human antigens CD4, CD8, CD25 and CD45 was employed. It was found that, among microscopic intraepithelial mononuclear leukocytes, only few bear the immunohistochemical markers of activity. The distribution of intraepithelial mononuclear leukocytes in the stomach of a cancer patient is changed.

RESUMEN

Eight patients with sarcoidosis of the parotid salivary gland and submandibular area were examined. Primary involvement of the intra- or intra- and extraglandular lymph nodes with a probable secondary involvement of the salivary gland parenchyma was detected. Clinical, x-ray, and cytological findings were nonspecific, typical rather of a tumor of chronic sialadenitis. Histological findings were decisive for the diagnosis of sarcoidosis.

RESUMEN

Concentration of von Willebrand's factor antigen (WFAg) was measured by solid phase enzyme immunoassay in the blood serum of 17 female patients (mean age 56.7 +/- 12.1 years) with primary Sjogren's syndrome (PSS) lasting, on the average, 4.1 +/- 1.5 years. Mean WFAg level in patients with PSS was equal to 2.56 +/- 1.4 IU/ml and exceeded significantly that of donors' (1.06 +/- 0.34, p < 0.001). In 9(53%) of 17 patients WFAg concentration was higher than normal (> 2.1 IU/ml). WFAg mean levels and frequency of its elevation did not differ significantly in patients with chronic parotitis, enlarged salivary glands, arthralgia/arthritis, lymphadenopathy, lung lesions, polyneuropathy and patients free of the above symptoms. In patients with myalgia, Raynaud's syndrome, skin vasculitis and vascular diseases WFAg concentrations were higher than in patients without them. There was no significant correlation between WFAg level and ESR, concentration of C-reactive protein, presence of rheumatoid and antinuclear factors.

140 rectal mucosa biopsies were obtained from 86 patients with rheumatoid arthritis (RA), from 54 RA patients prior to or after a year basic therapy with either methotrexate or sulfa drug (sulfasalazine or salazopyridazine). 80% of the examinees exhibited large macrophages, lymphoid follicules, IgM- and IgG-containing cells. The knowledge of the initial morphologic changes in the rectal mucosa from RA patients helps predict efficacy of methotrexate or sulfonic drug treatment. Rectoromanoscopy with biopsy proved important diagnostic tool in RA capable of differentiating the disease variants, prompting valid therapeutic approach with control of the treatment results.

A new model for vascularized and nonvascularized iliac bone autograft transfer to a femoral shaft osteotomy site is presented in this experimental study. As demonstrated on roentgenograms taken at 4 to 6 weeks postoperatively, callus formation between vascularized grafts and recipient sites was 1.5 to 2 times faster than in the nonvascularized graft transfer. According to densitometric analysis, conventional free grafts lost more hydroxyapatite and restored their mineral structure at a slower rate than vascularized grafts. There was no difference between free vascularized and pedicled grafts in the rate of mineral deposition changes, but the pedicle grafts had more significant loss of bone density. Formation and mineralization of periosteal callus between the femoral-shaft fragments took place more rapidly in the first 12 weeks after pedicled bone transplantation. The correlation between the densitometric results and morphologic behavior of transplanted bones was examined. It was demonstrated that all of the grafts at the recipient site underwent remodeling into cortical bone. However, vascularized, and especially pedicled, grafts maintained their cancellous structure relatively longer.

The data obtained upon sialography of the parotid glands (PG) in 28 patients with Sjogren's disease (SD) were compared to clinical SD manifestations, PG regional circulation and histological picture of the labial salivary glands. 24 patients exhibited classical sialography characterized by sialoectasias, enlarged ducts, obscure outlines of the latter. 4 sialograms displayed the duct narrowing, noncontrast parenchyma outlines, unclear images of sialoectasias. Severe xerostomia, marked dryness of the vermilion border, rare enlargement of the salivary glands and parotitis recurrences occurred more frequently in patients with narrow efferent PG ducts. These changes are attributed to vascular sclerosis, devascularization, stromal sclerosis and parenchymal PG atrophy.
