RESUMEN

The growing use of digital technologies (DTs) has a myriad of implications to socio-technical systems, which are not yet fully recognised. This paper investigates the contributions and drawbacks of DTs to resilient performance (RP), an aspect that so far has received less attention in comparison to others such as efficiency. To this end, a survey questionnaire was applied to 79 academics and practitioners linked to resilience engineering. Data analysis involved descriptive statistics and a thematic analysis of the open-text responses to the survey. Mixed impacts were identified, with 10 themes related to contributions and 16 to drawbacks. Regarding the contributions, the results highlighted the use of DTs for monitoring and anticipating system performance. Machine learning seems to be the most promising approach for this purpose. A key drawback is the need for developing new skills across the workforce so that they can make sense of the outputs of DTs and are aware of their strengths and weaknesses. The human role is expected to remain crucial for RP, which makes the current coordination difficulties with DTs even more important to address. A research agenda composed of five topics is proposed, encompassing description, prescription, and assessment. The agenda emphasizes the need for mapping the attributes or functionalities of DTs onto resilience concepts, models, and frameworks.

Building maintenance encompasses multiple tightly inter-connected agents (e.g., technicians, occupants, supervisors, and equipment). Variable working conditions and limited resources may affect the safety and sustainability of the activities. Although recent studies have explored how complex systems can perform resilient behavior in facing the complexity of everyday activities, the factors that effectively contribute to resilient performance are still paired with limited empirical evidence. We studied the performance of the maintenance team during sudden breakdowns of air-conditioning devices in a large university campus, using the Functional Resonance Analysis Method (FRAM). A FRAM diagram containing 30 functions was organized including six macro-cognitive functions (expertise, sensemaking, communication, coordination, collaboration, and adaptation/improvisation), examining their role in anticipating, and responding to emergencies, and eight functional units that are directly impacted by disturbances were analyzed in more detail. Results indicate that macro-cognitive functions can greatly impact the functionality of the maintenance team in pursuit of their goals. Moreover, we noted those macro-cognitive functions here analyzed depend on each other to produce resilient performance.

BACKGROUND: Although slack is an asset to resilient hospitals, it is usually explicitly discussed only in terms of the quantity and quality of beds and staff. This paper expands this view by addressing slack in four infrastructures of intensive care units (ICUs) (physical space, electricity supply, oxygen supply, and air treatment) during the COVID pandemic. METHODS: The study occurred in a leading private hospital in Brazil, aiming at the identification of slack in four units originally designed as ICUs and two units adapted as ICUs. Data collection was based on 12 interviews with healthcare professionals, documents, and comparison between infrastructures and regulatory requirements. RESULTS: Twenty-seven instantiations of slack were identified, with several indications that the adapted ICUs did not provide infrastructure conditions as good as the designed ones. Findings gave rise to five propositions addressing: relationships intra and inter infrastructures; the need for adapted ICUs that match as closely as possible the designed ICUs; the consideration of both clinical and engineering perspectives in design; and the need for the revision of some requirements of the Brazilian regulations. CONCLUSIONS: Results are relevant to both the designers of the infrastructures and to the designers of clinical activities as these must take place in fit-for-purpose workspaces. Top management might also benefit as they are the ultimate responsible for decision-making on whether or not to invest in slack. The pandemic dramatically demonstrated the value of investing in slack resources, creating momentum for this discussion in health services.

Post-operative delirium (POD) is a health hazard condition for the patients and it is associated with increased costs for the healthcare system. Following a system-theoretic approach, firstly a model, then a questionnaire, have been designed to probe the collective awareness about POD throughout the entire patient's perioperative pathway. The 58 reported answers pointed out that most of the information, specifically associated with POD, are routinely recorded but not used to stratify the patients' individual risk to develop POD. The results suggest the need for design a new socio-technical role within modern health care systems: the POD analyst. A Systems-Theoretic Accident Model and Processes (STAMP) model is proposed both to propel the awareness about POD and as a template for future POD risk factors collections.

Although the modelling of interactions has long been at the core of socio-technical systems theory, and is a key for understanding resilience, there is a lack of a holistic taxonomy of interactions. This study introduces a taxonomy of interactions to be used in association with the Functional Resonance Analysis Method (FRAM). The taxonomy has nine criteria: nature of agents, output nature, levelling, waiting time, distance, degree of coupling, visibility, safety and/or security hazards, and parallel replications. For each criterion, two descriptors are proposed: what the interaction looks like; and - when applicable - the variability level of the interaction. The use of the taxonomy is presented for three systems with clearly distinct complexity characteristics: cash withdrawal from an ATM, teaching a university course, and manufacturing operations. These case studies indicate the usefulness of the taxonomy for the identification of leverage points in work system design. They also show the value of modelling the variability of the interactions in FRAM models, in addition to the traditional modelling of the variability of the outputs of functions. Implications of the taxonomy for resilience engineering are discussed.

In a socio-technical work domain, humans, device interfaces and artefacts all affect transformations of information flow. Such transformations, which may involve a change of auditory to visual information & vice versa or alter semantic approximations into spatial proximities from instruments readings, are generally not restricted to solely human cognition. This paper applies a joint cognitive system approach to explore a socio-technical system. A systems ergonomics perspective is achieved by applying a multi-layered division to transformations of information between, and within, human and technical agents. The approach uses the Functional Resonance Analysis Method (FRAM), but abandons the traditional boundary between medium and agent in favour of accepting aircraft systems and artefacts as agents, with their own functional properties and relationships. The joint cognitive system perspective in developing the FRAM model allows an understanding of the effects of task and information propagation, and eventual distributed criticalities, taking advantage of the functional properties of the system, as described in a case study related to the cockpit environment of a DC-9 aircraft. Practitioner Summary: This research presents the application of one systemic method to understand work systems and performance variability in relation to the transformation of information within a flight deck for a specific phase of flight. By using a joint cognitive systems approach both retrospective and prospective investigation of cockpit challenges will be better understood. RATIONALE, AIMS, AND OBJECTIVES: The application of resilience in health care requires the shift from a cause-effect approach to a systemic approach, yet few tools have been developed to measure resilience potential in this specific context. This study tests a resilience assessment grid (RAG) questionnaire to measure the resilience of anaesthesiologists, with a cross-country survey. METHOD: A study was conducted with an analytic hierarchy process (AHP) questionnaire containing 57 detailed questions; 16 nations and 172 respondents were involved in the study. The data were statistically analysed to identify insights from the questionnaire, main improvements for further assessment, and confirmation of the design of the questionnaire. The questionnaire reliability was assessed by Cronbach analysis. Weak items were identified by a detailed correlations analysis and through a weight-polarization matrix. Construct validity was confirmed by principal component analysis (PCA) and factor analysis (FA). RESULTS: The α level of Cronbach analysis is 0.910. PCA and FA confirmed the absence of underlying unexpected factors, with less than 8% from the first factor and a total of just 54% of variability explained by 17 factors. Suggestions for revising the questionnaire ensue from the analysis, with improvements for the questionnaire's significance. CONCLUSION: The questionnaire shows the potential to assess proxy measures of resilience, even confirming the relevance of a structured weighting approach based on the AHP. The exemplar statistical cross-country analyses encourage the widespread use of a centralized resilience questionnaire to support standardized analyses and the diffusion of best practices among organizations.

BACKGROUND: Resilience engineering is a paradigm for safety management that focuses on coping with complexity to achieve success, even considering several conflicting goals. Modern sociotechnical systems have to be resilient to comply with the variability of everyday activities, the tight-coupled and underspecified nature of work, and the nonlinear interactions among agents. At organizational level, resilience can be described as a combination of four cornerstones: monitoring, responding, learning, and anticipating. METHODS: Starting from these four categories, this article aims at defining a semiquantitative analytic framework to measure organizational resilience in complex sociotechnical systems, combining the resilience analysis grid and the analytic hierarchy process. RESULTS: This article presents an approach for defining resilience abilities of an organization, creating a structured domain-dependent framework to define a resilience profile at different levels of abstraction, and identifying weaknesses and strengths of the system and potential actions to increase system's adaptive capacity. An illustrative example in an anesthesia department clarifies the outcomes of the approach. CONCLUSION: The outcome of the resilience analysis grid, i.e., a weighed set of probing questions, can be used in different domains, as a support tool in a wider Safety-II oriented managerial action to bring safety management into the core business of the organization.
