RESUMEN

Although lupus is, by definition, associated with genetic and immunological factors, its molecular mechanisms remain unclear. The up-to-date research findings point out that various genetic and epigenetic factors, especially gene-specific and site-specific methylation, are believed to contribute to the initiation and development of systemic lupus erythematosus (SLE). This review presents and summarizes the association between abnormal DNA methylation of immune-related cells and lupus-like diseases, as well as the possible mechanisms of immune disorder caused by DNA methylation, aiming at a better understanding of the roles of aberrant DNA methylation in the initiation and development of certain forms of lupus and providing a new insight into promising therapeutic regimens in lupus-like diseases.

OBJECTIVE: To assess the prevalence of AIDS-related sexual behaviors and HIV infection status in young men who have sex with men(YMSM)in China. METHODS: Literature retrieval was conducted to obtain the related literature indexed by CNKI, Wangfang and PubMed databases from January 2005 to February 2015. Two reviewers independently screened the literature according to the inclusion and exclusion criteria, extracted data and assessed the methodological quality. Meta-analysis was performed by the R software. RESULTS: Thirty seven studies involving 32 110 YMSM were included. 75.5%(95% CI: 68.8%-81.2%)YMSM sought sexual partners through Internet and 65.5%(95%CI: 60.0%-70.6%)had more than 2 sexual partners. In the last 6 months, 89.1%(95%CI: 84.2%-92.6%)YMSM had anal intercourse and 42.0%(95%CI: 37.4%-46.6%)used condom consistently; 11.4%(95%CI: 8.4%-15.4%)had vaginal intercourse and 36.9%(95%CI: 32.4%-41.6%)used condom consistently. The pooled prevalence of HIV infection was 5.0%(95%CI: 3.9%-6.5%), sub-group analysis showed that the prevalence of HIV infection in the eastern, central and western China were 4.2%(95%CI: 2.6%-6.6%), 1.9%(95%CI: 0.8%-4.6%), 6.3%(95% CI: 4.7%-8.5%), respectively, and there were significant differences among different areas. CONCLUSION: The prevalence of AIDS-related risk sexual behaviors and incidence of HIV infection in YMSM were high in China, it is necessary to conduct AIDS intervention in this population.

The aim of this study was to validate the automatic tracking of facial landmarks in 3D image sequences. 32 subjects (16 males and 16 females) aged 18-35 years were recruited. 23 anthropometric landmarks were marked on the face of each subject with non-permanent ink using a 0.5mm pen. The subjects were asked to perform three facial animations (maximal smile, lip purse and cheek puff) from rest position. Each animation was captured by the 3D imaging system. A single operator manually digitised the landmarks on the 3D facial models and their locations were compared with those of the automatically tracked ones. To investigate the accuracy of manual digitisation, the operator re-digitised the same set of 3D images of 10 subjects (5 male and 5 female) at 1 month interval. The discrepancies in x, y and z coordinates between the 3D position of the manual digitised landmarks and that of the automatic tracked facial landmarks were within 0.17mm. The mean distance between the manually digitised and the automatically tracked landmarks using the tracking software was within 0.55 mm. The automatic tracking of facial landmarks demonstrated satisfactory accuracy which would facilitate the analysis of the dynamic motion during facial animations.

RESUMEN

Flavonoids are a group of polyphenolic compounds, diverse in chemical structure and characteristics, found ubiquitously in plants. Until now, more than 9000 different flavonoid compounds were described in plants, where they play important biological roles by affecting several developmental processes. There has been increasing interest in the research of flavonoids from dietary sources, due to growing evidence of the versatile health benefits of flavonoids including anti-inflammatory, antioxidant, antiproliferative and anticancer activity, freeradical scavenging capacity, antihypertensive effects, coronary heart disease prevention and anti-human immunodeficiency virus functions. This paper reviews the current advances in flavonoids in food with emphasis on mechanism aspects on the basis of the published literature, which may provide some guidance for researchers in further investigations and for industries in developing practical health agents.

The applications of membrane processes in anaerobic biological wastewater treatment still have some limitations due to severe membrane scaling and fouling, although they have been proven to achieve superior COD removal and biomass retention. An innovative anaerobic membrane process for wastewater treatment was conducted to control the membrane scaling problems. The process comprises an anaerobic reactor, an aerobic reactor, and a membrane separation tank. Anaerobic sludge from a full-scale UASB reactor treating food wastewater was inoculated to anaerobic and aerobic reactor to purify synthetic wastewater consisting of glucose and sodium acetate. The anaerobic reactor was operated in a sludge bed type without three-phase separator. The aerobic reactor can eliminate residual organics from the anaerobic reactor effluent using facultative microorganisms. To provide solid-liquid separation, hollow fiber ultrafiltration module was submerged in the separation tank. The results clearly show that the anaerobic membrane process combined methanogenic and aerobic COD reduction is a stable system. No fatal scaling was found after two months of operation even without chemical cleaning for the membrane. It was also found that inorganic precipitates formed in the aerobic reactor were reduced due to CO2 stripping in aerobic reactor. Another important finding was that the inorganic precipitates were entrapped into facultative aerobes floc. The ash/SS ratio of aerobes floc increased from 0.17 to 0.55 after 50 days of operation, which confirms this phenomenon. Based on our investigation, the new process can control scaling effectively to extend the membrane application in anaerobic treatment.

Interstitial lung disease (ILD) in patients with myositis is defined by the presence of interstitial changes on radiographic examination. The reported prevalence of ILD varies from 0% to nearly 50%. However, only rarely has the pathological pattern of diffuse alveolar damage (DAD) associated with idiopathic inflammatory myopathy (IIM) been reported. We report five patients with IIM (one with dermatomyositis, one with polymyositis, and three with amyopathic dermatomyositis) and respiratory failure. Four underwent open lung biopsy with pathological proof of diffuse alveolar damage (DAD). Despite intensive immunosuppressive therapy, all of them died. In addition to the case reports, we discuss DAD in patients with IIM.

STUDY OBJECTIVE: The purpose of this study was to determine whether the administration of sodium bicarbonate to normal individuals would increase their PaCO2 and thereby decrease the ventilatory requirements at a given workload. DESIGN: In this double-blind crossover study, six normal men ingested either 3 mEq/kg NaHCO3 or 1 mEq/kg NaCl once a day for 5 days, in addition to 40 mg of furosemide and 40 mEq KCl. After each 5-day treatment, the subjects underwent a symptom-limited maximal bicycle ergometer exercise test while breathing through external dead space (with a volume of approximately 50% of their FEV1), a second exercise test without any external dead space, and an assessment of their respiratory response to hypercapnia. RESULTS: The administration of the NaHCO3 resulted in a significant increase in the arterial HCO3- from 20.8 to 24.0 mEq/L and a significant increase in the PaCO2 from 31.7 to 36.9 mm Hg at rest that persisted during exercise. During exercise periods with the added dead space, the Borg scores were significantly lower at each workload after the subjects received bicarbonate, but the maximal exercise level did not increase. The mean (+/-SD) slope of the mouth occlusion pressure response to hypercapnia was significantly lower after the administration of NaHCO3 than after NaCl, respectively: 0.73+/-0.41 vs 1.27+/-0.97 cm H2O/mm Hg. CONCLUSION: From this study we conclude that the administration of NaHCO3 results in a significant increase in the PaCO2, decreases the ventilation and the Borg score at equivalent workloads, and decreases the hypercapnic response in normal individuals.

Pleuritis or pleural effusion frequently develops in patients with pneumonia or heart failure. Most of these pleural changes regress without intrapleural intervention. The detailed mechanisms of the regression of the pleural changes in humans are not well documented. We studied the parietal pleura of nine patients with lung cancer and two patients with coronary artery disease by scanning electron microscopy (SEM). All patients had neither radiographic nor gross evidence of pleural disease but all had mixed surface alterations by SEM. Focal denudation of mesothelial cells was common. Deeper injuries exposed thick and thin interweaving collagen bundles. Patchy depositions of amorphous or crystallized fibrin covered normal and damaged pleural surfaces, frequently admixed with macrophages, red blood cells, and tissue debris. Reactive mesothelial cells appeared to proliferate over the fibrin. Our findings suggest that subclinical pleural alterations occur often in patients with pulmonary or cardiac diseases and that an intact pleural surface in those patients is restored mainly by the proliferation of reactive mesothelial cells.

Extramedullary hematopoiesis is a compensatory process for some hematologic diseases, such as spherocytosis, myelofibrosis and thalassemia. We report a 34-year-old man with beta-thalassemia intermedia who developed intrathoracic extramedullary hematopoiesis with recurrent pleural effusion. Diagnosis of extramedullary hematopoiesis of the pleura and soft tissue was established after thoracoscopic biopsy. Subsequently, the pleural effusion was controlled with intrapleural minocycline instillation. To our knowledge, this is the first case of extramedullary hematopoiesis described in the parietal pleura associated with massive pleural effusion and successfully treated with minocycline.

Coronary artery bypass grafting (CABG) is commonly performed with a saphenous vein graft (SVG) and/or an internal mammary artery graft (IMA). We hypothesized that there would be a higher incidence of pleural changes after CABG in patients who underwent IMA grafting because pleurotomy is usually performed. In the present study, the pre and the 6th postoperative day chest roentgenograms of 122 patients who received CABG were reviewed. The incidence of effusion in the patients who received only SVG was 43 percent (23/54) and did not differ significantly (p greater than 0.05) from the incidence in the patients who also had IMA 41 percent (28/68). Almost all of the patients (43/51) had unilateral left-sided pleural effusions. Most of the effusions were small and did not require treatment. The incidence of effusion was not higher in patients with enlargement of their cardiac silhouette or atelectasis and was not related to the presence of chest tubes. The incidence of pleural thickening was higher in the IMA group (49 percent) than in the SVG group (31 percent) but the difference did not achieve statistical significance (p greater than 0.05). We conclude that there is a high (approximately 40 percent) incidence of small effusions and thickening after CABG. The incidence of pleural effusion and pleural thickening do not appear to be influenced by the type of surgery (IMA vs SVG). We speculate that the effusions are due to pericardial inflammation.

This study was undertaken to understand the myopathic manifestations as well as the electrophysiological, pathological, and histochemical changes in muscle tissues of chronic alcoholics. Twenty eight cases of chronic alcoholics with neurological complications and confirmed myopathy were included in this study. Of these, 11 cases (39.3%) belonged to the subclinical forms with either Korsakoff-Wernicke encephalopathy or cerebellar atrophy. The remaining 17 cases (60.7%) were clinical forms with various manifestations, such as hypokalemic periodic paralysis, alcoholic rhabdomyolysis, vacuolar myopathy with phosphorylase deficiency, chronic proximal limb-girdle myopathy and neuro-myopathy. Electromyography (EMG) study showed both myopathic and neurogenic changes. The single fiber EMG revealed increased jitter and blocking rate suggesting reinnervation. In muscle biopsy, the histological and histochemical abnormalities included random atrophic fibers, necrotic fibers, fibers with internal nuclei, phagocytosis and moth-eaten fibers. There was selective atrophy of type 2 muscle fibers, particularly of type 2B fiber. Total or partial myophosphorylase deficiency was observed in some cases (33%). All these findings indicate typical myopathic changes, which might be caused by defects in the energy pathways, especially in the glycolytic processes. In some muscle specimens, there were angular atrophic fibers and fiber type groupings suggesting denervation and reinnervation changes. In conclusion, both myopathic and neurogenic alterations were frequently observed in muscles of chronic alcoholics.

Infection due to Staphylococcus aureus continues to be a source of significant morbidity and mortality. However, its treatment is increasingly complicated by the rising prevalence of resistance to antibiotics. Apart from the two recognized modes of staphylococcal resistance, namely, penicillinase production and intrinsic resistance, Sabath and associates have described a third type in which resistance is manifested by susceptibility to growth inhibition but tolerance to the lethal action of bactericidal agents. The mechanism of tolerance is attributed to a deficiency of autolytic enzyme activity in the part of bacteria, possibly secondary to an inhibition of autolysins in the tolerant staphylococcal strains. These strains are found in patients with infections responding poorly to treatment with cell-wall active antibiotics including vancomycin. Because of its unique mechanism of action and pharmacokinetic properties, rifampin has been reported to be the most active among 65 antistaphylococcal agents tested and have the capacity to kill intraleukocytic staphylococci. We present 2 cases who were cured following the addition of rifampin to previously established regimens. Case 1 was a 40-year-old male who had fever, cough, dyspnea, a right elbow abscess and left leg swelling for 2 weeks prior to admission. Culture of purulent material from the elbow abscess grew staphylococcus aureus. Chest X-ray showed bilateral septic embolism and phleborheography showed partial deep vein occlusion of the left ileofemoral vein. Case 2 was 22-year-old female with fever, chills and cough for 3 weeks. Blood culture grew staphylococcus aureus, and Chest X-ray revealed bilateral septic embolism with pneumonia. Neither of them responded to standard antibiotics which were judged adequate by in vitro sensitivity tests. Clinical cure was later obtained after rifampin was added to the regimens. These results suggest that rifampin may be a useful adjunct in the therapy of staphylococcal infections.

We report three cases of neuro-Behçet's disease, including one woman and two men (age range 52-56 years), who displayed symptoms of high fever, headache and confusion. The EEG showed generalized slowing in two cases, and periodic lateralized epileptiform discharges in one. Brain CT scans from two patients revealed cortical atrophy with multiple low density areas in one case, and focal contrast enhancement in the other. The CSF study revealed pleocytosis in all cases, but only one showed lymphophagocytosis. All of the patients responded dramatically to steroid therapy, although administration of the drug was somewhat delayed in two cases, during which initial management was aimed at the CNS infection of bacterial origin. Recurrent attacks of confusion or mental dullness were found in two patients, and high cortical dysfunction persisted on follow-up for some time. The clinical manifestations, laboratory findings, and neuroradiological features of neuro-Behçet's disease in the reported cases are reviewed.
