Resumen Objetivo: El objetivo de este estudio es demostrar la utilidad de una secuencia tardía post-contraste en la resonancia magnética multiparamétrica de próstata (RMMP) para caracterizar lesiones PI-RADS II. Materiales y métodos: Se analizaron en forma retrospectiva las RMMP realizadas entre enero de 2015 y diciembre de 2016. El protocolo de la RMMP fue basado en las recomendaciones del PI-RADS versión 2, y se agregó una adquisición tardía luego del dinámico post-contraste. Los reportes fueron revisados bajo la versión 2.1. Resultados: Se seleccionaron 31 pacientes que presentaron lesiones categorizadas como PI-RADS II en la zona periférica, los cuales se encontraban en seguimiento del antígeno prostático específico y presentaron confirmación histológica de prostatitis crónica. Se evidenció un realce tardío de la lesión en todos los pacientes. Según los resultados histopatológicos, 30 presentaban prostatitis crónica y el restante tejido benigno (tejido fibromuscular). Discusión: La prostatitis crónica no muestra realce temprano, y presenta realce tardío debido al tejido conectivo fibroso que la compone. En la RMMP, la prostatitis puede imitar el cáncer de próstata. Agregar una adquisición tardía solo adiciona 150 segundos al estudio y podría ayudar a resolver aquellos casos inciertos categorizados como PI-RADS III empleando las secuencias convencionales, debido a que el realce tardío de la lesión es altamente sugestivo de un proceso inflamatorio (PI-RADS II). Conclusión: La presencia de realce tardío es una herramienta útil para realizar un adecuado diagnóstico de una lesión PI-RADS II en la zona periférica, pudiendo evitar una biopsia innecesaria.


Abstract Objective: The aim of this study is to demonstrate the utility of a post-contrast late sequence in multiparametric magnetic resonance imaging (RMMP) to characterize PI-RADS II lesions. Materials and methods: The RMMPs performed between January 2015 and December 2016 were retrospectively analyzed. The RMMP protocol was based on the recommendations of the PI-RADS version 2, and a late acquisition was added, after the dynamic post-contrast. The reports were reviewed under the version 2.1. Results: 31 patients with PI-RADS II lesions in the peripheral zone were selected, who were in prostate specific antigen follow-up and had histological confirmation of chronic prostatitis. A late enhancement of the lesion was evidenced in all patients. According to the histopathological results, 30 had chronic prostatitis and the remaining benign tissue (fibromuscular tissue). Discussion: Chronic prostatitis does not show early enhancement, and presents late enhancement due to its fibrous connective tissue. In RMMP, prostatitis may mimic prostate cancer. Adding a late sequence only adds 150 seconds to the study and could help to resolve those uncertain cases categorized as PI-RADS III using traditional sequences because the late enhancement of the lesion is highly suggestive of an inflammatory process (PI-RADS II). Conclusion: The presence of late enhancement is a useful tool to perform an adequate diagnosis of a PI-RADS II lesion in the peripheral zone, helping to avoid an unnecessary biopsy.

RESUMEN

Resumen La inflamación de la aorta (aortitis) es una patología poco frecuente, con etiología infecciosa (pseudoaneurisma micótico, sífilis) y no infecciosa (arteritis, aortitis idiopática, espondilitis anquilosante, entre otras) de difícil diagnóstico clínico y variable pronóstico. Por esa razón, la utilización de diversos métodos por imágenes, tales como la tomografía computada multidetector (TCMD), la tomografía computada por emisión de positrones (PET-TC), la resonancia magnética (RM) y ultrasonido (US) facilitan la identificación, seguimiento y tratamiento de esa entidad. El siguiente trabajo tiene como objetivo realizar una revisión y actualización bibliográfica acerca de la aortitis y sus diversas etiologías, ejemplificando con casos de nuestra institución.


Abstract Aortic inflammation (aortitis) is a rare pathology, with infectious (fungal pseudoaneurysm, syphilis) and noninfectious etiology (arteritis, idiopathic aortitis, ankylosing spondylitis, among others), it has a difficult clinical diagnosis and a variable prognosis. The use of various imaging methods such as multidetector computed tomography (MDCT), magnetic resonance imaging (MRI), positron emission tomography-computed tomography (PET-CT) and ultrasound (US) facilitate the identification, monitoring and treatment of this entity. The following paper aims to perform a literature review and update about aortitis and its various etiologies, exemplifying cases of our institution.

Resumen Los síndromes de compresión vascular constituyen un grupo de patologías poco frecuentes con prevalencia incierta, no solo por el desconocimiento que se tiene de estos, sino también por su presentación clínica inespecífica. Eso conlleva a que sean infradiagnosticados o detectados en etapas tardías, con secuelas a largo plazo. Su fisiopatología y clínica dependen del tipo de vaso comprometido, el sitio anatómico afectado y la causa subyacente que lo esté produciendo. La anamnesis, el examen físico dirigido y el uso de métodos complementarios como la ultrasonografía Doppler y la angiografía por tomografía computarizada (ATC) o angiografía por resonancia magnética (ARM), son herramientas vitales para su correcto diagnóstico, planificación terapéutica y seguimiento.


Abstract Vascular compression syndromes constitute a group of rare pathologies with uncertain prevalence not only because of their unawareness, but also for its nonspecific clinical presentation. This leads to their underdiagnosis, or detection in late stages with long-term sequelae. Its pathophysiology and presentation depends on the type of vessel involved, the affected anatomical site and the underlying cause producing it. Clinical history, physical examination and use of complementary methods such as Doppler ultrasonography, Computed Tomography Angiography (CTA) or Magnetic Resonance Angiography (MRA), are essential tools for its proper diagnosis, therapeutic planning and follow-up.

La resonancia magnética cardíaca (RMC) es un método no invasivo que provee información acerca de la anatomía, función y caracterización tisular del miocardio, llegando a ser de gran utilidad en el diagnóstico y diferenciación de trastornos infiltrativos como la amiloidosis. La amiloidosis cardíaca (AC) es una miocardiopatía restrictiva resultado del depósito de amiloide en el corazón, que determina una semiología característica en la RM que permite establecer el diagnóstico en la mayoría de los casos. Los hallazgos por RMC incluyen hipertrofia miocárdica del ventrículo izquierdo (HVI) con realce tardío positivo en el ventrículo izquierdo (VI) y el resto de las cámaras cardíacas, asociado a alteración en la cinética del gadolinio con anulación del pool sanguíneo y hallazgos adicionales, como derrame pleural o pericárdico o ambos, que apoyan el diagnóstico1-3. Presentamos el caso de una paciente con diagnóstico de amiloidosis sometida a RMC en donde se demuestran los hallazgos característicos de esta patología. (AU)


Cardiac magnetic resonance imaging (MRI) is a noninvasive method of image that provides information about the anatomy, function and tissue characterization, becoming very useful in the diagnosis and differentiation of infiltrative disorders such as amyloidosis. Cardiac amyloidosis (CA) is a restrictive cardiomyopathy result of amyloid deposition in the heart. MRI findings include myocardial hypertrophy with positive late gadolinium enhancement (LGE) in the left ventricle (LV) associated with the altered kinetics of gadolinium and additional findings as pleural and pericardial effusion that support the diagnosis. We report the case of a patient diagnosed with amyloidosis showing the characteristic MR findings in this pathology. (AU)

UNLABELLED: IgG4-related hypophysitis is a recently described entity belonging to the group of IgG4-related diseases. Many other organs can also be affected, and it is more common in older men. To date, 32 cases of IgG4-related hypophysitis have been reported in the literature, 11 of which included confirmatory tissue biopsy and the majority affecting multiple organs. The aim of this report is to present two cases of biopsy-proven IgG4-related hypophysitis occurring in two young female patients with no evidence of involvement of other organs at the time of diagnosis. LEARNING POINTS: IgG4-related hypophysitis belongs to the group of IgG4-related diseases, and is a fibro-inflammatory condition characterized by dense lymphoplasmacytic infiltrates rich in IgG4-positive plasma cells and storiform fibrosis.It is more common in older men, but young women may also present this type of hypophysitis.Although involvement of other organs is frequent, isolated pituitary disease is possible.Frequent clinical manifestations include anterior hypopituitarism and/or diabetes insipidus.THE DIAGNOSIS MAY BE CONFIRMED WITH ANY OF THE FOLLOWING CRITERIA: a pituitary biopsy with lymphoplasmacytic infiltrates, with more than ten IgG4-positive cells; a sellar mass and/or thickened pituitary stalk and a biopsy-proven involvement of another organ; a sellar mass and/or thickened pituitary stalk and IgG4 serum levels >140 mg/dl and sellar mass reduction and symptom improvement after corticosteroid treatment.Glucocorticoids are recommended as first-line therapy.

BACKGROUND: Myocarditis is a relatively common inflammatory disease that affects the myocardium. Infectious disease accounts for most of the cases either because of a direct viral infection or post-viral immune-mediated reaction. Cardiovascular magnetic resonance (CMR) has become an established non-invasive diagnosis tool for acute myocarditis. A recent large single centre study with patients with biopsy-proven viral myocarditis undergoing CMR scans found a high rate of mortality. The aim of this study was to assess the rate of clinical events in our population of patients with diagnosed myocarditis by CMR scan. METHODS: Patients who consulted to the emergency department with diagnosis of myocarditis by CMR were retrospectively included in the study from January 2008 to May 2012. A CMR protocol was used in all patients, and were followed up to assess the rate of the composite endpoint of all-cause death, congestive heart failure, sudden cardiac death, hospitalization for cardiac cause, recurrent myocarditis or need of radiofrequency ablation or implantable cardiac defibrillator (ICD). A descriptive statistical analysis was performed. RESULTS: Thirty-two patients with myocarditis were included in the study. The mean age was 42.6±21.2 years and 81.2% were male. In a mean follow up of 30.4±17.8 months, the rate of the composite endpoint of all-cause death, congestive heart failure, sudden cardiac death, hospitalization for cardiac cause, recurrent myocarditis or need of radiofrequency ablation or ICD was 15.6% (n=5). Two patients had heart failure (one of them underwent heart transplant), one patient needed ICD because of ventricular tachycardia and two other patients were re-hospitalized, for recurrent chest pain and for recurrent myocarditis respectively. CONCLUSIONS: In our series of acute myocarditis diagnosed by CMR we found a low rate of cardiovascular events without mortality. These findings might oppose data from recently published myocarditis trials.

Takotsubo cardiomyopathy (TC) is a disease that can be misinterpreted as a more serious acute coronary syndrome. Its clinical characteristics resemble those of a myocardial infarct, while its imaging characteristics are critical on correctly characterizing and diagnosing the disease. From angiography, where coronary anatomy is evaluated, to cardiac magnetic resonance (CMR), where morphology and tissue characterization is assessed, the array of imaging options is quite extent. In particular, CMR has achieved great improvements (stronger magnetic fields, better coils, etc.) in the last decade which in turn has made this imaging technology more attractive in the evaluation and diagnosis of TC. With its superior soft tissue resolution and dynamic imaging capabilities, CMR is currently, perhaps, the most useful imaging technique in TC as apical ballooning or medio-basal wall motion abnormalities (WMA), presence of wall edema and late gadolinium enhancement (LGE) characteristics are critical in the diagnosis and characterization of this pathology. In this review, CMRs role in TC will be evaluated in light of the current available evidence in medical literature, while also revising the clinical and physiopathologic characteristics of TC.

Prolactinomas in men are usually macroprolactinomas and other investigators have attributed bigger size of tumors in men to delay in diagnosis. A retrospective study of 71 macroadenomas (42 men) was carried out. Parameters studied were age, signs and symptoms at presentation, time of onset of symptoms, basal prolactin, estradiol, and total testosterone levels, tumor size and Ki 67 expression in tumor tissue. Male patients were older. Visual defects were significantly more prevalent in men. Hardy 4 stage tumors were found only in men. We found no significant correlation between tumor size and the patients age nor between tumor size and the onset of symptoms. Whereas basal E2 levels (21.2+/-12.9 vs. 33.3+/-43.3 pg/ml, p=n.s.) were very similar in male and female patients, testosterone levels were significantly higher in men (0.6+/-0.5 vs. 1.8+/-1.2 ng/ml, p=0.02). The rate of cell proliferation represented by Ki 67 was significantly higher in tumors in men (3.5+/-1.2 vs. 1.5+/-0.5%, p=0.0001). This is the first study focused in macroprolactinomas that shows that they are clinically and biologically more aggressive in men. Hypogonadism in men could appear later in the progression of prolactinomas and this might explain why men were older at the time of diagnosis. Furthermore, testosterone could be a source for E2 in situ aromatization giving male tumors an advantage in cell proliferation.

Propósito: Determinar si la RM puede predecir el compromiso tumoral del margen de resección circunferencial (MRC) en pacientes con cáncer de recto. Material y Métodos: Entre abril del 2005 y marzo del 2008, se evaluaron por resonancia magnética (RM), en forma consecutiva, 70 pacientes (40 M y 30 H, edad promedio de 64 años, rango de 34-78 años), con diagnóstico endoscópico y por biopsia de cáncer rectal inferior o medio. Se realizó una RM sin contraste E.V. en un equipo Siemens Avanto 1.5T, con bobina phase array de superficie. Se efectuaron secuencias con cortes finos ponderadas en T2 (TR¬-TE 4200-88, espesor de 3mm, gap 0, matriz de 256 x 256, FOV de 150x150 mm) en los planos axial, sagital y coronal. Los pacientes recibieron un enema rectal de 150 ml de glicerina previo al examen. No se realizó insuflación colónica ni administración de antiespasmódicos. Se midió la distancia más corta desde el borde del tumor hasta el MRC. Una distancia ≤ 2 mm en el plano axial se consideró como compromiso del MRC. Resultados: El MRC fue ≤ 2 mm tanto por RM como por anatomía patológica en 26 pacientes. En 8 casos, el MRC fue menor por RM que por anatomía patológica. En 32 pacientes el MRC estaba respetado por ambas metodologías diagnósticas y 4 pacientes fueron considerados positivos para compromiso del MRC en histología pero negativos por RM. La sensibilidad, especificidad, valor predictivo positivo y negativo de la RM para el compromiso tumoral del MRC fue de 86%, 80%, 76% y 88% respectivamente. Conclusión: La RM brinda información confiable del compromiso tumoral del MRC en pacientes con cáncer rectal, aportando una ayuda en la evaluación de este factor pronóstico de riesgo en pacientes previo al tratamiento quirúrgico.


Purpose: To determine whether magnetic resonance imaging (MRI) can predict tumor involvement of the circumferential resection margin (CRM) in patients with rectal cancer. Materials and methods: Between april 2005 and march 2008, 70 consecutive patients (mean age 64, range 34-78 years), 40 F and 30 M, with endoscopy and biopsy- proven middle and lower rectal cancer. Non contrast enhanced MRI was performed on a Siemens Avanto 1.5 T. A phased array coil was used and T2 weighted thin section sequences (TR/TE 4200/88, slice thickness 3mm, gap 0, matrix 256 x 256, field of view 150 x 150 mm) were performed in axial, sagittal and coronal orientations. Patients received a 150 ml glycerin enema before examination. No air insufflations or intramuscular antispasmodic was used. The shortest distance from the tumor edge to the circumferential margin was measured. A distance ≤ 2 mm, analyzed in axial slices, was considered as definition of circumferential margin involvement. Results: The CRM was 2 mm in both MRI and histopathological findings in 26 patients. In 8 cases the CRM was shorter on MRI than in histopathological sections. In 32 patients the CRM was respected in both exams and 4 patients were considered positive on histopathological findings but negative in MRI. The sensitivity, specificity, positive and negative predictive values for prediction of tumor involvement of CRM were 86%, 80%, 76% and 88%, respectively. Conclusions: MRI gives reliable information on tumor involvement of the CRM in patients with rectal cancer. This may provide accurate identification of an important prognostic risk factor in patients prior to surgical treatment.

BACKGROUND: There are not many studies that address the selection of patients harboring malignant brain tumors for open surgery. It is necessary, especially in developing countries, to establish the standards because of their impact not only on the efficacy but also on the cost-effectiveness of surgery. With the concern to add information that may help in future studies about the decision making, we proposed to analyze factors associated with surgical complications and evaluate their influence on the functional status at 30 days after surgery. METHODS: A consecutive series of 236 surgeries performed between June 1999 and June 2005 were retrospectively analyzed (168 gliomas, 65 metastases, 3 others). Variables evaluated were age, sex, pre- and postoperative KPS, ASA status, anatomic localization, extent of tumor resection, tumor histology, and number of surgeries. RESULTS: The incidence of complicated craniotomies was 15.68% and mortality was 2.97%. Postoperatively, 92% of the patients improved or maintained the functional status, whereas 8% worsened. In multivariate analysis, only preoperative KPS (P = .009), ASA status (P = .02), and histology type (P = .03) showed significant association with postoperative complications. CONCLUSIONS: We found that the neurologic and clinical preoperative condition and grade III gliomas were factors related to postoperative complications, whereas age, extent of resection, and number of surgeries were not risk factors. We believe that these conclusions provide an additional benchmark for future multicentric studies that focus on the selection criteria for resection of malignant brain tumors.

Ectopic acromegaly represents less than 1% of the reported cases of acromegaly. Although clinical improvement is common after treatment with somatostatin (SMS) analogs, the biochemical response and tumor size of the growth hormone-releasing hormone (GHRH)-producing tumor and its metastases are less predictable. Subject A 36-year-old male was referred because of a 3-year history of acromegaly related symptoms. He had undergone lung surgery in 1987 for a "benign" carcinoid tumor. Endocrine evaluation confirmed acromegaly Plasma IGF-1: 984 ng/ml (63-380), GH: 49.8 ng/ml (<5). MRI showed a large mass in the left cerebellopontine angle and diffuse pituitary hyperplasia. Pulmonary, liver and bone metastases were shown by chest and abdominal CT scans. Ectopic GHRH secretion was suspected. Methods Measurement of circulating GHRH levels by fluorescence immunoassay levels and immunohistochemical study of the primary lung tumor and metastatic tissue with anti-GHRH and anti-somatostatin receptor type 2 (sst2A) antibodies. Results Basal plasma GHRH: 4654 pg/ml (<100). Pathological study of liver and bone biopsy material and lung tissue removed 19 years earlier was consistent with an atypical carcinoid producing GHRH and exhibiting sst2A receptor expression. Treatment with octreotide LAR 20-40 mg q. month resulted in normalization of plasma IGF-1 levels. Circulating GHRH levels decreased dramatically. The size of the left prepontine cistern mass, with SMS receptors shown by a radiolabeled pentetreotide scan, decreased by 80% after 18 months of therapy. Total regression of pituitary enlargement was also observed. No changes were observed in lung and liver metastases. After 24 months of therapy the patient is asymptomatic and living a full and active life.

La resonancia magnética cardíaca (RMC) es una técnica con alta resolución espacial que puede medir con exactitud elgrado de extensión del infarto de miocardio con técnica de realce tardío con gadolinio. La medición de la extensión delinfarto de miocardio es importante para determinar el grado de viabilidad del miocardio. El objetivo de este estudio fuedeterminar si la extensión del infarto de miocardio medido por RMC se asocia al grado de disfunción sistólica delventrículo izquierdo. En 13 pacientes se midió la extensión del infarto de miocardio por RMC determinado el grado detransmuralidad del infarto en un modelo de 17 segmentos. La fracción de eyección fue de 35 ± 12 y el tamaño promediodel infarto de miocardio fue de 34 ± 15 del total de la masa del ventrículo izquierdo. La extensión de infarto secorrelacionó con el grado de disfunción del ventrículo izquierdo (-0,77, p= 0,013). En conclusión, identificamos unacorrelación inversa entre la extensión de infarto de miocardio y el grado de disfunción del ventrículo izquierdo. La RMCes una técnica útil para determinar el grado de viabilidad del miocardio y el grado de disfunción del ventrículo izquierdodespués de un infarto de miocardio.
