Objective: To evaluate the presence of joint effusion and morphology of the articular disc (AD) viewed in the sagittal plane in patients with disc displacement with reduction (DDWR) and to correlate the results with clinical findings.Methods: The sample consisted of 116 patients with DDWR who were evaluated clinically and with magnetic resonance imaging. The AD's morphology was assessed from the sagittal view with the mouth both open and closed. The statistical analysis demonstrated a significance level of 5%.Results: With a mean age of 35 years, 79 patients (68.10%) showed joint effusion, and the female gender was most prevalent (p < 0.05). The results showed a relationship between joint effusion and DDWR in both sides (p < 0.05).Conclusion: Clinically, the present study can infer that DDWR is associated with joint effusion, and females are the most affected. It can be suggested that the pain may be associated with joint effusion.

OBJECTIVE: The aim of this study was to evaluate the correlation among age, gender and the number of temporomandibular disorder (TMD) findings. METHODS: The records from 228 patients with TMD were analyzed for the presence of these findings: morphological changes, disc displacement with reduction (DDWR) and without reduction (DDWOR), bone edema, effusion, and avascular necrosis. Statistical analyses were conducted using multinomial regression with a 5% significance level. RESULTS: DDWR was the most frequent finding. Group 1 was composed of 94 patients (41.22%), Group 2, of 67 patients (29.38%), and Group 3, of 67 patients (29.38%). Men were significantly less likely to belong to Group 3 than women (p = 0.5517). Older patients were slightly more likely to fall in Groups 2 and 3 than in Group 1. DISCUSSION: Women were shown to be more susceptible to developing a higher number of concomitant conditions than men, and the number of findings tended to increase with age.

Objective: To correlate the clinical aspects of symptomatic patients with chronic articular disc displacement with reduction with alterations in the articular disc (AD) morphology and sagittal position. Methods: Records from 109 patients were selected that included data on AD morphology and sagittal position as determined by MRI. According to the MRI results, the sagittal position and AD morphology with opened and closed mouth were correlated with many clinical variables. Results: More than half of the patients studied were female, and the biconcave and hemiconvex morphologies were most common. Thirty-four patients (31.3%) presented with restricted maximum interincisal distance (MID). The biplanar morphology was associated with eccentric bruxism and MID (p < 0.05). Visual analog scale (VAS) scores between 2 and 7 were shown to be risk factors (p < 0.05). Conclusion: The mouth position can influence AD morphology and eccentric bruxism. VAS scores and unknown etiology were risk factors.

RESUMEN

Neurofibromatosis type 1 (NF1) is an autosomal dominant inherited disorder caused by mutations of chromosome 17. The NF1 clinical diagnosis is based on pre-established criteria, including the presence of cutaneous neurofibromas, café au lait spots, and iris (Lisch) nodules. Early detection and a multidisciplinary approach are essential for the prevention of complications, including problems of function, aesthetics, and self-esteem, as well as the occurrence of malignant transformation. This study reports a case of an exuberant NF1 manifestation diagnosed by a dental surgeon, whose investigation of a family led to the diagnosis of NF1, with different expressivities, in 3 generations(AU).

PURPOSE: The aim of this retrospective study was to evaluate the accuracy of two-dimensional (2D) virtual surgical planning (VSP) of pharyngeal airway space (PAS) in patients submitted to bimaxillary orthognathic surgery. METHODS: This study was conducted with lateral cephalograms acquired through cone-beam computed tomography records of 33 patients, divided into group 1-patients submitted to maxillary advancement and mandibular setback (n = 17) and group 2-patients submitted to maxillomandibular advancement (n = 16). Records were taken 1 to 2 months prior to surgery, which was used to perform the 2D VSP (Tp), and 6 to 8 months after surgery (T1). In Dolphin Imaging software, the anteroposterior size of the PAS was calculated at the level of four craniometric points: A, occlusal plane (Mx), B, and pogonion (Pog). Two previously calibrated examiners performed these measurements. Statistical analyses were conducted using Kendall and t tests at a 5% level of significance. RESULTS: There was a concordance between the two examiners at all points and times. In group 1, points A and B have statistically significant differences between the PAS measurements performed in Tp and T1, while in group 2, none of the PAS points showed statistically significant differences when comparing Tp to T1. CONCLUSIONS: 2D computer-based cephalometric prediction in Dolphin Imaging software offers a good orientation to professionals during the surgical procedure of bimaxillary surgeries since its use is considered clinically relevant in daily practice.

A aparência dos dentes e do tecido gengival ao redor dos mesmos tem um papel muito importante na estética da região anterior da maxila tendo em vista que, anormalidades na simetria e no contorno podem afetar significativamente a harmonia de uma dentição natural. O aumento de coroa clínica estético seguido de reanatomização dos dentes vem se tornando uma opção viável para pacientes com discrepâncias gengivais e dentárias. O objetivo do presente trabalho é apresentar o planejamento integrado periodontal e restaurador proposto em um caso clínico. O paciente foi diagnosticado com alterações na linha gengival do sorriso e presença de discrepâncias dentárias. Ele foi submetido à instrução de higiene bucal e rigoroso controle de placa bacteriana. Após o estudo do caso com auxílio de modelos de gesso, optou-se pela realização do aumento de coroa clínica estética e reanatomização dos dentes antero-superiores com resina composta. Este trabalho evidenciou a necessidade de um conhecimento multidisciplinar do cirurgião-dentista para realização de um plano de tratamento adequado, visando maior harmonia e estética do sorriso, buscando a satisfação do paciente para o sucesso final do tratamento.


The appearance of the teeth and the soft tissue around them have an important role in aesthetics of the maxillary anterior region since abnormalities in symmetry and contour can significantly affect the harmony of a natural dentition. The surgical crown lengthening and reconstruction of anterior teeth are becoming a viable option for patients with dento-gingival discrepancies. The aim of the present study is to show the integrated Periodontal and Restorative treatment planning porposed for the patient. The one showed excessive gingival display and tooth discrepancies. First of all the patient got oral hygiene instruction and underwent a rigorous plaque control. After analysing the case with the aid of study models, aesthetics crown lengthening was perfomed and reconstruction of anterior teeth with composite resin restorations were done. This paper has highlighted the need for a multidisciplinary knowledge of the dentist to suggest an appropriate treatment plan, seeking greater harmony and aesthetics of the smile and patients’ satisfaction for the success of treatment.

A estética tem sido considerada um fator determinante no direcionamento do desenvolvimento da implantodontia, envolvendo além da anatomia do dente a ser substituído, a aparência saudável e harmônica do tecido peri-implantar. O enxerto de tecido conjuntivo tem sido empregado com alto índice de sucesso para obtenção de estética, função e saúde dessa mucosa. Objetivo: apresentar um caso clínico que ilustre a técnica de enxerto de tecido conjuntivo associada a retalho reposicionado lateralmente como uma opção viável para regiões peri-implantares esteticamente comprometidas. Materiais e Métodos: paciente do gênero feminino, 51 anos de idade, compareceu a clínica odontológica com queixa de desarmonia entre dentes, implante e gengiva na região superior-anterior. Optou-se por um retalho deslocado lateralmente seguido de enxerto de tecido conjuntivo na região peri-implantar do dente 11 com o objetivo de ganho de tecido em altura e volume gengival. Resultados: em um pós-operatório de 30 dias observou-se completa cobertura da superfície peri-implantar e ganho de tecido queratinizado. Em um controle pós-operatório de 1 ano, a estética e harmonia foram obtidas com a opção de tratamento escolhida. Conclusão: o relato de caso evidenciou a necessidade do reconhecimento pelo Cirurgião-Dentista das alternativas de cirurgias periodontais para suprir esses defeitos gengivais além de ilustrar uma das técnicas viáveis para solucionar este tipo de problema...


Esthetics has been considered one of the dominating factors for the development of the implant therapy, involving not only the anatomy of the tooth to be replaced, but also the healthy and harmonious appearance of the periimplant tissue. Connective tissue grafting has been employed with high success rates to obtain esthetics, function and health of the soft tissue mucosa. Objective: To present a clinical case that illustrates a connective tissue grafting technique associated with a laterally-displaced flap approach as an option for esthetically compromised periimplant regions. Materials and Methods: A Female patient, 51 years, attended our dental office complaining of lack of harmony between teeth, implant and gingiva in the anterosuperior region of the mouth. A laterally-displaced flap approach followed by connective tissue grafting was performed in the periimplant region of tooth 11 in order to increase gingival tissue height and volume. Results: Thirty days post-operatively, the patient demonstrated complete coverage of the periimplant surface and gains in keratinized tissue. In the one-year follow-up visit, esthetics and harmony were obtained with the chosen treatment. Conclusion: This case report demonstrated the need to recognize the different periodontal surgeries alternatives by the dentist in order to deal with gingival defects, and to illustrate one of the possible techniques to solve this type of problem...
