BACKGROUND: In our country, transplantation centers differ in the age limit for allogeneic hematopoietic transplantation (ALOHT). In our program, transplants with age- adjusted conditioning are performed in patients until 70 years old. Currently more than 60% of ALOHT reported to the Center for International Bone Marrow Transplantation Research (CIBMTR) are performed in patients older than 40 years. AIM: To report our experience with ALOHT in acute myelogenous leukemia (AML), analyzing patient age at transplantation in different periods and transplant results in different age groups. MATERIAL AND METHODS: A retrospective analysis of the database of adult hematopoietic transplants in AML patients was performed. Demographic data, disease characteristics, transplant data, survival and relapse times, and mortality were collected. RESULTS: In our program, 1030 transplants were performed in adults and 119 ALOHT were performed in AML patients, between 1990 and 2020. The median age of patients in all periods was 41 years, (range 16-69). The median age was 33 and 45 years, in the periods 1990-2000 and 2000-2020 respectively (p < 0.01). Seventy-eight patients received myeloablative conditioning (median age 44 years) and 41 reduced intensity conditioning (median age 53 years). Five-year overall survival was 44.6% (confidence intervals (CI) 41-48). Non relapse mortality of all periods was 19% (CI 17 - 40%) and relapse rate was 17 % (CI 16-22). No difference in five years overall survival among patients younger than 40, 41 to 50 and over 51 years was observed. CONCLUSIONS: Overall Survival, non-relapse mortality and relapse rate were similar in younger and older patients in our program and similar to those previously reported in other centers.

OBJETIVOS: Analizar el impacto de una intervención educacional sobre la calidad de la dieta mediterránea (DM), la actividad física y el estado ponderal en adolescentes. MÉTODO: Ensayo clínico aleatorizado y controlado con intervención educacional multimodal (grupo control [n=36] y grupo experimental [n=46]). Recogida de datos, al inicio y al final del estudio, en población adolescente de la ciudad de Cáceres. En ambos grupos se determinaron medidas antropométricas y datos sociodemográficos. Se evaluó la calidad de la DM mediante el test KIDMED, el grado de actividad física a través del cuestionario PAQ-A y el estado ponderal con las tablas de crecimiento de la Fundación Faustino Orbegozo Eizaguirre. RESULTADOS: Obtuvimos un aumento significativo en el grupo experimental en el cuestionario PAQ-A que valora la actividad física (p = 0,029). No se observaron diferencias significativas entre grupos en el estado ponderal (p = 0,916). Al comparar la calidad de la DM (calidad alta vs moderada o baja) con la actividad física (pcontrol=0,730; pexperimental=0,495) y con el estado ponderal (pcontrol=0,838; pexperimental=0,372), no se observaron diferencias significativas. CONCLUSIONES: La intervención educacional no mejoró la calidad de la DM ni la actividad física, aunque la mayoría de nuestra muestra presentaba normopeso y actividad física aceptable. Debemos continuar mejorando el patrón alimentario saludable de nuestros adolescentes para garantizar un estado de salud adecuado en el futuro

People with eating disorders show impaired health-related quality of life (HRQoL). We aimed to investigate the relative role of physical and mental factors and stage of change as possible predictors of HRQoL in a group of Spanish women (n = 124) with eating disorders. For this purpose, initial and follow-up data were obtained after 6 months from patients attending an outpatient treatment unit for eating disorders. The determinants of the physical and mental domains of the Medical Outcomes Survey Short-form Health Survey (SF-36) questionnaire were investigated in the total sample and separately based on the eating disorder diagnosis by multiple linear regression. Lower scores in the physical component of the SF-36 questionnaire were associated with the presence of a higher body mass index (BMI) at follow-up as well as a higher score in the "action" component of the Attitudes towards Change in Eating Disorders Questionnaire (ACTA). Conversely, a higher index in the EuroQoL-5D overall quality of life questionnaire (EQ-5D) and the presence of obsessive compulsive disorder were associated with a higher score in the physical dimension. The instrument used demonstrated the ability to assess changes associated with the physical component of these patients over the period studied, and the analysis provided more information and specific data on different aspects of HRQoL, thus allowing a more detailed analysis of the information.

AIM: We aim to investigate the association between the presence of eating disorders and both Internet addiction (IA) and Facebook addiction (FA) in women suffering from eating disorders. METHODS: A total of 124 women completed three instruments: the Internet Addiction Test (IAT), the Bergen Facebook Addiction Scale (BFAS) and a sociodemographic questionnaire. RESULTS: The proportion of FA was 37.9%. The distribution of risk of IA was 21.8%. When the risk of Internet or Facebook addiction was compared with respect to eating disorders, no significant differences were found between groups (P = 0.146 and P = 0.086, respectively). Age and Body Mass Index (BMI) were predictors of BFAS scores; the standardized beta coefficient (ß) for age was -0.463 (P ≤ 0.001), while for BMI it was 3.44; (P = 0.001) being a positive predictor of BFAS scores. For IAT scores, ß age (negatively) = -0.415; (P < 0.001) and ß for weight (positively) 3.657; (P < 0.001) were identified. CONCLUSIONS: The presence of an eating disorder does not seem to be a factor that characterizes the risk of Internet or Facebook addiction in our sample. As information regarding the potential association between Internet and Facebook addiction and the presence of eating disorders is limited, we encourage further studies on this topic.

OBJETIVO: Determinar la calidad de vida relacionada con la salud, la capacidad funcional y el estado de ánimo previo a la fractura de cadera y al mes de la intervención quirúrgica. MÉTODO: Estudio descriptivo prospectivo desarrollado desde enero de 2015 a enero de 2016 en el Hospital Campo Arañuelo (Navalmoral de la Mata, Cáceres). Se recogieron variables sociodemográficas, capacidad funcional (índice de Barthel), dependencia (escala de Lawton y Brody), calidad de vida relacionada con la salud (cuestionario Euroqol-5D) y estado de ánimo (escala de depresión geriátrica de Yesavage) con anterioridad a la fractura de cadera y al mes de la intervención. RESULTADOS: En 96 pacientes mayores de 65años se produjo un descenso significativo de la capacidad funcional: la media fue de 86,66 antes de la fractura frente a 58,72 al mes de la intervención (p < 0,001). La dependencia media fue de 4,95, y al mes, de 2,24 (p < 0,001). Las puntuaciones medias sobre calidad de vida basal y posterior a la intervención en EQ-5D fue de 72,95 vs 59,45, respectivamente (p = 0,004). Aunque antes de la fractura el 94,10% no tenían alteración del estado de ánimo, al mes de la intervención presentaban probable depresión el 20,50% y depresión establecida el 11%. CONCLUSIONES: La fractura de cadera afecta a la funcionalidad de los ancianos y provoca dependencia para las actividades básicas e instrumentales de la vida diaria. También afecta a la calidad de vida, y esta disminuye de forma significativa al mes de la intervención quirúrgica. El estado de ánimo depresivo está inversamente relacionado con la calidad de vida percibida

Abstract Introduction Patients with benign or malignant blood disorders, who require allogeneic stem cell transplantation and lack an identical human leukocyte antigen HLA identicalHL sibling donor, could be transplanted with hematopoietic stem cells from unrelated adult or umbilical cord donors. However, in our country, both approaches are costly and time-consuming options. Methods Over the last few years, haploidentical modalities have been investigated as an alternative donor source, showing similar results to those obtained with identical HLA donors. We started using T-cell-replete haploidentical with post-transplant cyclophosphamide in 2012 and we presented our experience with patients undergoing haploidentical ransplantation compared to SIB. Results Since January 2012 to date, 91 allogeneic transplants have been performed, of which 49 were haploidentical and 42 were HLA identical. The mean age of the patients was 35 years (range: 17-62). The mean CD34/kg × 106 infused per group was 5.93 and 5.89, respectively. Time to granulocyte and platelet engraftment was 11 and 15 days, respectively, for haploidentical, and 12 and 14 days, respectively, for HLA identical (p = 0.10). The 100-day cumulative incidence of global acute GVHD was 34% for haploidentical and 29% for SIHLA identical (p = 0.9). The 2-year overall global graft-versus-host disease was 43% for haploidentical and 41% for HLA identical (p = 0.8). Overall survival, relapse, and transplant and relapse-related mortality were similar between both groups. Conclusion Our experience showed that haploidentical has similar outcomes to those obtained with HLA idential and can be performed in our country safely.

Objetivo: Explorar las vivencias y experiencias sobre las relaciones sexuales en el tercer trimestre de gestación en primíparas. Diseño: Estudio cualitativo fenomenológico. Emplazamiento: Cáceres (Extremadura). Participantes: Mujeres embarazadas primíparas en el tercer trimestre de embarazo. Método: Se realizó un muestreo teórico en embarazadas primíparas. Se incluyeron 15 participantes. Los datos se recopilaron mediante entrevistas en profundidad, grabadas en audio y posteriormente transcritas. El análisis se llevó a cabo mediante la propuesta de Giorgi. Resultados: Los resultados muestran 3 temas principales. Temor a hacer daño, mediado por los antecedentes obstétricos y el deseo de tener el hijo tan esperado. Explorando nuevas rutas: las formas de expresión sexual son modificadas por los cambios físicos, los temores y la movilidad, resaltando la importancia de otras muestras de afecto y amor (besos y las caricias). El tabú del sexo: la falta de información frente a la sexualidad durante el embarazo sigue siendo frecuente. Conclusiones: La mujer en el tercer trimestre de embarazo deja al margen su deseo sexual y el de su pareja y se centra en el bienestar del bebé. Destaca el rol incipiente de madre frente al de pareja. Cuando el embarazo es muy deseado y "costoso", más se reduce la vida sexual. Los profesionales sanitarios debemos aconsejar e informar a las parejas con una actitud abierta y amplia, para mantener la calidad y la salud sexual de la pareja

The Health Professionals must advise and inform the couples with an open-minded attitude

PARTICIPANTS: Primiparous women in the third trimester of their pregnancy. METHODS: We use theoretical sampling, was conducted on pregnant primiparous. The study included 15 participants. The data was collected using in-depth interviews, that were voiced recorded and later transcribed. The analysis was made using Giorgi's proposal. RESULTS: The results show three main points. Fear of doing damage, mediated by the obstetric history and the desire to have the long-awaited child. Exploring new routes: forms of sexual expression are modified by the physical changes, the fears, and the mobility. Consumption of vegetables grown in arsenic (As)-contaminated soils is an important exposure route to the element for humans. The present study is focused on locally-grown, frequently-consumed vegetables, such as carrots (Daucus carota), beets (Beta vulgaris) and quinoa (Chenopodium) from the As-polluted Chiu Chiu area in Northern Chile. The latter region is affected both by As discharge from copper mining activity and natural As contamination, leading to a high As content in local food and water. For the selected vegetables, the following aspects were investigated: i) Their total As, Cu, Pb, Cr, Cd and Mn content; ii) Arsenic speciation in the edible part of the vegetables by liquid chromatography inductively-coupled plasma mass spectrometry (LC-ICPMS) analysis; iii) Arsenic bioaccessibility in the vegetables during in vitro gastrointestinal digestion; iv) Arsenic species present in the extracts obtained from in vitro gastrointestinal digestion; and v) Arsenic dietary exposure estimates for the assessment of the risk posed by the vegetables consumption. A significant degree of As contamination was found in the vegetables under study, their metal content having been compared with that of similar Spanish uncontaminated products. In vitro gastrointestinal digestion of the studied vegetables led to quantitative extraction of As from carrots and beets, whereas efficiency was about 40% for quinoa. For carrots, only As(III) and As(V) species were found, being their concentration levels similar. In the case of quinoa, around 85% of the element was present as As(V). For beets, inorganic As(V) and unknown overlapped As species (probably arsenosugars) were found. No significant transformation of the original As species was observed during in vitro gastrointestinal digestion. Arsenic dietary exposure values obtained for the three vegetables (0.017-0.021µg As person(-1)day(-1)) were much lower than the JFCFA's safety limit of 50µg As person(-1)day(-1). Therefore, no toxicological risk would be expected from the intake of these vegetables.

BACKGROUND/AIMS: Hemorrhagic complications during ECMO may affect a large proportion of the patients depending on the clinical setting. To guarantee optimal delivery of blood products to these patients, blood banks require updated information on the transfusion requirements. Few studies to date provide this information. This work assesses transfusion requirements in neonates and children during ECMO during the past 9 years. METHODS: We reviewed blood bank and hospital records of patients who underwent ECMO at our institution between May 2003 and May 2012. Data obtained included age, weight, diagnosis, type, length of ECMO, and daily transfusion requirements during ECMO. Descriptive and non-parametric inferential statistic analyses were performed. Our series included 98 patients. RESULTS: Mean time of patients on ECMO was 9.2 days, with the longest treatment spanning 22 days. Mean daily transfusion requirements were 39.5 ml/kg of RBC, 12.9 ml/kg of plasma, 34.3 ml/kg of platelets and 1.4 ml/kg of cryoprecipitate. Patients who underwent ECMO due to cardiac disease or congenital diaphragmatic hernia (CDH) required significantly higher transfusion volumes of plasma (p<0.05), platelets (p< 0.05) and cryoprecipitate (p<0.05) when compared to patients underwent ECMO due to respiratory disease. Concomitant with the aging of ECMO circuits, patients showed increased requirements of RBC, plasma, and CRYO around the seventh day of the ECMO run. This effect was not observed for platelets, which remained nearly consistent around 2.2 transfusions/day. CONCLUSIONS: ECMO patients required significant transfusion support, which was particularly higher among patients who underwent ECMO due to cardiac disease or congenital diaphragmatic hernia.

Public cord blood banks are a source of hematopoietic stem cells for patients with hematological diseases who lack a family donor and need allogeneic transplantation. In June 2007 we started a cord blood bank with units donated in three maternity wards in Santiago, Chile. We report the first three transplants done with cord blood units form this bank. Cord blood units were obtained by intrauterine collection at delivery. They were depleted of plasma and red cells and frozen in liquid nitrogen. Tests for total nucleated cells, CD34 cell content, viral serology, bacterial cultures and HLA A, B and DRB1 were done. Six hundred cord blood units were stored by March 2012. Three patients received allogeneic transplant with cord blood from our bank, two with high risk lymphoblastic leukemia and one with severe congenital anemia. They received conditioning regimens according to their disease and usual supportive care for unrelated donor transplantation until full hematopoietic and immune reconstitution was achieved. The three patients had early engraftment of neutrophils and platelets. The child corrected his anemia and the leukemia patients remain in complete remission. The post-transplant course was complicated with Epstein Barr virus, cytomegalovirus and BK virus infection. Two patients are fully functional 24 and 33 months after transplant, the third is still receiving immunosuppression.

Introducción: La población de la Región de Antofagasta se encuentra expuesta a contaminación ambiental por metales pesados (MP). Los elementos traza (ET) podrían actuar con As como agonistas o antagonistas. El presente estudio detalla la determinación de las concentraciones totales (CT) de V, Cr, Mn, Co, Ni, Cu, As, Se y Boro en aurícula derecha (AD), arteria mamaria izquierda (AMI), vena safena (VS) y tejido adiposo (TA) de personas que padecen patologías cardiovasculares (CV) y han requerido cirugía cardíaca. Métodos: Se analizaron AMI, VS, AUD, y TA en un grupo de 50 pacientes con cardiopatía coronaria de la Región de Antofagasta que han sido sometidos a cirugía cardíaca (grupo expuesto, E), y en un grupo de 20 pacientes control sometidos a cirugía cardíaca en el mismo período en Santiago (grupo no expuesto, NE). Las determinaciones de ET y MP se realizaron mediante Técnicas Ópticas y Atómicas. Resultados: En el grupo de pacientes E residentes en la Región de Antofagasta Boro presenta correlaciones interelementales significativas (p=0,05), que no se presentan o son escasas en el grupo NE. El rango de CT de B en AD fue de 24,721 - 932,032 μg/g, niveles muy superiores a los encontrados en el resto de los elementos en estudio. El rango de CT de B encontrada para AD fue de 25,205 - 56,873 μg/g. Conclusión: Individualmente, el elemento más enriquecido en todos los tipos de tejidos del grupo E de la Región de Antofagasta es B, le siguen As y Mn. Cu está empobrecido en el grupo NE, en particular en AD y AMI.


The Antofagasta population is exposed to environmental contamination with heavy metals (HM). Trace elements (TE) might interact with As either as agonists or antagonists. The total concentracions of V, Cr, Mn, Co, Ni, Cu, Se and B in the right atrium (RA), left internal mammary artery (LIMA), saphenous vein (SV) and adipose tissue (AT) from patients undergoing cardiac surgery are reported. Methods: LIMA, SV, RA and AT from 50 patients with myocardial infarction submitted to revascularization surgery in Antofagasta (exposed group, E) or Santiago (non exposed group NE, 20 patients) were analyzed. Optical and atomic techniques were used for analysis of HM and TE. Results: Significant inter elemental correlations were observed for B in group E, (p<0.05), which are not present in group NE. The RA concentrarion of B in group A ranged from 24.72 to 932.03 ug/g, substancially higher than concentracion of the other elements under study. Corresponding concentra-rions of B in the NE group were 25.21 to 56. 87 ug/g. Conclusion: B was the most enriched element in the exposed group, followed by As and Mn. Cu content was significantly lower in RA and LIMA from patients in the non exposed group.

Introducción: Los efectos de la intoxicación con Arsénico (As) como enfermedades cardiovasculares (CV), pigmentaciones y oclusiones arteriales coronarias están asociados con la ingestión de As inorgánico a través del agua de bebida y a exposiciones ambientales. La unión del As (III) a proteínas y la metilación del As podría ser una primera etapa en el mecanismo de detoxificación. Objetivo: Evaluar la unión de As a proteínas en aurícula derecha y vena safena (VS) en sujetos expuestos de la Región de Antofagasta. Métodos: Se estudió la asociación As-proteína en el citosol de AD y VS de 6 pacientes con enfermedad coronaria grave de la Región de Antofagasta. Para el fraccionamiento del citosol se utilizaron columnas de exclusión molecular de tres diferentes rangos de masas. El perfil del As se detectó por Espectrometría de Masas Inductivamente Acoplado (ICP-MS) y por Espectrosco-pía Ultra Violeta - Visible de las fracciones moleculares (enlaces As- tiolatos de proteínas). Resultados: En todos los casos el As estuvo ampliamente distribuido en todo el intervalo de fracciones para AUD y VS. Los porcentajes de As colectado en las fracciones de las diferentes columnas usadas fueron 10, 25 y 50 por ciento. En la especiación de As en el citosol, por Cromatografía Líquida de Alta Resolución acoplada a la Espectrometría de Masas (IC-HPLC-ICP-MS), solamente se encontró As(III) y As(V) con una distribución Gaussiana para ambas especies, siendo la relación As(III)/As(V) constante para AUD y VS. Conclusión: En los tejidos CV existe asociación As - proteína lo cual podría implicar que el As está unido a biocompuestos de diferente peso molecular a través de grupos sulfhidrilos vecinales. Es probable que el As en AUD y VS se una a fracciones proteicas de masa molecular superior a 80 kDA y a subunidades de la estructura cuaternaria de la proteína nativa.


Background: The effects of arsenic (As) toxi-city - cardiovascular disease, pigmentation, coronary artery occlusion- come from ingestion of contaminated drinking water and environmental exposure. Protein linkage or As(III) and As methylation may be a first step in detoxification. The aim of this study is to evaluate protein linkage of As in the right atrium (RA) and saphenous vein (SV) of As exposed subjects from Antofagasta, Chile Method: As-protein linkage was studied in the cytosol of AD and SV obtained from 6 patients operated on for coronary artery disease. Molecular exclusion columns of 3 different mass ranges were used to obtain the cytosol fraction. As species were detected by induction coupled mass spectrometry and visible ultraviolet spectrometry (links of As and protein thyolates). Results: As was widely distributed in AD and SV in all subjects. As collected in the 3 different columns used were 10 per cent, 25 per cent and 50 per cent. Only As(III) and As(V) were obtained through the method used (IC-HPLC-ICP-MS); a normal distribution was evident for both As species. The relation As(III)/As(V) was similar in AD and SV. Conclusion: A linkage of As and proteins through neighbor sulphidryl groups is present in cardiovascular tissues of exposed subjects. It is likely that As is linked to >80 kDA protein fractions and to quaternary subu-nits or the native protein.

BACKGROUND: Arsenic exposure increases the risk of non-cancerous and cancerous diseases. In the Antofagasta region in Chile, an established relationship exists between arsenic exposure and the risk of cancer of the bladder, lung and skin. Platinum-based drugs are first-line treatments, and many works recognise selenium as a cancer-fighting nutrient. We characterised the short-term urinary excretion amounts of arsenic, selenium and platinum in 24-h urine samples from patients with lung cancer and those with cancer other than lung treated with cisplatin or/and carboplatin. As - Se - Pt inter-element relationships were also investigated. RESULTS: The amounts of platinum excreted in urine were not significantly different between patients with lung cancer and those with other cancers treated with cisplatin, despite the significant variation in platinum amounts supplied from platinum-based drugs. In general, the analytical amounts of excreted selenium were greater than those for arsenic, which could imply that platinum favours the excretion of selenium. For other types of cancers treated with drugs without platinum, excretion of selenium was also greater than that of arsenic, suggesting an antagonist selenium-anti-cancer drug relationship. CONCLUSIONS: Regards the baseline status of patients, the analytical amounts of excreted Se is greater than those for As, particularly, for cisplatin chemotherapy. This finding could imply that for over the As displacement Pt favours the excretion of Se. The analytical amounts of excreted Se were greater than those for As, either with and without Pt-containing drugs, suggesting an antagonist Se-anti-cancer drug relationship. However, it seemed that differences existed between As - Se - Pt inter-element associations in patients treated for lung cancer in comparison with those treated for cancer other than lung. Therefore, knowledge obtained in this work, can contribute to understanding the arsenic cancer mechanism and the As - Se - Pt inter-element association for lung cancer and other types of cancer, which in some cases respond at a linear mathematical model.
