Studies on the effects of non-nutritive sweeteners (NNSs) among pregnant women are scarce and have produced mixed results. One of the major challenges is to accurately assess NNS intake, especially in countries that have implemented policies to prevent obesity and where many foods and beverages have been progressively reformulated to partially or totally replace sugar with NNSs. This study aimed to develop and assess the relative validity of a food frequency questionnaire (FFQ) for use in pregnant women. We developed an FFQ to examine the intake of seven NNSs (acesulfame-k, aspartame, cyclamate, saccharin, sucralose, steviol glycosides, and D-tagatose). This questionnaire was piloted in 29 pregnant women (median age = 31.2 y; 25th-75th percentile: 26.9-34.7) to assess NNS intake over the previous month, compared to 3-day dietary records (3-DR). The validity of this dietary method was assessed using Spearman's correlation coefficient, Lin´s concordance correlation coefficient (CCC), and Bland-Altman plots. Spearman's correlations between the FFQ on NNSs and 3-DR ranged from 0.50 for acesulfame K to 0.83 for saccharin. CCC ranged between 0.22 and 0.66. The Bland-Altman plots showed an overestimation of saccharin, sucralose, and steviol glycosides intake by the FFQ on NNSs compared with 3-DR, and an underestimation of acesulfame K and aspartame. Overall, the NNSs most frequently consumed were sucralose, and none of the participants exceeded the acceptable daily intake for any of the NNSs evaluated. The FFQ on NNSs seems to be reasonably valid in the assessment of NNSs among pregnant women.

BACKGROUND AND OBJECTIVE: The perception of quality of life about health status is subjective; assessment of the impact includes well-being while eating, speaking, smiling, interacting with others, and feeling good about the esthetic appearance of teeth and mouth. The objective of the present study was to cross-culturally adapt and determine the validity and reliability of the Mexican version of the Iowa Fluoride Study Oral Health for adolescents. MATERIAL AND METHODS: A cross-sectional study was carried out in a sample of 240 Mexican adolescents aged 15 to 19 years. The questionnaire was translated, back-translated, and administered through the Google Forms platform. The cultural adaptation consisted of the evaluation of the grammatical, conceptual, and linguistic equivalences. The evaluation of the utility and acceptability was carried out through an analysis of semantic equivalence. The utility of the questionnaire was also evaluated by analyzing its grammatical readability. Reliability tests, Kaiser-Meyer-Olkin (KMO), factor analysis, and Pearson's correlation were performed. RESULTS: The mean age of all participants was 16.4±1.4; 65.3% (n=158) were female. Face validity was considered adequate. The wording of the objective and instructions of the questionnaire were improved. The confidentiality assurances were highlighted. The questions were clear, understandable, and pertinent, and they showed adequate syntax. The INFLESZ index corresponds to a "fairly easy" level of readability. In the quantitative validation, the correlation of items was greater than 0.4. The KMO was 0.930 (p=0.001), and Bartlett sphericity was 2466.5 (p=0.001). Through the exploratory factorial analysis, we evaluated the emotional well-being (12 items), social welfare (five items), and oral symptoms (OS) (three items) dimensions. Internal consistency was high (Cronbach's α=0.942). CONCLUSION: The culturally translated and adapted questionnaire is valid and reliable for use in research on Mexican adolescents.

A depressão é definida como um distúrbio que provoca alterações de ordem social, psicológica, fisiológica e biológica. Pessoas diagnosticadas como depressivas apresentam impactos no funcionamento psicossocial, saúde física, mortalidade e qualidade de vida. Por esta razão, este estudo teve como objetivo demonstrar os mecanismos fisiológicos envolvidos nos transtornos de depressão e sua relação com o neurotransmissor dopamina. Trata-se de uma revisão bibliográfica narrativa realizada no período de março a maio de 2023, através de pesquisas nas bases de dados Scientific Electronic Library Online (SciELO), PubMed via Medical Literature Analysis and Retrieval System Online (MEDLINE), Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS) e Google Scholar. Através desta revisão foi possível evidenciar a importância no conhecimento acerca dos mecanismos fisiopatológicos envolvidos nos transtornos de depressão, sendo essencial para o entendimento e aplicação em tratamentos de pacientes diagnosticados com este transtorno.


Depression is defined as a disorder that causes social, psychological, physiological and biological changes. People diagnosed as depressive have an impact on psychosocial functioning, physical health, mortality, and quality of life. For this reason, this study aimed to demonstrate the physiological mechanisms involved in depression disorders and their relationship with the neurotransmitter dopamine. It is a narrative bibliographic review conducted in the period from March to May 2023, through searches in the databases Scientific Electronic Library Online (SciELO), PubMed via Medical Literature Analysis and Retrieval System Online (MEDLINE), Latin American and Caribbean Literature in Health Sciences (LILACS) and Google Scholar. Through this review it was possible to highlight the importance in the knowledge about the physiopathological mechanisms involved in depression disorders, being essential for the understanding and application in treatments of patients diagnosed with this disorder.


La depresión se define como un trastorno que causa cambios sociales, psicológicos, fisiológicos y biológicos. Las personas diagnosticadas como depresivas tienen un impacto en el funcionamiento psicosocial, la salud física, la mortalidad y la calidad de vida. Por esta razón, este estudio tuvo como objetivo demostrar los mecanismos fisiológicos involucrados en los trastornos de la depresión y su relación con el neurotransmisor dopamina. Esta es una revisión bibliográfica narrativa realizada entre marzo y mayo de 2023, a través de la investigación en las bases de datos Scientific Electronic Library Online (SciELO), PubMed via Medical Literature Analysis and Retrieval System Online (MEDLINE), Latin American and Caribbean Literature in Health Sciences (LILACS) y Google Scholar. A través de esta revisión, se pudo destacar la importancia en el conocimiento de los mecanismos fisiopatológicos involucrados en los trastornos de la depresión, y es esencial para entenderlos y aplicarlos al tratamiento de pacientes diagnosticados con este trastorno.

BACKGROUND: Worldwide, the prevalence of obesity and related non-communicable chronic diseases is high and continues to grow. In that sense, anthocyanins (ANC) have shown beneficial health effects in preventing obesity and metabolic risk factors. Moreover, the demand for functional foods incorporating these compounds has risen significantly in the past years. Thus, there is a need for validations of the functional properties of these formulations; nevertheless, in vivo assays are complex and require a lot of resources. One approach for estimating bioactive compounds' functionality and health benefits is to evaluate their bioaccessibility on a specific food matrix, determined by various factors. This article aims to review different factors influencing the bioaccessibility of ANC evaluated on in vitro digestion models as a functionality parameter, elucidating the effect of chemical composition, raw materials, food matrices, and vehicles for the delivery of ANC. METHODS: Study searches were performed using PubMed, Web of Science, Scopus, and Science Direct databases. RESULTS: Different factors influenced bioaccessibility and stability of ANC studied by in vitro digestion: i) the raw material used for ANC obtention; ii) food processing; iii) other food components; iv) the extraction method and solvents used; v) the structure of ANC; vi) delivery system (e.g., microencapsulation); vii) pH of the medium; viii) the digestion stage. CONCLUSION: Simulated digestion systems allow to determine free or encapsulated ANC bioaccessibility in different food matrices, which offers advantages in determining the potential functionality of a food product.

The BH-3 mimetic venetoclax overcomes apoptosis and therapy resistance caused by high expression of BCL2 or loss of BH3-only protein function. Although a promising therapy for hematologic malignancies, increased expression of anti-apoptotic MCL-1 or BCL-XL, as well as other resistance mechanisms prevent a durable response to venetoclax. Recent studies demonstrate that agents targeting epigenetic mechanisms such as DNA methyltransferase inhibitors, histone deacetylase (HDAC) inhibitors, histone methyltransferase EZH2 inhibitors, or bromodomain reader protein inhibitors may disable oncogenic gene expression signatures responsible for venetoclax resistance. Combination therapies including venetoclax and epigenetic therapies are effective in preclinical models and the subject of many current clinical trials. Here we review epigenetic strategies to overcome venetoclax resistance mechanisms in hematologic malignancies.

The prevalence of obesity among Chilean adults and children is one of the highest worldwide. To fight the constant increase of non-communicable diseases and the growing sales of sugar-sweetened beverages, the Chilean government recently enacted a new Law of Food Labeling and Advertising imposing the application of front-of-package warning labels in foodstuffs whose composition exceeds limits for critical nutrients including sugar. Accordingly, food companies have been reformulating their products, incorporating non-caloric sweeteners (NCSs) in partial or total replacement of sucrose. The number of NCS-containing foods and beverages, therefore, has been increasing in the last years. This study aims to identify the NCS-containing products from different food/beverage categories currently available on the Chilean market. Nineteen supermarkets and 13 food web pages were visited by trained dietitians to carry out a systematic search of ingredient information from the different food categories. Overall, 1,489 products were analyzed, of which 815 (55.5%) contained at least one NCS, being this proportion particularly high, compared to other countries. 67.1% of the dairy products, 31.5% of the cereal products, 49% of the processed fruits, 74.3% of the non-alcoholic beverages, and 46.2% of sweets and other desserts contained NCS. Considering the food categories more specifically oriented to children, NCSs were present in 98.8% of powder juices, 98.3% of the flavored milks, 91.2% of jellies, and 79% of the dairy desserts. Sucralose and steviol glycosides were the most widely used NCSs, these sweeteners being present, alone or mixed with other, in 73.5 and 39.7% of the NCS-containing products, respectively, while the use of saccharin and cyclamate was low. In addition, 80 tabletop NCSs were available in the local market, 91.2% of them being sucralose and steviol glycosides (alone or combined). The high number of food products containing steviol glycosides makes very plausible that the daily consumption of this NCS in the pediatric populations could exceed its acceptable daily intake (ADI). The fact that there are no NCS-free foods alternatives for certain food categories, especially for children, is worrying.

RESUMEN La obesidad es una enfermedad multifactorial definida como acumulación patológica de grasa. Su prevalencia ha aumentado enormemente en el mundo. Chile presenta una de las mayores prevalencias de obesidad de la OCDE. Su casuística simplificada comprende una diferencia sostenida entre gasto e ingesta de energía, manteniendo un delta positivo traducido en mayor acumulación de grasa. No obstante, la etiología completa de esta enfermedad comprende también factores psicológicos, genéticos, ambientales, etc. El ambiente juega un papel clave en la predisposición al consumo de alimentos, a la realización de ejercicio físico, incluso afectando la susceptibilidad genómica, exacerbando o disminuyendo la carga genética. Esta modificación de susceptibilidad genética por el ambiente se conoce como epigenética, que se refiere a una serie de modificaciones por "sobre" la genética que son altamente modificables por factores ambientales. Se ha descrito que algunas de estas modificaciones pueden heredarse de una generación a otra, lo que otorga otro nivel de complejidad al estudio de nuevas terapias complementarias para frenar la tendencia al sobrepeso. En la presente revisión se describe cuales son las modificaciones epigenéticas más frecuentes encontradas, su relación con obesidad y dieta, y finalmente como se relaciona con la transmisión transgeneracional de esta patología.


ABSTRACT Obesity is a multifactorial disease defined by a pathological accumulation of body fat. Its prevalence has increased greatly across the world. Chile has one of the highest prevalence of obesity among OCDE countries. It is caused by a sustained difference between energy expenditure and intake, keeping a positive delta, which drives fat accumulation. However, its etiology is comprised several factors: psychological, genetics, environmental, etc. The environment plays a key role in the predisposition towards food consumption, the adoption of exercise, and genetic susceptibility, increasing or decreasing the genetic load towards obesity. This modification of susceptibility is known as epigenetics, which refers to modifications "over" genetics, which are highly modifiable by environmental factors. Some of these modifications can be inherited from one generation to another, granting a higher complexity level regarding designing novel complementary therapies against obesity. Thus, the present review described which epigenetic modifications are related to obesity and different dietary patterns, and finally how epigenetic modifications can be related to transgenerational transmission of obesity.

RESUMEN Las representaciones sociales producto de la forma de percibir, interpretar y significar la realidad social a partir del sentido común generan conductas que influyen directamente en la práctica y solicitud de la atención odontológica, bajo la influencia de las condiciones socioeconómicas actuales del país. Los niveles de pobreza se incrementan de manera acelerada, generando mayores desigualdades sociales, con distribución de beneficios económicos polarizados con concentración de los ingresos más altos en unos pocos. Con ello la salud-enfermedad es determinada por los niveles de ingreso, limitando el acceso a los servicios de salud para los grupos sociales más desfavorecidos. El objetivo de este estudio es conocer las representaciones sociales de salud-enfermedad odontológica y su relación con la atención y el acceso a servicios odontológicos y el tipo de atención al que acuden personas con padecimientos estomatológicos y las características de las prácticas odontológicas de quienes les atienden. Se realizó un estudio cualitativo con profesionales y pacientes del ámbito privado en una zona de la Ciudad de México. Los resultados muestran características de concepción y práctica influidas por el modelo médico hegemónico y de descontextualización de los problemas de atención odontológica de los pacientes. Se concluye que las representaciones sociales de profesionales y pacientes se construyen en el contexto de vida donde prevalece la polarización y exclusión social.


ABSTRACT The social representations resulting from the way of perceiving, interpreting and signifying social reality from the common sense, generate behaviors that directly influence the practice and application of dental care, under the influence of the current socioeconomic conditions of the country. Poverty levels increase rapidly, creating greater social inequalities, with a distribution of polarized economic benefi ts with a concentration of higher incomes in a few. With this, health- dental disease is determined by income levels, limiting access to health levels for the most disadvantaged social groups. The objective of this study is to know the social representations of health-dental disease and its relationship with care and access to dental services and the type of care that people with dental conditions access, and the characteristics of the dentistry practice of those who care for them. A qualitative study was carried out with professionals and patients from the private sector in an area of Mexico City. The results show characteristics of conception and practice infl uenced by the hegemonic medical model and decontextualization of the problems of dental care patients. It is concluded that the social representations of professionals and patients are built in the context of life where polarization and social exclusion prevail.

ABSTRACT The number of fixed segments in the surgical treatment of thoracolumbar burst fractures remains controversial. This study aims to compare the results of short and long fixation in thoracolumbar burst fractures through a meta-analysis of studies published recently. MEDLINE and Cochrane databases were used. Randomized controlled trials and non-randomized comparative studies (prospective and retrospective) were selected. Data were analyzed with the software Review Manager. There was no statistically significant difference in the Cobb angle of preoperative kyphosis. Long fixation showed lower average measurements postoperatively (MD = 1.41; CI = 0.73-2.08; p<0.0001) and in the last follow-up (MD = 3.98; CI = 3.22-4.75; p<0.00001). The short fixation showed the highest failure rates (RD = 4.03; CI = 1.33-12.16; p=0.01) and increased loss of height of the vertebral body (MD = 1.24; CI = 0.49-1.98; p=0.001), with shorter operative time (MD = -24.54; CI = -30.16 - -18.91; p<0.00001). There was no significant difference in blood loss and clinical outcomes. The high rates of kyphosis correction loss with short fixation and the lower correction rate in the immediate postoperative period were validated. There was no significant difference in the blood loss rates because arthrodesis was performed in a short segment in the analyzed studies. The short fixation was performed in a shorter operative time, as expected. No study has shown superior clinical outcomes. The short fixation had worse rates of kyphosis correction in the immediate postoperative period, and increased loss of correction in long-term follow-up, making the long fixation an effective option in the management of this type of fracture.


RESUMO A quantidade de segmentos fixados no tratamento cirúrgico das fraturas toracolombares tipo explosão continua controverso. Este estudo tem como objetivo comparar os resultados da fixação curta e da longa nas fraturas toracolombares do tipo explosão, por meio de uma metanálise dos estudos publicados recentemente. Foram utilizadas as bases de dados MEDLINE e COCHRANE. Foram selecionados estudos controlados randomizados e estudos comparativos não randomizados (prospectivos e retrospectivos). Os dados foram analisados com o software Review Manager. Não houve diferença estatisticamente significante na medida do ângulo de Cobb da cifose pré-operatória. A fixação longa apresentou medidas médias inferiores no pós-operatório (MD = 1,41; IC = 0,73-2,08; p < 0,0001) e no último seguimento (MD = 3,98; IC =3,22-4,75; p < 0,00001). A fixação curta apresentou taxas de falha maiores (RD = 4,03; IC = 1,33-12,16; p = 0,01) e maior perda de altura do corpo vertebral (MD = 1,24; IC = 0,49-1,98; p = 0,001), com menor tempo operatório (MD = -24,54; IC = -30,16 - -18,91; p < 0,00001). Não houve diferença estatisticamente significante na perda sanguínea e nos desfechos clínicos. As taxas elevadas de perda da correção da cifose na fixação curta e a menor taxa de correção no pós-operatório imediato foram validadas. Não houve diferença estatisticamente significante quanto às taxas de perda sanguínea, porque a artrodese foi realizada em um segmento curto nos trabalhos analisados. A fixação curta teve menor tempo operatório, como esperado. Nenhum estudo demonstrou superioridade dos resultados clínicos. A fixação curta apresentou taxas piores de correção da cifose no pós-operatório imediato e maior perda da correção no seguimento a longo prazo, fazendo da fixação longa uma opção efetiva no manejo deste tipo de fratura.


RESUMEN El número de segmentos fijados en el tratamiento quirúrgico de las fracturas toracolumbares tipo explosión sigue siendo controvertido. Este estudio tiene como objetivo comparar los resultados de fijación corta y larga en las fracturas toracolumbares tipo explosión, a través de un meta-análisis de estudios publicados recientemente. Se utilizaron las bases de datos MEDLINE y COCHRANE. Se seleccionaron los ensayos controlados aleatorios y estudios comparativos no aleatorios (prospectivos y retrospectivos). Los datos fueron analizados con el software Review Manager. No hubo diferencia estadísticamente significativa en cuanto a la medida del ángulo de Cobb de la cifosis preoperatoria. Lar fijación larga mostró mediciones promedio más bajas en el postoperatorio (MD = 1,41, IC = 0,73-2,08; p < 0,0001) y en el último seguimiento (MD = 3,98, IC = 3,22-4,75; p < 0,00001). La fijación corta mostró las tasas de fracaso más altas (RD = 4,03, IC = 1,33-12,16; p = 0,01) y una pérdida mayor de la altura del cuerpo vertebral (MD = 1,24, IC = 0,49-1,98; p = 0,001), con un menor tiempo operatorio (MD = -24,54; IC = -30,16 - -18.91; p < 0,00001). No hubo diferencia significativa en la pérdida de sangre y los resultados clínicos. Se validaron las altas tasas de pérdida de corrección de la cifosis con la fijación corta y la menor tasa de corrección en el postoperatorio inmediato. No hubo diferencia significativa en las tasas de pérdida de sangre porque la artrodesis se realizó en un segmento corto en los estudios analizados. La fijación corta tuvo menor tiempo operatorio, como se esperaba. Ningún estudio ha demostrado resultados clínicos superiores. La fijación corta tuvo peores tasas de corrección de la cifosis en el postoperatorio inmediato y mayor pérdida de corrección en el seguimiento a largo plazo, por lo que la fijación larga es una opción efectiva en el manejo de este tipo de fracturas.

ABSTRACT. Objective: To analyze the incidence of spinal injuries between 2000-2010 due to motorcycle accidents and the relation to the increase in motorcycle sales in the same period, as well as the anatomical distribution of these spinal injuries. Methods: Data were collected from 1,295 records of patients who have suffered spinal injury resulting from motorcycle accidents admitted to the ward TRM (Spinal Cord Trauma) of the Hospital Geral do Estado da Bahia from 2000 to 2010 in this retrospective study. We selected 110 medical records and collected information on sex, age, neurological deficit on admission (according to Frankel scale), diagnosis, and level of injury. Results: Between 2000 and 2010 there was an increase of almost five times in the incidence of patients who have suffered spinal injury due to motorcycle accidents. More than half (51.4%) had cervical spine injury, 37.2% thoracic spine injury and 11.34% had lumbar spine injury. Only 34.3% of patients had no neurological deficit on admission and patients with thoracic spine fracture had a higher incidence and severity of lesion. The average age of patients was 30 years. Conclusions: The increased incidence of spinal injuries due motorcycle accidents occurred in the same period in which there was an increase in motorcycle sales in the country. Patients who have suffered those injuries were young, with higher incidence in the cervical and thoracic spinal levels and high rates of neurological deficit.


RESUMO. Objetivo: Analisar a incidência de lesões na coluna vertebral entre 2000 e 2010 devido a acidente motociclístico e sua relação com o aumento da venda de motos nesse período, assim como a distribuição anatômica dessas lesões na coluna. Métodos: Trata-se de estudo retrospectivo, no qual foram coletados dados de 1.295 prontuários de pacientes que sofreram lesão da coluna decorrente de acidente motociclístico internados na enfermaria de TRM (Trauma Raquimedular) do Hospital Geral do Estado da Bahia no período de 2000 a 2010. Foram selecionados 110 prontuários e catalogadas as informações com relação ao sexo, idade, déficit neurológico na apresentação (de acordo com a escala de Frankel), diagnóstico e nível da lesão. Resultados: Entre os anos de 2000 e 2010 houve um aumento de quase cinco vezes na incidência de pacientes que sofreram lesão da coluna devido a acidente motociclístico. Mais da metade (51,4%) apresentou lesão na coluna cervical, 37,2% na torácica e 11,4% na lombar. Somente 34,3% dos pacientes não tinham déficit neurológico na admissão e pacientes com fratura da coluna torácica tiveram maior incidência e gravidade de lesão medular. A média de idade dos pacientes foi de 30 anos. Conclusões: O aumento da incidência de lesões na coluna devido acidente motociclístico ocorreu no mesmo período em que houve aumento das vendas de motos no país. Os pacientes que sofreram essas lesões eram jovens, com incidência maior nos níveis cervical e torácico e altos índices de déficit neurológico.


RESUMEN. Objetivo: Analizar la incidencia de lesiones de la columna entre 2000-2010 debido a los accidentes de motocicleta y su relación con el aumento de las ventas de motocicletas en ese período, y la distribución anatómica de las lesiones de la columna vertebral. Métodos: Estudio retrospectivo en el que se recogieron datos de 1.295 registros de pacientes que han sufrido lesiones de la columna resultantes de accidentes de motocicleta ingresados en la sala de hospital TRM (Trauma de la Médula Espinal) del Hospital Geral do Estado da Bahia entre 2000 y 2010.Se seleccionaron 110 historiales médicos y se recogió información sobre el sexo, edad, déficit neurológico al ingreso (según la escala de Frankel), el diagnóstico y el nivel de la lesión. Resultados: Entre 2000 y 2010 hubo un aumento de casi cinco veces en la incidencia de pacientes que han sufrido lesiones de la columna debido a los accidentes de motocicleta. Más de la mitad (51,4%) tuvo lesiones de la columna cervical, el 37,2% tenían lesión en la columna torácica y el 11,4%, en la lumbar. Sólo el 34,3% de los pacientes no tuvieron déficit neurológico al ingreso y los pacientes con fractura de la columna torácica tuvieron una mayor incidencia y severidad de la lesión de la médula espinal. La edad promedio de los pacientes fue 30 años. Conclusiones: El aumento de la incidencia de lesiones en la columna debido a los accidentes de motocicleta se produjo en el mismo período en que hubo un aumento en las ventas de motocicletas en el país. Los pacientes que han sufrido esas lesiones eran jóvenes, con mayor incidencia en los niveles cervicales y torácicos y altas tasas de déficit neurológico.
