RESUMEN

Recent work suggests that between 50 and 75% of small-cell lung cancer (SCLC) tumours have specific high-affinity binding sites for somatostatin. This study evaluated the potential role of the radiolabelled somatostatin analogue, [111In]pentetreotide, in the detection and staging of SCLC in patients prior to and after chemotherapy using scintigraphic imaging techniques. Thirteen patients were studied prior to chemotherapy. Following standard staging six patients had limited stage disease and seven extensive disease. [111In]pentetreotide imaging led to the detection of all primary sites of disease, including a primary site of disease not detectable with chest radiograph or computerised tomography (CT) of the thorax. Five of ten metastatic sites detected by standard staging were also imaged. Furthermore, a cerebellar metastasis was detected in a patient thought to have disease confined to the right hemithorax. This was subsequently confirmed with a CT brain scan. Following chemotherapy [111In]pentetreotide imaging detected residual intrathoracic disease in two of three patients with complete remissions by standard staging and in two patients who had had a partial response to chemotherapy. These results suggest that [111In]pentetreotide imaging may have a role to play in the clinical evaluation of patients with SCLC. Specifically, this technique may be of particular value in detecting residual intrathoracic disease in patients thought to be in complete remission by conventional staging methods.

RESUMEN

Neuroendocrine tumours are characterised by the expression of high affinity binding sites for somatostatin. The detection of bronchial carcinoid tumours through scintigraphic imaging is described in two patients using the novel radiolabelled somatostatin analogue indium-111 pentetreotide.

We investigated lung pathology in 61 HIV-positive patients who underwent 79 bronchoscopies. There were 27 cases of pneumocystis carinii pneumonia (PCP), 22 cases of pyogenic pulmonary infection and six cases of mycobacterial infection. Eleven cases of non-specific interstitial pneumonitis were found and 10 intravenous drug abusers had birefringent pulmonary talc granulomata in biopsy specimens. Women and intravenous drug abusers had higher rates of pyogenic pulmonary infection than men and other risk groups respectively. When compared with a previous survey from the same hospital using the same methodology the present study showed a change in the risk groups with 50% more intravenous drug abusers, 42% more haemophiliacs and 45% fewer patients with sexually transmitted infection. However, apparent changes in the spectrum of diseases encountered were not significant.

RESUMEN

The measurement of the complex respiratory impedance at a high temporal resolution is described. The measurement of impedance is correlated with the breathing cycle using electrical transthoracic impedance. This allows the determination of the complex impedance at any fixed point in the respiratory cycle, which results in lower variability in the measured data when compared to the original device. The respiratory impedance in normal subjects fluctuates during the breathing cycle and has two maxima during a single breath, which occur at peak inspiratory and expiratory flow. In addition, the measurement of impedance at a high temporal resolution enables the observation of novel features in the impedance which appear to be associated with certain respiratory disorders.

RESUMEN

We describe the results of 43 bronchoscopic examinations carried out on 35 patients who were HIV positive. Of these, 13 were intravenous drug abusers, 13 were homosexual or bisexual, six were haemophiliacs, two were homosexual intravenous drug abusers and one was a heterosexual contact of a drug abuser. All underwent investigation because of respiratory symptoms. Aetiological diagnosis could not be made from physical examination, x-ray or routine laboratory tests but, was usually made using bronchoscopy and its associated investigations. Using these techniques, Pneumocystis carinii pneumonia was the commonest diagnosis (19 cases) followed by bacterial infection. Mycobacterial infection accounted for two cases and non-specific interstitial pneumonitis accounted for three. Kaposi's sarcoma was found in one instance and eight investigations were non-diagnostic.

RESUMEN

A patient with advanced emphysema and cor pulmonale had the changes of alpha 1 antitrypsin deficiency in a liver biopsy specimen and was shown to have the phenotype PiFZ. This case supports the contention that the F allele of alpha 1 antitrypsin predisposes to the development of emphysema, particularly when it occurs in conjunction with the Z allele.

RESUMEN

A prominent sarcoid like pulmonary granulomatous reaction to Hodgkin's disease was diagnosed six months before extrapulmonary Hodgkin's disease was confirmed histologically. It recurred with exacerbations of the lymphoma. The reaction is similar to that often seen at pathological staging of intra-abdominal organs not affected by Hodgkin's disease.

RESUMEN

A patient survived acute alcohol intoxication with an unprecedented blood alcohol level of 1,500 mg/dL. Treatment included peritoneal dialysis and intravenously administered fructose. Alcohol elimination followed first-order kinetics, in which the elimination rate is proportional to concentration, rather than zero order in which the metabolic rate is independent of concentration. The report provides further evidence for first-order elimination kinetics with high blood alcohol levels and for an adaptive increase in alcohol metabolism with long-term alcohol consumption.

We have compared the macrophage elastolytic activity of a group of current and former smokers with irreversible airflow obstruction. Elastolytic activity was determined in an initial bronchoalveolar lavage cell population and in alveolar macrophages cultured for three days, to investigate whether enhanced macrophage elastolytic activity alone is a determining factor in the susceptibility of some smokers to obstructive lung disease. Twenty current smokers and 12 former smokers who had abstained from smoking for at least three years were studied. All patients had spirometric evidence of irreversible air flow obstruction. Current smokers had a cell yield (mean +/- SD) of 138.7 +/- 36.4 X 10(6) cells (alveolar macrophages 94.2% +/- 2.1%) compared with 31.4 +/- 14.1 X 10(6) cells (macrophages 86.5% +/- 4.7%) in former smokers. Elastolytic activity in the initial lavage cell population from current and former smokers, measured with the synthetic elastase substrate succinyl-L-alanyl-L alanyl-L-alanine-p-nitroanilide, and expressed as the equivalent of 1 microgram of porcine pancreatic elastase, was respectively 0.113 +/- 0.003 and 0.096 +/- 0.004 microgram pancreatic elastase/mg cell protein. After three days in culture macrophage elastolytic activity in the current and former smokers' cells was respectively 0.107 +/- 0.006 and 0.011 +/- 0.001 microgram pancreatic elastase/mg cell protein (p less than 0.05). The elastase activity of the cultured alveolar macrophages from five current smokers had the inhibitor profile of a metalloproteinase. Our results indicate that enhanced macrophage elastolytic activity alone is not a determining factor in the susceptibility of some smokers to develop obstructive lung disease.

RESUMEN

1. A method for measuring the pulmonary water compartments in vivo in anaesthetized dogs is described. 2. The technique depends upon the sequential intravenous injection of markers labelled with gamma-ray-emitting isotopes. Each is allowed to become evenly distributed throughout the compartment to which it is confined. A calibrated gamma-counter placed at the surface of the inflated lung then records a count rate delated to the total quantity of isotope in unit volume of tissue. The size of each water compartment can be calculated from the external count rate and the isotopic concentration of the marker in plasma. 3. The markers used were 125I-labelled serum albumin for plasma, [125I]iodide for extracellular water and [125I]iodoantipyrine for total water. 4. The method was extended to calculate trans-capillary flux rates of serum albumin and showed that rapid exchange of serum albumin takes place between the plasma and interstitial space. 5. Results obtained non-invasively by the isotopic method were compared with direct measurements. Agreement was good.

RESUMEN

Thirty-two EEGs from six cases of Aicardi's syndrome were reviewed. A characteristic EEG pattern was found in all cases. This consists of multifocal epileptiform abnormalities occurring on a burst-suppression pattern showing complete asynchrony between the two hemispheres. This pattern has been described so far only in Aicardi's syndrome. These characteristic EEG features are more readily found early in the course of the disease and occur less frequently six months after from the onset of symptoms, at which time they are often replaced by multiple epileptic foci on a severely disorganized background. The EEG sleep pattern was profoundly altered in all stages of the disease. The EEG is considered a helpful tool in the diagnosis of Aicardi's syndrome.
