BACKGROUND: Oncology care has made the shift to the outpatient setting. The authors' cancer center is a basic life support-certified facility using an affiliated hospital for emergent transfers. A nurse-led initiative was developed in response to expressed anxieties and lack of comfort by staff related to their role in emergency management. Mock drills help staff retain knowledge and skills necessary during an emergency. OBJECTIVES: This project aimed to create simulation-based scenarios to practice outpatient emergency management skills. METHODS: Targeted objectives for each scenario were developed by the team with staff input. An evaluation form following the mock drill is used to assess comfort level and attained knowledge. FINDINGS: Practicing an emergency scenario in a controlled and simulated environment improves provider confidence, communication, teamwork, and patient safety. It is an adaptable strategy that fosters team cohesiveness during actual emergencies. A successful mock drill team encompasses organization, clearly identified roles, and frequent team practice.

RESUMEN

Recent approaches in treating pancreatic adenocarcinoma, an aggressive disease with limited survival, include the use of liposomal irinotecan as an option when first-line therapy has failed. Liposomal irinotecan has been approved in combination with 5-fluorouracil and leucovorin for patients with metastatic pancreatic cancer. Liposomal irinotecan is a newer therapy requiring oncology nurses to obtain knowledge and skills for proper administrating, monitoring of hypersensitivity reactions during infusion, managing side effects, and providing patient education. Nursing considerations when administering this drug include infusion time, premedication, risk for hypersensitivity reactions and adverse events, and side effects.

RESUMEN

Fosaprepitant dimeglumine (Emend IV®) is an IV antiemetic that may be beneficial to patients receiving highly emetogenic regimens. Aprepitant (Emend®) is an oral medication that is administered for three consecutive days, whereas fosaprepitant is a single-dose IV medication that is administered on the day of chemotherapy for 20-30 minutes (depending on the IV access type). Fosaprepitant may be useful, yet it can also present a risk for hypersensitivity reactions and phlebitis. Oncology nurses must be aware of the signs and symptoms of these potential adverse events to properly care for their patients.
