RESUMEN

The concept of precision oncology entails molecular profiling of tumors to guide therapeutic interventions. Genomic testing through next-generation sequencing (NGS) molecular analysis provides the basis of such highly targeted therapeutics in oncology. As radiomic analysis delivers an array of structural and functional imaging-based biomarkers that depict these molecular mechanisms and correlate with key genetic alterations related to cancers. There is an opportunity to synergize these two big-data approaches to determine the molecular guidance for precision therapeutics. Colorectal cancer is one such disease whose therapeutic management is being guided by genetic and genomic analyses. We review the rationale and utility of radiomics as a combinative strategy for these approaches in the management of colorectal cancer.

RESUMEN

Infertility in a large proportion of infertile couples is due, in part, to the male factor. Spermatozoa must survive the unique environment of the female reproductive tract in their path to fertilize the ovum. This fact is even more problematic for men with oligospermia. Of the many environmental factors that effect sperm motility in the female reproductive tract, we have decided to concentrate on the effect of pH and viscosity on bovine spermatozoa in the laboratory setting. Follicular fluid was harvested from heifer ovaries to serve as a chemo-attractant. Through image analysis, our data shows trends of sperm motility as a function of pH and viscosity. There is a significant increase in the number of immotile sperm seen as pH decreases from 6.5 to 6.0. Furthermore, an exponential relationship between sperm speed and environmental viscosity was observed in vitro. This suggests that modulating vaginal pH and vaginal secretion viscosity could greatly affect spermatozoa motility and therefore male fertility.

RESUMEN

BACKGROUND: Although breast surgery, including mastectomy, is increasingly being performed on an outpatient basis, skepticism remains about the safety of outpatient mastectomy with immediate breast reconstruction. Studies have demonstrated a psychologic benefit to outpatient breast surgery in addition to the clear financial benefit. We sought to determine whether or not the postoperative complication rate after outpatient mastectomy with immediate reconstruction is low enough to consider the procedure safe and effective. STUDY DESIGN: Charts were retrospectively reviewed for all patients who underwent outpatient mastectomies with immediate breast reconstruction at St Vincent's Comprehensive Cancer Center between December 2000 and June 2004. The presence or absence of postoperative complications was determined from records during the postoperative period and subsequent office visit. RESULTS: Of 29 outpatient mastectomies with immediate reconstruction performed on 28 patients (one had independent procedures on each breast), only one procedure (3%) required subsequent admission to the hospital (for bleeding). Other complications included three seromas, two cases of cellulitis requiring antibiotics, and one hematoma. The overall complication rate was 24% (7 of 29), with only 14% (1 of 7) of the complications requiring hospitalization. CONCLUSIONS: These results demonstrate that outpatient mastectomy with immediate reconstruction is a safe and effective procedure for carefully selected patients. The complication rates for our patient population are similar to those in other published reports on outpatient operations, most notably, those of outpatient mastectomy without immediate reconstruction.
