RESUMEN

OBJECTIVES: Pulpal inflammation is often associated with odontogenic pain. Dental pulp is abundantly innervated with sensory fibers encompassing neuropeptides. Neurokinin A (NKA) and substance P (SP) are important neuropeptides in the dental pulp that can cause neurogenic inflammation. Since no previous study has assessed dental pulp neuropeptides in children, this study aimed to compare the level of NKA and SP in gingival crevicular fluid (GCF) of sound and symptomatic carious primary teeth. MATERIALS AND METHODS: Samples of GCF were obtained of 20 sound and 20 painful carious primary teeth. Enzyme-linked immunosorbent assay (ELISA) was used to quantify neuropeptides in GCF. Data were analyzed using paired t-test, ANOVA, Kolmogorov-Smirnov test and correlation coefficient test. RESULTS: A significant difference was noted in the level of NKA in GCF of painful and sound teeth (2.23 pg/ml in painful, and 1.84 pg/ml in sound teeth, P<0.05). The difference between the two groups regarding SP was not significant (2.23 pg/ml in painful, and 2.02 pg/ml in sound teeth, P>0.05). CONCLUSIONS: The results showed that the level of NKA and SP was higher in GCF of painful teeth compared to that of sound teeth. This difference was statistically significant with regard to NKA. Thus, these neuropeptides can serve as indicators for pathological activities in teeth with symptomatic irreversible pulpitis.

OBJECTIVES: This study aimed to assess and compare the microleakage of two bulk fill and one conventional composite in class II restorations of primary posterior teeth. MATERIALS AND METHODS: This in vitro, experimental study was conducted on 60 primary mandibular second molars, which were randomly divided into three groups. Standard class II cavities were prepared in teeth and restored with 3M bulk fill composite in group 1, SonicFill bulk fill composite in group 2 and Z250 conventional composite in group 3. Single Bond 2 bonding agent was used in all cavities. The teeth were then thermocycled and immersed in 1M silver nitrate solution. The teeth were then mesiodistally sectioned and evaluated under a stereomicroscope at×10 magnification. Dye penetration depth was recorded in microns and data were analyzed using one-way ANOVA. RESULTS: The mean (± standard deviation) dye penetration depth in the gingival margins was 543±523µm, 343±290µm and 597±590µm for 3M bulk fill, SonicFill and Z250 conventional composite, respectively. These values were 214±93µm, 302±127µm and 199±145µm in the occlusal margins, respectively. The three groups were not significantly different in terms of occlusal or gingival microleakage (P>0.05), but gingival margins showed significantly higher microleakage than occlusal margins in all three groups (P<0.05). CONCLUSIONS: Bulk fill composites are not significantly different from conventional composites in terms of microleakage.

BACKGROUND: It is shown that neuropeptides can be transported from pulp chamber to periodontal ligament through apical foramen and accessory canals. Therefore, clinical pulpal pain leads to expression of preinflammatory neuropeptides such as substance P (SP) and neurokinin A (NKA) in gingival crevicular fluid (GCF). This study aimed to evaluate levels of SP and NKA in GCF of carious and healthy permanent teeth, comparatively. MATERIALS AND METHODS: This cross-sectional study was performed on twenty children referred to Department of Pediatric Dentistry, Tehran University of Medical Sciences, who had a painful permanent first molar. Sampling was done by sterile paper cone from GCF of the mentioned teeth and the intact tooth of the other side of the jaw in the same patient. Values of SP and NKA were measured by ELISA test. RESULTS: The mean concentration of SP in GCF of painful carious and healthy teeth was 2.65 ± 0.56 and 1.83 ± 0.65 pcgr/ml, respectively. This value was 2.29 ± 0.29 and 1.61 ± 0.35 pcgr/ml for NKA concentration in carious and healthy teeth as well. CONCLUSION: Significant higher levels of both SP and NKA in GCF of painful carious teeth were observed, which is in line with previous studies' findings.

BACKGROUND: There is a variation in trends of oral cancers all over the world. Many investigations have reported evidence of an increasing incidence in oral cancers during recent years. The purpose of this study was to investigate time trend and changes in demographic distribution of oral cancers incidence in Isfahan during 1991-2010. MATERIALS AND METHODS: In this retrospective analytic study archive of Oral Pathology Department of School of Dentistry, Isfahan University of Medical Sciences from 1991 to 2010 were reviewed. A total of 231 Pathology reports were analyzed. Age, sex, primary site, histologic type of cancer, and the referral year were recorded. Data were analyzed by using Jointpoint Regression Program 3 and SPSS 18. P value less than 0.05 consider as significant level. RESULTS: Out of all malignancies, 55% were male and 45% were female. The most frequent cancer was squamous cell carcinoma. Comparing the two time intervals (1991-2000) and (2001-2010) showed that the ratio of carcinomas and salivary gland tumors had decreased while there was an increase in incidence of sarcomas and lymphomas. Among young persons, the occurrence of oral carcinomas (mostly SCC) is rare but sarcomas were more common in younger patients. Gingiva was the most frequently involved in oral cancers with (46%), followed by tongue with (18%). CONCLUSION: : According to this study it revealed that some changes in trends of oral cancer have happened in Isfahan that calls for more study and evaluation of etiologies of these changes.
