RESUMEN

PURPOSE: The cellular distribution of ghrelin positive cells is not well defined. The aim of the study is to quantify and determine the distribution of ghrelin cells in gastric mucosa in patients with morbid obesity and relate this finding to gender, comorbidities, gastritis, and plasmatic levels of acyl and desacylghrelin before and after laparoscopic sleeve gastrectomy. PATIENT AND METHODS: We performed a study on 61 patients with BMI≥50 kg/m2 operated by laparoscopic sleeve gastrectomy. Three gastric regions were analyzed by histopathology and immunohistology. Blood sample was taken before surgery, and at 6 and 12 months post-surgery, to study the plasma levels of ghrelin isoforms. RESULTS: Ghrelin cells are present in all regions of the gastric mucosa, with a greater number in the body and the fundus. Difference was found in the antrum between male and female patients (p=0.018). Patients with arterial hypertension also showed a lower level of cells in antrum (p<0.05). Acylghrelin levels after surgery were significantly lower (32.83+5.6 pg/mL to 10.09+11.8 pg/mL, p<0.05). Values of desacylghrelin tended to decrease but no significant variation was observed (207.4+39.3 pg/mL to 188.84+52.3 pg/mL). CONCLUSION: Our patients show ghrelin cells in all areas of the stomach. Gender, comorbidities, and gastritis are determinant on gastric ghrelin-producing cells distribution.

RESUMEN

Malignant melanoma (MM) and blue nevi of the uterine cervix are an extremely rare neoplasm, probably derived from embryologic migration of melanocytes from the neural crest. MM displays aggressive behavior with a poor prognosis. We report the case of a 76-year-old postmenopausal woman abnormal vaginal bleeding. She underwent a hysterectomy and bilateral salpingo-oophorectomy with paraaortic-iliac lymphadenectomy. Histopathological and immunohistochemical studies were consistent with the diagnosis of MM and blue nevi in the uterine cervix. Although it is extremely rare, this case suggests that MM of the uterine cervix should be considered in the differential diagnosis of undifferentiated neoplasm. Early diagnosis is essential in order to warrant a better prognosis, although there are no cases of cure described.

RESUMEN

PURPOSE: To report a case of choroidal melanoma in which the absence of 18FDG-positron emission tomography (PET)/computed tomography (CT) uptake in the whole body scan excluded the presence of any foci of metastases. Additionally, the PET/CT study correctly diagnosed the necrotic character of the lesion. METHODS: A 76-year-old woman with neovascular glaucoma, and without vitreous or retina visualization, was studied by our ophthalmology service. RESULTS: Ocular ultrasound showed an 18 × 16 mm vitreous exophytic image compatible with choroidal melanoma; magnetic resonance gave a 17 × 15 mm exophytic image, highly intense at T1 and hypointense at T2, with cystic and necrotic images after a contrast injection. An 18FDG-PET/CT study revealed a clearly hypometabolic lesion, representing absence of cell viability and excluding active melanoma cells. No other uptake lesions were detected at the whole-body PET/CT scan, assuming the absence of extraocular metastases. The eye was enucleated, and the pathology study showed a 16 × 15 mm necrotic choroidal melanoma. CONCLUSIONS: The 18FDG-PET/CT study correctly depicted the necrotic character of the choroidal melanoma and was able to exclude other foci of metastases.
