RESUMEN

Coral reef decline is an issue of concern around the globe. Remote and uninhabited coral areas are not exempt from facing changes in species composition and functionality due to global drivers. Quitasueño is a remote atoll within the Seaflower Biosphere Reserve, in the Southwestern Caribbean Sea. To evaluate the current status of the coral reefs in Quitasueño we sampled 120 stations through Rapid Ecological Assessment and evaluated four stations through Planar Point Intercept to compare the current percent cover of benthic groups with previous studies in the area. We found pronounced changes in coral and macroalgae covers in time, and great conspicuousness of multiple conditions of deterioration along Quitasueño, including diseases, coral predation, and aggression and invasion of coral colonies by macroalgae and sponges. The reef ecosystem seems to be facing a phase shift, in which the benthic cover previously dominated by hard corals is currently dominated by fleshy macroalgae. It is essential to evaluate the possible drivers of the extent of degradation of Quitasueño to understand the process of deterioration and mitigate the impacts.

Fish populations that bear considerable pressure levels tend to show a decline in the average size of individuals, with the small and unexploited species replacing the large and exploited ones. It is important to carry on with their characterization in areas where they are becoming an important source of food for local human populations. An example of such species are parrotfishes, whose responses to external factors such as fishing need to be understood and predicted. In this study, we used a diver-operated stereo-video to examine individual body size, sex ratios and proportion of species of the parrotfish assemblage and analyze them on a qualitative fishing pressure gradient at four oceanic islands in the Colombian Caribbean. We reported over 10,000 occurrences of eleven parrotfish species, of which we estimated the total length of over 90%, grouping them into three size categories (large, medium, and small). Our data showed a spatial variation of parrotfishes' abundances, biomass, and individual body size. Observed differences are size-category-dependent throughout the qualitative fishing pressure. In general, the medium-bodied species had smaller sizes, lower abundances, and thus lower contribution to the total parrotfish biomass at the most heavily fished island. Unexpectedly, we found evidence of possible indirect effects over the small-bodied species Scarus iseri and Scarus taeniopterus with significantly greater abundances, and larger sizes of males of S. iseri, at the higher fishing pressure sites. Overall, our data highlights the extent of the spatial variation in the parrotfish communities at relatively short distances, and present new insights into the responses of parrotfish species on a spectrum of body sizes along a gradient of human pressure.

The Latin American Society of Nephrology and Hypertension conducted a prospective cohort, multinational registry of Latin American patients with kidney impairment associated to COVID-19 infection with the objective to describe the characteristics of acute kidney disease under these circumstances. The study was carried out through open invitation in order to describe the characteristics of the disease in the region. Eight-hundred and seventy patients from 12 countries were included. Median age was 63 years (54-74), most of patients were male (68.4%) and with diverse comorbidities (87.2%). Acute kidney injury (AKI) was hospital-acquired in 64.7% and non-oliguric in 59.9%. Multiorgan dysfunction syndrome (MODS) due to COVID-19 and volume depletion were the main factors contributing to AKI (59.2% and 35.7% respectively). Kidney replacement therapy was started in 46.2%. Non-recovery of renal function was observed in 65.3%. 71.5% of patients were admitted to ICU and 72.2% underwent mechanical ventilation. Proteinuria at admission was present in 62.4% of patients and proteinuria during hospital-stay occurred in 37.5%. Those patients with proteinuria at admission had higher burden of comorbidities, higher baseline sCr, and MODS was severe. On the other hand, patients with de novo proteinuria had lower incidence of comorbidities and near normal sCr at admission, but showed adverse course of disease. COVID-19 MODS was the main cause of AKI in both groups. All-cause mortality of the general population was 57.4%, and it was associated to age, sepsis as cause of AKI, severity of condition at admission, oliguria, mechanical ventilation, non-recovery of renal function, in-hospital complications and hospital stay. In conclusion, our study contributes to a better knowledge of this condition and highlights the relevance of the detection of proteinuria throughout the clinical course.

Colobanthus quitensis (Kunt) is one of the two vascular plant species present in Antarctica and develops under severe environmental conditions, being found in both pristine and human-threatened environments. We determined the Cd, Cr, Cu, Mn, Ni, Pb, and Zn levels in C. quitensis roots, leaves, and soils of origin using flame atomic absorption spectroscopy. In January 2017, we collected samples from four geographical zones on the longitudinal gradient along which C. quitensis is distributed, starting from Punta Arenas (PAR) at the extreme south of mainland Chile and moving southwards to the Antarctic territory from King George Island (KGI) to Hannah Point Peninsula (PHA) and finally Lagotellerie Island (LAT). We used certified reference material to validate the plant tissues and soil samples we collected. The highest concentrations of metals that we measured in the soils and in the C. quitensis roots and leaves were in samples we collected at the KGI station, the zone with the greatest human activity. The lowest concentrations we measured were at the LAT station, an island with little human intervention and scarce fauna. The mean concentrations of metals in the roots and leaves of C. quitensis followed a similar order at all sampling locations: Mn > Zn > Cu > Ni > Pb > Cr > Cd. In contrast, in soil, they followed the following order: Mn > Zn > Cu > Cr > Pb > Ni > Cd. The concentration levels obtained for the different metals in the soil and plants tissue samples in this region of Antarctica indicated that the area was non-polluted. However, the metallic trace element (MTE) concentrations may be at an early stage of contamination, as described in other areas of the Antarctic, being a new threat to this continent.

Scleredema adultorum of Buschke is a rare skin disease characterized by skin thickening and tightening typically at the neck and the upper part of the body. This thickening results from increased mucin deposition in the reticular dermis. Three variants are recognized. Scleredema diabeticorum is one subtype associated with diabetes mellitus. We report a man with a history of poorly controlled diabetes presenting with extensive scleredema adultorum of the trunk in a "cuirasse" pattern associated with restrictive lung disease. Cutaneous ultrasonography revealed a marked thickening of the dermis as well as various echogenic spots in the dermis. Cutaneous hardness secondary to scleredema may cause limited mobility. Therefore, respiratory manifestations may be associated and assessed, mainly in cases of extensive scleredema involving trunk and shoulders. Moreover, cutaneous ultrasonography may be useful to monitor the evolution of scleredema and confirm the diagnosis.

La osificación heterotópica es un trastorno infrecuente que consiste en el depósito de tejido óseo extraesquelético. En la piel, puede ser primario, en el contexto de síndromes genéticos, o secundario a trastornos muy diversos. Dentro de las formas secundarias, la osificación subcutánea de las piernas por insuficiencia venosa crónica es una complicación infrecuente y poco reportada. Presentamos un paciente con osificación subcutánea de las piernas secundaria a insuficiencia venosa y revisamos la literatura

Manifestations of chronic cutaneous lupus erythematosus are variable. Periorbital and facial swelling occurs in dermatomyositis and systemic lupus, but it has been rarely reported as a manifestation of exclusively cutaneous lupus. A 48-year-old woman presented with a 16-year history of asymptomatic, bilateral swelling and erythema of her face with marked worsening after sun exposure. No systemic symptoms were associated. A complete evaluation did not reveal other findings. Cutaneous biopsy showed features of lupus erythematosus. She was treated with photoprotection, topical tacrolimus, hydroxychloroquine and azathioprine with a partial response. Facial swelling with erythema represents quite an unusual manifestation of chronic cutaneous lupus erythematosus. Dermatomyositis, systemic lupus and Morbihan disease are the main differential diagnoses. LEARNING POINTS: Periorbital and facial swelling with erythema are clinical manifestations of dermatomyositis and systemic lupus erythematosus. However, these manifestations represent quite an unusual presentation of chronic cutaneous lupus erythematosus.The periorbital area is most frequently affected, while extensive facial involvement is much more unusual.A complete evaluation and cutaneous biopsy are essential to make the diagnosis and to rule out other disorders such as dermatomyositis, systemic lupus erythematosus and Morbihan disease.

In the skin, it can be primary, in association with genetic syndromes, or be secondary to different disorders. The latter include subcutaneous ossification of the legs in chronic venousinsufficiency, an infrequent and unrecognized complication. We report the case of a patient with subcutaneous ossification of both legs secondary to venous insufficiency and review the literature.

We present a methodology for the automatic recognition of negated findings in radiological reports considering morphological, syntactic, and semantic information. In order to achieve this goal, a series of rules for processing lexical and syntactic information was elaborated. This required development of an electronic dictionary of medical terminology and informatics grammars. Pertinent information for the assembly of the specialized dictionary was extracted from the ontology SNOMED CT and a medical dictionary (RANM, 2012). Likewise, a general language dictionary was also included. Lexicon-Grammar (LG), proposed by Gross (1975; Cahiers de l'institut de linguistique de Louvain, 24. 23-41 1998), was used to set up the database, which allowed an exhaustive description of the argument structure of predicates projected by lexical units. Computational framework was carried out with NooJ, a free software developed by Silberztein (Silberztein and Noo 2018, 2016), which has various utilities for treating natural language, such as morphological and syntactic grammar, as well as dictionaries. This methodology was compared with a Spanish version of NegEx (Chapman et al. Journal of Biomedical Informatics, 34(5):301-310 2001; Stricker 2016). Results show that there are minimal differences in favor of the algorithm developed using NooJ, but the quality and specificity of the data improves if lexical-grammatical information is added.

RESUMEN

Plasmacytoid blast dendritic cell neoplasia (NCDBP) is an uncommon malignant neoplasm, presenting clinically with cutaneous involvement and subsequent lymph node and bone marrow extension. It characteristically expresses the markers: CD56, CD4, and CD123. There is no optimal treatment, relapses are frequent, and the survival time is short. We present the case of an elderly patient with NCDBP who initially presented with cutaneous lesions, but experienced rapid systemic progression and did not, respond to treatment.

Sacrococcygeal chordoma is a malignant tumour originating from remnants of the notochord. Chordomas are slow-growing tumours whose symptoms develop insidiously. We present the case of a 72-year-old woman with a 6-month history of genital pain radiating to the perianal area and exacerbating when she was in a sitting position. MRI and PET studies revealed a large mass in the sacrococcygeal region causing bone destruction and invasion of neurovascular structures. The immunohistochemical study of the surgical specimen determined it to be chordoma. This is the first published case of pudendal neuralgia as a form of presentation of sacrococcygeal chordoma.

Intimate partner violence (IPV) is a significant worldwide problem whose magnitude and risk factors vary across different settings and cultures. Nowadays, it is a priority to improve the knowledge on this issue in order to formulate better evidence-based policy responses. This study aims to contribute to a better understanding of non-fatal IPV against women in Spain. A retrospective analysis of IPV cases with a final judicial decision was carried out. The period under study extended from January 2005 to December 2012, with a total of 582 files included in the investigation. Most IPV victims were young adult women of Spanish origin, either married or single, with children, unemployed and with a low family income level. The majority of alleged perpetrators were young adults, employed, with a middle-low income level, a history of alcohol consumption/abuse, but no criminal records. Most victims had previous history of IPV, were engaged in a long-term relationship with their abuser and lived with him at the time of assault. The combination of psychological and physical abuse was the most frequent form of violence. The most common mechanisms of assault consisted in minor acts of physical violence, which resulted in mild injuries, most of them in the upper limbs and face. Nearly half of women sought medical care, but physician's injury report was only made in about a quarter of these cases, even though it is mandatory for health professionals. The majority of criminal proceedings were initiated by the victim's report and ended in conviction, most of them being considered occasional mistreatment. This study confirms the heterogeneity of the phenomenon of intimate partner violence. The importance of adopting standard IPV concepts and promoting the recognition and assessment of this form of violence amongst health care professionals, criminal investigators and forensic personnel is emphasized.
