RESUMEN

Com o avanço das técnicas e materiais restauradores, o cirurgião-dentista tornou-se capaz de solucionar problemas estéticos de forma conservadora. Esse relato de caso descreve a associação das técnicas de microabrasão e fechamento de diastemas com resinas compostas diretas. A paciente do sexo feminino, 16 anos de idade, foi encaminhada pelo ortodontista à clínica de Dentística Restauradora para o fechamento de diastema. Após anamnese e exame clínico detalhado, constatou-se, também, a presença de manchas brancas de fluorose na face vestibular dos dentes anterossuperiores, sendo também proposto o procedimento de microabrasão. Por meio de modelos de estudo da paciente, foi realizado o enceramento diagnóstico, de forma a simular o fechamento dos diastemas. O modelo encerado foi moldado com silicona de condensação para servir como guia de orientação durante os procedimentos restauradores. O tratamento resultou na elevação da autoestima da paciente,pela obtenção de um sorriso mais harmonioso por meio da associação de técnicas estéticas conservadoras.


With the improvements in restorative techniques and materials, the dentist is able to conservatively solve esthetic problems. This case report describes the association of techniques for microabrasion and closure of diastema with direct composite resins. The patient, aged 16, was referred to the restorative dental clinic by the orthodontist for diastema closure. After a detailed clinical examination, it was also noticed the presence of fluorosis white stains on the buccal surface of anterior-superior teeth, and microabrasion treatment was proposed. Through the study cast models of the patient, diagnostic waxing was performed to simulate the closure of diastema. The wax model was cast with condensation silicone to serve as a guideline during the restorative procedures. The treatment resulted in patient's improved self-esteem by obtaining a more harmonious smile through the association of conservative esthetic techniques

The aim of this study was to evaluate the influence of different restorative procedures on the fracture resistance of endodontically treated teeth submitted to intracoronal bleaching. Fifty upper central incisors were distributed into 5 groups: GI - healthy teeth; GII - endodontically treated teeth sealed with Coltosol; GIII - endodontically treated teeth bleached and sealed with Coltosol; GIV - endodontically treated teeth bleached and restored with composite resin; and GV - endodontically treated teeth bleached and restored with a fiberglass post and composite resin. In the bleached specimens, a cervical seal was made prior to bleaching with 38% hydrogen peroxide. The gel was applied on the buccal surface and in the pulp chamber, and was then light-activated for 45 s. This procedure was repeated three times per session for four sessions, and each group was submitted to the restorative procedures described above. The specimens were submitted to fracture resistance testing in a universal testing machine. There were statistically significant differences among the groups (p < 0.05). The mean value found for GIII was the lowest (0.32 kN) and was significantly different from the values found for GI (0.75 kN), GII (0.67 kN), GIV (0.70 kN), and GV (0.72 kN), which were not significantly different from each other (p > 0.05). The restorative procedures using composite resin were found to successfully restore the fracture resistance of endodontically treated and bleached teeth.

Este estudo teve o objetivo de investigar a influência de diferentes procedimentos restauradores diretos com resina composta e pinos de fibra de vidro, na resistência à fratura de dentes com cavidades Classe IV, tratados endodonticamente e submetidos ao clareamento interno. Quarenta (n=40) incisivos laterais superiores humanos hígidos foram utilizados. Todos os dentes, com exceção do Grupo controle, foram submetidos ao tratamento endodôntico e então divididos em 5 Grupos (n=8): GI: Dentes hígidos; GII: Dentes tratados endodonticamente + resina composta (RC); GIII: Dentes tratados endodonticamente + clareamento com peróxido de hidrogênio (PH) a 35% + RC; GIV: Dentes tratados endodonticamente + Clareamento com PH a 35% + pino de fibra de vidro (Everstick/StichTeck) + RC; GV: Dentes tratados endodonticamente + Clareamento PH a 35% + RC e pino de fibra de vidro (Fibrekor Post; Jeneric Pentron Inc., Wallingford, CT, USA);. Os dentes tratados endodônticamente receberam cortes na coroa simulando uma classe IV e posteriormente restaurações de resina composta. O sistema LED-LASER Whitening Lase Light Plus (DMC Equipamentos Ltda, São Carlos, SP, Brasil) foi utilizado para ativação do agente clareador. Após o processo de clareamento, a restauração provisória e o tampão cervical dos dentes submetidos ao clareamento foram removidos para que cada Grupo fosse submetido ao procedimento restaurador. Após a confecção das restaurações as mesmas foram submetidas ao teste de resistência a fratura, em máquina de ensaios (EMIC). A Análise de Variância apontou efeito significativo de Grupos sobre a resistência (p=0,041). O teste de Tukey foi aplicado nas comparações múltiplas das médias de resistência, identificando somente a média de GIII (dentes tratados endodonticamente + clareamento com PH a 35% + RC) significativamente menor do que a GI ( dentes Hígidos) (p=0,026). Concluiu-se que os dentes com cavidade classe IV, tratados endodonticamentes, submetidos ao clareamento dental interno com PH a 35% com aceleração de luz e restaurados apenas com RC, apresentam resistência a fratura comprometida quando comparado aos dentes hígidos. Quando os dentes tratados endodonticamente forem submetidos ao claremaento interno, o uso de pinos é recomendado, quando o clareamento interno não for realizado, a restauração direta pode ser realizada apenas com RC


This study aimed to investigate the influence of restorative procedures on fracture resistance of teeth with Class IV cavities endodontically treated and submitted to internal bleaching with light source acceleration using LED-LASER system. Forty (n = 40) lateral incisors and healthy humans were used. All teeth, except for the control Group, were submitted to endodontic treatment and then divided into 5 Groups (n = 8): GI: healthy teeth, GII: endodontically treated teeth + composite, GIII: Teeth whitening endodontically treated with PH 35% + composite, GIV: Teeth whitening endodontically treated with PH 35% glass fiber post (Everstick / StichTeck) + composite, GV: Teeth whitening endodontically treated PH 35% + composite fiber and glass post (Fibrekor Post; Jeneric Pentron Inc., Wallingford, CT, USA). The crow of endodontically treated tooth was cut simulating a Class IV. The system LED LASER-Light Plus Whitening Lase (DMC Equipment Ltda, São Carlos, SP, Brazil) was used to activate the bleaching agent. After the whitening process, the buffer cervical and the temporary restoration of teeth submitted to bleaching have been removed so that each Group was subjected to the restorative procedure. After restorations the specimens were subjected to the same fracture toughness test, in a testing machine (EMIC). Analysis of Variance (ANOVA) showed significant effect on the resistance Group (p = 0.041). The Tukey test was applied in multiple comparisons for resistance means values, identifying only the mean of GIII (endodontically treated teeth bleaching with PH 35% + composite) significantly lower than GI (healthy teeth) (p = .026). It was concluded that teeth endodonticamentes treated with cavity class IV, subjected to internal dental bleaching with PH 35% and only restored with RC, have lower fracture toughness when compared to healthy teeth. When endodontically teeth were submitted to internal bleaching use of pins is recommended. And when internal bleaching is not carried out, the direct restoration can be done only with RC

PURPOSE: The use of different light sources as an adjunct to in-office bleaching has been questioned. Thus, the aim of this study was to evaluate the color changes of teeth after application of bleaching techniques with different products, with and without activation by a LED-laser system. METHODS: Twenty-four bovine teeth surfaces were submitted to three bleaching techniques with two commercially available 35 percent hydrogen peroxide bleaching agents (n=8). The specimens were immersed in red wine for 48 h at 37°C and submitted to the bleaching techniques. Color changes were measured before and after staining as well as immediately after and 24 h after the bleaching treatments, with two different methods of color evaluation, software ScanWhite V1.1 and intra-oral spectrophotometer (Vita Easyshade). Data were analyzed by ANOVA and Kruskal-Wallis test. RESULTS: The statistical analysis showed that there was no statistically significant difference at 5 percent of significance level between the different groups, independently of the evaluation time, evaluation methods or the use of LED-laser systems. CONCLUSION: The results suggested that the use of light in the bleaching techniques did not influence the color changes.


OBJETIVO: o uso de diferentes fontes de luz como auxiliar no clareamento de consultório tem sido descrita na literatura.Assim, o objetivo deste estudo foi avaliar as alterações da cor dos dentes após a aplicação de técnicas de clareamento com diferentes produtos, com e sem ativação por um sistema de LED-laser. METODOLOGIA: Vinte-quatro superfícies de dentes bovinos foram submetidas a três técnicas de clareamento com dois agentes clareadores disponíveis comercialmente à base de peróxido de carbamida a 35 por cento (n=8). Os espécimes foram inicialmente imersos em vinho tinto por 48 ha 37°C, e submetidos às técnicas de clareamento. As alterações de cor foram mensuradas antes e após o manchamento, bem como imediatamente e 24 hs após as técnicas de clareamento, por meio de dois diferentes métodos de avaliação da cor: software ScanWhite V1.1 e espectrofotômetro intra-oral Vita Easyshade. Os dados foram analisados pelos testes de ANOVA e Kruskal-Wallis. RESULTADOS: A análise estatística não apontou diferenças significantes entre os diferentes grupos testados, independentemente do período de avaliação, método de avaliação ou uso do sistema LED-laser. CONCLUSÃO: Os resultados sugeriram que o uso da luz não influenciou nas mudanças de cor.
