OBJECTIVES: To investigate the factors associated with the uptake of influenza A/H1N1 vaccination by university students, and to examine the relationship between intention and actual vaccination. STUDY DESIGN: Prospective cohort study. METHODS: A baseline survey was conducted among students at the Chinese University of Hong Kong in 2009 to collect data on demographics, pandemic risk perceptions and self-reported intention to be vaccinated in the future. A follow-up survey was conducted in 2011 using an online survey platform collecting information on actual vaccine uptake behaviour, and vaccine attitudes, knowledge and perceptions. RESULTS: Self-reported intention to be vaccinated in 2009 was significantly associated with actual vaccine acceptance. Vaccine perceptions (attitudes and knowledge) were found to be a better predictor of vaccine acceptance than disease risk perceptions. Being a medical or science student and receiving health advice about vaccination from a doctor or school-endorsed advertisement were also found to be predictors of vaccine acceptance. CONCLUSIONS: University students in Hong Kong were wary of the A/H1N1 vaccination campaign, as revealed by their low uptake rate and doubts about the vaccine. Knowledge of the pandemic and vaccine was high in this population, but feelings of susceptibility were low. The results indicate a need to provide tailored messages emphasizing the importance of vaccination and the efficacy of the vaccine in the future.

Introducción: El virus de influenza pandémica A (H1N1), cuya circulación se inició en abril del año 2009 en México y Estados Unidos, se constituyó en el último virus pandémico desde los casos detectados en Hong Kong en 1968. El genoma del virus de influenza A está formado por 8 segmentos ARN de cadena simple (polaridad negativa), que codifican para 10 proteínas. Los genes hemaglutinina y neuraminidasa codifican para dos proteínas de superficie y son los utilizados en los análisis de variabilidad genética. Objetivos: a) Detectar la circulación del virus pandémico en pacientes con sospecha clínica de infección por influenza, y b) Diseñar una estrategia para amplificar de forma completa los genes hemaglutinina y neuraminidasa. Materiales y Métodos: Fueron analizados por Real-Time RT-PCR (transcripción reversa y reacción en cadena de la polimerasa en tiempo real) un total de 181 muestras de hisopado faríngeo, colectadas o remitidas al Hospital de Clínicas, del 6 de agosto al 11 de octubre de 2009. Para el diseño de amplificación de los genes hemaglutinina y neuraminidasa, se han utilizado herramientas bioinformáticas y reacción en cadena de la polimerasa. Resultados: Del total de muestras analizadas, 27 (14.9 %) dieron resultado positivo para el nuevo virus pandémico. Por otra parte, la amplificación completa de ambos genes proporcionó los resultados esperados: 1678-pares de bases (pb) para la hemaglutinina, y 1427-pb para la neuraminidasa. Conclusiones: La implementación de esta tecnología de amplificación permitirá posteriormente la secuenciación de estos genes a fin de determinar las variaciones genéticas del virus que podrían tener un impacto en la salud humana.


Introduction: The pandemic influenza A (H1N1) virus, whose circulation was detected in April 2009 in Mexico and the United States, is the latest pandemic virus since the cases reported in Hong Kong in 1968. The genome of the influenza A virus consists of 8 segments of single-stranded RNA of negative polarity, coding for 10 proteins. The hemagglutinin and neuraminidase genes encode for two surface proteins and are used in the analysis of genetic variability. Objectives: a) to detect circulation of the pandemic virus in patients with clinical suspicion of influenza infection and b) design a strategy to fully amplify the hemagglutinin and neuraminidase genes.Materials and Methods: A total of 181 pharyngeal swabs were collected and sent to the Hospital de Clínicas for analysis using Real-Time RT-PCR (reverse transcription and polymerase chain reaction in real time) between 6 August and 11 October 2009. To design the amplification of hemagglutinin and neuraminidase genes, we used bioinformatic tools and polimerase chain reaction. Results: Of the samples analyzed, 27 (14.9%) were positive for the new pandemic virus. Moreover, the complete amplification of both genes provided the expected results: 1678-base pairs (bp) for the hemagglutinin, and 1427-bp for neuraminidase. Conclusions: The use of this technology for amplification will eventually allow sequencing to identify genetic variations of the virus that could have an impact on human health.

La determinación del antígeno prostático específico (PSA) forma parte del diagnóstico del cáncer de próstata. Como en condiciones patológicas sus niveles aumentan, es considerado marcador tumoral útil de diagnóstico de cáncer de próstata en forma precoz. Determinamos los niveles séricos de PSA, dentro de la campaña “Semana de la Próstata” organizado por la Cátedra de Urología del Hospital de Clínicas en Octubre 2007. De los 89 pacientes, el 86,5% presentó niveles de PSA entre 0 y 4ng/ml, 10,1% entre 4 y 10 ng/ml y el 3,4% entre 10 y 40 ng/ml respectivamente. Se realizó una distribución por edad y se determinaron las medias de los valores de PSA en los mismos. El 12,4% del grupo E1 (41 a 50 años) con 0,5ng/ml de PSA, el 52,8% del grupo E2 (51 a 60 años) con 7,4ng/ml de PSA, el 28,1% del grupo E3 (61 a 70 años) con 5,2ng/ml de PSA y el 6,7% del grupo E4 (71 a 80 años) con 1,5 ng/ml de PSA. Hallándose valores más elevados de PSA en el grupo E2 y E3, no así en el grupo E4. En relación al tacto rectal (TR) y los valores del PSA, el 31,5%(28) presentaron TR normal con un valor medio de PSA de 3,4. Mientras que el 65,1% (58) presentaban TR patológico con valores medios de PSA de 7,17 en 55 pacientes y sólo 3 pacientes con TR patológico presentaron niveles de PSA por debajo de 2,5 ng/ml. El TR resultó ser la variable con mayor poder de discriminación, con respecto al resultado de PSA en estos pacientes.


The determination of prostate-specific antigen (PSA) is part of the diagnosis of prostate cancer.It is considered an useful tumor marker for early diagnosis of porostate cancer because in pathological conditions its levels increase.Serum levels of PSA were determined within the campaign "Prostate Week" organized by the Department of Urology of the Hospital de Clínicas in October 2007.;Of the 89 patients, 86.5% had PSA levels between 0 and 4 ng/ml, 10.1% between 4 and 10 ng/ml and 3.4% between 10 and 40 ng/ml respectively. An age distribution was made and the mean of PSA values were determined in each group. Twelve point four percent of group E1 (41 to 50 years) had 0.5 ng/ml of PSA, 52.8% of group E2 (51 to 60 years) 7.4 ng/ml PSA, 28.1% of E3 group (61 to 70 years) 5.2 ng/ ml of PSA and 6.7% of the E4 group (71 to 80 years) had 1.5 ng/ml of PSA.The highest values of PSA were found in E2 and E3 groups, but not in the E4 group. In relation to digital rectal examination (DRE) and PSA values, 31.5% (28) showed normal DRE with a mean value of PSA of 3.4 while 65.1% (58) had pathological DRE with mean values of PSA of 7.17 in 55 patients and only 3 patients had pathological TR with PSA levels below 2.5 ng/ml. The DRE was the variable with the greatest ability to discriminate in relation to the results of PSA in these patients.

RESUMEN

UNLABELLED: Over 100 years viruses have fascinated scientists around the world. Although biologists, chemists, physicians, veterinarians, and even physicists attempted to elucidate the nature of viruses, the question still remains "Are viruses alive?" Different theories have aimed at unifying our views of virology to provide an answer. However, the discovery of a mimivirus, its genome organization and replication cycle, in addition to the recently found virophage challenged the established frontier between viruses and parasitic cellular organisms. Consequently, the old controversy whether viruses are inert agents at the threshold of life or a different form of life was reignited. This review reopens the debate about the living nature of viruses from the classical concepts to the recent discoveries in order to rationally discuss our beliefs about the living or non-living character of viruses. La vacunación rutinaria de difteria, pertussis y tétanos a células enteras (DPTw) está presente desde 1940 y con elevadas coberturas en muchos países del mundo. La Organización Mundial de la Salud ha aprobado el uso universal de la vacuna anti-hepatitis B (HB) y combinaciones con DPT, en los últimos años ha sido incorporada la vacuna anti-Haemophilus influenzae tipo b (Hib) en programas de vacunación del niño. Es aplicada en Paraguay desde el 2002 a través del Programa Ampliado de Inmunizaciones del Ministerio de Salud Pública y Bienestar Social. Determinamos la reactoinmunogenicidad secundaria a la vacunación primaria pentavalente combinada en infantes concurrentes al Hospital Distrital de Lambaré­Paraguay en los años 2007-2008. Estudio longitudinal, observacional prospectivo de los efectos secundarios y los aspectos inmune- específicos de la vacuna Berna DTPw-HepB-Hib (QUINVAXEMTM) en lactantes menores de un año, a los 2 meses de edad, datos basales y post vacunales (1 mes luego de 3ª dosis). Efectos locales: 30(75%); rubor 17 (42.5%); tumefacción (menos 20 mm); 13 (32.5%); calor local 11 (27.5%). Efectos generales: fiebre: 37 (92.5%) llanto fuerte y persistente: 32(80%); irritabilidad: 23 (57.5%); hiporreactividad 16 (40%), anorexia 8 (20%); Inmunogenicidad: antes de la 1ª dosis; antitetánica IgG (+) 38/40 (95%), anti-difteria (+) IgG 29/40 (72.5%); anti-HBsAg 0/40 (0%) negativos. Respuesta post-vacuna penta comb. (7m. edad): antitetánica IgG 14/14 (100%) (+); anti-difteria IgG 12/14 (83%) (+); anti-HBsAg 14/14 (100%) positivos. Se evidencia la reactogenicidad de grado variable y decreciente. Niveles de anticuerpos de los componentes DT, satisfactorios y la Hep B excelentes


The combined cellular pentavalent vaccine is one the greatest achievements of human kind in the 20th century and is still successful in the 21st century. It is made up of five components and protects against the following diseases: diphtheria, tetanus, pertussis, hepatitis B and Haemophilus influenzae type b. The vaccine provides specific active immunization against infections caused by Corynebacterium diphtheriae, Clostridium tetani, Bordetella pertussis, Haemophilus influenzae type B and the Hepatitis B virus in children from six weeks of age. In Paraguay, it has been used since 2002 through the Widened Program of Immunization (PAI in Spanish) of the Ministry of Public Health. The objective of this work was to determine the secondary reactoinmunogenicity to the primary combined pentavalent vaccination in children attending the District Hospital of Lambaré, Paraguay in 2007 and 2008. This is a longitudinal, prospective observational study to evaluate the secondary effects and immune-specific aspects of the Berna DTPw-HepB-Hib combined pentavalent vaccine (QUINVAXEMTM) applied to breastfed babies under one year old, of both sexes, all races, origins and nutrition statuses in the Hospital of Lambaré. The vaccination was free, according to PAI's guidelines and by consecutive sampling previous written consent of the parents, at 2 months of age. Data were collected at baseline an post-vaccination after one month and a third dose. In a total population of 40 breastfed babies, good nutrition was found in 36 (90%). Local effects were seen in 30 (75%) distributed as follows: blush in 17 (42.5%), tumefaction (less than 20 mm) in 13 (32.5%) and local heat in 11 (27.5%). General effects were distributed as follows: fever in 37 (92.5%), loud and persistent cry in 32 (80%), irritability in 23 (57.5%), hyporeactivity in 16 (40%), anorexia in 8 (20%), hypotonicity and allergy, 2 each (5%), convulsions and paralysis in none. The effects are an average after the three doses with a decreasing trend at the end. The immunogenicity basal data before the 1st dose were as follows: antitetanic IgG (+) 38/40 (95%), antidiphtheria IgG (+) 29/40 (72.5%) and anti HBs Ag (-) 0/40 (0%). Post - vaccination responses (at 7 months old) were as follows: antitetanic IgG (+) 14/14 (100%), antidiphtheria IgG (+) 12/14 (83%) and anti HBs Ag (+) 14/14 (100%). These results show a reactogenicity of variable and decreasing degree. The antibodies levels of the DT components were satisfactory and those of Hep B excellent

El dengue en Paraguay es una infección de circulación relativamente reciente ya que la transmisión fue detectada por primera vez en los años 1988-89. El comportamiento de la enfermedad y la circulación de los tres serotipos (DEN 1, 2 y 3), entre los años 2000 y 2006, muestra el elevado riesgo de epidemias y de aparición del dengue hemorrágico. Nuestro objetivo fue determinar la prevalencia de IgG e IgM del virus dengue en individuos adultos, de ambos sexos, provenientes de áreas urbanas y rurales del Paraguay, que concurrieron al Instituto de Investigaciones en Ciencias de la Salud entre febrero y abril del 2007. Se realizó un estudio observacional, descriptivo donde fueron incluidos 605 pacientes. Las determinaciones de IgM e IgG se realizaron por el método de ELISA de captura e indirecto respectivamente. De los 605 pacientes, 64% (388/605) pertenecían al sexo femenino, con edad promedio de 31,9 ? 13,5 años. Del área urbana provinieron el 80% (482/605), 13% (79/605) del área rural y del 7% restante no se obtuvieron datos. Del total de pacientes estudiados, 20 % (121/605) fueron positivos para IgM, de quienes 55 % (67/121) eran del sexo femenino. La determinación de IgG, se realizó en 157 pacientes (según solicitud), de los cuales 61% (96/157) resultaron negativos y 39 % (61/157) positivos. Según la frecuencia encontrada, es recomendable un plan integral sostenible en la lucha contra el dengue, para desarrollar estrategias de vigilancia epidemiológica, prevención y control de esta enfermedad considerando el alto riesgo para la ocurrencia de brotes de dengue hemorrágico


In Paraguay, dengue is an infection with a relatively recent circulation because it was first detected in the period 1988-89. The disease behaviour and circulation of the three serotypes (DEN 1, 2 and 3) between 2000 and 2006 show the high risk of epidemics and the appearance of the hemorragic dengue. Our objective was to determine the prevalence of IgG and IgM antibodies against dengue virus in male and female adult subjects, from urban and rural areas of Paraguay, which attended the IICS between February and April 2007. An observational, descriptive study was carried out including 605 patients and IgG and IgM antibodies were determined by using MAC ELISA and indirect ELISA respectively. Out of the 605 studied patients, 64% (388/605) was women and the population average age was 31.9±13.5 years. Eigthy percent (482/605) came from urban areas and 13% (79/605) from rural areas. Out of the 605 subjects, 20% (121/605) were IgM positive being 55% (67/121) of them females. The IgG determination was made in 157 patients (by request), 61% (96/157) were negative and 39% (61/157) positive. According to the frequency found in this study, it is recommendable to implement an integral and sustainable plan against dengue in order to develop epidemiologic surveillance, prevention and control strategies considering the high risk of occurrence of hemorrhagic dengue outbreaks

La Fiebre Amarilla (FA) es una de las más importantes zoonosis que afecta a poblaciones humanas. La FA silvestre es imposible de ser erradicada, manteniéndose activa en zonas tropicales en África y Sudamérica. Todas las especies de primates son susceptibles y se consideran reservorios en el medio silvestre. La mortalidad es baja, se desconoce su valor con precisión, sin embargo existen epizootias con alta mortalidad, en humanos varía entre 20-50%. El objetivo de este trabajo fue buscar evidencias de FA en primates capturados en áreas de brote de FA de los departamentos de San Pedro y Central del Paraguay mediante la técnica de Neutralización por reducción de placas para FA cepa vacunal 17 D. Los resultados en los 35 primates estudiados fueron negativos, quizás por lo tardío del momento en la toma de muestras y bajo número de primates capturados.


Yellow Fever (YF) is one of the most important zoonotic diseases affecting human population. It is impossible to eradicate wild YF remaining active in tropical zones of Africa and South America. All species of primates are susceptible and are considered reservoirs in wild regions. Mortality is low and its precise value is unknown though there are epizootics with high mortality rates and in humans varies between 20-50%. The objective of this study was to search for evidence of YF in primates caught in YF outbreaks areas of the departments of San Pedro and Central in Paraguay through the neutralization technique by plates reduction for YF vaccine strain 17 D. The results in the 35 primates studied were negative, perhaps because of the lateness of the time sampling and the low number of captured primates.

The disease behaviour and circulation of the three serotypes (DEN 1, 2 and 3) between 2000 and 2006 show the high risk of epidemics andthe appearance of the hemorragic dengue. Our objective was to determine the prevalence of IgG and IgM antibodies against dengue virus in male and female adult subjects, fromurban and rural areas of Paraguay, which attended the IICS between February and April 2007. An observational, descriptive study was carried out including 605 patients and IgG and IgM antibodies were determined by using MAC ELISA and indirect ELISA respectively. Out of the 605 studied patients, 64% (388/605) was women and the population averageage was 31.9±13.5 years. Eigthy percent (482/605) came from urban areas and 13% (79/605) from rural areas. To determine clinical and epidemiological features of scorpion stings in two departments of Colombia, a descriptive study was performed in the hospitals of 10 towns from Antioquia (2 256 071 inhabitants) and five from Tolima (630 424 inhabitants). One hundred and twenty-nine cases were admitted during one year, 51 in Antioquia, 78 in Tolima and 41 were children less than 15 years old. Most stings (70.5%) occurred inside the house; 27.9% were on the hands and 26.4% on the feet. The scorpion species involved were Tityus pachyurus (51), Centruroides gracilis (31), T. fuehrmanni (29), T. asthenes (7) and Chactas spp. (1). In 10 cases the scorpion involved was not identified. Systemic envenoming signs (e.g. vomiting, tachypnea) were significantly more frequent in children than in adults (P < 0.05). Four children had hypertension, but none developed pulmonary oedema. One 3-year-old girl, stung by T. asthenes, had acute oedematous pancreatitis. Ninety-eight patients had mild envenoming. Moderate (27 patients) and severe (four patients) envenoming was significantly more frequent in children than in adults (P = 0.003; relative risk = 2.97). A pepsin-digested anti-Centruroides spp. antivenom was administered to 19 of 31 patients presenting systemic envenoming signs. No adverse reactions to antivenom were observed.

Lymphocytic chorimeningitis virus (LCMV), the prototype arenavirus, and Lassa virus (LASV), causative agent of Lassa hemorrhagic fever (LHF), belong to the Old World group of the family Arenaviridae. Both viruses have extensive strain diversity and significant variations in lethality and pathogenicity for man and experimental animals. We have shown that the LHF-like infection of rhesus macaques with the WE strain of LCMV affects liver functions, induces hepatocyte proliferation, and causes a rise in IL-6 and soluble TNF receptors (sTNFR) concomitant with a rise in viremia. The levels of IL-6 and sTNFR can serve as an additional diagnostic tool for liver involvement in pathogenesis of arenavirus infection. Mucosal inoculation of rhesus macaques with LCMV-WE can result in attenuated infection with a transient viremia and liver enzyme abnormalities. The ARM strain of LCMV shares 88% amino acid homology with WE. In contrast to LCMV-WE, ARM strain does not induce manifested disease in monkeys, does not affect liver functions, and does not induce hepatocyte proliferation. Previously we demonstrated that LCMV-ARM infection protected rhesus macaques challenged with LCMV-WE. Here we have shown that the protected animals have no signs of hepatitis and hepatocyte proliferation.

Objetivo: comparar el modo de presentación y manejo de la enfermedad en dos Servicios de cirugía de poblaciones geográficas distintas. Diseño: estudio observacional, retrospectivo. Pacientes y método: 203 casos de la 2ª Cátedra de Cirugía del Hospital Universitario de Clínicas de Asunción, Paraguay y 150 casos del Hospital Universitario de Lleida, España. Se analizaron: motivo de ingreso, antecedentes, tratamiento y morbi-mortalidad postoperatoria. Resultados: serie paraguaya: edad media hombres 53 (3484) años y 62 (36-92) mujeres. 55 por ciento ingresaron por complicaciones hemorrágicas y 45 por ciento por diverticulitis aguda. El tratamiento médico se inició en 109 casos y quirúrgico en 110, 72 de urgencia y 38 electivas. La morbilidad fue del 31,8 por ciento (40,2 por ciento en urgencia y 16 por ciento programada, (p<0,05) y la mortalidad del 15,5 por ciento, 22,2 por ciento urgentes y 2,6 por ciento programados p<(0,003).Serie de Lleida: edad media hombres 65,5 (38-85) p<0,005 y mujeres 71,4(30-93) p<0,01) años. 86 por ciento ingresaron por diverticulitis aguda, 10,7 por ciento por hemorragia (p<0,001). Presentaron crisis previa el 16 por ciento (p<0,001). El tratamiento fue médico en 111 casos y quirúrgico en 39 (p<0,001); 33 de urgencia y 6 electivas (p<0,04). La morbilidad fue de 41 por ciento y la mortalidad de 5 (12,8 por ciento), todos de urgencia. La técnica operatoria utilizada fue similar: resección con anastomosis primaria en las electivas y mayoría de Hartmann, en las urgencias, con una proporción de anastomosis inmediata del 33 y 21 por ciento. Conclusiones : posiblemente factores dietéticos y de calidad de vida influyen en la aparición de la enfermedad diverticular. Hay que prevenir la cirugía de urgencia. Cada cirujano debe adaptar su estrategia quirúrgica al medio socio-económico-cultural de la población (AU)

OBJECTIVES: Lassa fever virus (LAS) is transmitted to man by rodent carriers and is fatal in a third of untreated cases. Our goal is to provide immune protection from Lassa fever by mucosal vaccination. STUDY DESIGN/METHODS: Mice were vaccinated intragastrically with control vectors or with vectors (vaccinia or Salmonella) expressing LAS nucleocapsid protein (NP). Mice were challenged intracranially with a lethal dose of the related arenavirus, lymphocytic choriomeningitis virus (LCMV), as a measure of the vaccine's ability to elicit cross-protection. RESULTS: Salmonella and vaccinia vectors expressing LAS NP each protected a third of the mice from lethal challenge with LCMV. All mice vaccinated with a vector expressing LCMV NP were protected as expected. CONCLUSIONS: The LAS recombinant Salmonella vector is comparable to the LAS recombinant vaccinia vector in its ability to cross-protect mice from lethal challenge. Nucleocapsid protein is an inadequate immunogen on its own, but provides sufficient cross-protection to make it a useful component of a broadly reactive arenavirus vaccine.

Lymphocytic choriomeningitis virus (LCMV) induces type I interferon (alpha and beta interferon [IFN-alpha and IFN-beta]) upon infection and yet is sensitive to the addition of type II interferon (gamma interferon [IFN-gamma]) to the culture media. This sensitivity is biologically important because it correlates inversely with the ability of certain LCMV strains to persist in mice (D. Moskophidis, M. Battegay, M. A. Bruendler, E. Laine, I. Gresser, and R. M. Zinkernagel, J. Virol. 68:1951-1955, 1994). The cellular oncoprotein PML is induced by both IFN-alpha/beta and IFN-gamma, and PML binds the LCMV Z protein and becomes redistributed within cells from nucleus to cytoplasm upon LCMV infection. In the present study, increased PML expression results in diminished LCMV replication, implicating PML in the IFN sensitivity of LCMV. Virus production in PML -/- murine embryonic fibroblasts (MEF) exceeds virus production in PML +/+ MEF, and this difference is exacerbated by IFN treatment that upregulates PML expression. IFN-gamma also diminishes LCMV production in PML -/- cells; therefore, viral IFN sensitivity is not entirely due to PML. Both viral mRNA production and viral protein production decrease as PML expression increases. Here we propose that PML reduces LCMV transcription through its interaction with the Z protein.

OBJECTIVE: To compare the form of presentation and management of the disease in two surgical units covering geographically different populations. DESIGN: Observational retrospective study. PATIENTS AND METHOD: 203 cases from the 2nd Chair of Surgery of the Hospital Universitario de Clínicas de Asunción, Paraguay and 150 cases from the Hospital Universitario de Lleida, Spain. We analyzed the cause of admission, medical history, treatment and post-operative morbidity and mortality. RESULTS: Paraguay series: average age: 53 years (range 34-84) for men and 62 years (range 36-92) for women. Fifty-five per cent were hospitalized because of hemorrhagic complications and 45% because of acute diverticulitis. Medical treatment was provided in 109 cases and surgery in 110, 72 of which were emergencies and 38 elective procedures (p < 0.05). Morbidity was 31.8% (40.2% in emergencies and 16% in elective procedures, p < 0.05) and mortality was 15.5% (20.2% in emergencies and 2.6% in elective procedures, p < 0.003). LLEIDA SERIES: Average age: 65.5 years (range 38-85, p < 0.01) for men and 71.4 years (range 30-93, p < 0.01) for women. Eighty-six per cent were hospitalized because of acute diverticulitis and 10.7% because of acute hemorrhage (p < 0.001). Sixteen per cent have had previous attacks (p < 0.001). Medical treatment was provided to 111 patients and surgery to 39 (p < 0.001), 33 of which were emergencies and 6 elective procedures (p < 0.04). Morbidity was 41% and mortality 12.8% (5 cases), all of which were emergencies. The surgical technique was similar in both groups: resection with primary anastomosis in elective procedures and Hartmann's procedure in most emergencies, with a rate of immediate anastomosis of 33 and 21%, respectively. CONCLUSIONS: Factors related to nutrition and quality of life may influence the development of diverticular disease. Emergency surgery should be prevented. Surgeons must adapt their surgical approach to the socioeconomic and cultural medium of the population.

Aedes albopictus was introduced into the United States in used tires in 1985. Its successful colonization of the upper Midwest has potential to alter the current epidemiology of bunyaviruses that circulate in the region. It is permissive for the replication of several arboviruses, including La Crosse (LACV) and Jamestown Canyon (JCV) bunyaviruses. In this study, we demonstrate the ability of LACV and JCV to coinfect Ae. albopictus mosquitoes and to form all six possible reassortant genotypes. All reassortant viruses infect Ae. albopictus orally and can be transmitted to suckling mice. All reassortants are neurovirulent in mice. However, reassortant viruses carrying the LACV M segment in the foreign genetic background of JCV are more neuroinvasive than JCV, or any other reassortant genotype. In addition, these reassortants can replicate in gerbils and infect Ae. triseriatus, characteristics of LACV, but not JCV. Because Ae. albopictus is spreading into new geographic areas and feeds on a variety of mammals, including humans, it has the potential to transmit new, emerging bunyaviruses in nature.

Malignant myoepitheliomas (myoepithelial carcinomas) are uncommon, and we know of only 29 reported cases. We present a new case together with its clinical, histological, and immunohistochemical features. The tumour was located in the inferior vestibular sulcus of a 64-year-old woman. She was treated by wide local resection. Malignant myoepitheliomas are distinguished from benign myoepithelial neoplasms by their infiltrating and destructive growth. The tumour cells may be spindle-shaped or more rounded (plasmacytoid cells) and contain cellular pleomorphism and mitotic activity. The clinical and biological behaviour of this tumour is not yet known and there is little information about treatment and prognosis.

Eosinophilic ulcer of the oral mucosa (EUOM) is an uncommon lesion that usually affects the tongue. The lesion is benign and self-limiting, develops rapidly and usually disappears after several weeks without treatment. Microscopically, it contains a diffuse polymorphous cell infiltrate composed mainly of eosinophils. The cause of EUOM is unknown, but the condition has been attributed to trauma. We now describe a case of EUOM on the lateral surface of the tongue with recurrence on its dorsal surface 3 years later. The occurrence of multiple lesions, whether at the same time or subsequently as reported here, makes a purely traumatic origin rather unlikely and suggests that there may be a predisposition of some patients to develop EUOM. Eosinophilic ulcer of the oral mucosa (EUOM) is an uncommon lesion that is very rarely reported in the dermatological literature. It usually affects the tongue and presents clinically as an ulcer with elevated and indurated borders. Recognition is important because it often mimicks tongue cancer; however, the lesion is benign and self-limiting, develops rapidly and usually disappears after several weeks without treatment. Usually the ulcer occurs in a single episode, but the possibility of recurrence at other sites has been poorly emphasized.
