BACKGROUND: Previous retrospective results are evaluated prospectively and blinded. METHODS: A total of 221 eyes previously classified as normal (G1), 279 as moderate risk of glaucoma (G2) and 217 as high risk (G3) according to the Globin Discriminant Function (GDF) Laguna-ONhE index were examined with OCT Spectralis- Results: In G1, the Bruch's Membrane Opening Minimum Rim Width (BMO-MRW) was 332 ± 55 microns; in G2, it was 252 ± 47 (p < 0.0001); and in G3, 231 ± 44 (p < 0.0001). In G1, the 1% and 5% percentiles were 233 and 248, respectively; in G2, they were lower in 28.80% and 42.29% of cases, respectively; and in G3, in 50.23% and 63.59% of cases, respectively. Most of the cases were normal-tension glaucomas. Laguna-ONhE indices showed a curvilinear correlation with BMO-MRW results. The Retinal Nerve Fibre Layer (RNFL) showed a poor relationship with BMO. Assuming G1 to be truly normal, BMO-MRW would have a Receiver operating characteristic (ROC) curve area of 0.901 for G2 and G3 and 0.651 for RNFL. A significant reduction in pixels corresponding to vessels was found in G2 and G3 vs. G1 (p < 0.0001). CONCLUSIONS: In some cases, these defects appear to be mainly glaucomatous, and in others, they are associated with diabetic microangiopathy. In normal tension glaucoma, RNFL defects may be less severe than those inside the nerve.

BACKGROUND: Parkinson's disease is characterised by the loss of balance and the presence of walking difficulties. The inclusion of rehabilitation therapies to complement pharmacological therapy allows for comprehensive management of the disease. In recent years, virtual reality has been gaining importance in the treatment of neurological diseases and their associated symptoms. Therefore, the objective of this systematic review was to analyse the effectiveness of virtual reality on balance and gait in patients with Parkinson's disease. METHODS: This study is a systematic review conducted following PRISMA's statements. An electronic search of the literature was carried out in the following databases: PubMed, Cochrane, Dialnet, Scopus, Web of Science, PsycINFO and Science Direct PEDro. The inclusion criteria were controlled and non-controlled clinical trials published in the last 12 years in English or Spanish, in which virtual reality was applied to treat balance and gait impairments in patients with Parkinson's disease. RESULTS: 20 studies were finally included in this review. A total of 480 patients participated in the included studies. All patients were diagnosed with Parkinson's disease. Most of the investigations used the Nintendo Wii + Balance Board or the Microsoft Kinect TM combined with the Kinect Adventures games as a virtual reality device. CONCLUSIONS: According to the results of this literature review, virtual reality-based interventions achieve good adherence to treatment, bring innovation and motivation to rehabilitation, and provide feedback as well as cognitive and sensory stimulation in patients with Parkinson's disease. Therefore, virtual reality can be considered an alternative for personalised rehabilitation and for home treatment.

Introduction: COVID-19 is an infectious disease, transmitted by diverse viral strains from the Coronaviridae family. It can manifest as a mild-moderate condition with general symptoms such as malaise, body aches and fever. However, when it finds a patient with other comorbidities it can enhance its virulence, potentially leading to a severe disease with pulmonary-systemic effects. Obesity recognized as a pandemic nowadays, displays and imminent increase in the risk of developing complications during the course of COVID-19 process. Objective: Identify the prevalence of obesity in patients admitted to the Intensive Care Unit (ICU) that displayed an unfavorable prognostic. Methodology: Observational, retrospective study, data was retrieved from a single medical center in Dominican Republic. In the period of time from March to December 2020 a total of 382 patients hospitalized in ICU with a positive PCR for SARS-CoV-2 was extracted from the electronic medical records. Conclusion: Obesity leads to a homeostatic imbalance that severely affects the body´s immune system, making the patient more susceptible to present severe cases of COVID-19, requiring higher levels of hospitalization and ICU admissions.


Introducción: el COVID-19 es una enfermedad de origen infeccioso viral, transmitida por diversas cepas de la familia Coronaviridae. Su manifestación puede ser leve-moderada con síntomas generales como malestar, dolor corporal y fiebre, sin embargo, la presencia de comorbilidades en pacientes la convierte potencialmente en una enfermedad severa con afectación pulmonar-sistémica. La obesidad, también reconocida como pandemia, representa un factor de riesgo inminente ante una mayor susceptibilidad de adquirir complicaciones durante la enfermedad por COVID-19. Objetivo: identificar la prevalencia de obesidad en pacientes admitidos a Unidad de Cuidados Intensivos (UCI) que mostraron un pronóstico desfavorable. Metodología: se realizó un estudio retrospectivo de tipo observacional, se recolectó la información de un solo centro de salud de la República Dominicana. Durante el período de marzo a diciembre del 2020, se recopiló de los récords médicos electrónicos un total de 382 pacientes hospitalizados en UCI con una prueba positiva de SARS-CoV-2. Conclusión: la obesidad produce un desbalance homeostático afectando especialmente la inmunidad del organismo haciendo al paciente más susceptible a presentar casos severos de COVID-19, requiriendo mayores niveles de hospitalización e ingresos a UCI.

Mapping B and T cell epitopes constitutes an important action for peptide vaccine design. PLD and CP40 virulence factors of Corynebacterium pseudotuberculosis biovar ovis, a causal agent of Caseous Lymphadenitis, have been evaluated in a murine model as good candidates for vaccine development. Therefore, the goal of this work was to in silico analyze B and T cell epitopes of the PLD and CP40 proteins of a Mexican isolate of Corynebacterium pseudotuberculosis ovis. The Immune Epitope Data Base and Resource website was employed to predict the linear and conformational B-cell, T CD4+, and T CD8+ epitopes of PLD and CP40 proteins of Corynebacterium pseudotuberculosis ovis Mexican strain 2J-L. Fifty B cell epitopes for PLD 2J-L and forty-seven for CP40 2J-L were estimated. In addition, T CD4+ and CD8+ cell epitopes were predicted for PLD 2J-L (MHC I:16 epitopes, MHC II:10 epitopes) and CP40 2J-L (MHC I: 15 epitopes, MHC II: 13 epitopes). This study provides epitopes, paying particular attention to sequences selected by different predictor programs and overlap sequences as B and T cell epitopes. PLD 2J-L and CP40 2J-L protein epitopes may aid in the design of a promising peptide-based vaccine against Caseous Lymphadenitis in Mexico.

BACKGROUND: Burns are mild or severe lesions produced in living tissue, due to the action of different agents. This pathology is considered the third cause of accidental death in the world by the World Health Organization. Among the most disabling sequelae in these patients, pain and range of motion have the greatest impact. A recommended tool to complement the treatment or management of the symptoms associated with burns is virtual reality. OBJECTIVE: The objective of this study was to analyse the effectiveness of virtual-reality therapy for pain relief and the improvement of the range of joint movement in patients who have suffered burns. METHODOLOGY: This study is a systematic review conducted following the PRISMA statements. An electronic literature search was performed in the following databases: PubMed, Cochrane, Dialnet, Scopus and Science Direct. The inclusion criteria were: participants with burns in any part of the body, interventions with virtual reality with or without complementary treatment, studies in both Spanish and English, and outcome measures of pain and range of motion. RESULTS: Finally, 10 studies were included in the review. The sample consisted of one pilot study, three randomized controlled clinical trials, one prospective randomized controlled clinical trial, one control group and treatment group trial, one interventional clinical trial and three comparative studies. The most commonly used assessment tools for pain were the graphic rating scale (GRS) and for range of motion the goniometer. The use of virtual-reality games significantly reduced pain scores during physiotherapy and occupational therapy treatments as well as in nursing care. The range of motion improved significantly during virtual-reality exercises performed during a physiotherapy treatment in 33% of studies included in this review. CONCLUSION: The results of the studies analysed in this systematic review suggest that the use of virtual reality for the management of pain and range of movement limitations associated with burn injuries could control these symptoms and decrease their negative consequences on the person.

OBJETIVO: Explorar la percepción de mujeres en edad reproductiva con respecto a las vías del parto reportada en la evidencia disponible. MÉTODO: Se llevó a cabo una revisión sistematizada de artículos en las bases de datos PubMed, SciELO, SCOPUS, Web of Science, LILACS, Cochrane Library y Biblioteca Virtual de Salud. La búsqueda se realizó entre agosto y noviembre del año 2020. Los términos MeSH usados fueron "Perception", "Delivery Obstetric", "Cesarean Section" y "Woman", junto con los términos DECS "Percepción", "Parto Obstétrico", "Cesárea" y "Mujeres". Además, se utilizaron los términos libres "Perceptions", "Vaginal Delivery", "Obstetric Deliveries", "Abdominal Deliveries", "Caesarean Section" y "Womens Groups". En la totalidad de las bases de datos utilizadas se aplicaron como límites de búsqueda "Last 5 Years" y "Free Full Text". RESULTADOS: Respecto a la percepción de las vías de parto, se constató la preferencia de la vía vaginal sobre la cesárea. Además, se evidenciaron factores que influyen en el proceso, tales como miedo, experiencia previa, influencia de terceros, religión, cultura, nivel socioeconómico y zona demográfica. CONCLUSIONES: La evidencia revisada sugiere que las mujeres prefieren la vía del parto vaginal sobre la cesárea, siendo el miedo el factor más influyente al momento de su elección, seguido por los factores asociados a experiencias previas, el nivel socioeconómico y las creencias.


OBJECTIVE: To explore the perception of women of reproductive age regarding delivery routes reported in the available evidence. METHOD: A systematized review of articles in PubMed, SciELO, SCOPUS, Web of Science, LILACS, Cochrane Library, Biblioteca Virtual de Salud databases, was carried out. The search was conducted between August and November 2020. MeSH descriptors "Perception", "Delivery Obstetric", "Cesarean Section" and "Woman", along with DECS descriptors "Percepciones", "Parto Obstétrico", "Cesárea" and "Mujeres", were used. Also, free terms "Perceptions", "Vaginal Delivery", "Obstetric Deliveries", "Abdominal Deliveries", "Caesarean Section" and "Womens Groups". In the totality of databases limits of search "Last 5 years" and "Free Full Text", were used. RESULTS: Regarding the perception of the delivery routes, the preference of the vaginal route over caesarean section was verified. In addition, factors that influence the process such as fear, previous experience, influence of third parties, religion, culture, socioeconomic level and demographic area were evidenced. CONCLUSIONS: The reviewed evidence suggests that women prefer the vaginal delivery route over caesarean section with fear being the most influential factor at the time of their choice, and secondly, the factors associated with previous experiences, socioeconomic level and beliefs.

Equine infectious anemia is a worldwide distributed disease that affects the Equide family. Commercial effective vaccine is not available, for that reason control of the disease depends on diagnostic tools. To improve the efficiency of the diagnostic program in Cuba, LABIOFAM Group, developed an indirect enzyme-linked immunosorbent assay (ELISA), ELISA kit, to complement the diagnostic system that currently uses the agar gel immunodiffusion (AGID) kit. The ELISA AIE-LAB Kit was evaluated in a Mexican context, compared with the gold standard test Agar gel immunodiffusion, AGID AIE-LABIOFAM, and commercial AGID kit. The analytical sensitivity was determined using serial dilutions twofold of the positive control serum to establish the range of detected antibodies in relation to the cutoff value of the plate (OD 0.300). A precision study was carried out to evaluate repeatability, intermediate precision, and reproducibility by estimating the standard deviation and coefficient of variation. The precision results were satisfactory and the values of the coefficient of variation were considered adequate to guarantee an excellent consistency of the ELISA AIE-LAB. The diagnostic performance of the ELISA AIE-LAB involved the evaluation of specificity, sensitivity, and concordance in comparison with both AGID tests. The diagnostic sensitivity was 100% and the specificity 97.6%, with a very good degree of concordance (Kappa = 0.9). The results suggest that the ELISA AIE-LAB test could be used in Mexico as a diagnostic system for the detection of specific antibodies against the equine infectious anemia virus, as per current international norms.

INTRODUCCIÓN Dado que el conocimiento sobre la anticoncepción de emergencia puede ser de gran utilidad en la promoción de la salud sexual y salud reproductiva, especialmente en los adolescentes, la deficiencia del mismo podría disminuir su prescripción y uso. OBJETIVO Describir el conocimiento sobre los métodos anticonceptivos de emergencia de un grupo de mujeres adolescentes estudiantes de enseñanza media de la comuna de Hualqui, Chile. MATERIAL Y MÉTODO Se realizó un estudio descriptivo de corte transversal. Se encuestó a 115 mujeres adolescentes estudiantes de primer a cuarto año de enseñanza media del liceo San Juan Bautista de Hualqui, Octava Región de Chile. Se exploraron antecedentes generales de salud sexual y salud reproductiva, y conocimientos sobre la anticoncepción de emergencia, su uso y formas de adquisición. RESULTADOS La edad promedio fue de 16,1 años. El 39% de las estudiantes eran sexualmente activas, y en promedio iniciaron su vida sexual a los 15,3 años y tuvieron 2,1 parejas sexuales. El método anticonceptivo que más utilizaban fue el anticonceptivo hormonal inyectable (31,1%), seguido de la anticoncepción oral combinada (20,0%). Con respecto a la anticoncepción de emergencia, el 80,9% declaró saber de su entrega liberada, mientras que el 11,1% declaró haberlos usado alguna vez. El 91,3% de las encuestadas consideró necesario tener mayor información. CONCLUSIÓN Las adolescentes participantes del estudio mostraron algún grado de conocimiento respecto a la anticoncepción de emergencia, pese a no utilizarla habitualmente. No obstante, la mayoría expresó la necesidad de tener mayor acceso a información relacionada al tema.


BACKGROUND Given that knowledge about emergency contraception can be very useful in promoting sexual health and reproductive health, especially in adolescents, its deficiency could decrease its prescription and use. AIM To describe the knowledge about emergency contraceptive methods of a group of female adolescent secondary education students from the Hualqui commune, Chile. METHOD A descriptive cross-sectional study was carried out. 115 adolescent women students from first to fourth year of high school at San Juan Bautista high school in Hualqui, Eighth Region of Chile, were surveyed. General sexual health and reproductive health backgrounds, and knowledge of emergency contraception, its use, and forms of acquisition were explored. RESULTS The average age was 16,1 years. 39% of the students were sexually active, and on average started their sex life at 15,3 years and had 2,1 sexual partners. The most widely used contraceptive method was injectable hormonal contraception (31,1%), followed by combined oral contraception (20,0%). Regarding the emergency contraception, 80,9% stated that they knew of their free delivery, while 11,1% stated that they had ever used them. 91,3% of respondents considered it necessary to have more information. CONCLUSIONS The adolescents participating in the study showed some degree of knowledge regarding emergency contraception, despite not using it regularly. However, the majority expressed the need for greater access to information related to the topic.

RESUMEN

En este artículo se profundiza sobre los beneficios que la musicoterapia puede aportar al nivel cognitivo y/o conductual de los ancianos con demencia. Para ello se realizó una búsqueda bibliográfica de artículos de ensayos controlados aleatorios, ensayos casos-control y estudios pilotos, publicados desde enero de 2000 a enero de 2012 en las bases de datos Cochrane, MEDLINE, Dialnet y CSIC, centrados en la comparación de la musicoterapia como tratamiento no farmacológico en las personas mayores de 65 años con demencia moderada, frente a su tratamiento terapéutico-ocupacional habitual. Se seleccionaron 10 artículos en función de los criterios de inclusión. El análisis de los resultados sugiere que la musicoterapia mejora en las personas con demencia su nivel conductual, cognitivo y de comportamiento social (AU)

An in-depth review is presented the possible benefits of music therapy in relation to the cognitive and/or behavioural level of elderly patients with dementia. We have carried out a systematic review of randomized controlled trials, case-control and pilot studies published from January 2000 to January 2012 using the Cochrane Database of Systematic Reviews, MEDLINE, Dialnet and CSIC. We focused on comparison of music therapy as non-pharmacological therapy, in patients over 65 years of age with moderate dementia, with regular therapeutic and occupational treatment. Ten articles were selected based on the inclusion criteria. The analysis of the results suggest that music Therapy influences the elderly people with dementia in a positive way by improving levels of behavioural and cognitive functioning and social participation.

The purpose of this study was to assess the effects of chronic oral anticoagulant (OAC) treatment on bone mass using quantitative ultrasound (QUS) techniques. A total of 120 patients (47 women and 73 men) undergoing treatment with OAC and 57 healthy subjects (27 women and 30 men) participated in this study. Bone status was assessed using QUS devices that measure the amplitude-dependent speed of sound (Ad-SoS) in phalanges and the broadband ultrasound attenuation (BUA) in the calcaneus. Men undergoing OAC treatment had lower Ad-SoS, Z-score, T-score, and BUA values (all p < 0.005) and higher levels of undercarboxylated osteocalcin (u-OC) and tartrate-resistant acid phosphatase (TRAP) (both p < 0.0001) than controls. Women receiving OAC treatment had lower BUA values (p < 0.005) and total osteocalcin (OC) levels (p < 0.0001) and higher levels of u-OC and TRAP (both p < 0.0001) than controls. There was a statistically significant negative correlation between u-OC levels and Ad-SoS values in both men (r = -0.432, p = 0.0328) and women (r = -0.332, p = 0.0269) undergoing OAC treatment. In conclusion, patients undergoing OAC treatment had a loss of trabecular and cortical bone mass, possibly due to a decrease in the γ-carboxylation of osteocalcin resulting from the vitamin K antagonism of these drugs.
