RESUMEN

Hypertension is a most common health problem worldwide and its incidence seems to be increasing on global scale. The objective of the study was to assess the relationship of serum Total Cholesterol with hypertensive patients in order to compare this parameter with normotensive subjects. This analytical type of cross sectional study was carried out in the Department of Physiology, Mymensingh Medical College, Mymensingh, Bangladesh from July 2017 to June 2018. A total number of 120 male subjects, age ranged from 30-65 years was included in this study. Among them, sixty (60) hypertensive subjects were taken as study group (Group II) and sixty (60) age matched normotensive male subjects were taken as control group (Group I). Data were expressed as mean±SD and statistical significance of difference among the group was calculated by unpaired students' 't' test. In this study we found that serum total cholesterol was significant in study group (229.62±17.49mg/dl) in comparison with control group (166.32±18.04mg/dl). Therefore, by this study we recommended that routine estimation of these parameters is important for prevention of complication related to hypertension for leading a healthy life.

Type-2 diabetes mellitus accounts for 90-95% of all diabetes. There is a Correlation between Fasting Serum Glucose and Serum Zinc level in patients with Type-2 Diabetes Mellitus. This study was done to assess the relationship of Fasting Serum Glucose and Serum Zinc in type-2 diabetic patients in order to compare this parameter with healthy subjects. This analytical type of cross sectional study was carried out in the Department of Physiology, Mymensingh Medical College, Mymensingh, Bangladesh from July 2018 to June 2019. A total number of 140 subjects, age ranged from 35-65 years was included in this study. Among them, seventy (70) type-2 diabetic patients were taken as study group (Group II) and seventy (70) ages matched healthy subjects were taken as control group (Group I). Group I again subdivided into control group male (Group IA) and control group female (Group IB). Group II also subdivided into study group male (Group IIA) and study group female (Group IIB). Pearson correlation coefficient test is done to correlate the relationship between fasting serum glucose (FSG) and serum zinc. In this study we found that serum zinc level was decreased in study groups in comparison with control groups. Therefore, by this study we recommended that routine estimation of this parameter is important for prevention of complication related to diabetes for leading a healthy life.

Menopause occupying one-third of women's life and women face various physiological, psychological and biochemical changes during this period. The systolic and diastolic blood pressure was significantly elevated among postmenopausal women as compared with reproductive women. To assess the blood pressure changes in healthy postmenopausal women in order to compare this parameter with healthy reproductive women, this analytical type of comparative study was carried out in the Department of Physiology, Mymensingh Medical College, Mymensingh, Bangladesh from January 2017 to December 2017. Two hundred healthy women (100 female were postmenopausal as study group and 100 female were reproductive women as control group) aged between 25 to 65 years were enrolled in this study. Blood pressure of the individual was measured by an android sphygmomanometer and stethoscope with appropriate size of cuff. Data were expressed as mean±SD and statistical significance of difference among the group was calculated by unpaired students' 't' test. The mean±SD of systolic and diastolic blood pressure were higher in postmenopausal group in comparison to the reproductive women group. This study concludes systolic and diastolic blood pressure increases in postmenopausal women.

Aim of the study was to assess the relationship of serum HDL-C and triglyceride with hypertensive patients in order to compare this parameter with normotensive subjects. This analytical type of cross sectional study was carried out in the Department of Physiology, Mymensingh Medical College, Mymensingh, Bangladesh from July 2017 to June 2018. A total number of 120 male subjects, age ranged from 30-65 years was included in this study. Diabetes mellitus is a complex disorder involving multiple organ systems and is the primary modifiable risk factor for heart disease. Patients with type 2 diabetes mellitus have an increased prevalence of lipid abnormalities. A timely intervention to normalize circulating lipids could decrease the probability of cardiovascular complications. Aim of the study was to assess the relationship of serum High Density Lipoprotein Cholesterol (HDL-C) and triglyceride (TG) with type 2 diabetic patients in order to compare this parameter with non-diabetic subjects. This analytical type of cross sectional study was carried out in the Department of Physiology, Mymensingh Medical College, Mymensingh, Bangladesh from July 2017 to June 2018. A total number of 160 female subjects, age ranged from 35-65 years was included in this study. Among them, eighty (80) type 2 diabetic patients were taken as study group (Group II) and eighty (80) ages matched non-diabetic female healthy subjects were taken as control group (Group I). Data were expressed as mean±SD and statistical significance of difference among the group was calculated by student's unpaired (t) test. The mean±SD of serum HDL-C of control group and study group were 49.60±2.18mg/dl & 28.58±2.80mg/dl respectively. In study group serum HDL-C was decreased. The mean±SD of serum Triglyceride of Group I and Group II were 122.06±11.65mg/dl & 221.84±11.79mg/dl respectively. In group II serum triglyceride was increased. Serum HDL cholesterol and serum triglyceride were significant in study group in comparison with control group. Therefore, by this study we recommended that routine estimation of these parameters is important for prevention of complication related to type 2 diabetes mellitus for leading a healthy life.

BACKGROUND: This study aimed to investigate the prevalence of otomycosis and aetiological agents in Khouzestan province, south-west Iran. METHODS: This cross-sectional study examined and cultured 881 swabs from suspected external otitis cases, collected from throughout Khouzestan province. Fungal agents were identified by slide culture and complementary tests when necessary. RESULTS: The mean patient age was 37 years. The 20-39 year age group had the highest prevalence of otomycosis: 293 cases, comprising 162 (55.3 per cent) women and 131 (44.7 per cent) men. The seasonal distribution of cases was: summer, 44.7 per cent; autumn, 28.7 per cent; winter, 14.7 per cent; and spring, 11.9 per cent. The fungal agents isolated were Aspergillus niger (67.2 per cent), Aspergillus flavus (13 per cent), Candida albicans (11.6 per cent), Aspergillus fumigatus (6.2 per cent) and penicillium species (2 per cent). CONCLUSION: Fungal otomycosis is still one of the most important external ear diseases. In this study in south-west Iran, Aspergillus niger was the predominant aetiological agent. However, clinicians should be cautious of candidal otomycosis, which has a lower rate of incidence but is more prevalent among 20-39 year olds.

Mirtazapine (Remeron) is a member of the relatively new class of tetracyclic antidepressants. There are published cases of mirtazapine's detection as an incidental finding in postmortem cases; however, case reports with associated tissue concentrations and interpretations are not available. This report documents the tissue distribution of mirtazapine in eight postmortem cases in which it was identified but did not contribute to the cause or manner of death. The following mean mirtazapine concentrations (milligrams per liter or milligrams per kilogram) were found: heart blood 0.12 (range, < 0.01-0.33, n = 7); peripheral blood 0.09 (< 0.01-0.14, n = 4); urine 0.61 (0.01-3.2, n = 7); liver 0.88 (0.04-3.6, n = 6), kidney 0.21 (0.02-0.48, n = 5); and bile 0.62 (0.11-1.6, n = 6). In each case, the mirtazapine concentration in heart blood was approximately equal to that of peripheral blood, indicating that postmortem redistribution was not a factor in evaluating postmortem blood concentrations in these cases. However, because the liver mirtazapine concentrations were 5-30 times the blood concentrations, the potential for postmortem redistribution cannot be excluded. Additionally, because urine concentrations of the parent compound were consistently greater than the blood concentrations, urine is an adequate screening specimen for mirtazapine.

Twenty-seven patients with advanced testicular cancer underwent cisplatin-based combination chemotherapy and followed by salvage surgery. Complete necrosis and/or fibrosis (necrosis) was found in 12 (44%) operative specimens, mature teratoma in 5 (19%) and residual cancer in 10 (37%) patients. Ten of the 12 (83%) patients with necrosis and all 5 with mature teratoma were rendered disease-free (NED). Only 6 of the 10 (60%) patients with residual cancer were continuously NED. These findings suggest that if carcinoma is found in the resected specimen, further salvage chemotherapy is required. Salvage surgery should be avoided in patients with elevated serum tumor marker levels.

RESUMEN

Long-term (from 10 to 25 years) clinical follow up of 60 patients suffering from schizophrenia was carried out. On the basis of observing groups of patients with various gravity of the disease course it was found that of importance for the prognosis were (apart from the structure of the paroxysms in the manifest period) premorbid peculiarities of the patient's personality and some pre-manifest disturbances, such as early personality changes, transitory hallucinatory and delusional disturbances, etc.
