RESUMEN

OBJECTIVES: A clinically practical tool to assess skin biomechanical properties rapidly and accurately is still lacking. Our aim was to examine the intra- and inter-observer reproducibility of a myotonometer for objective skin property assessment in systemic sclerosis (SSc), comparing it with the modified Rodnan skin score (mRSS), and distinguishing patients from healthy controls. METHOD: Thirty-four patients (21 limited and 13 diffuse SSc), and 31 age and gender-matched healthy controls were enrolled. Skin tone and stiffness were measured at four different anatomical sites (the forearm, hand, leg, and foot) using a myotonometer. The correlation between the mRSS and skin properties was assessed. Also, hand functionality was evaluated for possible correlations between the variables. The differences in skin properties between dcSSc and lcSSc patients, and healthy controls were assessed using variance analysis. RESULTS: Intra- and inter-examiner reproducibility were excellent (ICC = 0.70 to 0.98) for tone and stiffness except for non-dominant hand tone, which showed good reliability (ICC = 0.64 to 0.74). Stiffness and tone values of the hands, forearms, and feet significantly correlated with mRSS total score (r = 0.40 to 0.71, p < 0.05). Additionally, tone and stiffness of the hands and forearms moderately correlated with hand function (p < 0.05). Tone and stiffness values increased in patients with dcSSc compared to healthy controls, or patients with lcSSc, at the hands, forearms, and legs (p < 0.05). CONCLUSIONS: Our findings emphasize the potential utility of the myotonometer for assessing skin properties and differentiating SSc patients from controls, demonstrating its promise as a valuable clinical evaluation tool in this context. Key Points •The myotonometer displayed excellent intra- and inter-examiner reproducibility for assessing skin properties. •Skin tone and stiffness parameters well correlated with the mRSS scores. •The myotonometer can distinguish patients with diffuse cutaneous SSc from healthy controls.

AIMS: The primary objective was to compare patients with type 2 diabetes mellitus (T2DM) and healthy peers in terms of plantar fascia (PF) stiffness, fall risk, and gait speed. The second objective was to examine the relationship between stiffness of PF and fall risk, gait speed. METHODS: Fifty patients diagnosed with T2DM (mean duration = 10.74 ± 7.07 years) were included. Myotonometer was used to evaluate the stiffness of PF. To assess the risk of falling, and gait speed, the International Fall Efficiency Scale (FES-I) and the 4-Meter Gait Speed Test (4mGST) were used, respectively. RESULTS: Compared to healthy controls, PF stiffness (right foot mean difference = 148.99 N/m, left foot mean difference = 113.13 N/m p < .001) was higher in the T2DM group. The FES-I and 4mGST scores were worse in the group with T2DM (p < .05). 12.8 % of FES-I and 23.4 % of 4mGST variance were explained by stiffness of PF. CONCLUSIONS: The results of the study showed that the stiffness of PF changed in patients with T2DM. There was a decrease in gait speed and an increase in the risk of falling as PF stiffness increased.

OBJECTIVES: The whole-body vibration (WBV) effects on muscle strength show inconsistent results. Moreover, there is no study about the WBV effect on stiffness, elasticity, and muscle strength. Therefore, the study aimed to examine the effect of WBV exposure with static squat posture on the stiffness, elasticity, and strength of the lower-limb extensor muscles. MATERIAL AND METHODS: Forty healthy untrained young adults were divided into WBV and control groups. The experimental group received WBV exposure on 2 nonconsecutive days of the week, for 6 weeks. The MyotonPRO device was used for the assessment of the knee extensor and the ankle dorsiflexors' stiffness and elasticity. Isometric muscle strength was evaluated with a hand-held dynamometer. All measurements were done by the same assessor at baseline, and the following 6 weeks. RESULTS: Significant group-by-time interactions were found for the elasticity scores of the right (d = 0.84, P = .01) and left (d = 0.77, P = .02) ankle dorsiflexors. Similar to the elasticity measurements, significant group-by-time interactions were observed in the muscle strength scores of the right (d = 0.45, P = .046) and left (d = 1.25, P < .001) ankle dorsiflexors. No significant effects were observed in any of the evaluated muscle stiffness measurements (P > .05), and there was no significant group-by-time interaction in knee-extensor muscle strength and elasticity scores (P > .05). CONCLUSIONS: The study results indicate that if the ankle dorsiflexor strength and elasticity are desired to be increased, the 6-week WBV exposure in a static squat posture can be used in healthy individuals.

OBJECTIVE: The purpose of this study was to compare the effect of body posture and apnea severity on the tone, stiffness, and elasticity of upper airway muscles in awake patients with obstructive sleep apnea syndrome (OSAS). METHODS: Eighty adult patients with OSAS from the Sleep Laboratory of the Bitlis State Hospital between April and December 2021 were included in the study. The tone, stiffness, and elasticity of the genioglossus and accessory muscles (sternocleidomastoid, masseter) were measured while the patients were awake, sitting upright, and in supine posture. According to polysomnography results and the Apnea-Hypopnea Index, patients were classified as mild, moderate, or severe OSAS. RESULTS: There were 56 male (70%) and 24 female (30%) patients (age 45.0 ± 11.0 years). Tone of genioglossus, masseter, and sternocleidomastoid muscles were higher in supine than in sitting posture (all P < .05). There was no difference in elasticity scores of the bilateral genioglossus muscle in sitting and supine posture (P > .05). The elasticity scores of the sternocleidomastoid and masseter muscles were higher in sitting (all P < .05). An interaction effect of postural change and apnea severity was observed on stiffness scores of the genioglossus muscle (P < .05). CONCLUSION: Results showed that the genioglossus muscle and accessory muscles take on the inspiratory load that develops with postural strain, regardless of the severity of apnea, in awake patients with OSAS. For participants measured in this study, accessory muscles supported respiration by increasing their elasticity.

CONTEXT: The research on the change in properties of the lower leg muscles by different intensity sinusoidal vertical whole-body vibration (SV-WBV) exposures has not yet been investigated. OBJECTIVE: The purpose of this study was to determine effect of a 20-minute different intensity SV-WBV application to the ankle plantar flexor and dorsiflexor muscles properties and hamstring flexibility. DESIGN: Prospective preintervention-postintervention design. SETTING: Physiotherapy department. PARTICIPANTS: A total of 50 recreationally active college-aged individuals with no history of a lower leg injury volunteered. INTERVENTIONS: The SV-WBV was applied throughout the session with an amplitude of 2 to 4 mm and a frequency of 25 Hz in moderate-intensity vibration group and 40 Hz in a vigorous-intensity vibration group. MAIN OUTCOME MEASURES: The gastrocnemius and tibialis anterior muscle tone was assessed with MyotonPRO, and the strength evaluation was made on the same lower leg muscles using hand-held dynamometer. The sit and reach test was used for the lower leg flexibility evaluation. RESULTS: The gastrocnemius muscle tone decreased on the right side (d = 0.643, P = .01) and increased on the left (d = 0.593, P = .04) when vigorous-intensity vibration was applied. Bilateral gastrocnemius muscle strength did not change in both groups (P > .05). Without differences between groups, bilateral tibialis anterior muscle strength increased in both groups (P < .01). Bilateral gastrocnemius and tibialis anterior muscle tone did not change in the moderate-intensity vibration group (P > .05). Flexibility increased in both groups (P < .01); however, there was no statistically significant difference between the groups (d = 0.169, P = .55). CONCLUSIONS: According to study results, if SV-WBV is to be used in hamstring flexibility or ankle dorsiflexor muscle strengthening, both vibration exposures should be preferred. Different vibration programs could be proposed to increase ankle plantar flexor muscle strength in the acute results. Vigorous-intensity vibration exposure is effective in altering ankle plantar flexor muscle tone, but it is important to be aware of the differences between the lower legs.
