[Mesocolon liposarcoma: a case report]. / Un liposarcome du mésocôlon.

Liposarcoma is the second most common soft tissue sarcoma in adults after fibrosarcoma. Extremities and retroperitoneum are the most common sites. Intraabdominal and particulary mesocolic liposarcoma is a rare localisation. We report the case of a 64-year-old woman with a localized grade 1 liposarcoma of the right mesocolon treated by an extended right hemicolectomy. No further treatment was done because of the low grading and the disease-free margins. The patient is without any evidence of recurrence after one year.

The goal of this text is to remind the place of histoplasmosis in the differential diagnosis of chronical buccal lesions, even if this pathology is extremely rare in our countries. The diagnosis is easy and fast because we can do biopsy in a place easily accessible.

Natural killer-like T-cell lymphoma of the stomach.

We report the case of a 69-year-old white woman who developed a natural killer (NK)-like T-cell lymphoma involving primarily the stomach. The tumour consisted of large and pleomorphic lymphocytes infiltrating the gastric mucosa. Immunohistochemistry performed on paraffin sections showed the neoplastic cells to be CD3+, CD5-, CD8-, CD43+, CD45RO+, and CD57+. In addition, these cells also expressed HLA-DR, granzyme B, and, to a lesser extent, the CD30 activation marker. No pathologic features suggesting Helicobacter pylori, Epstein-Barr virus infection, or lymphocytic gastritis were found within adjacent normal mucosa. The patient had no previous history of coeliac disease, and her serology for H. pylori was negative. Since lymphomas are usually considered the neoplastic counterpart of normal lymphocytic subsets, it is possible that in this case the tumour cells originate from a distinct cytotoxic T-cell population normally present within the gastric mucosa. The pathogenesis of this highly unusual neoplasm, however, remains a mystery.

Combined therapeutic approach for extraorbital sebaceous carcinoma in a Torre's syndrome.

Extraocular sebaceous carcinomas are extremely uncommon neoplasms. More exceptional is their association with an internal malignancy as first described separately by Torre and Muir in 1967. We report the first use of combined adjuvant radio- and chemotherapy in a sebaceous carcinoma of the anal margin in a Torre's syndrome. An interesting partial response was attained before the patient died from general debility. The therapeutic approach is discussed.

We present a case of dramatic radiation enterocolitis inducing portal venous air diagnosed by Doppler sonography only. The sonographic pattern consisted of multiple irregular hyperechoic areas into the liver, with internal repetitive noisy bidirectional peaks superimposed on the usual continuous Doppler display of the portal flow. Although portal hyperechoic moving foci alone may reflect only slow portal velocity, they do not create any Doppler distortion as do moving bubbles. Portal air may have multiple causes such as abdominopelvic abscesses, sepsis, intestinal distension, fulminant hepatitis, cholangitis, cholecystitis, diabetic acidosis..., but mesenteric infarct, necrotic enterocolitis, and radiation enteritis are life-threatening conditions that have to be diagnosed as soon as possible. Although large quantities of portal air may be demonstrated on plain film of the abdomen or by computed tomography, Doppler sonography may detect smaller quantities, allowing earlier diagnosis of intestinal pathology requiring immediate surgical treatment. Therefore, Doppler sonography of the liver should be performed in any patient with acute abdominal pain or distension, especially if being treated by abdominal radiotherapy.

[A new complication of cystic neoplasms of the pancreas: cysto-duodenal fistula]. / Nouvelle complication des néoplasies kystiques du pancréas: la fistule cysto-duodénale.

Pancreatic cystadenomas and cystadenocarcinomas are rare tumors. Their uncommon complications are usually due to compression of adjacent structures or to pancreatic and bile duct fistulas. Authors report the 1st case of duodenal fistula formation and discuss the reported complications.

[Interstitial pneumopathy caused by melphalan]. / Pneumopathie interstitielle au melphalan.

The Authors have described a case of interstitial pneumonia due to 1-phenyl alanine (Melphalan). This case report, where a diagnosis of myeloma of the lung was excluded, was characterised by contact with a single cytotoxic agent in low doses and a short delay before the appearance of the pneumopathy. The different cytotoxic substances capable of inducing such pulmonary lesions are recalled as well as the mechanisms responsible for the phenomenon. The Authors compare their observations to the 5 well documented cases in the literature and suggest that hypersensitivity may have been a contributory factor in their case.

Angioimmunoblastic lymphadenopathy with paraproteinemia. Analysis of the clinical and biological characteristics and their prognostic significance.

We report a case of a double M component appearing in the course of angioimmunoblastic lymphadenopathy with dysproteinemia (AILD). Lymphocyte function, especially the decrease in T-suppressor cells, could play an essential role in the mechanisms of the disease. In the presence of an M component, the prognosis for AILD seems to be more pessimistic. Arguments for considering AILD as a premalignant disease are reviewed.
