RESUMEN

Given the importance of maximizing resection for prognosis in patients with HGG and the potential risks associated with ventricle opening, this study aimed to assess the actual increase in post-surgical complications related to lateral ventricle opening and its influence on OS and PFS. A retrospective study was conducted on newly diagnosed HGG, dividing the patients into two groups according to whether the lateral ventricle was opened (69 patients) or not opened (311 patients). PFS, OS, subependymal dissemination, distant parenchymal recurrences, the development of hydrocephalus and CSF leak were considered outcome measures. A cohort of 380 patients (154 females (40.5%) and 226 males (59.5%)) was involved in the study (median age 61 years). The PFS averaged 10.9 months (±13.3 SD), and OS averaged 16.6 months (± 16.3 SD). Among complications, subependymal dissemination was registered in 15 cases (3.9%), multifocal and multicentric progression in 56 cases (14.7%), leptomeningeal dissemination in 12 (3.2%) and hydrocephalus in 8 (2.1%). These occurrences could not be clearly justified by ventricular opening. The act of opening the lateral ventricles itself does not carry an elevated risk of dissemination, hydrocephalus or cerebrospinal fluid (CSF) leak. Therefore, if necessary, it should be pursued to achieve radical removal of the disease.

Seasonal hunger is the most common food insecurity experience for millions of small dryland farmers. This study tests the relationships between food insecurity, farm forests, and biomass poverty using a longitudinal dataset from the Amhara region of Ethiopia. These data form part of the Ethiopia Socioeconomic Survey, which collected panel data over three survey rounds from 530 households between 2011 and 2016. This dataset represents a collection of unique socioeconomic, wellbeing, and micro-land use measures, including farm forests. Hierarchical mixed effect regression models assessed the relationship between food insecurity and farm forests as well as the conditional effects of biomass poverty among the poorest farmers and women-headed households. Over a six-year study period, farmers reported increased stress from smaller land holdings, higher prices, and climate-related shocks. A clear trend towards spontaneous dispersed afforestation is observed by both researchers and satellite remote sensing. Model results indicate, dedicating approximately 10% of farm area to forest reduces months of food insecurity by half. The greatest reductions in food insecurity from farm forests are reported by ultra-poor and crop residue-burning households, suggesting that biomass poverty may be a major constraint to resilient food security on these farms. This research provides novel quantitative evidence of induced intensification and food security impacts of farm management preserving and building stores of biomass value as green assets. The results reported here have important implications for nature-based solutions as a major strategy to achieve sustainable development in some contexts.

Testing density-dependence and path-dependence in long-term population dynamics under differentiated local contexts contributes to delineate the changing role of socioeconomic forces at the base of regional disparities. Despite a millenary settlement history, such issue has been rarely investigated in Europe, and especially in highly divided countries such as those in the Mediterranean region. Using econometric modeling to manage spatial heterogeneity, our study verifies the role of selected drivers of population growth at ten times between 1921 and 2021 in more than 8000 Italian municipalities verifying density-dependent and path-dependent dynamics. Results of global and quantile (spatial) regressions highlight a differential impact of density and (lagged) population growth on demographic dynamics along the urban cycle in Italy. Being weakly significant in the inter-war period (1921-1951), econometric models totalized a high goodness-of-fit in correspondence with compact urbanization (1951-1981). Model's fit declined in the following decades (1981-2021) reflecting suburbanization and counter-urbanization. Density-dependence and path-dependence were found significant and, respectively, positive or negative, with compact urbanization, and much less intense with suburbanization and counter-urbanization. A spatial econometric investigation of density-dependent and path-dependent mechanisms of population dynamics provided an original explanation of metropolitan cycles, delineating the evolution of socioeconomic (local) systems along the urban-rural gradient.

In the current accelerated process of global warming, forest conservation is becoming more difficult to address in developing countries, where woodlands are often fueling the illegal economy. In Colombia, the issue of narcodeforestation is of great concern, because of the ramification of narcoactivities that are affecting forests, such as agribusinesses and cattle ranching for money laundering. In this study, we use spatially explicit regressions incorporating a factor decomposition of predictors through principal component analysis to understand the impact of coca plantations on global and local-scale deforestation in Colombia. At national level we find a positive and statistically significant relationship between coca crops and deforestation. At the regional level, in two out of four regions, it appears that coca is causing deforestation, especially in the Department of Northern Santander and on the Pacific coast. The spatial models used reveal not only a direct effect but also positive and significant spillover effects, in line with the conjecture that narcodeforestation is not only due to the quest for new areas to expand coca-cultivation, which would determine a loss of forest only in the municipality where coca cultivation increases, but also to the need to launder illegal profits, or create clandestine routes and airplane strips, which can affect forests also in nearby municipalities.

In light of global change, research on ecosystem dynamics and the related environmental policies are increasingly required to face with the inherent polarization in areas with low and high human pressure. Differential levels of human pressure are hypothesized to reflect development paths toward ecological stability of local systems vis à vis socioeconomic resilience. To delineate the latent nexus between socioeconomic development paths and ecological stability of local systems, we proposed a multidimensional, diachronic analysis of 28 indicators of territorial disparities, and ecological stability in 206 homogeneous administrative units of Czech Republic over almost 30 years (1990-2018). Mixing time-invariant factors with time-varying socio-environmental attributes, a dynamic factor analysis investigated the latent relationship between ecosystem functions, environmental pressures, and the background socioeconomic characteristics of the selected spatial units. We identified four geographical gradients in Czech Republic (namely elevation, economic agglomeration, demographic structure, and soil imperviousness) at the base of territorial divides associated with the increased polarization in areas with low and high human pressure. The role of urbanization, agriculture, and loss of natural habitats reflective of rising human pressure was illustrated along the selected gradients. Finally, policy implications of the (changing) geography of ecological disturbances and local development paths in Czech Republic were briefly discussed.

A short-term issue that has been occasionally investigated in the current literature is if (and, eventually, how) population dynamics (directly or indirectly) driven by COVID-19 pandemic have contributed to enlarge regional divides in specific demographic processes and dimensions. To verify this assumption, our study run an exploratory multivariate analysis of ten indicators representative of different demographic phenomena (fertility, mortality, nuptiality, internal and international migration) and the related population outcomes (natural balance, migration balance, total growth). We developed a descriptive analysis of the statistical distribution of the ten demographic indicators using eight metrics that assess formation (and consolidation) of spatial divides, controlling for shifts over time in both central tendency, dispersion, and distributional shape regimes. All indicators were made available over 20 years (2002-2021) at a relatively detailed spatial scale (107 NUTS-3 provinces) in Italy. COVID-19 pandemic exerted an impact on Italian population because of intrinsic (e.g. a particularly older population age structure compared with other advanced economies) and extrinsic (e.g. the early start of the pandemic spread compared with the neighboring European countries) factors. For such reasons, Italy may represent a sort of 'worst' demographic scenario for other countries affected by COVID-19 and the results of this empirical study can be informative when delineating policy measures (with both economic and social impact) able to mitigate the effect of pandemics on demographic balance and improve the adaptation capacity of local societies to future pandemic's crises.

Wildfire regimes affected by global change have been the cause of major concern in recent years. Both direct prevention (e.g., fuel management planning) and land governance strategies (e.g., agroforestry development) can have an indirect regulatory effect on wildfires. Herein, we tested the hypothesis that active land planning and management in Italy have mitigated wildfire impacts in terms of loss of ecosystem services and forest cover, and burned wildland-urban interface, from 2007 to 2017. At the national scale, we assessed the effect size of major potential fire drivers such as climate, weather, flammability, socio-economic descriptors, land use changes, and proxies for land governance (e.g., European funds for rural development, investments in sustainable forest management, agro-pastoral activities), including potential interactions, on fire-related impacts via Random Forest modelling and Generalized Additive Mixed Model. Agro-forest districts (i.e., aggregations of neighbouring municipalities with homogeneous forest and agricultural characteristics) were used as spatial units of analysis. Our results confirm that territories with more active land governance show lower wildfire impacts, even under severe flammability and climatic conditions. This study supports current regional, national, and European strategies towards "fire resistant and resilient landscapes" by fostering agro-forestry, rural development, and nature conservation integrated policies.

Vulnerability to land degradation in southern Europe has increased substantially in the last decades because of climate and land-use change, soil deterioration, and rising human pressure. The present work focuses on a quantitative evaluation of changes over time in the level of vulnerability to land degradation of a Mediterranean country (Italy) using a composite indicator, the environmentally sensitive area index (ESAI), which is the final outcome of a complex model conceived to assess land vulnerability on the basis of climate, soil, vegetation, and human pressure. Considering four different levels of vulnerability to land degradation (not affected, potentially affected, fragile, and critical), the main trajectories of this index were highlighted in a long-time perspective (1960-2010), discriminating dynamics over two sub-periods (1960-1990 and 1990-2010). The empirical results at a very detailed spatial scale (1 km2 grid) reflect spatial consolidation of degradation hot-spots over time. However, aggregated trajectories of change indicate an overall improvement in the environmental conditions between 1990 and 2010 compared with what is observed during the first period (1960-1990). Worse environmental conditions concerned southern Italian regions with a dry climate and poor soil conditions in the first time interval, large parts of northern Italy, traditionally recognized as a wet and affluent agricultural region, experienced increasing levels of land vulnerability in the second time interval. Being classified as an unaffected region according with the Italian national action plan (NAP), the expansion of (originally sparse) degradation hot-spots in northern Italy, reflective of an overall increase in critical areas, suggests a substantial re-thinking of the Italian NAP. This may lead to a redesign of individual regional action plans (RAPs) implementing place-specific approaches and comprehensive measures to be adopted to mitigate land degradation.

We presented an operational rationale grounded on complex system thinking to quantify structural and functional landscape transformations along three stages representative of post-war metropolitan development in Rome, Italy (urbanisation with population/settlement densification, 1949-1974; suburbanisation with medium-density settlement expansion, 1974-1999; counter-urbanisation with settlement sprawl, 1999-2016). A mathematical morphology approach assessing the geometric form of land patches and a multi-way factor analysis (MFA) of landscape metrics were used to investigate the joint evolution of urban form and land-use functions over time. The empirical results of the MFA delineated the multivariate relationship between nine land-use classes (with distinctive socioeconomic functions) and seven morphological types (reflecting different landscape structures) according to four observation times (1949, 1974, 1999, 2016). Taken as an intrinsic attribute of complex landscape systems experiencing intense transformations, an estimation of the 'rapidity-of-change' in the form-functions relationship at a given development stage was derived from MFA outcomes separately for urbanisation, suburbanisation, and counter-urbanisation. A simplified form-functions relationship, reflecting the spatial polarisation in compact settlements and rural (low-density) landscapes, was observed with compact urbanisation. By stimulating urban sprawl into fringe farmland, suburbanisation resulted in patchy and heterogeneous rural landscapes. Counter-urbanization was associated with the fragmentation of built-up settlements leading to a chaotic mosaic of land structures that mixes urban and rural traits. Rapidity-of-change in form-function relationships was greater during suburbanisation than urbanisation and counter-urbanisation. It reflects the intrinsic pressure of economic growth in contemporary cities.

Desertification risk depends on the interplay of biophysical and socioeconomic drivers, among which climate change, soil depletion, landscape modifications, and biodiversity decline are key factors of change in Southern Europe. The present study introduces a diachronic analysis of desertification risk in Italy adopting a multidimensional approach based on four dimensions (ecological, economic, demographic, and administrative) assessed at three dates (1961, 1991, and 2011). These risk components were evaluated separately in Southern Italy, a formerly affected region (sensu United Nations Convention to Combat Desertification), and Northern/Central Italy, a nonaffected region in the country. All risk measures document how the divide between affected and nonaffected regions in Italy has gradually reduced. Because of local warming and rising human pressure, Northern Italy has recently displayed a level of desertification risk close to those observed in Southern Italy over the last 30 years. These results suggest a thorough revision of the national classification of risky areas, that may inform more specific mitigation and adaptation policies responding effectively to recent socioenvironmental trends and local (economic) dynamics. The intrinsic system's evolution observed at both regional and national level in Italy may be generalized to a broader European context. Our work finally documents the appropriateness of a multidimensional definition of desertification risk grounded on the joint analysis of ecological, demographic, economic, and administrative indicators. A comprehensive knowledge of socioeconomic patterns and processes of change contributes to more precise scenario modeling and design of integrated strategies mitigating desertification risk.

Earlier studies relating form and functions of cities address an intriguing and complex research issue, especially for specific urban typologies. Although with inherent differences on a local scale, Mediterranean cities represent diversified settlement morphologies and multifaceted socioeconomic contexts. The present study investigates the socioeconomic structure at the base of rapid development of a large Mediterranean agglomeration (Athens, Greece). Results of a multivariate analysis of the spatial distribution of average (per-capita) declared income and non-parametric correlations of contextual indicators suggest that the characteristic socio-spatial structure of the 1970s and the 1980s in Athens had slightly changed in recent times. A remarkable segregation in wealthy and disadvantaged communities - well beyond the urban-rural divide usually observed in Mediterranean regions - consolidated in recent decades. Despite economic transformations shaping urban design and infrastructural networks, persistent disparities between affluent and economically depressed neighborhoods still characterize the socio-spatial structure of contemporary Athens.

BACKGROUND: Cerebral cavernous malformations (CCMs) are a major type of cerebrovascular lesions of proven genetic origin that occur in either sporadic (sCCM) or familial (fCCM) forms, the latter being inherited as an autosomal dominant condition linked to loss-of-function mutations in three known CCM genes. In contrast to fCCMs, sCCMs are rarely linked to mutations in CCM genes and are instead commonly and peculiarly associated with developmental venous anomalies (DVAs), suggesting distinct origins and common pathogenic mechanisms. CASE REPORT: A hemorrhagic sCCM in the right frontal lobe of the brain was surgically excised from a symptomatic 3 year old patient, preserving intact and pervious the associated DVA. MRI follow-up examination performed periodically up to 15 years after neurosurgery intervention demonstrated complete removal of the CCM lesion and no residual or relapse signs. However, 18 years after surgery, the patient experienced acute episodes of paresthesia due to a distant recurrence of a new hemorrhagic CCM lesion located within the same area as the previous one. A new surgical intervention was, therefore, necessary, which was again limited to the CCM without affecting the pre-existing DVA. Subsequent follow-up examination by contrast-enhanced MRI evidenced a persistent pattern of signal-intensity abnormalities in the bed of the DVA, including hyperintense gliotic areas, suggesting chronic inflammatory conditions. CONCLUSIONS: This case report highlights the possibility of long-term distant recurrence of hemorrhagic sCCMs associated with a DVA, suggesting that such recurrence is secondary to focal sterile inflammatory conditions generated by the DVA.

Natural population growth is an intrinsic property of demographic systems that depends on (spatially) non-stationary processes of fertility and mortality. Assuming distinctive demographic dynamics as a characteristic attribute of urban, suburban and rural systems, analysis of spatial variability in natural population growth delineates nonlinear stages of metropolitan expansion, possibly reflecting divergent responses to socioeconomic stimuli. The present study investigates endogenous population growth (1956-2019) and the relationship with demographic density as basic attributes of individual stages of the city life cycle in Athens (Greece), a mono-centric metropolitan region in Southern Europe. A spatially explicit analysis of natural balance rates at local scale identified two stages of growth, namely compact urbanization (mid-1950s to late 1970s)-with agglomeration strengthening the polarization in demographically dynamic and shrinking districts-and spatially decentralized suburbanization (early 1980s to late 2010s)-with a less defined role of agglomeration economies and more heterogeneous demographic processes. However, the impact of population density on endogenous growth was stronger in recent decades, suggesting how demographic dynamics may still respond to agglomeration stimuli, at least during recessions. At the same time, the spatial structure of natural balance rates became more mixed, likely reflecting the importance of heterogeneous demographic behaviors at the individual level.

Despite the multidisciplinary management in the treatment of glioblastomas, the average survival of GBM patients is still 15 months. In recent years, molecular biomarkers have gained more and more importance both in the diagnosis and therapy of glial tumors. At the same time, it has become clear that non neoplastic cells, which constitute about 30% of glioma mass, dramatically influence tumor growth, spread, and recurrence. This is the main reason why, in recent years, scientific research has been focused on understanding the function and the composition of tumor microenvironment and its role in gliomagenesis and recurrence. The aim of this review is to summarize the most recent discovery about resident microglia, tumor-associated macrophages, lymphocytes, and the role of extracellular vesicles and their bijective interaction with glioma cells. Moreover, we reported the most recent updates about new therapeutic strategies targeting immune system receptors and soluble factors. Understanding how glioma cells interact with non-neoplastic cells in tumor microenvironment is an essential step to comprehend mechanisms at the base of disease progression and to find new therapeutic strategies for GBM patients. However, no significant results have yet been obtained in studies targeting single molecules/pathways; considering the complex microenvironment, it is likely that only by using multiple therapeutic agents acting on multiple molecular targets can significant results be achieved.

Vulnerability to land degradation in Mediterranean Europe increased substantially in the last decades because of the latent interplay of climate and land-use change, progressive soil deterioration, and rising human pressure. The present study provides a quantitative evaluation of the intrinsic change over time in the level of vulnerability to land degradation over a representative Mediterranean area (Italy) using a normative indicator, the percentage of land classified as 'critical' in total area. This indicator derives from a spatially explicit elaboration of the ESA (Environmental Sensitive Area) Index (ESAI), a standard methodology of land classification considering different levels of vulnerability to degradation at a particularly refined spatial scale (1 km2). This indicator was calculated over a relatively long time interval (1960-2010) and aggregated at the geographical scale of administrative regions in Italy, a relevant domain in the implementation of the National Action Plan (NAP) to combat desertification and the adoption of individual Regional Action Plans (RAP). A significant - but spatially heterogeneous - increase in 'critical' land was observed in Italy, leading to distinctive dynamics in northern/central regions and southern regions. Climate aridity and anthropogenic pressure leveraged the sudden vulnerability in some marginal land of Northern Italy - a region classified as unexposed to desertification risk - paralleling the levels observed in some districts of Southern Italy, an 'affected' region to desertification risk. These results suggest a re-thinking of mitigation policies proposed in the Italian NAP and a redesign of the RAPs toward place-specific adaptation measures, especially in the 'less exposed' Northern Italian region.

The United Nations Convention to Combat Desertification (UNCCD) assumes spatial disparities in land resources as a key driver of soil degradation and early desertification processes all over the world. Although regional divides in soil quality have been frequently observed in Mediterranean-type ecosystems, the impact of landscape configuration on the spatial distribution of sensitive soils was poorly investigated in Southern Europe, an affected region sensu UNCCD. Our study proposes a spatially explicit analysis of 16 ecological metrics (namely, patch size and shape, fragmentation, interspersion, and juxtaposition) applied to three classes of a landscape with different levels of exposure to land degradation ('non-affected', 'fragile', and 'critical'). Land classification was based on the Environmentally Sensitive Area Index (ESAI) calculated for Italy at 3 time points along a 50-year period (1960, 1990, 2010). Ecological metrics were calculated at both landscape and class scale and summarized for each Italian province-a relevant policy scale for the Italian National Action Plan (NAP) to combat desertification. With the mean level of soil sensitivity rising over time almost everywhere in Italy, 'non-affected' land became more fragmented, the number of 'fragile' and 'critical' patches increased significantly, and the average patch size of both classes followed the same trend. Such dynamics resulted in intrinsically disordered landscapes, with (i) larger (and widely connected) 'critical' land patches, (ii) spatially diffused and convoluted 'fragile' land patches, and (iii) a more interspersed and heterogeneous matrix of 'non affected' land. Based on these results, we discussed the effects of increasing numbers and sizes of 'critical' patches in terms of land degradation. A sudden expansion of 'critical' land may determine negative environmental consequences since (i) the increasing number of these patches may trigger desertification risk and (ii) the buffering effect of neighboring, non-affected land is supposed to be less efficient, and this contains a downward spiral toward land degradation less effectively. Policy strategies proposed in the NAPs of affected countries are required to account more explicitly on the intrinsic, spatio-temporal evolution of 'critical' land patches in affected regions.

Climate change and landscape transformation have led to rapid expansion of peri-urban areas globally, representing new 'laboratories' for the study of human-nature relationships aiming at land degradation management. This paper contributes to the debate on human-driven land degradation processes by highlighting how natural and socioeconomic forces trigger soil depletion and environmental degradation in peri-urban areas. The aim was to classify and synthesise the interactions of urbanisation-driven factors with direct or indirect, on-site or off-site, and short-term or century-scale impacts on land degradation, focussing on Southern Europe as a paradigmatic case to address this issue. Assuming complex and multifaceted interactions among influencing factors, a relevant contribution to land degradation was shown to derive from socioeconomic drivers, the most important of which were population growth and urban sprawl. Viewing peri-urban areas as socio-environmental systems adapting to intense socioeconomic transformations, these factors were identified as forming complex environmental 'syndromes' driven by urbanisation. Based on this classification, we suggested three key measures to support future land management in Southern European peri-urban areas.

Southern Europe is a hotspot for desertification risk because of the intimate impact of soil deterioration, landscape transformations, rising human pressure, and climate change. In this context, large-scale empirical analyses linking landscape fragmentation with desertification risk assume that increasing levels of land vulnerability to degradation are associated with significant changes in landscape structure. Using a traditional approach of landscape ecology, this study evaluates the spatial structure of a simulated landscape based on different levels of vulnerability to land degradation using 15 metrics calculated at three time points (early-1960s, early-1990s, early-2010s) in Italy. While the (average) level of land vulnerability increased over time almost in all Italian regions, vulnerable landscapes demonstrated to be increasingly fragmented, as far as the number of homogeneous patches and mean patch size are concerned. The spatial balance in affected and unaffected areas-typically observed in the 1960s-was progressively replaced with an intrinsically disordered landscape, and this process was more intense in regions exposed to higher (and increasing) levels of land degradation. The spread of larger land patches exposed to intrinsic degradation brings to important consequences since (1) the rising number of hotspots may increase the probability of local-scale degradation processes, and (2) the buffering effect of neighbouring (unaffected) land can be less effective on bigger hotspots, promoting a downward spiral toward desertification.

BACKGROUND: Gliomas are among the most challenging pathologies for neurosurgeons due to their infiltrative and recurrent nature in functionally relevant regions. Current knowledge confirms that gross total resection highly influence survival in patient with glioma. However, surgery performed in eloquent brain area, could seriously compromise the quality of life in patient with reduced life expectancy even more if it concerns the language function. METHODS: 18 right-handed patients with perisylvian gliomas on the left hemisphere were prospectively analyzed over a period of 12 months. Standardized preoperative Diffusion-Tensor-Imaging based tractography of the five main language Tracts (Arcuate Fasciculus, Frontal Aslant Tract, Inferior Fronto-Occipital Fasciculus, Inferior Longitudinal Fasciculus, Uncinate Fasciculus) was navigated during the surgical procedure. Using a validated method, correlations were made between the pre-operative fascicles and their possible infiltration and surgical damage. The language status was assessed using the Aachen Aphasia Test. RESULTS: In all nine patients who developed a permanent disorder there was pre-operative involvement of at least one fascicle and resection of at least one of these. In this way, areas of high risk of permanent language damage have emerged as a result of surgical injury: the temporoparietal junction, the middle portion of the FAT and the temporal stem. CONCLUSIONS: Navigated tractography has proven to be a user-friendly tool that can assess perioperative risk, guide surgical resection, and help the neurosurgeon to find that balance between tumor resection and function preservation.

The COVID-19 pandemic has rapidly evolved into an acute health crisis with extensive socioeconomic and demographic consequences. The severity of the COVID-19 pandemic requires a refined (and more comprehensive) understanding of virus dissemination over space, transmission mechanisms, clinical features, and risk factors. In line with this assumption, the present study illustrates a comparative, empirical analysis of the role of socioeconomic and demographic dimensions in the early stages of the COVID-19 pandemic grounded on a large set of indicators comparing the background context across a global sample of countries. Results indicate that-in addition to epidemiological factors-basic socioeconomic forces significantly shaped contagions as well as hospitalization and death rates across countries. As a response to the global crisis driven by the COVID-19 pandemic, all-embracing access to healthcare services should be strengthened along with the development of sustainable health systems supported by appropriate resources and skills. The empirical findings of this study have direct implications for the coordination of on-going, global efforts aimed at containing COVID-19 (and other, future) pandemics.
