OBJECTIVE: To assess maternal mortality (MM) in Brazilian Black, Pardo, and White women. METHODS: We evaluated the maternal mortality rate (MMR) using data from the Brazilian Ministry of Health public databases from 2017 to 2022. We compared MMR among Black, Pardo, and White women according to the region of the country, age, and cause. For statistical analysis, the Q2 test prevalence ratio (PR) and confidence interval (CI) were calculated. RESULTS: From 2017 to 2022, the general MMR was 68.0/100,000 live births (LB). The MMR was almost twice as high among Black women compared to White (125.81 vs 64.15, PR = 1.96, 95%CI:1.84-2.08) and Pardo women (125.8 vs 64.0, PR = 1.96, 95%CI: 1.85-2.09). MMR was higher among Black women in all geographical regions, and the Southeast region reached the highest difference among Black and White women (115.5 versus 60.8, PR = 2.48, 95%CI: 2.03-3.03). During the covid-19 pandemic, MMR increased in all groups of women (Black 144.1, Pardo 74.8 and White 80.5/100.000 LB), and the differences between Black and White (PR = 1.79, 95%CI: 1.64-1.95) and Black and Pardo (PR = 1.92, 95%CI: 1.77-2.09) remained. MMR was significantly higher among Black women than among White or Pardo women in all age ranges and for all causes. CONCLUSION: Black women presented higher MMR in all years, in all geographic regions, age groups, and causes. In Brazil, Black skin color is a key MM determinant. Reducing MM requires reducing racial disparities.

RESUMEN

This is a scoping review on mapping the use of digital tools to assess food consumption in Brazil. Searches were carried out in nine electronic databases (Medline, Lilacs, Scopus, Embase, Web of Science, Science Direct, Ovid, Free Medical Journal and Crossref) to select studies published from October 2020 to December 2023. This review identified forty-eight digital tools in the 94 publications analyzed, the most frequent being web-based technologies (60%) and mobile devices (40%). Among these studies, 55% (n = 52) adopted a population-based approach, while 45% (n = 42) focused on specific regions. The predominant study design observed was cross-sectional (n = 63). A notable trend observed was the increasing frequency of validation studies in recent years. Although the use of digital tools in the assessment of food consumption in Brazil has grown in recent years, studies did not describe the process of creating and validating the tools, which would contribute to the improvement of data quality. Investments that allow the expansion of the use of the internet and mobile devices; the improvement of digital literacy; and the development of open-access tools, especially in the North and Northeast regions, are challenges that require a concerted effort towards providing equal opportunities, fostering encouragement, and delving deeper into the potential of digital tools within studies pertaining to food consumption in Brazil.

BACKGROUND: Chronic cerebral hypoperfusion in vascular dementia leads to memory and motor deficits; Physical exercise improves these aspects and promotes neuroprotection. Sexual dimorphism may significantly influence both ischemic and exercise outcomes. AIMS: The aim of this study was to investigate the effects of 2VO (Two-Vessel occlusion) and the acrobatic training on motor function, functional performance, and tissue loss in male and female rats. METHODS: Male and female rats were randomly divided into 4 groups: sham acrobatic, sham sedentary, 2VO acrobatic and 2VO sedentary. After 45 days of 2VO surgery, the animals received 4 weeks of acrobatic training. At the end, open field, beam balance and horizontal ladder tests were performed. Brain samples were taken for histological and morphological evaluation. RESULTS: Spontaneous motor activity in the open field was not affected by 2VO, on the other hand, an impairment in forelimb placement was observed after 2VO and acrobatic training prevented errors and improved hindlimb placement. Neuronal loss was found in the motor cortex and striatum after 2VO, especially in females, which was prevented by acrobatic training. CONCLUSION: Mild motor damage was found in animals after 2VO when refined movement was evaluated, probably associated to neuronal death in the motor cortex and striatum. The acrobatic exercise showed a neuroprotective effect, promoting neuronal survival and attenuating the motor deficit.

OBJECTIVES: Since peri-implantitis is an increasing and prevalent concern in clinical practice and there is no consensus regarding the best therapeutic protocol, this study evaluated the knowledge and behaviours of dentists working in Implantology regarding implant-related infections modulating factors and therapeutic protocols used in the management of peri-implantitis. METHODS: Cross-sectional study was conducted with 86 Brazilian Implantology clinicians. Data were collected using a structured and online questionnaire evaluating socioeconomic characteristics, education, work/clinical practice, knowledge and attitudes regarding the risk factors and management of peri-implantitis. The reliability of the questionnaire was evaluated by test-retest technique. The questionnaire was developed based on the last consensus on peri-implant diseases (2018) and the current evidence related to implant-related infections. Descriptive, bivariate and logistic regression analyses were conducted adopting a significance level of 5%. RESULTS: In this study, 89.5% of included dentists reported that already treated patients with peri-implantitis. Approximately 80% of dentists use antibiotics and mouth rinses during the treatment, and surgical procedures seem the main choice to treat peri-implantitis (91.8%) by dentists. As a preventive approach, 94.2% of dentists reported that routinely assessed biofilm accumulation in the follow-up visits after implant placement. Logistic regression showed that the self-reported ability to treat peri-implantitis was statistically (p < 0.05) higher among dentists who reported abilities to diagnose the disease and use laser for peri-implantitis treatment. CONCLUSION: Dentists working in Implantology have a good level of knowledge and behaviors in the management of peri-implantitis. However, the lack of consensus regarding the best treatment protocols may reflect dentist's behaviours because different treatment protocols have been used by evaluated clinicians.

Sarcopenia (the loss of muscle mass, strength and skeletal muscle function) increases mortality and the risk of hospitalization in the older population. Although it is known that older adults with type 2 diabetes mellitus (T2DM) have a higher risk of dynapenia and sarcopenia, few studies have investigated these conditions in middle-aged populations. The objective of this study was to investigate whether T2DM, its duration, the presence of albuminuria, and glycemic control are associated with sarcopenia and its components in adults. The cross-sectional analysis was based on data from visit 2 of the Brazilian Longitudinal Study of Adult Health (2012-2014). The 2018 European Working Group on Sarcopenia in Older People criteria were used to define dynapenia, low appendicular muscle mass (LAMM), and sarcopenia (absent/probable/confirmed). The explanatory variables were: T2DM; duration of T2DM; T2DM according to the presence of albuminuria; and glycemic control (HbA1C < 7%) among people with T2DM. A total of 12,132 participants (mean age = 55.5, SD: 8.9 years) were included. The odds ratio for LAMM was greater among those with T2DM, T2DM duration from 5 to 10 years, and T2DM without albuminuria. Chances of dynapenia were higher among those with T2DM, T2DM duration > 10 years, and T2DM with and without albuminuria. The variables T2DM, T2DM ≥ 10 years, and T2DM with albuminuria increased the odds of probable sarcopenia, and T2DM duration from 5 to 10 years increased the odds of confirmed sarcopenia. The results support the importance of frequently monitoring the musculoskeletal mass and strength of individuals with T2DM to prevent sarcopenia and related outcomes.

BACKGROUND: Peripheral blood analysis is a non-invasive and low-cost technique of prognostic value for several diseases, including oral cancer. Considering the role of inducible nitric oxide synthase in tumor-associated inflammation, this study purposed to evaluate the influence of this enzyme on peripheral blood parameters and systemic inflammatory biomarkers during murine oral carcinogenesis. METHODS: A 50 µg/mL solution of 4-nitroquinoleine-N-oxide was provided to 15 C57BL/6J (Nos2+/+ ) and 16 B6.129P2-Nos2tm1Lau /J (Nos2-/- ) for 16 weeks. Animals were followed for 8 weeks after treatment. Blood samples and tongues were collected for hematological and histopathological analyses. Red blood cells, white blood cells, and platelet cell parameters were analyzed. The neutrophil-to-lymphocyte ratio, platelet-to-lymphocyte ratio, and the systemic immune-inflammation index were also calculated. The depth of invasion of all carcinomas was measured. RESULTS: Differences were found in several blood parameters. The depth of invasion in Nos2-/- was lower than in Nos2+/+ (p = 0.009), and strong correlations were found between depth of invasion and neutrophil count (ρ = -0.68, p = 0.017), lymphocyte count (ρ = 0.72, p = 0.011), neutrophil-to-lymphocyte ratio (ρ = -0.65, p = 0.025), platelet-to-lymphocyte ratio (ρ = -0.73, p = 0.013), and systemic immune-inflammation index (ρ = -0.67, p = 0.037) in Nos2-/- mice. CONCLUSION: Inducible nitric oxide synthase seems to have an important role in OSCC invasion and progression, which might be associated to alterations in immune-inflammatory cell dynamics evidenced by peripheral blood and systemic inflammatory biomarkers.

Idosos com diabetes mellitus tipo 2 (DMT2) apresentam risco elevado de desenvolver dinapenia e sarcopenia, mas poucos estudos investigaram populações de meia-idade. O objetivo foi investigar se DMT2, sua duração, a presença de albuminúria e o controle glicêmico estão associados à sarcopenia e seus componentes em adultos. Análise transversal baseada nos dados da segunda visita do Estudo Longitudinal de Saúde do Adulto (2012-2014). Os critérios do European Working Group on Sarcopenia in Older People [Grupo de Trabalho Europeu sobre Sarcopenia em Pessoas Idosas] de 2018 foram usados para definir dinapenia, baixa massa muscular apendicular e sarcopenia (ausente/provável/confirmada). As variáveis explicativas foram: DMT2; duração do DMT2; DMT2 de acordo com a presença de albuminúria; e controle glicêmico (HbA1c < 7%) entre pessoas com DMT2. Foram incluídos 12.132 participantes (idade média de 55,5; DP: 8,9 anos). A razão de chances para baixa massa muscular apendicular foi maior entre pessoas com DMT2, duração do DMT2 entre 5 e 10 anos e DMT2 sem albuminúria. As chances de dinapenia foram maiores entre pessoas com DMT2, duração do DMT2 > 10 anos e DMT2 com e sem albuminúria. DMT2, DMT2 ≥ 10 anos e DMT2 com albuminúria aumentaram as chances de sarcopenia provável e duração do DMT2 entre 5 e 10 anos aumentaram as chances de sarcopenia confirmada. Os resultados reforçam a importância do monitoramento frequente da massa e da força muscular em indivíduos com DMT2 para prevenir a sarcopenia e desfechos relacionados.

El objetivo fue investigar si la DMT2, su duración, la presencia de albuminuria y el control glucémico están asociados con la sarcopenia y sus componentes en adultos. Análisis transversal basado en los datos de la visita 2 del Estudio Longitudinal de Salud del Adulto en Brasil (2012-2014). Se utilizaron los criterios del European Working Group on Sarcopenia in Older People [Grupo de Trabajo Europeo sobre Sarcopenia en Personas Mayores] del 2018 para definir dinapenia, baja masa muscular apendicular y sarcopenia (ausente/probable/confirmada). Las variables explicativas fueron las siguientes: DMT2; duración de la DMT2; DMT2 según la presencia de albuminuria; y control glucémico (HbA1c < 7%) entre personas con DMT2. Se incluyeron 12.132 participantes (edad media = 55,5, DE: 8,9 años). La razón de probabilidades de masa muscular apendicular baja fue mayor entre personas con DMT2, duración de la DMT2 entre 5 y 10 años y DMT2 sin albuminuria. Las probabilidades de dinapenia fueron mayores entre las personas con DMT2, duración de la DMT2 > 10 años y DMT2 con y sin albuminuria. Las condiciones de DMT2, DMT2 ≥ 10 años y DMT2 con albuminuria aumentaron las probabilidades de sarcopenia probable y la duración de la DMT2 entre 5 y 10 años las probabilidades de sarcopenia confirmada. Los resultados refuerzan la importancia del monitoreo frecuente de la masa y de la fuerza musculoesquelética en individuos con DMT2 para prevenir la sarcopenia y los desenlaces relacionados.

Resumo O objetivo do estudo foi avaliar o uso de medicamentos associados à Covid-19 por usuários da Atenção Primária à Saúde (APS). Trata-se de um estudo transversal com coleta de dados nas unidades de Estratégia Saúde da Família do município de Rondonópolis, MT, Brasil. Os dados foram obtidos com aplicação de um formulário semiestruturado. Dentre os participantes, 36% afirmaram ter consumido fármacos para prevenção da Covid-19, como ivermectina (89,58%), vitamina D (13,88%) e azitromicina (6,94%). Residir em domicílio em que duas ou mais pessoas precisaram sair para trabalhar na pandemia foi associado ao uso de medicamentos preventivos da Covid-19 (OR: 1,7; IC95%: 1,09 - 2,60). Os resultados indicam a necessidade de realizar ações para a promoção do uso racional de medicamentos nos territórios da APS, com vistas a capacitação profissional, educação popular em saúde e combate à desinformação.


Abstract The study aimed to evaluate the use of medications associated with Covid-19 by users of Primary Health Care (PHC). This is a cross-sectional study with data collection in Family Health Strategy units in the city of Rondonopolis, MT, Brazil. Data were obtained using a semi-structured form. Among the participants, 36% said they had consumed drugs to prevent Covid-19, such as ivermectin (89.58%), vitamin D (13.88%) and azithromycin (6.94%). Living in a household where two or more people had to go out to work during the pandemic was associated with the use of Covid-19 preventive medications (OR: 1.7; 95% CI: 1.09 - vccccccc 2.60). The results indicate the need to carry out actions to promote the rational use of medicines in PHC territories, with a view to professional training, popular health education and combating misinformation.

Este artigo tem como objetivo descrever as experiências de comunicação sobre saúde, desenvolvidas com comunidades quilombolas na Amazônia, através de mídias sociais durante a pandemia de covid-19. A disseminação de mensagens educativas foi uma das alternativas utilizadas pelas organizações quilombolas e por seus parceiros, para proporcionar informação e orientação sobre saúde. São apresentadas experiências acumuladas a partir das ações desenvolvidas por voluntários, no período de 2020 a 2022. Entre as mídias digitais e as estratégias utilizadas, destacam-se os cards digitais produzidos e distribuídos, principalmente, por meio do aplicativo de mensagens WhatsApp. Refletimos sobre a importância do uso das mídias sociais como instrumentos de comunicação educacional para a promoção da saúde e prevenção da covid-19, em uma era tecnológica que potencializa trocas intensas de informações e conhecimentos. No caso abordado, a educação sobre saúde, através de mídias sociais/digitais, mostrou-se um instrumento essencial para a minimização dos impactos da covid-19 na população quilombola


This article aims to describe the health communication experiences developed with quilombola commu-nities in the Amazonia through social media in the covid-19 pandemic. The dissemination of educational messages was one of the alternatives used by quilombola organizations and their partners to provide health information and guidance. Accumulated experiences are presented based on the actions carried out by volunteers in the period from 2020 to 2022. Among the digital media and strategies used, most important are the digital cards produced and distributed mainly through the WhatsApp Messenger. We reflect on the importance of using social media as educational communication tools for health promotion and prevention of covid-19, in a technological era that intensifies intense exchanges of information and knowledge. In the case approached here, health education through social/digital media proved to be an essential tool for minimizing the impacts of covid-19 on the quilombola population


Este artículo tiene como objetivo describir las experiencias de comunicación en salud desarrolladas con comunidades quilombolas de la Amazonia a través de las redes sociales durante la pandemia de covid-19. La difusión de mensajes educativos fue una de las alternativas utilizadas por las organizaciones quilombolas y por sus aliados para proporcionar información y orientación sobre salud. Se presentam experiencias acumuladas a partir de las acciones desarrolladas por voluntarios en el periodo de 2020 a 2022. Entre los medios digitales y estrategias utilizados, se destacan las tarjetas digitales producidas y distribuidas principalmente a través de la aplicación de mensajería WhatsApp. Reflexionamos sobre la importancia de utilizar los medios sociales como herramientas de comunicación educativa para la promoción de la salud y la prevención de la covid-19, en una era tecnológica que potencia intensos intercambios de información y conocimiento. En el caso relatado, la educación para la salud a través de medios sociales/digitales demostró ser una herramienta esencial para minimizar los impactos de la covid-19 en la población quilombola

OBJECTIVES: To identify the possible causes of low adherence to vaccination campaigns in Brazil, find and analyze campaigns regarding human papillomavirus (HPV) in Brazil and abroad, and apply quality tools to prepare proposals to increase vaccination coverage (VC) and prevent HPV in the country. METHOD: This is a qualitative and deductive-hypothetical research. A narrative review of the literature (especially on the narratives and formats applied in vaccination campaigns in Brazil) was the technique used to develop our method. RESULTS: Brazil had a 49.6% VC in 2019, unlike countries such as Australia (80.2% in 2017), Mexico (97.5% in 2019), and Peru (91% in 2019). This study found evidence of the use of social marketing strategies to engage communities as good practices in the vaccination campaigns of these countries. CONCLUSION: With the retrieved information, three quality tools (Ishikawa diagram, Pareto graph, and 5W2H) classified and quantified the causes of low VC in Brazil and enabled proposals that can direct its Ministry of Health to take more effective strategies to achieve the HPV VC goal recommended by the WHO.

RESUMEN

Tingui biochar (TB) activated with potassium hydroxide (TB-KOH) was synthesized in the present study. The adsorption capacity of TB-KOH was evaluated for the removal of acetaminophen and caffeine in monocomponent and bicomponent solutions. As a result, the study of the TB-KOH characterization as well as the adsorption kinetics, isotherm, thermodynamics, and a suggestion of the global adsorption mechanism are presented. TB-KOH was characterized through physical-chemical analysis to understand its surface morphology and how it contributes to the adsorption of these drugs. Furthermore, modelling using advanced statistical physical models was performed to describe how acetaminophen and caffeine molecules are adsorbed in the active sites of TB-KOH. Through the characterizations, it was observed that the activation with KOH contributed to the development of porosity and functional groups (-OH, C-O, and C = O) on the surface of TB. The monocomponent adsorption equilibrium was reached in 90 min with a maximum adsorption capacity of 424.7 and 350.8 mg g-1 for acetaminophen and caffeine, respectively. For the bicomponent solution adsorption, the maximum adsorption capacity was 199.4 and 297.5 mg g-1 for acetaminophen and caffeine, respectively. The isotherm data was best fitted to the Sips model, and the thermodynamic study indicated that acetaminophen removal was endothermic, while caffeine removal was exothermic. The mechanism of adsorption of acetaminophen and caffeine by TB-KOH was described by the involvement of hydrogen bonds and π-π interactions between the surface of TB-KOH and the molecules of the contaminants.

Human activities significantly impact the aquatic biota in lacustrine environments, leading to negative environmental consequences. Due to this, this study was carried out to evaluate the genotoxic and histological biomarkers in the gills and kidneys of Prochilodus lacustris. The collection of fish and water was conducted in three distinct stations (S1, S2, S3) in an international protected area in northeast Brazil. The histological alterations index was used to assess changes in the gills and kidneys at a histological level. In addition, micronucleus and nuclear abnormalities tests were employed to examine damage to the genetic material. According to the frequency of Micronucleus, nuclear injuries, and the histopathology of gills and kidneys, it can be inferred that station S1 had the highest impact on the fish, causing significant harm. Based on our findings, the organisms residing in Lake Açu are adversely affected by the poor environmental conditions. This raises doubts about the effectiveness of the Protection Area by Humid Zone of International Interest in Northeast Brazil.

Immunoglobulin Y (IgY) could be used in serological diagnosis focused on several infectious agents. This study aims to produce IgY anti-hepatitis B virus surface antigen (anti-HBs) and to assess its use in enzyme immunoassays. Antibodies were produced by immunizing chickens with Hepatitis B vaccine associated (group A), or not, with adjuvant CpG-ODN (group B). Eggs were collected for 20 weeks, yolks were purified based on using polyethylene glycol and affinity chromatography. IgY anti-HBs was featured based on SDS-PAGE and Western Blot techniques. Total protein concentration was measured through spectrophotometry. In-house ELISA used to detect HBsAg was developed based on using IgG/HRP conjugate and IgY-anti-HBs sensitized microplates. Thus, IgY anti-HBs were confirmed through molecular pattern based on SDS-PAGE, whereas specificity of anti-HBs was confirmed through Western Blot. Mean total protein reached 3.27 ± 3.00 mg/mL and 3.11 ± 3.12 mg/mL in groups A and B, respectively. In-house ELISA was developed based on using a panel of HBV positive and negative serum samples; it recorded 100 % sensitivity and 78.9 % specificity to detect HBsAg. In conclusion, it was possible producing anti-HBs IgY by immunizing chickens with HBV vaccine; this molecule could be used as capture antibody to help detecting HBsAg in-house ELISA.

RESUMEN

Mayaro virus (MAYV), the etiologic agent of Mayaro fever, leads patients to severe myalgia and arthralgia, which can have a major impact on public health in all the countries where the virus circulates. The emergence and dissemination of new viruses have led the scientific community to develop new in vivo models that can help in the fight against new diseases. So far, mice have been the most used animal model in studies with MAYV and have proved to be an adequate model for recapitulating several aspects of the disease observed in humans. Mice are widely used in in vivo research and, therefore, are well known in the scientific community, which has allowed for different strains to be investigated in the study of MAYV. In this review, we summarize the main studies with MAYV using mice as an experimental model and discuss how they can contribute to the advancement of the understanding of its pathogenesis and the development of new drugs and vaccines.

This study addresses the current trend of essential oils in alternative medicine using the non-chordate model Drosophila melanogaster. Following the three R's principles, it proposes non-chordate models to fill knowledge gaps on essential oil toxicity. Copaiba, lavender, and ginger essential oils are evaluated for effects on D. melanogaster lifespan, climbing ability, and brain structure, while their anti-inflammatory properties are also analyzed. Results show dose-related differences: higher concentrations (0.25% v/v) cause brain deterioration and impaired climbing, while lower concentrations (0.0625% v/v for copaiba and ginger; 0.125% for lavender) have no effect on climbing or brain structure. Lavender oil significantly extends lifespan and maintains anti-inflammatory activity when ingested, underscoring its therapeutic potential. These findings highlight the importance of D. melanogaster as a model for studying essential oil properties, potentially replacing chordate models. In addition, this research advances alternative remedies for currently incurable diseases, with lavender oil emerging as a promising candidate for drug discovery.

In this work, the synthesis of activated carbon from the bark of the Magonia pubescens (known as Tingui) and its efficiency in the removal of diclofenac sodium through batch adsorption tests and physical-chemical characterizations were investigated. The phytotoxicity of this material was also evaluated through germination and root growth of Lactuca sativa seeds. According to the experimental design performed for the synthesis of Tingui carbon, the optimized temperature and residence time for the production of this adsorbent were 550 °C and 120 min, respectively. The equilibrium time was reached in 600 min and the theoretical model that best fitted the kinetic data was the Elovich model. The BET was the best fit for the adsorption isotherm dataThis indicates that the adsorption process of sodium diclofenac by activated carbon can occur by two different mechanisms, monolayer and/or multilayer adsorption, depending on the conditions employed in the process, such as temperature and adsorbate concentration. The thermodynamic study showed that the process was favourable and spontaneous in the temperature range evaluated. Furthermore, the characterizations showed by TG/DTG and FTIR analyses that the temperature throughout the process had a marked impact on the degradation of the organic constituents of the biomass and the appearance of distinct functional groups that contributed to the adsorption process of diclofenac sodium. Finally, the toxicity tests recognized that this adsorbent does not affect the germination of L. sativa species. Thus, this adsorbent may become a novel and viable option to be used in the removal of sodium diclofenac.

Objetivou-se identificar as estratégias de enfrentamento utilizadas pelos gestores da Atenção Primária à Saúde durante a pandemia da Covid-19. Trata-se de um estudo qualitativo, realizado com 10 profissionais da Atenção Primária à Saúde do município de Ibiporã, localizado no Norte do estado do Paraná. Foram realizadas entrevistas individuais, semiestruturadas, audiogravadas, transcritas na íntegra e submetidas ao Discurso do Sujeito Coletivo, adotando como referencial teórico a Teoria das Representações Sociais. As questões que permearam a identificação das estratégias de enfrentamento adotadas pelos gestores abrangeram quatro ideias centrais: questionamentos acerca dos sentimentos frente à experiência em ser gestor durante a Covid-19; tomada de decisões sobre a reorganização dos serviços frente à Covid-19; principais dificuldades e limitações durante o processo de trabalho; e estratégias pessoais e profissionais que auxiliaram no enfrentamento da Covid-19 enquanto gestor municipal de saúde. Verificou-se que a busca pelas estratégias de enfrentamento deu-se pelos sentimentos de desconhecimento acerca da Covid-19, o que desencadeou medo, cansaço e estresse nos gestores e equipes de saúde, com a necessidade de planejamento e readequações no processo de trabalho. As principais estratégias de enfrentamento utilizadas pelos gestores foram: diálogo com a equipe, escuta ativa, apoio dos familiares e momentos de oração.


The objective was to identify the management strategies used by Primary Health Care managers during the Covid-19 pandemic. This is a qualitative study, carried out with 10 professionals from Primary Health Care in the city of Ibiporã, located in the north of the State of Paraná. interviews, semi-structured audios, transcribed in full-recording and presented for individual Discourse Representations, transcribed in integral theory and presented as Social Discourse Representations were referenced. The questions that permeate the identification of the four management strategies planned by the managers cover central: questions about the feelings facing the experience of being Covid-19; decision-making on the reorganization of services in the face of Covid-19; main difficulties and guarantees during the work process; and personal and professional strategies that help face Covid-19 as a municipal health manager. It was found that fear seeks coping strategies, due to feelings of ignorance about Covid-19, which triggered fatigue and stress in managers and health teams, with the need for planning and readjustments in the work process. The main coping strategies used by the managers were: dialogue with the team, active listening, support from family members and moments of prayer.

Hepatitis E virus (HEV) circulation in humans and swine has been extensively studied in South America over the last two decades. Nevertheless, only 2.1% of reported HEV strains are available as complete genome sequences. Therefore, many clinical, epidemiological, and evolutionary aspects of circulating HEV in the continent still need to be clarified. Here, we conducted a retrospective evolutionary analysis of one human case and six swine HEV strains previously reported in northeastern, southern, and southeastern Brazil. We obtained two complete and four nearly complete genomic sequences. Evolutionary analysis comparing the whole genomic and capsid gene sequences revealed high genetic variability. This included the circulation of at least one unrecognized unique South American subtype. Our results corroborate that sequencing the whole capsid gene could be used as an alternative for HEV subtype assignment in the absence of complete genomic sequences. Moreover, our results substantiate the evidence for zoonotic transmission by comparing a larger genomic fragment recovered from the sample of the autochthonous human hepatitis E case. Further studies should continuously investigate HEV genetic diversity and zoonotic transmission of HEV in South America.

Abstract Background Tuberous sclerosis (TS) is a multisystem genetic disease in which epilepsy is a frequent manifestation and is often difficult to control. Everolimus is a drug with proven efficacy in the treatment of other conditions related to TS, and some evidence suggests that its use benefits the treatment of refractory epilepsy in these patients. Objective To evaluate the efficacy of everolimus in controlling refractory epilepsy in children with TS. Methods A literature review was conducted in the Pubmed, BVS, and Medline databases, using the descriptors Tuberous sclerosis, Children, Epilepsy, and Everolimus. Original clinical trials and prospective studies published in Portuguese or English in the last decade that evaluated the use of everolimus as an adjuvant therapy in the control of refractory epilepsy in pediatric patients with TS were included. Results Our search screened 246 articles from electronic databases, 6 of which were chosen for review. Despite the methodological variations between the studies, most patients benefited from the use of everolimus to control refractory epilepsy, with response rates ranging from 28.6 to 100%. Adverse effects were present in all studies leading to dropouts of some patients; however, the majority were of low severity. Conclusion The selected studies suggest a beneficial effect of everolimus in the treatment of refractory epilepsy in children with TS, despite the adverse effects observed. Further studies involving a larger sample in double-blind controlled clinical trials should be performed to provide more information and statistical credibility.


Resumo Antecedentes A esclerose tuberosa (ET) é uma doença genética multissistêmica na qual a epilepsia é a manifestação neurológica mais frequente, sendo muitas vezes de difícil controle. O everolimo é uma droga com eficácia comprovada no tratamento de outras condições relacionadas à ET, e indícios sugerem benefícios de seu uso também no controle da epilepsia refratária nesses pacientes. Objetivo Avaliar a eficácia do everolimo no controle da epilepsia refratária em crianças com ET. Métodos Revisão de literatura nas bases de dados Pubmed, BVS e Medline, utilizando os descritores Tuberous sclerosis, Children, Epilepsy e Everolimus. Incluíram-se ensaios clínicos originais e estudos prospectivos publicados em português ou inglês na última década e que avaliassem o uso do everolimo como terapia adjuvante no controle da epilepsia refratária em pacientes pediátricos com ET. Resultados Nossa busca rastreou 246 artigos nas bases de dados, dos quais 6 foram escolhidos para a revisão. Apesar das variações metodológicas entre os estudos, a maioria dos pacientes tiveram benefício no uso do everolimo para controle da epilepsia refratária, com taxas de resposta variando entre 28.6 e 100%. Os efeitos adversos estiveram presentes em todos os estudos, levando à desistência de alguns pacientes, contudo a maioria foi de baixa gravidade. Conclusão Os estudos selecionados sugerem efeito benéfico do everolimo no tratamento da epilepsia refratária em crianças com ET, apesar dos efeitos adversos observados. Novos estudos envolvendo uma amostra maior em ensaios clínicos controlados duplo-cegos devem ser realizados para fornecer mais informações e credibilidade estatística.
