Introduction: Gastrostomies can be performed percutaneously by interventional radiology (PRG) or endoscopy (PEG). Methods: Retrospective analysis of patients with advanced cancer who underwent a gastrostomy in 2017 in an oncology center. Results: In 2017, 164 patients underwent gastrostomies, and 137 (84%) were male. The median age was 60 years (range: 38-91). The predominant Eastern Cooperative Oncology Group (ECOG) performance status stage was 1, with 73 (45%) patients. Head and neck cancer was the most common diagnosis, with 127 (77%) cases. The most frequent reason for performing a gastrostomy was dysphagia, 132 (81%). Most gastrostomies were PEG, 121 (74%), followed by PRG, 41 (25%), and surgery, 2 (1%). Early complications occurred in 86 (52%) patients, and the most frequent of them were local pain in 69 (80%) patients and minor local bleeding in 13 (15%). Late complications occurred in 90 (55%) patients, and the most frequent was also local pain in 57 (63%) patients, followed by local infection in 8 (9%), tube extrusion in 7 (8%), and stomal leakage in 7 (8%). In the multivariable analysis, the factors associated with survival were lymph node metastases and the ECOG performance status. Until June 30th, 2022, 123 (75%) patients had died, and 41 (25%) were still alive. Conclusion: Gastrostomies were performed predominantly in ECOG performance stage 1 patients with head and neck cancer and symptoms of dysphagia, and PEG was the most common procedure.

The scope of this study was to understand the effects resulting from Integrative and Complementary Practices (ICPs) in the treatment of people with obesity cared for at a state center of reference of the Brazilian Unified Health System, based on the reports of users. Qualitative exploratory-descriptive methodology was used, employing semi-structured interviews as a technique for data production. The empirical universe was composed of eight male and female members in the adult age group, with a medical diagnosis of obesity and being monitored at the ICP Outpatient Clinic. Feeling well was observed as a significant and pivotal sensation for the ongoing experience in the ICPs, resulting from the therapy, materialized in the different effects produced by the practices and bringing about a reorganization of the subject's life, the care of oneself and of others. It was possible to observe that the organic presence of the ICPs assumes a hybrid and dynamic place in the process of care, although a perspective has emerged that links the ICPs to obesity through the control of anxiety, the body, and food. Furthermore, the ICPs seem to collaborate with the displacement of the focus of body weight management to the person as a whole, also acting as mediators in the process of body acceptance.


O estudo buscou compreender os efeitos produzidos pelo uso de Práticas Integrativas e Complementares no cuidado a pessoas com obesidade atendidas em um centro de referência estadual do Sistema Único de Saúde, a partir da narrativa dos usuários. Utilizou-se a metodologia qualitativa exploratório-descritiva, tendo como técnica de produção de dados as entrevistas semiestruturadas. O universo empírico foi composto por oito pessoas, dos gêneros masculino e feminino, na faixa etária adulta, com diagnóstico médico de obesidade e em acompanhamento no Ambulatório de PICS. Observou-se o sentir-se bem como sentimento marcante e definidor para a continuidade da vivência nas PICS, resultante da terapêutica, concretizado nos diferentes efeitos produzidos pelas práticas e articulador de uma reorganização da vida do sujeito, do cuidado de si e do outro. Foi possível observar que a presença orgânica das PICS assume um lugar híbrido e dinâmico no fluxo do cuidado, ainda que tenha se sobressaído uma perspectiva que vincula as PICS à obesidade por meio do controle da ansiedade, do corpo e da alimentação Além disso, as PICS parecem colaborar com o deslocamento da centralidade da gestão do peso corporal para a pessoa em sua integralidade, atuando ainda como mediadoras do processo de aceitação corporal.

Resumo O estudo buscou compreender os efeitos produzidos pelo uso de Práticas Integrativas e Complementares no cuidado a pessoas com obesidade atendidas em um centro de referência estadual do Sistema Único de Saúde, a partir da narrativa dos usuários. Utilizou-se a metodologia qualitativa exploratório-descritiva, tendo como técnica de produção de dados as entrevistas semiestruturadas. O universo empírico foi composto por oito pessoas, dos gêneros masculino e feminino, na faixa etária adulta, com diagnóstico médico de obesidade e em acompanhamento no Ambulatório de PICS. Observou-se o sentir-se bem como sentimento marcante e definidor para a continuidade da vivência nas PICS, resultante da terapêutica, concretizado nos diferentes efeitos produzidos pelas práticas e articulador de uma reorganização da vida do sujeito, do cuidado de si e do outro. Introdução: A compreensão do que seja obesidade vai influenciar a maneira como estruturamos as políticas públicas, o processo formativo dos profissionais de saúde e, principalmente, as práticas de cuidado. Objetivo: Analisar as representações sociais sobre obesidade para profissionais de saúde da Atenção Primária do estado da Bahia, Brasil. Método: Esta comunicação breve refere-se a uma pesquisa qualitativa, ancorada na Teoria das Representações Sociais, desenvolvida no contexto de um curso sobre a qualificação do cuidado às pessoas com sobrepeso e obesidade. Para a produção dos dados, utilizaram-se um questionário semiestruturado on-line e a Técnica de Associação Livre de Palavras, a partir do estímulo "Escreva as três primeiras palavras que vêm a sua mente quando você pensa em obesidade". A análise prototípica foi realizada com a ajuda do software OpenEvoc. Resultados: Os resultados indicam que o núcleo central das representações sociais sobre obesidade foi formado pelos elementos doença, alimentação, sobrepeso e gordura, enquanto os vocábulos saúde mental, qualidade de vida, atividade física, estigma, saúde e multifatorialidade compõem o sistema periférico. Revelaram que há predominância da perspectiva patológica e individualizada, em que pesem a ampliação do conhecimento científico moderno e das orientações institucionais sobre a obesidade; a assunção dos fatores psicoemocionais no desenvolvimento da obesidade; assim como a incipiência da abordagem multifatorial, ecológica e/ou sindêmica da obesidade. Conclusão: Este estudo sugere que pesquisas aprofundem o estudo de tais representações sociais, suas motivações no campo formativo, laboral e social, bem como a análise do que consolida e provoca as novas narrativas anunciadas.


Introduction: The understanding of whatobesity is will influence the way we structure public policies, the training process of health professionals, and, mainly, care practices. Objective: To analyze the social representations of obesity among Primary Health Care professionals in the state of Bahia, Brazil. Method: This brief communication refers to qualitative research, anchored in the Theory of Social Representations, developed in the context of a course on the qualification of care for people with overweight and obesity. For data production, we used an online semi-structured questionnaire and the Free Word Association Technique, based on the stimulus "Write the first three words that come to your mind when you think about obesity". The prototypical analysis was carried out with the assistance of the OpenEvoc software. Results: The results indicate that the central core of social representations about obesity was formed by the elements disease, food, overweight and fat, while the words mental health, quality of life, physical activity, stigma, health, and multifactorial constitute the peripheral system. They revealed that the pathological and individualized perspective is predominant, despite the enhancement of modern scientific knowledge and institutional guidelines on obesity; the assumption of psycho-emotional factors in the development of obesity; as well as the incipience of the multifactorial, ecological, and/or syndemic approach to obesity. Conclusion: This study suggests that research deepens the study of such social representations, and their motivations in the educational, labor, and social field, like the analysis of what consolidates and provokes the newly announced narratives.

Objetivo: descrever os casos de violência doméstica contra mulheres usuárias de serviços de saúde na Atenção Primária nas Zonas Leste e Centro-Sul de Manaus. Metodologia: tratou-se de um estudo descritivo, com abordagem quantitativa realizado nas Unidades Básicas de Saúde Gebes de Melo Medeiros Filho e Theomário Pinto da Costa, localizadas em bairros de Zonas distintas de Manaus-AM. A amostra foi constituída por 20 mulheres cadastradas em cada UBS, somando ao todo 40 participantes. Na coleta de dados, foi utilizado um formulário com roteiro estruturado. Resultados: a maioria das entrevistadas têm 50 anos ou mais, possuem ensino médio completo e estão solteiras; o tipo de agressão prevalente foi a violência psicológica (87,5%); a maioria das participantes não denunciou a violência sofrida (85%); predominou como local de ocorrência da violência, a própria residência (90%) da vítima. Conclusão: diante do impacto social ocasionado às vítimas, é necessário que o profissional atuante na Atenção Primária saiba identificar, orientar e encaminhar os casos de violência contra a mulher, a fim de contribuir não somente na interrupção do ciclo de violência, aumentando o número de denúncias e reduzindo os casos.


Objective: to describe the cases of domestic violence against women users of health services in Primary Care in the East and South-Central of Manaus. Methodology: it was a descriptive study, with a quantitative approach carried out in the Basic Health Units Gebes de Melo Medeiros Filho and Theomário Pinto da Costa, located in neighborhoods of distinct Zones of Manaus-AM. The sample consisted of 20 women registered in each BHU, adding up to 40 participants. In the data collection, a form with a structured script was used. Results: most of the interviewees are 50 years or older, have a full high school and are single; the type of aggression prevalent was psychological violence (87.5%); most of the participants did not report the violence suffered (85%); predominated as the place of occurrence of violence, the victim's own residence (90%). Conclusion: in view of the social impact caused to the victims, it is necessary that the professional acting in Primary Care know how to identify, guide and forward cases of violence against women, in order to contribute not only to the interruption of the cycle of violence, increasing the number of complaints and reducing the cases.

Metodología: se realizó un estudio descriptivo, con enfoque cuantitativo, en las Unidades Básicas de Salud Gebes de Melo Medeiros Filho y Theomário Pinto da Costa, ubicadas en barrios de distintas Zonas de Manaos-AM. La muestra estuvo conformada por 20 mujeres inscritas en cada BHU, sumando hasta 40 participantes. En la recolección de datos se utilizó un formulario con un script estructurado. Resultados: la mayoría de los entrevistados tienen 50 años o más, bachillerato completo y solteros; el tipo de agresión predominante fue la violencia psicológica (87,5%); la mayoría de los participantes no reportaron la violencia sufrida (85%); predominó como lugar de ocurrencia de la violencia, la propia residencia de la víctima (90%). Conclusión: en vista del impacto social causado a las víctimas, es necesario que el profesional que actúa en Atención Primaria sepa identificar, orientar y avanzar los casos de violencia contra las mujeres, con el fin de contribuir no solo a la interrupción del ciclo de violencia, aumentando el número de denuncias y reduciendo los casos.

Resumo A depressão, considerada uma doença grave e de alta prevalência, configura-se como problema de Saúde Pública em todas as regiões do mundo por ser a maior causa de incapacidade e por ter relações causais com as condições sociais estabelecidas em alguns países. O objetivo deste estudo foi analisar a experiência da depressão em um grupo virtual fechado do Facebook. Esta pesquisa, de natureza netnográfica, revelou os sentidos e particularidades de um grupo fechado do Facebook traduzidos por quem sofre de depressão. Como campo de investigação, possibilitou compreender a particularidade da experiência da depressão mediante debates coletivos entre os membros desse grupo por se sentirem com mais liberdade para falar de si e do outro.(AU)


Abstract Considered a serious and highly prevalent disease, depression is a worldwide public health problem, being related to social conditions in some countries and the leading cause of disability. The aim of this study was to investigate experiences of depression in a closed Facebook group. This netnographic study revealed the meanings and particularities of a closed Facebook group from the perspective of people suffering depression. The study allowed us to understand the particularities of the experience of depression from the collective discussions held between the group members, as they felt more free to talk about themselves and others.(AU)


Resumen La depresión, considerada una enfermedad grave y de alta prevalencia, se configura como un problema de Salud Pública en todas las regiones del mundo, por ser la mayor causa de incapacidad y por tener relaciones causales con las condiciones sociales establecidas en algunos países. El objetivo de este estudio fue analizar la experiencia de la depresión en un grupo virtual cerrado de Facebook. Esa investigación, de naturaleza netnográfica, reveló los sentidos y particularidades de un grupo cerrado de Facebook traducidos por quien sufre de depresión. Como campo de investigación, hizo posible comprender la particularidad de la experiencia de la depresión a partir de las discusiones colectivas entre los miembros de ese grupo, por sentirse con más libertad para hablar sobre sí mismos y sobre el otro.(AU)

Splanchnic venous thrombosis and cerebral venous thrombosis are uncommon manifestations of venous thromboembolism (VTE) that have been associated with inherited thrombophilias. Hyperhomocysteinemia is an established risk factor for thrombosis, and methylenetetrahydrofolate reductase (MTHFR) mutation is the most common genetic alteration in this condition. The association between MTHFR mutations, mild to moderate elevations in homocysteine, and the risk for thrombosis is controversial. Pylephlebitis, also known as suppurative portal vein thrombophlebitis, usually originates from an intra-abdominal infectious process. It is a condition with high morbidity and mortality, partly due to its late diagnosis, and antibiotics are the gold standard treatment. The purpose of anticoagulation is dubious. We describe the case of a 60-year-old male with a previous history of venous sinus thrombosis and MTHFR A1298C mutation with mild homocysteine ​​elevation who presented with signs and symptoms of intra-abdominal infection and whose abdominopelvic computed tomography (CT) with intravenous contrast showed splanchnic-vein thrombosis. Through this complex case, the authors present a review of the current state of the art on VTE, hyperhomocysteinemia, and pylephlebitis, emphasizing the need for a holistic view of the patient in the decision-making process.

Esta revisão integrativa objetivou caracterizar produções científicas que abordam interfaces entre a rede social Facebook e o campo da alimentação e nutrição. Entre agosto e outubro de 2018 as bases de dados Scientific Electronic Library Online, Literatura Latino-Americana e do Caribe em Ciências da Saúde, Web of Science, Scopus e MEDical Literature Analysis and Retrieval System Online foram consultadas, e 85 artigos foram selecionados. Os estudos são emergentes no cenário internacional e incipientes no Brasil. Observou-se expressiva utilização do Facebook para mediação de ações de educação alimentar e nutricional, e seus usos também foram situados como práticas de risco ao reforçar o marketing nutricional de alimentos considerados insalubres e ao difundir conteúdos imagéticos que suscetibilizam internautas com relação aos transtornos alimentares. Observa-se que o Facebook vem sendo apropriado de diversas formas, a partir de interesses e valores que refletem a sociedade de seu tempo e espaço e que imprimem suas características nesses processos de apropriação


This integrative review aimed to characterize scientific productions that address interfaces between the Facebook social network and the field of food and nutrition. Between August and October 2018, the databases Scientific Electronic Library Online, Latin American and Caribbean Literature in Health Sciences, Web of Science, Scopus and MEDical Literature Analysis and Retrieval System Online were consulted and 85 articles were selected. The studies are emerging in the international scenario and incipient in Brazil. Significant use of Facebook was observed to mediate actions of food and nutritional education, and its uses were also situated as risk practices by reinforcing the nutritional marketing of foods considered unhealthy and by the dissemination of imagery contents that cause internet users to eating disorders. The Facebook has been appropriated in various ways, through interests and values that reflect society in its time and space, printing its characteristics in these processes of appropriation


Esta revisión integradora tuvo como objetivo caracterizar las producciones científicas que abordan las interfaces entre la red social Facebook y el campo de la alimentación y la nutrición. Entre agosto y octubre de 2018 las bases de datos Scientific Electronic Library Online, Literatura Latinoamericana y del Caribe en Ciencias de la Salud, Web of Science, Scopus y MEDical Literature Analysis and Retrieval System Online fueron consultadas y se seleccionaron 85 artículos. Los estudios son emergentes en el escenario interna-cional e incipientes en Brasil. Se observó expresiva utilización de Facebook para mediación de acciones de educación alimentaria y nutricional, y sus usos también se situaran como prácticas de riesgo al reforzar el marketing nutricional de alimentos considerados insalubres y por la difusión de contenidos imáginarios que hacen que los internautas sean susceptibles a los trastornos alimentarios. El Facebook viene siendo apropiado de diversas formas, mediante intereses y valores que reflejan la sociedad en su tiempo y espacio, imprimiendo sus características en esos procesos de apropiación

Nocardiosis is a rare infection caused by gram-positive aerobic actinomycetes, which are common in soil. Inoculation occurs by inhaling agent fragments that cause localized or systemic suppurative lesions. The diagnosis is established based on isolation in cultural examinations. Trimethoprim-sulfamethoxazole (TMP-SMX) is the first-line treatment, and an antimicrobial susceptibility test is useful in severe cases or when there is no clinical response. The duration of treatment is determined by the affected site. However, the treatment cycles are long, and recurrence is common, which has a negative impact on the prognosis. We describe a case of an immunocompetent male with a recent diagnosis of pulmonary nocardiosis who, after starting therapy, presented with symptoms that could be explained by either disease progression or an adverse pharmacological reaction. Throughout this case, with atypical evolution, the authors review the diagnostic and therapeutic approach to Nocardia infection and alert to the importance of the differential diagnosis and available therapeutic options.

Thoracic outlet syndrome results from neurovascular compression at the thoracic outlet. Clinical presentation varies according to the predominantly compressed structure, determining its subtype: neurogenic, venous, or arterial. The neurogenic subtype is the most common, affecting 90% of patients, while the vascular subtype is rarely found in practice. We present two case reports of young patients with upper extremity deep vein thrombosis in the setting of venous thoracic outlet syndrome: one due to an anatomic variant, the second an effort thrombosis due to repeated upper arm exercise. These reports depict uncommon clinical scenarios, which imply significant morbidity if not identified and timely treated.

BACKGROUND: Weight stigma is a phenomenon associated with adverse behavioural and psychological consequences. Although experts suggest that its increase during the COVID-19 pandemic may be associated with worse health outcomes for people with obesity, a thorough analysis of the main findings and gaps is still needed when relating to this subject. OBJECTIVE: We aim to answer three questions: (1) How does weight stigma manifest in the COVID-19 pandemic? (2) How can weight stigma affect people with overweight or obesity in times of COVID-19? (3) What are the perceptions and experiences of weight stigma during the pandemic in individuals who experience overweight or obesity? METHODS: We conducted a scoping review of studies addressing weight stigma and the COVID-19 pandemic in electronic databases (Medline/PubMed, CINAHL, Embase, PsycInfo, BVS/Lilacs, Scopus, Web of Science, Google Scholar, and OpenGrey) published until 10th August 2021. All relevant studies were reviewed in full by two researchers. In addition, a narrative synthesis of the data was performed. RESULTS: The results included 35 studies out of 8,090 records and identified 13 original research publications, 14 text and opinion papers, and 6 narrative reviews. The results revealed the presence of weight stigma in the media, healthcare settings, interpersonal relationships, and public campaigns during the COVID-19 pandemic. The evidence of increasing weight stigma in the COVID-19 outbreak is limited, though. Many weight discrimination consequences were described during this time, such as impairment in accessing healthcare, worst COVID-19 outcomes, and maladaptive eating. However, only maladaptive behaviours and decline in mental health outcomes were demonstrated empirically in all age groups. This effect occurred regardless of body mass index, but people with high body weight were more likely to experience weight stigma. For some people with obesity, weight stigma in the pandemic has made activities of daily routine difficult. CONCLUSIONS: The results suggest that weight stigma in the COVID-19 pandemic occurs in several settings; moreover, although weight discrimination impacts mental health, whether before or during the pandemic, this influence between the pandemic and pre-pandemic scenario is still unclear. Therefore, more research is required in this field while the pandemic lasts, especially with people with obesity. Overall, people with overweight or obesity are more vulnerable to weight stigma than individuals without overweight. In addition, weight stigma refers to discrimination or prejudice based on a person's weight and relates to several consequences, for instance, poor healthcare treatment and mental health problems. In the COVID-19 outbreak, these weight stigma effects tend to become even more critical because they may be associated with unfavourable COVID-19 outcomes and eating disorder risks. Thus, it is crucial to investigate how weight stigma occurs during the pandemic and its impact on health, mainly for the most affected people. We investigated 35 studies published between 2019 and 2021 to map and explore how weight stigma was manifested and the related consequences for people with overweight or obesity in the COVID-19 pandemic. Only about a third of them were quantitative or qualitative, limiting the evidence of weight stigma in the COVID-19 context. The available evidence suggests that weight stigma manifests in several settings such as media, healthcare, public campaigns, and is more common in people with excess weight. However, weight discrimination experiences before or during the pandemic were associated with adverse psychological and behavioural consequences across all age groups, regardless of body weight. For some people with obesity, for instance, weight stigma made it difficult to accomplish their activities of daily routine. Nevertheless, it remains unclear whether weight stigma has increased in the pandemic, thus, more studies are required, especially about people with overweight or obesity.

Resumo Este artigo parte de uma abordagem qualitativa em páginas no Facebook (FB) para analisar sentidos do consumo de informações sobre alimentação saudável e dietas atribuídos por internautas. Foram realizadas observações com registro em diário de campo durante oito meses e entrevistas semiestruturadas com 10 internautas, norteadas por roteiro específico. As páginas refletem um mercado de emagrecimento que não é explícito para os consumidores. Todos os entrevistados acessam o FB diariamente, sendo que o consumo de informações variou entre os que buscam "o tempo todo", "às vezes" e "quando aparece na timeline". As informações são submetidas a processos de filtragem e validação mediados por experiências, por pares ou comparações em fontes diversas da Web- de modo a fazerem sentido. Internautas agenciam seus cuidados alimentares a partir de novos elementos "garimpados" na rede - as comunidades virtuais fazem parte de itinerários de autocuidado alimentar pautados no autocontrole e disciplinarização dos corpos. As análises podem contribuir para a compreensão e perspectivas de utilização das redes social por comunicadores em saúde e debates sobre o papel das mídias sociais no âmbito sanitário e sobre possibilidades de conciliação da lógica da mídia com a lógica da promoção da saúde.


Abstract This article starts from a qualitative approach on Facebook pages (FB) to analyze the meanings of consumption of information about healthy eating and diets attributed by Internet users. Observations were carried out with a field diary for eight months and semi-structured interviews were carried out with 10 internet users, guided by a specific script. The pages reflect a weight-loss market that is not explicit for consumers. All respondents access FB daily, and the consumption of information varied between those seeking "all the time", "sometimes" and "when it appears on the timeline". The information is subjected to filtering and validation processes mediated by experiences, pairs or comparisons in different sources on the Web - in order to make sense. Internet users organize their dietary care from new elements "mined" in the network - virtual communities are part of self-care itineraries based on self-control and discipline of bodies. The analyzes can contribute to the understanding and perspectives of the use of social networks by health communicators and debates on the role of social media in the health field and on possibilities for reconciling the logic of the media with the logic of health promotion.

The scope of this paper is to conduct a critical analysis on how perceptions of the notion of food culture have been presented in scientific papers on food and nutrition education in Brazilian Elementary schools. To achieve this, the use and application of this concept in the texts were recorded and the forms of the proposed relationship between the inclusion of food culture in the theoretical-methodological planning and the practice of the actions were critically read and analyzed. The paper is based on qualitative document research. The theoretical scope of the socio-anthropology of food is used as the focal point to support the reflections. Twenty scientific papers were selected on food and nutrition education published in various databases between 2010 and 2018. For data production and analysis, discourse analysis techniques were used. The conclusion drawn is that, despite being mentioned repeatedly in the texts, food culture is not consolidated as a legitimate dimension in the field of food and nutrition education. Although it is almost always claimed in the texts, it is not always explicitly and concretely mentioned. Therefore, it ends up losing the relevance and weight that, paradoxically, it already has.


O objetivo deste artigo é analisar criticamente como as compreensões a respeito da noção de cultura alimentar têm sido articuladas em artigos científicos sobre educação alimentar e nutricional (EAN) desenvolvidas em escolas brasileiras da educação básica. Para isso, fez-se um registro dos usos e aplicações desse conceito nos textos e leu-se criticamente as formas de relação propostas entre a inclusão da cultura alimentar no planejamento teórico-metodológico e a prática das ações analisadas. O artigo parte de uma pesquisa qualitativa de base documental. O escopo teórico da socioantropologia da alimentação serve de embasamento para sustentar as reflexões. Para a produção dos dados foram selecionados 20 artigos científicos sobre EAN publicados entre 2010 e 2018 em diferentes bases de dados. Para o processo de análise dos dados foram utilizadas técnicas da análise do discurso. Concluiu-se que a cultura alimentar, apesar de mencionada de forma repetida nos textos, não se consolida como uma dimensão legítima no campo da EAN. Ainda que na maior parte das vezes esteja reivindicada nos textos, nem sempre é referenciada de forma explícita e concreta. Por isso, acaba perdendo a relevância e o peso que, paradoxalmente, já tem.

The scope of this paper is to analyze the discursive strategies of a Ministry of Health publication, by virtue of the fact that another notion of food and nutrition care (FNC) emerges from them. Through discourse analysis, the results are interpreted mainly in the light of Edgar Morin and Anemarie Mol's studies. The analysis identified elements of the complex thinking constituting a broadened perspective about the food and FNC practices in the context of matrix support. As the main discursive strategy, narratives contextualize care in sinuous and inconclusive scenarios, emphasizing the immanence of unpredictability and uncertainty. Metaphorical resources reveal displacements that remove the centrality of Biomedicine and weave in the subject the practical wisdom of dealing with the coexistence between the generality of body and food metrification and the uniqueness of everyday life and eating. The stories contribute to the understanding of FNC as shared management, which aims to restore viable and ever provisional order. In conclusion, by exposing and legitimizing the plurality of ways of acting, the document that emerges from and relates back to it can have repercussions on health education and raise the theoretical repertoire surrounding FNC practices.


Este ensaio objetiva analisar as estratégias discursivas de uma publicação do Ministério da Saúde, considerando que delas emerge outra noção de cuidado em alimentação e nutrição (CAN). Por meio da análise de discurso, os resultados são interpretados principalmente à luz dos estudos de Edgar Morin e Annemarie Mol. A análise identificou elementos do pensamento complexo constitutivos de uma perspectiva ampliada acerca da alimentação e das práticas de CAN no contexto do apoio matricial. Como principal estratégia discursiva, narrativas contextualizam o cuidado em enredos sinuosos e inconclusos, enfatizando a imanência da imprevisibilidade e da incerteza. Recursos metafóricos revelam deslocamentos da centralidade da biomedicina e tecem no sujeito a sabedoria prática de lidar com a coexistência entre a generalidade da metrificação do corpo e da comida e a singularidade das vidas e do comer cotidiano. As histórias contribuem para a compreensão do CAN como manejo compartilhado, que visa a restituição da ordem - viável e sempre provisória. Como conclusão, ao expor e legitimar a pluralidade de modos de atuar, o documento que emerge da experiência e a ela se volta pode repercutir na formação em saúde e suscitar a ampliação do repertório teórico acerca das práticas de CAN.

Parkin (PK) is an E3-ligase harboring tumor suppressor properties that has been associated to various cancer types including glioblastoma (GBM). However, PK is also a transcription factor (TF), the contribution of which to GBM etiology remains to be established. Methods: The impact of PK on GBM cells proliferation was analyzed by real-time impedance measurement and flow cytometry. Cyclins A and B proteins, promoter activities and mRNA levels were measured by western blot, luciferase assay and quantitative real-time PCR. Protein-protein and protein-promoter interactions were performed by co-immunoprecipitation and by ChIP approaches. The contribution of endogenous PK to tumor progression in vivo was performed by allografts of GL261 GBM cells in wild-type and PK knockout mice. Results: We show that overexpressed and endogenous PK control GBM cells proliferation by modulating the S and G2/M phases of the cell cycle via the trans-repression of cyclin A and cyclin B genes. We establish that cyclin B is regulated by both E3-ligase and TF PK functions while cyclin A is exclusively regulated by PK TF function. PK invalidation leads to enhanced tumor progression in immunocompetent mice suggesting an impact of PK-dependent tumor environment to tumor development. We show that PK is secreted by neuronal cells and recaptured by tumor cells. Recaptured PK lowered cyclins levels and decreased GBM cells proliferation. Further, PK expression is decreased in human GBM biopsies and its expression is inversely correlated to both cyclins A and B expressions. Conclusion: Our work demonstrates that PK tumor suppressor function contributes to the control of tumor by its cellular environment. It also shows a key role of PK TF function in GBM development via the control of cyclins in vitro and in vivo. It suggests that therapeutic strategies aimed at controlling PK shuttling to the nucleus may prove useful to treat GBM.

Epstein-Barr virus (EBV) primary infection usually presents with classic symptoms of infectious mononucleosis (IM) like fever, lymphadenopathies and tonsillopharyngitis. Liver damage is frequently mild and self-limited and there are only a few cases of severe EBV-induced cholestatic hepatitis and jaundice reported in the literature. The authors present the case of a 22-year-old woman who was admitted with acute fever and jaundice. Physical examination revealed posterior cervical lymphadenopathies and painful hepatosplenomegaly. Laboratorial findings suggested an obstructive cause for jaundice but ultrasound and magnetic resonance cholangiopancreatography excluded biliary duct pathology. Heterophile antibodies were negative but EBV-specific antibodies revealed isolated positive viral capsid antigen (VCA) immunoglobin (Ig) M suggesting the diagnosis of early phase of EBV primary infection. The diagnosis of EBV-induced cholestatic hepatitis was confirmed after identification of EBV deoxyribonucleic acid (DNA) in blood and by liver biopsy. Supportive management was provided and, despite an initial clinical deterioration, the patient had a favorable outcome. EBV is a virus with a high prevalence worldwide, mainly subclinical, and jaundice is a rare manifestation of the infection. Although the majority of the patients recover without sequelae, progression to liver failure has been described and a careful assessment for complications is mandatory. Therefore, EBV infection should be included in the comprehensive differential diagnosis of jaundice in all age groups.
