RESUMEN

An experimental study was conducted using rodents at different doses to evaluate the effect of Phaseolus vulgaris (red beans) on cage crossing, head dip, open field, elevated plus maze, and light and dark apparatus for anxiety and forced swim test for depression. The corticosterone level and histopathological evaluation was also done to correlate the antidepressive impact of the red beans. The study also identified the components responsible for the effect using GCMS. Based on the findings, red beans could be a potential non-pharmacological therapy for mild to moderate depressive patients. The anxiety model was conducted on mice weighing 20-25 gms. Group I was taken as control, group II as 500 mg/kg and group III as administered 1000 mg/kg. The tests were performed on 0th, 7th, 15th, 30th, 45th, and 60th day. The depression model research was conducted on albino rats weighing between 180 and 200 g, divided into four groups: a control group, a 500 mg/kg Phaseolus vulgaris group, a 1000 mg/kg Phaseolus vulgaris group, and a standard group treated with fluoxetine. The forced swimming test was performed on days 0, 7, 15, 30, 45, and 60, after which histopathological evaluations were conducted and blood samples were taken to assess corticosterone levels. GCMS was used to identify the constituents present in red beans, while optical spectroscopy was used to detect minerals and ions. Results showed that both doses of Phaseolus vulgaris possess anxiolytic effect and increased the struggling time of rats in depression model significantly, with the 1000 mg/kg dose showing more significant results than the 500 mg/kg dose. The GCMS results identified the presence of erucic acid, which causes an increase in α-amylase, thus reducing depression. Optical spectroscopy also showed that red beans contain zinc, which may increase BDNF and help in treating depression.

BACKGROUND: Cataract surgery has become one of the most common surgeries in the world with one in every four surgeries performed being a cataract extraction and the numbers are expected to increase by 16 percent in the USA alone by 2024 as compared to the current statistics. The aim of the study is to analyze the visual outcomes of intraocular lens implants for various visual ranges. METHODS: This non-comparative interventional study was conducted at the Ophthalmology department of Al Ehsan Eye Hospital from Jan to Dec 2021. It included patients who underwent uneventful phacoemulsification with an intraocular lens implant and analysis of the visual outcomes for uncorrected distance (UDVA), uncorrected intermediate distance (UIVA), and uncorrected near distance (UNVA). RESULTS: Independent sample t-test was applied to observe the mean values of recorded far vision on the 1stday, 1 week, and 1 month after the trifocal intraocular lens implantation. It showed a significant difference with a p-value of 0.00 on 1st day, 1 week, and 1 month after was 0.3±0.1, 0.17±0.09, and 0.14±0.08 respectively. Mean improvement in near vision after 1 month was N6 with S.D 1.03 and in intermediate vision was N8±1.4 respectively. CONCLUSIONS: Trifocal Intraocular lens implantation offers an improved vision for near, intermediate, and distant visual ranges without the need for correction.

BACKGROUND: One of the major causes of cardiovascular morbidity and mortality is heart failure. The study aims to assess the effect of heart rate on the incidence of rehospitalization in patients with heart failure and reduced ejection fraction. METHODS: It is a cross-sectional, analytical research conducted over six months, from June to December 2022, at the cardiology department of a tertiary care hospital. Patients who satisfied the modified Framingham heart failure criteria at admission and were discharged with an initial diagnosis of heart failure and those readmitted within 6 months or less of their discharge were included in the study. Pregnant women, patients diagnosed with cognitive impairment, and patients who had contraindications for taking any of the beta-blockers, angiotensin-converting enzyme inhibitor/angiotensin receptor blocker /angiotensin receptor -neprilysin inhibitor, Sodium-Glucose co-transport inhibitor, and mineralocorticoid inhibitors were excluded from the study. RESULTS: A total of 77 patients were included in the study. At discharge, approximately 71 patients had a heart rate of less than 70 beats/min and had no readmissions whereas, 6 patients had a heart rate of greater than 70 beats/min with 5 patients requiring readmission in the following 6 months. This relationship was statistically significant with a p-value of 0.000. CONCLUSIONS: According to the study, heart rate is a significant factor in the rehospitalization of individuals with heart failure and a low ejection fraction. At discharge, if the heart rate is not optimized then the rate of readmissions is increased.

Purpose: Halloysite nanotubes (HNTs) are a versatile and highly investigated clay mineral due to their natural availability, low cost, strong mechanical strength, biocompatibility, and binding properties. The present work explores its role for retarding and controlling the drug release from the composite polymer matrix material. Methods: For this purpose, nanocomposite films comprising propranolol HCl and different concentrations of HNTs were formulated using the "solution casting method". The menthol in a concentration of 1% w/v was used as a permeation enhancer, and its effect on release and permeation was also determined. Quality characteristics of the nanocomposite were determined, and in vitro release and permeation studies were performed using the Franz diffusion system. The data was analyzed using various mathematical models and permeation parameters. Optimized formulation was also subjected to skin irritation test, FTIR, DSC, and SEM study. Systemic absorption and disposition of propranolol HCl from the nanocomposites were predicted using the GastroPlus TCAT® model. Results: The control in drug release rate was associated with the higher concentration of HNTs. F8 released 50% of propranolol within 8 hours (drug, HNTs ratio, 1:2). The optimized formulation (F6) with drug: HNTs (2:1), exhibited drug release 80% in 4 hours, with maximum flux of 145.812 µg/cm2hr. The optimized formulation was found to be a non-irritant for skin with a shelf life of 35.46 months (28-30 ℃). The in silico model predicted Cmax, Tmax, AUCt , and AUCinf as 32.113 ng/mL, 16.58 h, 942.34 ng/mL×h, and 1102.9 ng/mL×h, respectively. Conclusion: The study demonstrated that HNTs could be effectively used as rate controlling agent in matrix type transdermal formulations.

Vitamins are an essential component of the human body for growth and maintaining health. All vitamins have a significant role in metabolism, as a prophylactic in preventing various diseases, and maintaining health. Literature studies have predicted the positive impact of vitamin D on sugar level in blood of type 2 diabetes. Vitamin D plays a significant role in treating diabetes mellitus because it helps produce insulin and helps in the growth of beta cells of the pancreas. The present study was conducted for evaluating the impact of oral vitamin D in reducing the hyperglycemic conditions in patients after treatment of 1-6 months duration. 52 type 2 diabetes patients were enrolled in the study. The results showed that Vitamin D supplementation of 16 weeks reduced fasting blood glucose and HbA1C significantly in Vitamin D deficient Type 2 diabetes.

The impact of Algae supplements and its extract on blood pressure has not concluded yet. The aim of this systematic review meta-analysis is to evaluate the antihypertensive activity of this group marine organism on human. Alga was used in some studies as capsules (from 500-mg to 8-g) and the follow-up duration changed from 17 days to 9 months. The difference in standardized mean and its corresponding 95% confidence interval (CI) was applied as the effect size of algae supplementation on systolic and diastolic blood pressure. Based on the results, a meta-analysis of 10 studies with baseline effect control demonstrated that there was no difference in the mean systolic blood pressure in the 2 groups SMD (95%CI): -1.05 (-2.85,0.76), but a significant difference in the mean diastolic blood pressure was observed and showed that the mean diastolic blood pressure in the treatment group was lower than the control group SMD(95%CI): -2.23 (-4.35,-0.11). A meta-analysis of 4 studies with no baseline control effect did not show significant results on both blood pressure. The evidence to support this systematic review meta-analysis requires more investigation and future large scale RCT clinical trial to confirm the results.

Circular RNAs are a group of ancient but recently appreciated transcripts that affect carcinogenesis. An example of cancer-related circular RNAs is circ_CDR1as. It is mostly regarded as an oncogenic circular RNA, yet in bladder cancer and glioma it has the opposite effect. In gastric and ovarian cancer, both roles have been reported for this circular RNA. Circ_CDR1as has regulatory effects on miR-1270/AFP, miR-1287/Raf1, miR-7-5p/KLF4, miR-641/HOXA9, miR-219a-5p/SOX5, miR-7/HOXB13 and miR-876-5p/MAGE-A molecular axes. miR-7 is the most appreciated interacting miRNA with circ_CDR1as, since its interaction with circ_CDR1as has been validated in liver cancer, lung cancer, colorectal cancer, esophageal carcinoma, gastric cancer, pancreatic cancer, thyroid cancer, oral squamous cell carcinoma, nasopharyngeal carcinoma and osteosarcoma. The present article aims at summarization of the role of circ_CDR1as in neoplasms and its application as a biomarker in human cancers.

BACKGROUND: The retinal changes that develop in the eyes during amblyopia are still unclear. It has been proven that thickness of the nerve fiber layer of the retina is positively correlated with refractive error. The objective of the present study was to measure the Retinal Nerve Fiber Layer Thickness (RNFLT) using Ocular Coherence Tomography (OCT) in patients of amblyopia.. METHODS: A descriptive study was conducted in the Department of LRBT Free Eye Hospital, Lahore for 6 months. After meeting the inclusion criteria, 80 patients (40 amblyopic and 40 normal) were taken and grouped as A and B. Socio-demographic details of all the patients were recorded. They then underwent a complete ophthalmologic evaluation that included BCVA, slit lamp biomicroscopy, OCT examination was carried by TOPCON OCT. Each examination was done using the "fast retinal thickness" protocol for each test eye. Mean RNFLT was calculated for each eye. RESULTS: The mean age of patients was 12.58±4.44 years. There were 20 (50%) males and 20 (50%) females. In amblyopic eyes, the mean RNFLT was 125.82±13.06mm while in normal eyes, the mean RNFLT was 94.82±1.11mm. The overall mean RNFLT was significantly higher in amblyopic eyes as compared to normal eyes (p<0.01), as well as significant when data was stratified for age and gender. CONCLUSIONS: There was significant difference observed in RNFLT in amblyopic eyes as compared to normal eyes. So, these results can help us to enhance our knowledge and understand the causes of amblyopia hence improving diagnosis and management of disease.

Beta vulgaris L. is a vegetable most commonly consumed in salads and has been shown to possess multiple benefits. This research was carried out to observe the effects of Beta vulgaris powder at different doses orally in albino rabbits on liver biochemical parameters and coagulation. The study was carried out on albino rabbits which were divided into three groups designated as Group I (administered distilled water) Group II and III (administered beetroot powder at 500mg/kg and 1000mg/kg dose respectively) orally for 2 month duration. The sample was withdrawn at day 0, 30th and 60th day through cardiac puncture. The results showed that both doses of Beta vulgaris were considered safe for use as all the liver parameters were significantly decreased compared to control. Among both doses 500mg/kg dose was considered safer as it reduced the parameters significantly compared to 1000mg/kg dose. Blood coagulation factors at both the doses showed significant increase which was in reference range. Beta vulgaris is a highly beneficial dietary product with ample amount of flavonoids and anti-oxidant agents which might help in improving the liver function and also play a role in coagulation by increasing both fibrinogen levels and prothrombin time.

A diet comprising of nutrients that would control hypertension as well as hyperlipidemia would be very beneficial over all. This study aimed to assess the effect of lyophilized beet root powder at different doses on lipid profile and hyperlipidemia model. Albino rabbits weighing 1500-2000gms were taken for both studies. Beetroot powder was administered to animals at 500mg/kg and 1000mg/kg doses and after two month dosing the blood samples were withdrawn and lipid profile was assessed. Next a model of hyperlipidemia was created comprising of albino rabbits that were divided into five groups each containing n=6. Group I was considered as control, Group II was marked as Negative control, Group III was taken as standard, whereas Group IV and V were considered as treated and given different doses of beetroot. Blood samples were drawn at baseline, 45th day and at day 60th of study. Highly significant decrease in lipid profile (Cholesterol, LDL and TGS) and significant increase in HDL was observed by both doses after one month. HDL was increased more at 1000mg/kg dose. The presence of flavonoids and saponins in beetroot is responsible for hypolipidemic effect. From our research we came to the conclusion that beetroot powder reduced the lipid profile and could be beneficial in treatment of cardiovascular disease due to atherosclerosis and obesity.

Currently the increased focus is being given to reforming osteoporosis regimens. Optimizing the evaluation of pharmacological intervention occurs once a medicine has been approved. There is literature available on the use of alendronate in bone loss. The current study focuses on the efficacy assessment of alendronate on proximal femur bone density loss. Current work was carried out to analyze the data of the BMD. The study comprised of females who had received at least six months of Alendronate (70mg/week) for proximal femur osteoporosis. SPSS version-22 was used for analysis and a comparative change was regarded therapeutically significant. The reliability of the research was ensured by reporting cover-up and withdrawals. Among all the study participants who received Alendronate therapy the median height of females in centimeters (cms) was 155 (IQR=16) and the median weight was 55.5 Kilograms (Kgs) (IQR=15). The mean age of the population was 50.59±14.714. The study found the median T-score before therapy was -2.9 (IQR=0.7) and the median T-score after therapy was -2.51(IQR=1). The estimated difference of mean rank was statistically significant for pre- and post-therapy T-score (p=0.008). Hence, the results of this study indicate an improvement in BMD as a result of therapy. Alendronate at 70 mg per week is effective in reducing hip osteoporosis.

Retinal detachment (RD) describes the separation of neurosensory retina from the underlying pigment epithelium. There are various methods of treating RD but in many cases, an unusual delay between occurrence of retinal detachment and surgery has been observed. This study was conducted to find the extent of factors involved in delay in surgery. This cross sectional study was carried out at LRBT Eye Hospital, Lahore for 6 months. The non-probability, consecutive sampling technique was used. The demographic information was recorded. The patients were asked for causes of delay in retinal detachment surgery and all factors were measured. Data was analyzed by SPSS version 21. The mean age of patients was 52±9.86 years; the male to female ratio was 1.5:1. About 9.3% patients said that they do not know where to go, 30% patients thought that it was not a severe condition,36.4% patients thought that it would self-heal,17.1% patients didn't go to the doctor due to financial constraints whereas 7.1% patients did not have VR ophthalmologist near their residence. Statistically significant difference was found between the factors and education level of the patients i.e. p-value<0.05. Our study results concluded that people needed to be educated regarding the importance of retinal detachment and surgical procedures and complications associated with it.

The purpose of this study was to evaluate the anxiolytic and antidepressant activity of ethanolic fruit extract of Pyrus communis (pear), in comparison with escitalopram in rodents (rats and mice). Thirty Wistar rats of about 200-250gm and albino mice of 25-30gm, male gender were divided into three groups each comprising of (n=10) animal respectively. Control group received distilled water, positive control received 10mg escitalopram & treated group received 200mg/kg/day of Pyrus communis ethanolic fruit extract orally for 30 days. They were evaluated by using the open field test, forced swim test (FST), plus maze test, light and dark test, hole poking test, stationary rod test, water maze test & cage crossing activity. Results were expressed as mean ± SD. Data was analyzed by using SPSS software (VERSION 21) one way ANOVA followed by Tukey test was used for post hoc analysis. Our result showed that fruit extract had significant antidepressant-like behavior in FST (p<0.001), open field (p<0.05), cage crossing (p<0.001) , significant anxiolytic activity in light and dark box test, plus-maze activity and significantly enhanced learning in water maze and stationary rod test when compared with control. The Pyrus communis fruit extract showed the anxiolytic and antidepressant-like profile in rats and mice. However, further studies need to be carried out in clinical trials for its use in different neuropsychological disorders.

Frequent use of antibiotics has been developed resistance and the use of broad spectrum cephalosporin must be limited in children. The study evaluated the association of prescribing patterns of third generation cephalosporin with diagnosis, age, availability of cultural sensitivity report and gender. It is an observational study that was carried out in the duration of six months in a low socio-economic tertiary care hospital. The data of six hundred and eighty-five (685) patients were collected from the medical records of the tertiary hospital. The cephalosporin are the most prescribed antibiotics in children 118/217 (54.3%) followed by adults 119/403 (29.5%) and teenagers 18/65 (27.6%). Whereas, 75/217 (34.5%), 126/403 (31.2%) and 22/65 (33.8%) were prescribed cephalosporin with combination in patients respectively. The culture sensitivity was performed only in 25% of patients i.e., 173/685, Of 173 culture reports 70 and 91 cases from children and adults respectively. Blood is mostly examined specimen in children and urine in adults. Escherichia coli was highly recovered pathogen in culture sensitivity report. The study concluded broad spectrum cephalosporin antibiotics were highly prescribed in children. The culture sensitivity was performed in limited number of patients. Antibiotics stewardship programme will be implemented to reduce the prescribing of broad spectrum cephalosporin in young patients.

In the detection of coronary heart diseases amongst the non-invasive techniques the role of myocardial perfusion imaging is indexed. The rationale of this study was to compare the different parameters and the association between the exercise MPI and vasodilator MPI in the detection of coronary artery disease (that is prolonged Qt interval with the size of perfusion defects). It was a cross-sectional prospective study with purposive non-probability sampling technique which was conducted in a tertiary care hospital from January 2020 to June 2020 for a period of 6 months. All patients regardless of gender were included in this study and age ranging from 30 to 80 years with comorbidities ranging from diabetes, hypertension and smokers were also included. A total of 100 patients were included in this study out of which 81% were male, 50% were diabetics, 69% were hypertensive, 39% had a history of coronary artery disease, 25% were smokers and 63% had hyperlipidemia. For statistical analysis SPSS 21 was applied and significant association was observed between DTS treadmill score and the perfusion defect in vasodilator MPI, corrected Qt interval and DTS tread mill score and between corrected Qt and size of the perfusion defect (P value < 0.001). It was thus seen that the different components of the noninvasive nuclear stress test tend to co-relate and thus aid in the detection of coronary artery disease increasing the accuracy of results.

Herbal medicines have gained tremendous surge of interest in recent years. M. nigra leaves are a rich source of phenolics which are well-known for their antioxidant property. Morus nigra popularly known as black mulberry is considered to be the most significant species of genus Morus. This study was designed to evaluate its activity on seizure model in different doses. Five groups were made comprising of n=10 animals in each group respectively. Group I was on distilled water, Group II was administered with reference drug diazepam and Group III, IV and V were on 125mg/kg, 250mg/kg and 500mg/kg dose of Morus nigra for 15 days prior to experiment. On day 16th all animals were administered with strychnine after 30 minutes of respective treatments and three parameters were recorded i.e. duration, frequency and onset of seizures. M. nigra treatment showed significant seizure protection as noted by delayed latency of seizures (P<0.05), decrease in frequency and jerk's duration (P<0.05) in comparison to control and reference standard. Most significant (P<0.05) anticonvulsant effects were observed with 500mg/kg dose. Anticonvulsant activity of M. nigra could be due to potentiation of both Gabaergic and glycinergic activities. Antiepileptic potential of extract could also be amplified due to its antioxidant activity. This could serve as a non-pharmacological treatment for seizure management.

Pancytopenia is classified as low blood cell count. Low levels of hemoglobin, red blood cells, white blood cells and platelets are indicative of pancytopenic state. This pancytopenic state can be treatment (drug) or disease induced. Conventional approaches available to treat pancytopenia are usually associated with many undesirable adverse effects, are costly and parenterally administered. Interest in natural products has significantly increased due to their ability to stimulate cellular components of immune system. This study is designed to investigate the hematopoietic i.e. erythropoeitic, leucopoietic and thrombopoeitic potential of water distilled flowers of Rosa damascena Mill.

Infertility has recently been a common issue for which multiple medicinal as well as surgical treatments are available. Beta vulgaris commonly referred to as garden beet is one of the most utilized vegetable especially in salad dressings. The Present study was conducted to evaluate the effect of lyophilized beet root on fertility and reproductive hormones. The study was carried out in January 2017 on two different animal models. For fertility purpose albino mice of both genders weighing 20-25gm were divided into 5 pairs for each group, First group was control given distilled water, next was treated I being given beetroot 500mg/kg and last one was given 1000mg/kg beetroot. Similarly 5 pairs were made of buck and doe's weighing 1.75gm-2.5gm and labelled as Control, Treated I and Treated II and were administered distilled water, 500mg/kg and 1000mg/kg beetroot respectively. The pups were again mated after they grew up and in this way F1 generation was also observed. The kits were observed for presence of anomaly. The estrogen, progesterone and testosterone levels were evaluated at baseline, during pregnancy and post pregnancy in rabbit model. Improved fertility with no anomalies in the pups and kits were observed. The reproductive hormones were also highly significantly increased (p<0.001) as compared to baseline values. The study suggests that beetroot would be very beneficial in improving fertility and maintaining hormonal level during fertility hence it would prove to be very beneficial in maintaining pregnancy as well as treating infertility.

The objective of this study was to evaluate the effect of Foeniculum vulgare (FV) on fertility of mice and on serum sex hormone values of rats and rabbits. We used powdered form of Foeniculum vulgare seed and added it to food of mice, rats and rabbits in 2% and 4% ratio which was provided ad libitum throughout study period. Fertility study on mice was conducted for a period of nine months and 3 generations of mice fed on study drug were observed. Rats and rabbits were given Foeniculum vulgare seed powder containing diet for two month and then serum levels of FSH, LH, estrogen, progesterone and testosterone were checked. The number of pups increased in each succeeding generation and no gross abnormality was observed in any of the pups of all 3 generations. The sex hormone levels increased in both sexes of rats and rabbits with exception of FSH and LH levels which were less than 0.1mIU/mL in all study animals. Data was analysed on SPSS. Fertility and sex hormones increased in both male and female when given special diet containing 2% and 4% Foeniculum vulgare.

Coronary heart disease is the cause of 17.1 million deaths per year throughout the world. The rationale of this study was to determine the importance of 1st set of Troponin I in relation to age, duration of chest pain and left ventricular ejection fraction (LVEF) in patients presenting with acute ST elevation myocardial infarction along with the treatment protocol followed in emergency. It was a cross sectional prospective observational study which was conducted at a tertiary care hospital, at the Cardiology department for a period of 12 months. All patients regardless of gender, aged between 30-80 years with co-morbidities were included presenting with acute STEMI. A total of 150 patients were included in this study with a mean age of 61.2±10.3 years out of which males were (71%). Around 61% of the people presented to emergency >12 hours after onset of chest pain. There was non-significant difference in the treatment protocol given to all patients. For statistical analysis SPSS 21 was applied and significant relationship was observed between age, duration of chest pain and LVEF (p value <0.05). It was seen in our population that people older than 50 years tend to present to emergency department late with chest pain symptoms which results in a linear rising relationship with Troponin I and with increasing Troponin I there was significant reduction seen in LVEF.
