RESUMEN

The prevalence of overweight and obesity is continuously increasing, both in the adult and pediatric populations, posing a substantial challenge to public health. Understanding the epidemiological burden of metabolic syndrome (MetS) among children, particularly regarding its complications and long-term effects in adulthood, is crucial for identifying effective preventive measures and enhancing the clinical care of obese children. Therefore, by searching two databases, a systematic review was conducted in order to evaluate studies that specifically addressed the epidemiological MetS impact among overweight/obese European children and adolescents. Overall, 15 studies were considered. The epidemiological data concerning the MetS impact were contingent on the diagnostic criteria used and varied across countries, resulting in a prevalence range of 1.44% to 55.8%. Spanish studies were the most numerous (34%), revealing a country prevalence rate ranging from 2.5% to 19.6%. Males (prevalence range: 1.4-55.8%) and subjects with overweight/obesity (prevalence range: 12.9-55.8%) were mainly affected. Obesity emerged as the main risk factor in the MetS development and the consequent onset of cardiovascular complications and diabetes. Knowing the MetS burden and its risk factors could improve their prevention, detection, and treatment, and guide the development of targeted public health interventions to appropriately address the health needs of younger patients.

BACKGROUND: Peristomal skin complications (PSCs) are the most common skin problems seen after ostomy surgery. They have a considerable impact on a patient's quality of life and contribute to a higher cost of care. METHODS: A systematic review was conducted, querying three databases. The analysis was performed on international studies focused on the clinical-epidemiological burden of PSCs in adult patients with ileostomy/colostomy. RESULTS: Overall, 23 studies were considered. The main diseases associated with ostomy surgery were rectal, colon and gynecological cancers, inflammatory bowel diseases, diverticulitis, bowel obstruction and intestinal perforation. Erythema, papules, skin erosions, ulcers and vesicles were the most common PSCs for patients with an ostomy (or stoma). A PSCs incidence ranging from 36.3% to 73.4% was described. Skin complications increased length of stay (LOS) and rates of readmission within 120 days of surgery. CONCLUSIONS: PSCs data are still limited. A knowledge of their burden is essential to support health personnel and decision-makers in identifying the most appropriate responses to patients' needs. Proper management of these complications plays a fundamental role in improving the patient's quality of life. A multidisciplinary approach, as well as increased patient education and their empowerment, are priority measures to be implemented to foster a value-based healthcare.

PURPOSE: Despite the high and increasing prevalence of allergic diseases in athletes, allergy diagnostics is not part of the routine medical examination in sports medicine. This study reports the development and validation of an easy and reliable questionnaire for screening allergy in athletes. METHODS: AQUA was derived from the European Community Respiratory Health Survey Questionnaire. On the basis of open interviews with team doctors, coaches, and athletes, questions were added about: the type, duration, and intensity of training; exercise-related allergic and infectious symptoms; social habits (smoking); drug and food supplements intake; antidoping regulations. The final version of the questionnaire, made of 25 selected questions, was validated in 128 professional soccer players who underwent accurate history taking, medical examination, skin prick testing, and/or specific IgE determination. On the basis of the correlation with objective allergy (positive skin tests to at least one allergen), questions were scored from 1 to 5 according to their positive likelihood ratio. RESULTS: Skin tests (gold standard for validation) were positive in 46.8% of soccer players. Mean total AQUA score was 9.4 +/- 7.8 in allergic athletes versus 1.3 +/- 2.3 in nonallergic athletes. A total AQUA score of >or=5 was shown to have the best positive predictive value for allergy (0.94) with a specificity of 97.1% and a sensitivity of 58.3%. CONCLUSIONS: AQUA, produced in 10 European languages, is a validated, easy, and reliable tool for calling attention on the high prevalence of allergy in athletes.

UNLABELLED: The aim of the study is to investigate cognitive functions in boxers. The study population was composed of 2 groups of male boxers with no history of head concussions (except during boxing): a) 33 amateurs at their first examination; b) 27 professionals. Subjects were requested to: a) fulfill a questionnaire collecting demographic data, level of education, occupational status, boxing record (for professionals), and number of head concussions during boxing; b) undergo a computerized neuropsychological (NP) test (CogSport) measuring simple and complex reaction time (RT). Professionals were significantly (P < 0.0001) older (29.4 +/- 4.19 years) and started boxing at a younger age (14.5 +/- 3.94 vs 20.3 +/- 4.77 years, P < 0.0001) than debutants (24.1 +/- 5.13 years). Debutants showed significantly shorter simple RTs than professionals, both at the beginning (0.244 +/- 0.007 vs 0.249 +/- 0.007 s, P = 0.005) and the end (0.247 +/- 0.007 vs 0.251 +/- 0.008 s, P = 0.028) of NP test. Complex RTs did not differ between groups. Professionals showed a positive significant correlation between simple RT at the beginning of NP test and the total number of disputed (P = 0.043) and won (P = 0.035) bouts. In conclusion, professionals show a longer simple RT compared with debutants, with no difference regarding more complex cognitive tasks. A possible explanation may be found in an adaptation of skilled boxers to better (and longer) evaluate external stimuli before giving a motor response. RESUMEN

A 33-year-old man was admitted to our hospital with clinical and instrumental-diagnostic evidence (CT) of haemoperitoneum following a blunt abdominal trauma due to being butted by a bull. A full-thickness rupture of the pancreas, at the level of the isthmus, was found at laparotomy. The emergency procedure consisted in evacuation of the haemoperitoneum, haemostasis and suture of the pancreatic lesion plus drainage of the lesser sac. A high-output (800 ml/day) post-traumatic pancreatic fistula developed in the following days, with the need for a second operation. In the presence of a large pancreatic defect extensively communicating with the Wirsung duct, a singular surgical procedure was adopted in the form of a Roux-en-Y pancreaticojejunostomy with catheterisation of the pancreatic fistula using a small disposable silicone catheter. The post-operative course was uneventful and healing of the pancreatic fistula was documented at CT scan 39 days after the operation.
