RESUMEN

Improved detection and characterization of common focal liver lesions (FLL) are the main topics of the World Federation for Ultrasound in Medicine and Biology (WFUMB) guidelines on the use of contrast-enhanced ultrasound (CEUS). On stateof-the-art CEUS imaging, to create a library of rare FLL, especially concerning their atypical imaging characteristics, might be helpful for improving clinical diagnostic efficiency. In this review, we aim to summarize the ultrasound and CEUS features of rare benign FLL. Currently there are limited reports and images published.

RESUMEN

It is important to be familiar with the typical imaging features of the uncommon or even extremely rare focal liver lesions (FLL). Current guidelines of the World Federation for Ultrasound in Medicine and Biology (WFUMB) is aimed at assessing the usefulness of contrast enhanced ultrasound (CEUS) in the management of various FLL. In this review, we aim to summarize the ultrasound and CEUS characteristics with literature review of some extremely rare benign FLL, which might be helpful for improving diagnostic efficiency clinically.

RESUMEN

This review provides an overview of retinal vascular disorders that are less frequent in Germany and Europe compared to diabetic retinopathy and retinal venous or arterial occlusive disorders. The knowledge of these disorders is important for the differential diagnosis of retinal vascular disorders as well as potentially associated systemic disorders. In the current part one epidemiology, pathophysiology, clinical presentation, and therapy are discussed for hypertensive retinochoroidopathy, ocular ischemic syndrome, retinal alterations in sickle cell disease, Eales disease, radiation retinopathy, peripheral exudative hemorrhagic chorioretinopathy, and retinal disorders associated with pregnancy.

BACKGROUND: Medical information systems frequently use event logging, but these logs are not suitable for process mining as they are not logged in a standardized format. OBJECTIVES: Our goal is to enrich medical event logs for use in process mining. METHOD: We present an approach to convert events from standards- based repositories into the XES and OCEL formats commonly used in process mining. RESULTS: We tested this approach using simulated data from the Austrian breast cancer screening program. CONCLUSION: We aim to apply it to analyze care guidelines and improve hospital processes in the future.

The complexity of structural heart interventions has led to a demand for sophisticated periprocedural imaging guidance. Although traditional 2-dimensional (2D) transesophageal techniques are used widely, new-generation 3D ultrasound probes enable high temporal and spatial resolution. Multiplanar reconstruction of acquired 3D datasets has gained considerable momentum for precise imaging and to increase the validity of measurements. Previously, this technique was used after the acquisition of suitable 3D datasets. Recent advances in technology have enabled the use of live mode for multiplanar reconstruction across different ultrasound vendor platforms. The use of live multiplanar reconstruction can enhance the precision in real-time imaging, enable simultaneous visualization of structures of interest in multiple planes, reduce the need for probe manipulation, and thereby contribute to the success of the procedures. In this narrative review, the authors describe the rationale and utility for 3D transesophageal live multiplanar reconstruction, and outline its use for the structural heart interventions of mitral and tricuspid valve edge-to-edge repair, left atrial appendage occlusion, and the Lampoon procedure. A 3D transesophageal echocardiogram with live-multiplanar reconstruction has the potential to advance guidance of these complex interventions.

RESUMEN

BACKGROUND: HL7 Austria is a non-profit association dedicated to improving electronic data communication and interoperability in healthcare using the HL7 international standards. OBJECTIVES: We aim to provide an open infrastructure to develop, manage, and maintain HL7 FHIR implementation guides. METHODS: We utilize state-of-the-art open-source tooling developed by the FHIR community to support continuous integration. RESULTS: The implementation guides can be published as static HTML websites and maintained using GitHub. CONCLUSION: The solution supports all steps of a standard's lifecycle, from drafting and reviewing to balloting, publishing, and maintenance.

RESUMEN

Healthcare processes have many particularities captured and described within standards for medical information exchange such as HL7 FHIR. BPMN is a widely used standard to create readily understandable processes models. We show an approach to integrate both these standards via an automated transformation mechanism. This will allow us to use the various tools available for BPMN to visualize and automate processes in the healthcare domain. In the future we plan to extend this approach to enable mining and analyzing executed processes.

The hydroxide exchange membrane fuel cell (HEMFC) is a promising energy conversion technology but is limited by the need for platinum group metal (PGM) electrocatalysts, especially for the hydrogen oxidation reaction (HOR). Here we report a Ni-based HOR catalyst that exhibits an electrochemical surface area-normalized exchange current density of 70 µA cm-2, the highest among PGM-free catalysts. The catalyst comprises Ni nanoparticles embedded in a nitrogen-doped carbon support. According to X-ray and ultraviolet photoelectron spectroscopy as well as H2 chemisorption data, the electronic interaction between the Ni nanoparticles and the support leads to balanced hydrogen and hydroxide binding energies, which are the likely origin of the catalyst's high activity. PGM-free HEMFCs employing this Ni-based HOR catalyst give a peak power density of 488 mW cm-2, up to 6.4 times higher than previous best-performing analogous HEMFCs. This work demonstrates the feasibility of efficient PGM-free HEMFCs.

The power degradation and failure of photovoltaic (PV) modules can be caused by changes in the mechanical properties of the polymeric components during the module lifetime. This paper introduces instrumented nanoindentation as a method to investigate the mechanical properties of module materials such as polymeric encapsulants. To this end, nanoindentation tests were carried out on ethylene vinyl acetate (EVA) surfaces, which have been separated from the glass panel. Two types of time-dependent indentation cycle modes, the time domain (creep mode) and frequency domain (dynamic mode) were performed to determine the viscoelastic behavior. For each mode, a corresponding model was applied to calculate the main mechanical properties. The general capability of nanoindentation as cross-linking determination method is investigated with the methodological advantages over bulk mechanical characterization methods. A large number of Glass/EVA/Backsheet laminates were built using different lamination conditions resulting in different degrees of curing. Both indentation modes indicate good modulus sensitivity for following the EVA crosslinking in its early stages but could not reliably differentiate between samples with higher EVA branching. Additional dynamic mechanical analysis (DMA) characterization was used as an established method to validate the indentation measurements. Both nanoindentation and DMA tensile mode produce similar quantitative viscoelastic responses, in the form of the damping factor parameter, demonstrated for three different frequencies at room temperature. A statistical study of the data reveals the advantages for the investigation of multilayer PV laminates by using nanoindenation as a surface method while also being applicable to field aged modules.

BACKGROUND: There is a lack of secure official communication channels for peer review and peer feedback on medical findings. OBJECTIVES: We aimed to utilize the existing Austrian eHealth infrastructure to enable review and feedback processes. METHODS: We extended the IHE XDW workflow document to enable the exchange of text messages (i.e., comments on documents or images) over an XDS infrastructure. RESULTS: The workflow enables the exchange of comments on specific sections of CDA documents or radiological images and was verified in an XDS test environment. CONCLUSION: The presented solution is a proof of concept and the potential basis for the specification of a new IHE workflow definition.

Recently, the field of Metal-Insulator-Transition (MIT) materials has emerged as an unconventional solution for novel energy efficient electronic functions, such as steep slope subthermionic switches, neuromorphic hardware, reconfigurable radiofrequency functions, new types of sensors, terahertz and optoelectronic devices. Employing radiofrequency (RF) electronic circuits with a MIT material like vanadium Dioxide, VO2, requires appropriate characterization tools and fabrication processes. In this work, we develop and use 3D Smith charts for devices and circuits having complex frequency dependences, like the ones resulting using MIT materials. The novel foundation of a 3D Smith chart involves here the geometrical fundamental notions of oriented curvature and variable homothety in order to clarify first theoretical inconsistencies in Foster and Non Foster circuits, where the driving point impedances exhibit mixed clockwise and counter-clockwise frequency dependent (oriented) paths on the Smith chart as frequency increases. We show here the unique visualization capability of a 3D Smith chart, which allows to quantify orientation over variable frequency. The new 3D Smith chart is applied as a joint complex-scalar 3D multi-parameter modelling and characterization environment for reconfigurable RF design exploiting Metal-Insulator-Transition (MIT) materials. We report fabricated inductors with record quality factors using VO2 phase transition to program multiple tuning states, operating in the range 4 GHz to 10 GHz.

Low-dose CT has shown promise in detecting early stage lung cancer. However, concerns about the adverse health effects of radiation and high cost prevent its use as a population-wide screening tool. Effective and feasible screening methods to triage suspicious patients to CT are needed. We investigated human lung cancer metabolomics from 93 paired tissue-serum samples with magnetic resonance spectroscopy and identified tissue and serum metabolomic markers that can differentiate cancer types and stages. Most interestingly, we identified serum metabolomic profiles that can predict patient overall survival for all cases (p = 0.0076), and more importantly for Stage I cases alone (n = 58, p = 0.0100), a prediction which is significant for treatment strategies but currently cannot be achieved by any clinical method. Prolonged survival is associated with relative overexpression of glutamine, valine, and glycine, and relative suppression of glutamate and lipids in serum.

Hydroxide-exchange membrane fuel cells can potentially utilize platinum-group-metal (PGM)-free electrocatalysts, offering cost and scalability advantages over more developed proton-exchange membrane fuel cells. However, there is a lack of non-precious electrocatalysts that are active and stable for the hydrogen oxidation reaction (HOR) relevant to hydroxide-exchange membrane fuel cells. Here we report the discovery and development of Ni3 N as an active and robust HOR catalyst in alkaline medium. A supported version of the catalyst, Ni3 N/C, exhibits by far the highest mass activity and break-down potential for a PGM-free catalyst. The catalyst also exhibits Pt-like activity for hydrogen evolution reaction (HER) in alkaline medium. Spectroscopy data reveal a downshift of the Ni d band going from Ni to Ni3 N and interfacial charge transfer from Ni3 N to the carbon support. These properties weaken the binding energy of hydrogen and oxygen species, resulting in remarkable HOR activity and stability.
