It is known that exposure to solar radiation (SR) can cause various harmful effects on one's health, most of which are related to the ultraviolet (UV) component of SR. A considerably high number of people around the world who work outside are constantly exposed to SR for the majority of their working lives. The eye is the primary organ affected by short-term and long-term exposure to solar radiation. According to an ever-growing body of research, cataracts, pterygium, and macular degeneration are all possible side effects of prolonged exposure. Despite this, the danger of SR exposure is presently underestimated, if not completely overlooked, as a component of occupational risk for employees who do their duties outside. SR exposure is impacted by a wide range of individual and environmental factors; nevertheless, occupational exposure is among the most significant. The scarcity of affordable and accepted methods to measure SR worker exposure, particularly long-term exposure, is one of the key obstacles that must be overcome before a more excellent knowledge of this risk and the development of more effective preventive strategies can be accomplished. This review was conducted with the primary objectives of providing a comprehensive overview of the SR exposure risk of outdoor workers, including UV exposure extents and the chief approaches recently proposed for short-term and cumulative exposure, as well as providing an update on the information presented regarding the most common adverse eye effects. In conclusion, this article will be presenting preventative steps that may be taken to reduce occupational risk.

Chronic myeloid leukemia (CML) is a myeloproliferative neoplasm with raised granulocyte cell line. It arises in the hematopoietic stem cell and is characterized by the presence of BCR-ABL fusion gene as a result of translocation between chromosomes t(9,22) (q34; q11.2). CML affects bone marrow and peripheral blood. CML can be asymptomatic and is usually discovered on routine blood investigations. It can also present as symptoms associated with anemia and splenomegaly. Rarely do CML patients present with retinal abnormalities as an initial presentation. We cover a case of CML in this report that had retinal involvement as one of its earliest manifestations. Patient presented with gradual loss of vision in both eyes. Multiple bilateral intraretinal and preretinal hemorrhages with exudates were found during fundus examination. Blood tests revealed a raised leukocyte count along with lower hemoglobin levels. Peripheral smear revealed 4% blast cells. Bone marrow aspiration showed hypercellular marrow, marked granulocyte proliferation, and raised myeloid erythroid ratio. The diagnosis was further confirmed by fluorescence in-situ hybridization (FISH) which showed BCR-ABL1 fusion gene. Patient was immediately started on chemotherapy and has been on follow-up since then. Visual acuity of the patient improved and there was no progression of retinopathy. This case thus serves as an illustration of how early detection and management can significantly slow the progression of retinopathy and improve visual outcomes.

The greatest global root of irremediable amaurosis in the venerable is age-related macular degeneration (AMD), a complex eye condition. Clinically, AMD is characterized as being in an early stage to late stage and initially affects the macula, which is the center of the retina (advanced AMD). Age-related cellular and metabolic imbalance are made worse by the creation of excessive amounts of free radical species, which causes mitochondrial malfunction. As a result, in AMD-affected eyes, the deprivation of melanocytes, confection, and eventually atrophy within the retinal tissue are caused by the continued proliferation of oxidative stress caused by systemic antioxidant capacity depletion. In the urbanized, industrialized world, age-related macular degeneration (AMD) is one of the major causes of central vision loss in the older age group. Although several causes and mechanisms for the dysfunction and degeneration of the retinal pigment epithelium (RPE) have previously been identified, the condition's symptoms are still not fully understood. Etiopathogenesis is still not entirely understood. As a result, the RPE fails, allowing an accumulation of aberrant misfolded proteins, due to the loss of anatomical control over oppression, altered homeostasis, dysfunctional lipid homeostasis, and failure of mitochondria. Due to the multitude of interconnected processes, numerous complicated therapy combinations will probably be the best option to deliver the best visual outcomes; these combinations will vary depending on the kind and degree of the condition being treated. Undoubtedly, this will lead to the development of customized preventative medications and, hopefully, the revelation of a potential cure. All the mechanisms involved in the etiology of AMD should be continuously probed to create covariates for other contemporaneous or future problems.

Diabetes mellitus is a chronic metabolic disorder with increasing prevalence per hour. Cataracts are one of the most common eye complications, and they affect all structures of the eye. The incidence of cataracts is increasing in patients with diabetes by several mechanisms. With the advancement of technology, cataract surgery is now a necessary procedure for diabetic patients. High-risk complications, like diabetic macular oedema, diabetic retinopathy (DR), phakic, postoperative cyst, and postoperative macular oedema, and macular oedema and endophthalmitis following surgery for a pseudocyst, could result in blindness. The importance of preoperative, intraoperative, and postoperative factors cannot be overestimated in managing complications and improving visual outcomes. DR can be a severe problem if it worsens and causes non-proliferative or proliferative DR or if fluid accumulation in the eye is diagnosed as macular oedema. A woman progressing to sight-threatening DR during childbearing age experiences distress and often requires ocular treatment. Diabetes that has been present for a more extended period, as well as more significant hyperglycaemia, hypertension, cardiovascular diseases, and elevated blood pressure, substantially predict the development of DR. Oxidative stress can be caused by hyperglycaemia, irregular metabolic processes, and people with DR developing neurodegeneration. Therefore, controlling postprandial hyperglycaemia is crucial for preventing DR. Femtosecond laser technology, multifocal intraocular lenses, and other surgical innovations are popularly referred to as surgical management; it will be engaged in the coming era to determine whether there will be a continued reduction in the complication of cataract surgery. This article aims to review the correlation of DR with stroke and its screening and to outline the critical management strategies.

A 34-year-old male presented with a gradual, painless decrease in distant vision in the left more than the right in the past 15 days. He had a history of an episode of fever seven days prior to ocular symptoms and had been treated with oral Cefixime and antipyretics. On detailed fundus examination, both eyes showed creamy white superficial lesions with ill-defined margins, suggestive of retinitis, with a few hemorrhages along the inferior arcade associated with sheathing of vessels and telangiectatic vessels near the lesion. Fundus fluorescein angiography (FFA) of both eyes in the venous phase depicted blocked fluorescence at the site of retinitis lesions with hyperfluorescence at their borders. Optical coherence tomography (OCT) of both eyes showed inner retinal layer hyper-reflectivity with neurosensory detachment (NSD) with hyper-reflective deposits in it and dilated choroidal vessels. The diagnosis of both eyes' post-fever retinitis (PFR) was made, and he was investigated for complete blood count, peripheral smear, erythrocyte sedimentation rate, and HIV Tridot test, which were normal. He was treated with intravitreal triamcinolone acetonide (2 mg/0.05 ml) in both eyes.
