RESUMEN

Myocardial changes were studied in 239 patients with different diseases of the lungs associated with obstructive (84 patients) and restrictive (155 patients) ventilation disorders and in 109 healthy subjects (control group). Structural and electrical indices of the myocardium taken at echo- and vectorcardiography in patients with chronic pulmonary diseases differed from those of healthy subjects, depended on the type and severity of the ventilation disorders. Remodeling of the myocardium in obstructive ventilation disorders is accompanied by a moderate staged enlargement of atrial cavities, right ventricle, thickening of the walls of both ventricles in enhanced electric activity of all the heart compartment. The restrictive ventilation disorders are characterized by an early significant enlargement of both atria's cavities, and of both ventricles with maximal thickening of ventricular walls. Electric activity of the heart rose maximally at the initial stage, later it lowered. Variants of myocardial remodeling in abnormal external respiration were determined by differences in compensatory heart reactions. In the obstructive type, heart remodeling had better compensatory opportunities which manifested itself at all the stages of the ventilatory disorders. In the restrictive type, effective compensatory hyperfunction of the heart was seen only at the first stage of ventilatory disorders, at later stages it weakened.

RESUMEN

AIM: To examine relationships between the areas portions and hemodynamic parameters according to echocardiographic data in patients with various lung diseases accompanied by obstructive and restrictive ventilation disorders. MATERIALS AND METHODS: 154 patients aged 15 to 67 years who had various lung diseases were examined. Expiratory respiratory function and echocardiography were performed in all the patients. RESULTS: Despite the type of ventilation disorders, all cardiac areas are closely interrelated. The specific features of these relationships depending on the type and severity of ventilation disorders are revealed. Thus, the correlations of the right ventricle with other cardiac areas in the obstructive type increase with the greater severity of bronchial obstruction and they are determined by the diastolic size (volume) of this ventricle. CONCLUSION: The findings suggest that there are two types of compensatory reactions of the heart in lung diseases with obstructive and restrictive ventilation disorders.

RESUMEN

Echocardiographic correlations between cardiac compartments were studied in 51 patients with obstructive ventilation disorders due to pulmonary diseases. All the compartments were closely related depending on the severity of the bronchial obstruction. In aggravation of the latter, integration of the atria, right atrium and left ventricle, right and left ventricles of the heart increases. The integration also grows between structural-functional indices of the left ventricle. At initial stages of bronchial obstruction functional and hemodynamic performance of the left ventricle depends more on its size, while later more on the wall thickness. It is demonstrated that obstructive disorders of ventilation promote closer integration of cardiac compartments, mobilization of compensatory hemodynamic reactions. The left ventricle plays an essential role in realization of staged compensatory reactions of the heart. Remodeling of the heart in obstructive pulmonary diseases is reflected in altered intracardiac relationships.

RESUMEN

Twenty-two women with severe gestosis were examined during weeks 27-38. They were divided into 2 groups with different intensive care protocols: 1) osmooncotherapy and 2) 3-4 infusions of perfluorane in a dose of 3-4 ml/kg every other day. Addition of perfluorane to combined therapy promoted a more rapid stabilization of hemodynamics, metabolism, and helped prolong the pregnancy to 37-38 weeks.
