RESUMEN

A pharmacoepidemiological study of drug consumption was performed on 352 patients with III to IV NYHA functional class chronic heart failure (CHF) in different hospitals of Kursk region. The study found that only 51% of expenses related to pharmacological providence of CHF patients were associated with the drugs used directly for the treatment of CHF. Undesirable drug combinations are used more often in city hospitals and hospitals belonging to certain organizations than in the regional clinical hospital.

RESUMEN

For one hundred fifty eight patients with chronic obstructive pulmonary disease (COPD) were the subjects of a pharmacoepidemiological study. The study found that both pharmaceuticals indicated for the treatment of this disease, and drugs of low effectiveness, were used in treatment of these patients very intensively. Each hospital led its own policy of the distribution of expenses associated with COPD treatment.

RESUMEN

The results of a comparative clinicopharmacological study of adalate, foridone, siskor and lomir effects on cerebral and peripheral blood flow are presented for 46 patients with arterial hypertension stage I-III. It was found that all the four drugs relieve tonicity and enhance blood flow in a. cerebri media and a. carotis communis. Lomir and adalate had more potent effects on central hemodynamics. A maximal dilating effect on regional peripheral arteries was observed after a single intake of siskor and adalate, while lomir's and foridone's single doses were weaker. In course treatment with the above drugs these differences are less.

RESUMEN

A comparative study of pharmacodynamics and clinical efficacy of nimodipine, nifedipine and foridon in ischemic stroke (late recovery and residual period) was performed in 97 patients with ischemic stroke. It was found that nimodipine, nifedipine and foridon at late recovery or residual stages significantly improved higher psychic functions, promoted regression of neurologic disorders and optimized social rehabilitation. Nimodipine, primarily, decreased the intensity of neurological disorders, improved higher psychic functions and had a positive impact on cerebral hemodynamics. Nifedipine primarily improved central hemodynamics (increased stroke output and ejection fraction). Foridon is most effective selective pharmacodynamic impact on peripheral hemodynamics (stimulated arteriolar blood flow, diminished venous capacity and spasticity), i.e. normalized peripheral blood flow.

RESUMEN

Antianginal and hypotensive efficacy of etizem was studied in 40 patients with ischemic heart disease (IHD), stable angina of effort (moderate-to-severe) for 4 weeks and 43 patients with arterial hypertension (mild-to-moderate) for 12 weeks. A course treatment with etizem lowered mean 24 hour, mean diurnal and mean nocturnal blood pressure (both systolic and diastolic, normalized blood pressure in most cases of abnormal 24 hour diastolic blood pressure profile. Etizem also significantly improved left ventricular diastolic function but failed to do so in relation to myocardial mass of the left ventricle.

It was established that a regular administration of isradipine is accompanied by an increase in the mean values of Cmax (by 21.7%) and AUC (by 21.8%), which leads to extension of the pharmacodynamics effects without cumulation (the values of Clt, T1/2, and Tmax remain unchanged). The maximum hemodynamic effect of isradipine upon a single administration in a single dose of 5 mg coincides with the time or reaching the maximum drug concentration in the blood (Tmax). There is a reliable correlation between the hypotensive effect, the peripheral vascular resistance, and the concentration of isradipine.

RESUMEN

A total of 56 patients with coronary heart disease (CHD), angina pectoris of effort, functional classes II-III, were placed under observation. All the patients received 20 mg foridon (F), 20 mg corinfar (C), 80 mg anapriline (A). 24 patients were subjected to the continuous treatment with F (20 mg 4 times a day). The antianginal action of the drug was compared to the blood concentration on days 7, 14, 21, 28, 43 and 57 of the treatment with F. In 14 patients with angina pectoris and in 18 patients with essential hypertension, the efficacy of F and C was cross correlated. It has been demonstrated that F can be successfully used for the treatment of patients with CHD, angina pectoris of effort and (or) essential hypertension. The increase of the single dose of foridon from 20 to 40 mg does not result in the potentiation of its hypotensive effect. F and C provoke the highest rise of exercise tolerance with combined angina pectoris. Continuous concomitant administration of F and C with A leads to the potentiation of the antianginal action.
